Aims : The review was to determine the prevalence of needlestick injury especially among health care personnel and to determine the score of knowledge and compliance to the Standard Universal Precaution.
Method : Twelve theses were reviewed from year 1996 to 2007 of Master in Community Health Science, Master in Community Health and Postgraduate Diploma in Occupational Health of Community Health Department Universiti Kebangsaan Malaysia Medical Center involving of 1645 respondents of health care personnel, support staff and student of nursing and medical. Eleven studies were cross sectional design and only one study was retrospective.
Results : Respondents were dominated by female (74.6%) and Malay ethnicity (65%), as young as 19 years old to 56 years old of age. Majority had received Hepatitis B vaccination (79.4%) but only 37.1% had completed the 3 doses regime. The incidence of episodes of needle stick injury among health care personnel was double (53.7%) compared to final year medical students (20.9%). Needle stick injuries did not occur at random as there were a few health care personnel injured repeatedly. Those who had higher mean or median score for compliance to Standard Universal Precaution were non case of needlestick injury. Work practices had been highlighted in few studies of being risk factors for needlestick injury such as blood withdrawing related activities. Other risk factors were job category, predictive factor for compliance to Standard Universal Precaution, risk perception and training.
Conclusion : Even though the review could not extrapolated to general population of healthcare personnel but it gave some illustrated pictures to what had happened in small clustered locations. Episodes of needle stick injury was 53.7% for past 12 years, it was double in comparison to final year medical students in year 2001. 13.2% were injured repeatedly. The score of knowledge was more the 50% of range but compliance to Standard Precaution made differences in being cases or non cases among respondents. The seroconversion status till date was unknown.
A cross sectional study on work stress prevalence was carried out among 185 executives and managers in the head quarter of an international tobacco company. The aim of the study was to identify work stress prevalence in this company and work stressors that were associated with stress experienced by the workers. A questionnaire based on the Personal Stress Inventory by O'Donell (1984) was used. Data collected from participants included sociodemography factors, symptoms of stress and work stressors related to organizational policy, organizational structure, organizational process, and work environment. The study showed that the prevalence of stress among executives was 68.1% and managers were 67.9%. There was no significant difference in the level of stress between the executives and the managers in the company. The sociodemographic factors that were significantly associated with stress were salary, number of children and personal factors. The significant stressors in the workplace were lack of job recognition, over focusing on quality of work, heavy workload and long working hours.
In this paper, an improved method of reducing ambient noise in speech signals is introduced. The proposed noise canceller was developed using a computationally efficient (DFT) filter bank to decompose input signals into sub-bands. The filter bank was based on a prototype filter optimized for minimum output distortion. A variable step-size version of the (LMS) filter was used to reduce the noise in individual branches. The subband noise canceller was aimed to overcome problems associated with the use of the conventional least mean square (LMS) adaptive algorithm in noise cancellation setups. Mean square error convergence was used as a measure of performance under white and ambient interferences. Compared to conventional as well as recently developed techniques, fast initial convergence and better noise cancellation performances were obtained under actual speech and ambient noise.
Cranium dysraphism occur less commonly than spinal dysraphism. Overall, occipital encephalocoeles are more frequent than anterior encephalocoes. A large occipital encephalocoele possess difficulty in surgical intervention, not only the sac contains large amount of herniated brain tissue, but the appropriate positioning for successful intubation as well as the higher rate of surgical complications such as cerebrospinal fluid leakage and skin necrosis. In this case report, surgical management of large occipital encephalocoele is discussed. The use of antibiotics in ruptured encephalocoele is recommended.
Rapunzel syndrome is a syndrome whereby a gastric trichobezoar (hair ball) extends through the pylorus, in the form of a long tail, to cause gastric outlet obstruction. It was first described by Vaughan et al. in 1968. The syndrome is mostly seen in young females with psychiatric illness. Presentations can be non-specific especially in the early stages. If left untreated it may leads to severe complications, which may include gastric ulceration, intestinal obstruction, perforated viscus and obstructive jaundice. Treatment is essentially surgical and psychi- atric consultation is necessary to prevent relapses. We present the case of a 26-year-old mentally subnormal lady with gastric trichobezoar and provide a review of the literature.
Introduction: Diabetes Mellitus (DM) is a chronic metabolite disorder with high potential of multisystemic medical complications especially among poorly controlled patients. This study was conducted at Raja Perempuan Zainab II Hospital, Kota Bharu with objectives to explore the pattern of common diabetic mellitus type 2 complications and to compare between gender and races in relation to other risk factors during their hospitalization. Materials and Methods: This is a retrospective study based on patients’ record involving 215 patients (110 women and 105 men) who were admitted with diabetic complications in medical ward for a period of 6 months (January to June 2006) Results: Out of total 300 identifi ed subjects, 285 case notes were traceable. From 285, 84.6% (241 subjects) were diagnosed as DM type 2. 215 out of 241 subjects (89.2%) had one or more diabetic complications. In general the most common diabetic complication was diabetic nephropathy 34.9% (75 patients). When we compared between genders, diabetic foot ulcers was the most common diabetic complications among males (43.8%) and diabetic nephropathy for females (40.9%). A total of 199 patients (92.6%) had poor blood sugar control during their hospitalization and 103 patients (47.9%) had hypertension. Conclusion: Patients with DM type 2 had a high prevalence of complications. Common diabetic complications among genders were varied. Patients with diabetic complications had high prevalence of risk factors such as poor blood sugar control, concomitant hypertension and smoking habit.
Postural movements potentially affect aiming stability in archery, thus contributing to chances of inconsistent hits. According to the expertisenovice paradigm, the factor that sets winners apart from ordinary athletes is the former’s ability to control minute changes in their performance. The
present study seeks to determine the relationship between postural sway and shooting performance amongst Malaysian skilled recurve archers. Twenty one skilled Malaysian archers participated in this study, where performance level was measured by rank tournaments International Archery Federation (FITA) score. Postural sway was assessed in terms of anterior deviation (positive value) and posterior deviation (negative value) using ZEPHYR Bio-Harness. Postural sway was analysed at the following three phases; (i) setup, (ii) aiming, and (iii) release. Participants shot 12 arrows to a 30-meter target. Data yielded a significant relationship between postural sway and shooting performance. The correlation coefficients between shooting performance and postural sway value for skilled archers ranged between (r = -0.021 to 0.248) with the highest correlation recorded at the release phase, with the lowest at the aiming phase. The setup phase showed the only anterior deviation throughout the test. During the setup and release phases, correlation between postural sway with shooting performance was significantly noted (p < 0.001). Multiple regression analysis showed that postural sway during the setup and release phases were the significant indicators for shooting performance, accounting approximately 17% and 24% of the variances respectively. In sum, the results indicate that reducing postural sway
during the release phase can increase shooting performance of skilled archery athletes, thus establishing a significant relationship between the postural sway value with shooting performance of skilled archers.
Retinal detachment is one of the common complications of pathological myopia due to presence of retinal break.
However, retinal break commonly occurs in the peripheral retina. This case report illustrates the rare incidence of
retinal break adjacent to the optic disc, highlights the possible causes of poor visual outcome following surgical
repair as well as the possible measures to treat the complications.
The importance of the performance of concrete cannot be neglected since it is the early indicator of its physical and mechanical properties. It became more important when material with different physical properties than normal material such as rubber tire was used as concrete constituent. This paper presented apart of research result conducted on mortar and concrete with crumb rubber. Crumb rubber was replaced at 10%, 15% and 20% as sand replacement by volume. In addition, ordinary Portland cement was added to silica fume at 10% and 15% by weight. The properties measured in this study are air content and workability test. As for workability, superplasticizers were constantly used at 1% dosage for all mortar mixture, and 0.5% to 0.7% for concrete mixture. The air content was set at 4% to 6% and mortar flow test was conducted on a steel plate, shocked 15 times in 15 seconds and concrete slump test was carried out using slump cone equipment. Pressure method was used to measure air content. All mixes were done in a controlled room temperature. Results showed that when CR was added in the mixture segregation was observed in mortar requiring a high dose of superplasticizer to be added to improve the workability while air-modifying agent was used to reduce the mortar air content. In concrete mixture, low dosage of superplasticizers was required for workability and air-entrained agent was injected into the mixture to increase the air content between 4%-6%.
Simulating Lotka-Volterra model using numerical method require researchers to apply tiny mesh sizes to obtain an accurate result. This approach nevertheless increases the complexity and burden of computer memory and consume long computational time. To overcome these issues, we investigate and construct new two-step solver that could simulate Lotka-Volterra model using bigger mesh size. This paper proposes three new two-step schemes to simulate Lotka-Volterra model. A non-standard approximation scheme with trimean approach was adopted. The nonlinear terms in the model is approximated via trimean approach and differential equation via non-standard denominators. Four sets of parameters were examined to analyse the performance of these new schemes. Results show that these new schemes provide better simulation for large mesh size.
The increasing adoption of social media is a viable means in crowdsourcing. It can facilitate the connectivity of collaboration between different organisations, people and society to produce innovative and cost-effective solutions to many problems. Social media have opened up unprecedented new possibilities of engaging the public in meaningful ways through crowdsourcing. However, the growing number of security and privacy issues in social media may weaken the efficacy of crowdsourcing. This study aims to provide a basic understanding of security and privacy issues in line with the growth of crowdsourcing using social media platforms. This study also illustrates how crowdsourcing and social media data can lead to security and privacy issues in different environments. Lastly, this study proposes future works that may serve as direction for scholars to explore security and privacy in crowdsourcing through social media platforms. Secondary sources obtained from journals, conference papers, industry reports and books were reviewed to gather information.
Health care has emerged as one of the fastest growing industry worldwide. This induced health care costto rise tramendously. However, it is important to preserve high quality health care services that are equitable and affordable. In many countries, people are expected to contribute to the cost of the health care. Are populations ready to accept the concept and willing to pay for health financing scheme? What possible factors that may associate with their decision? This is the objective of the study, to examine the relevance evidence for this through a systematic review of literatures.We systematically searched Ovid MEDLINE and Google Schoolar databases until April 2016. We assessed the study population willingness to pay for health financing scheme and determine the significant variables that associate with WTP. 19 full-text articles were included in the review. Factors that were found significantly associated with WTP for health financing scheme by many studies were age, education, income and residential locality. Other factors that also found associated with WTP were health care services utilization and expenditure. The review findings showed that WTP for health financing scheme is beyond the households’ financial capacity and has multifactorial influences.
HIV/AIDS is a matter of concern and has become a pandemic disease that threatens the world population. It
has remained one of the priorities in global public health challenges. Caring for people with HIV/AIDS
requires clinical expertise as well as high-quality skills for health care providers. The purpose of the study was
to examine knowledge and attitude on HIV/AIDS patients among nursing students. A cross-sectional study
was conducted in Public University at Wast Cost Malaysia. The 126 respondents of nursing students agreed to
participate in the study using a self-administered questionnaire that was designed and modied after reading
and referring to articles, books, and journals. The results of the research revealed that nursing students, in
general, have a satisfactory knowledge (mean score 30.71), but had a moderate attitude toward HIV/AIDS
patient with total mean score 2.34. Most of the students (89.3%) get the knowledge regarding HIV/AIDS
through the internet. The majority of the students knew that blood transfusion (99.4%) and sharing
needles/syringe (99%) were the higher sources HIV/AIDS modes of transmission. Most of the students agreed
that peoples with HIV/AIDS should be treated in society with support from the community (97.0%). Although
the knowledge level of nursing students seems to be satisfactory, but they still showed the moderate attitude
towards HIV/AIDS patients. Thus, structured education programmes should be conducted by nursing faculty
to ensure that nursing students gain the necessary accurate knowledge and an appropriate attitude towards the
care of HIV/AIDS patients.
Cardiovascular disease (CVD) is a major cause of disability and premature death throughout the world. This disease is commonly experienced by people with unhealthy lifestyle, stress and physical inactivity. Cholesterol has received the most attention as single risk factor of CVD. Reducing the intake of cholesterol, saturated fat, and trans faty acids may be beneficial, yet controversy is still lingering to what constituents more beneficial dietary fats. The purpose of this article is to give an overview on the impact of major dietary fatty acids on cardiovascular morbidity and mortality and to give an insightful information regarding fatty acids composition in selected fruits oils in search for novel oils as potential therapy against CVD.
The aim of this paper is to review the potentialities and major methodological challenges
of integrating remote sensing (RS) and geographic information system (GIS) with socioeconomic data
from published articles or book chapters. RS and GIS combined with social science (SS)(termed as
geoinformation technology) serve many applications for sustainable management and monitoring of
the environment. This combined approach gives more accurate results than the single one. It makes
information available about the trend and pattern of land use and land cover change (LUCC) with
socioeconomic variables like population, demographic or income. This combined study which links
RS and GIS with socioeconomic data can also be used successfully for monitoring transmission rate
of disease and mapping or preparing vulnerability index. For impact assessment and modelling, this
combined technology provides better results than the single one. There are some methodological
problems for the researchers to link completely two different disciplines as the object of study and
observational unit is completely different. However, this interdisciplinary study is gaining popularity
day by day to researchers from different disciplines as well as decision makers.
Nitric oxide (NO) is a signaling molecule involved in numerous physiological processes in
both animals and plants. The bioactivity of NO is mainly transduced via post-translational modification
of cysteine residues of proteins termed S-nitrosylation. Interestingly, a number of key regulatory
components in plant defense responses have been found to be regulated by S-nitrosylation making this
type of protein modification an important modulator of plant immunity. As a signaling molecule, NO
intimately interact with other important molecules such as reactive oxygen species. Since the
identification of NO in plants, increasing number of papers isbeing published in the area of NO biology
each year. Here, a collection of papers describing the role of NO in plant immunity has been brought
together to provide a bird's-eye view on the focus area.
The mengkudu (Morinda citrifolia Linn.) plant is a well known medicinal plant. The fruits, leaves and roots of this plant have been widely used in traditional medicine. The purpose of this study was to evaluate the effects of an aqueous extract of the M. citrifolia fruits (25 mg/kg, 50 mg/kg and 100 mg/kg body weight given for a period of 30 days) on sperm quality and histology of testis in mice. Data from this study show that the epididymal sperm count, mortality, motility and sperm morphology in mice given the M. citrifolia fruit extract did not differ from that of the control group. Spermatogenesis was not affected by the treatment. The results obtained here suggest that the aqueous extract of the M. citrifolia fruit (up to 100 mg/kg) did not affect sperm quality and spermatogenesis in mice.
Mengkudu (Morinda citrifolia Linn.) adalah tumbuhan ubatan yang terkenal. Bahagian buah, daun dan akarnya digunakan secara meluas dalam perubatan tradisional. Kajian ini dijalankan untuk menentukan kesan pemberian ekstrak berakua buah M. citrifolia (dos 25mg/kg, 50 mg/kg dan 100 mg/kg berat tubuh mencit selama 30 hari) ke atas kuantiti sperma dan histologi testis mencit. Data daripada kajian ini menunjukkan bilangan sperma epididimis, mortaliti, motiliti dan morfologi sperma mencit kajian tidak berbeza berbanding kumpulan kawalan. Proses spermatogenesis juga tidak terjejas dengan pemberian M. citrifolia. Keputusan kajian ini mencadangkan ekstrak berakua M .citrifolia (sehingga 100 mg/kg) tidak mendatangkan kesan terhadap kualiti sperma dan spermatogenesis mencit.
Effect of Centella asiatica aqueous leaf extract was assessed on spermatogenesis and sperm quality in mice. The experiment was conducted by giving three different doses of C. asiatica extract orally; 25mg/kg, 50mg/ kg and 100mg/kg body weight daily for four weeks respectively. Control group was treated with normal saline (0.9% NaCl). Marked decreases in sperm count and motility were found in sperm collected from the cauda epididymis of treated mice. The treatment, however did not affect sperm mortality and morphology. Histologically, signs of inhibition of testicular cells appeared clearly at dose 100mg/kg, when the lumen of seminiferous tubules were only full with spermatocytes and spermatids. The present results show that the plant has antispermatogenic effect in mice. It is therefore, suggested that leaf extracts of C. asiatica possess antifertility activity in male mice.
Kesan ekstrak berakua daun Centella asiatica telah dikaji ke atas proses spermatogenesis dan kualiti sperma mencit. Kajian dijalankan dengan memberikan tiga dos C. asiatica secara oral masing-masing; 25mg/kg, 50mg/ kg and 100mg/kg berat tubuh mencit setiap hari selama empat minggu. Kumpulan kawalan diberikan larutan salin normal (0.9% NaCl). Pengurangan dalam bilangan dan motiliti sperma yang ketara telah diperhatikan pada semua kumpulan mencit yang diberikan ekstrak C. asiatica. Walau bagaimanapun pemberian ekstrak C. asiatica tidak mendatangkan kesan terhadap mortaliti dan morfologi sperma. Kajian histologi pula menunjukkan terdapat kesan perencatan sel-sel spermatogenik testis pada dos C. asiatica 100mg/kg di mana lumen pada tubul seminiferus hanya dipenuhi dengan sel-sel spermatosit dan spermatid. Hasil kajian menunjukkan tumbuhan ini mempunyai kesan antispermatogenik pada mencit. Oleh itu dicadangkan bahawa ekstrak daun C. asiatica mempunyai aktiviti antikesuburan pada mencit jantan.
Copper phthalocyanine (CuPc) thin films have been prepared using a simple spin coating method. The films were annealed at 5 different temperatures (323, 373, 473, 523 and 573 K) for one hour in air. Optical properties study using the UV-Vis spectrophotometer showed that in the range of wavelength of 300-800 nm, all of the films have identical absorption coefficient patterns and there was no systematic changes with respect to annealing temperature. The film annealed at 373 K showed the highest absorbance while the lowest absorbance was shown by the film annealed at 323 K. The results showed that the optical band gaps depended on the temperature. The film annealed at 373 K has the lowest optical energy gap. Using the five annealed films, solar cell with the configuration of Ag / n-Si / CuPc / Ag were fabricated. Under the 50 W/cm2 light illumination, the current voltage measurements at room temperature were carried out on the device. The device which consists of film annealed at 373 K exhibited the best photovoltaic characteristics. The different annealing temperature also affect the photovoltaic behavior of the devices in a non-systematic way.
Sanrego (Lunasia amara Blanco) digunakan masyarakat tempatan di Indonesia sebagai bahan afrodisiak dan mampu mengatasi masalah kesuburan lelaki. Penyelidikan ini mengkaji kesan pengambilan ekstrak akuas sanrego pada dos 30, 60 dan 90 mg/kg berat badan masing-masing terhadap bilangan, motiliti dan morfologi sperma serta tahap kesuburan dan afrodisiak tikus jantan. Kumpulan kawalan diberikan air suling. Semua perlakuan diberikan secara suap-paksa sekali sehari pada pukul 10.00 pagi-11.00 pagi selama 42 hari diikuti dengan analisis sperma dan ujian tahap kesuburan. Analisis sperma menunjukkan bahawa sanrego mempunyai kesan peningkatan bilangan sperma dengan gred motiliti terbaik (gred a) dengan signifikan tanpa mempengaruhi morfologi sperma. Pengujian tahap kesuburan menunjukkan peningkatan pada kumpulan haiwan perlakuan. Jumlah bilangan pemanjatan tikus jantan dengan perlakuan sanrego juga lebih tinggi berbanding kumpulan kawalan. Kajian ini mendapati tumbuhan sanrego berpotensi meninggikan kesuburan dan mempunyai kesan afrodisiak pada haiwan jantan.
