    This is a retrospective study to determine the distribution site, associated fracture and causes of mandibular fractures at HUSM, over a 5 year period, from 1st January 2002 - 31st December 2006. Records of patients who had mandibular fracture were reviewed. Data of age, sex, site of fracture, causes and associated fracture were recorded and analyzed using SPSS version 15.0. There were 113 (84.3%) males and 21 (15.7%) females. The mean age for male was 84.3% and female was 15.7%. The fracture occurs mostly at the age of 11-20 years (45.5%), followed by 21-30 years (30.6%). Motor vehicle accidents (MVA) were the commonest causes of mandibular fracture (92.5%), followed by fight and assault (3.7%), industrial accidents (3.0%), fall (0.7%). There were no cases recorded due to sport injury. The commonest site of mandibular fracture occurs at angle and para-symphysis (23%), followed by body (20.1%), symphysis (16.7%), condyle (15.5%) and ramus (1.7%). The most common associated fractures were head injury (23.5%), followed by clavicle fracture (17.2%) and fracture of radius (10.7%). Mandibular fracture was common in males with the mean age 24.63 years and mostly due to MVA. Angle and para-symphysis is the commonest site of mandibular fracture with most of the patient suffered from concomitant head injury.

    According to crash configuration and pre-crash conditions, traffic crashes are classified into different collision types. Based on the literature, multi-vehicle crashes, such as head-on, rear-end, and angle crashes, are more frequent than single-vehicle crashes, and most often result in serious consequences. From a methodological point of view, the majority of prior studies focused on multivehicle collisions have employed univariate count models to estimate crash counts separately by collision type. However, univariate models fail to account for correlations which may exist between different collision types. Among others, multivariate Poisson lognormal (MVPLN) model with spatial correlation is a promising multivariate specification because it not only allows for unobserved heterogeneity (extra-Poisson variation) and dependencies between collision types, but also spatial correlation between adjacent sites. However, the MVPLN spatial model has rarely been applied in previous research for simultaneously modelling crash counts by collision type. Therefore, this study aims at utilizing a MVPLN spatial model to estimate crash counts for four different multi-vehicle collision types, including head-on, rear-end, angle, and sideswipe collisions. To investigate the performance of the MVPLN spatial model, a two-stage model and a univariate Poisson lognormal model (UNPLN) spatial model were also developed in this study. Detailed information on roadway characteristics, traffic volume, and crash history were collected on 407 homogeneous segments from Malaysian federal roads. The results indicate that the MVPLN spatial model outperforms the other comparing models in terms of goodness-of-fit measures. The results also show that the inclusion of spatial heterogeneity in the multivariate model significantly improves the model fit, as indicated by the Deviance Information Criterion (DIC). The correlation between crash types is high and positive, implying that the occurrence of a specific collision type is highly associated with the occurrence of other crash types on the same road segment. These results support the utilization of the MVPLN spatial model when predicting crash counts by collision manner. In terms of contributing factors, the results show that distinct crash types are attributed to different subsets of explanatory variables.

    The child restraint legislation in Malaysia becomes mandatory from 1 January 2020. Prior to commencement of the rule, a survey showed that only 36% of Malaysian parents were aware of the importance of a child restraint system (CRS) and only 27% usage was reported during travel. The Malaysian Institute of Road Safety report showed that children transported in private vehicles were the leading groups of casualties among children aged 1 to 4 years old (43.8%) and 5 to 9 years old (30.2%), respectively. We performed a narrative review using the PubMed, ScienceDirect and Google Scholar databases using keywords such as child restraint system, unrestrained injuries, Malaysia and epidemiology. The objectives of this review were: (1) to determine the prevalence on the use of CRS in Malaysia, (2) to evaluate the injuries related to unrestrained children and (3) to show the nation's preparation towards implementation of the child restraint law. Six papers on prevalence, one paper on injury and six mainstream newspaper were included in this study. The prevalence of a CRS use was shown between 5% to 41.8%. In relation to injury, the only publication from this country showed that among 19 children involved in a car crash, five (26.3%) children had non-craniomaxillofacial (CMF) injuries, ten (52.6%) with CMF injuries only, two (10.5%) with both CMF and non-CMF injuries and two (10.5%) without any injury. Overall, the Injury Severity Score (ISS) range was between 0 to 13 (median, 1.00; interquartile range, 1). Preparation to comply with the best practice of the child restraint law is still ongoing, especially those addressing the issues related to the low-income parents in the country. Due to scarcity of publication and data on the CRS use and injuries related to its non-usage, it is advocated that parallel with the implementation legislation, vigorous forms of public education as well as good data management must be performed and monitored regularly by the road safety authority in this country.

    AIM: Previous studies have investigated home hazards as a risk factor for falls without considering factors associated with the presence of home hazards. The present study aimed to determine patterns of home hazards among urban community-dwelling older Malaysians, and to identify factors contributing to home hazards.

    METHODS: Cross-sectional data from the initial wave of the Malaysian Elders Longitudinal Research study were used. Basic demographics were obtained from the Global Questionnaire. Basic and instrumental activities of daily living were measured using the Katz and Lawton-Brody scales, and home hazards were identified using the Home Falls and Accidents Screening Tool. Participants were also asked if they had fallen in the previous 12 months.

    RESULTS: Data were analyzed from 1489 participants. Hazards were frequently identified (>30%) in the toilet and bathroom areas (no grab rail, no non-slip mat, distant toilet), slippery floors, no bedside light access and inappropriate footwear. Lower educational attainment, traditional housing, Chinese ethnicity, greater number of home occupants, lower monthly expenditure, poor vision and younger age were the factors independently associated with home hazards.

    CONCLUSIONS: This study provides evidence that home hazards are a product of the interaction of the individual's function within their home environment. Hazards are also influenced by local sociocultural and environmental factors. The relationship between home hazards and falls appears complex and deserves further evaluation.     This study had developed a passenger safety perception model specifically for buses taking into consideration the various factors, namely driver characteristics, environmental conditions, and bus characteristics using Bayesian Network. The behaviour of bus driver is observed through the bus motion profile, measured in longitudinal, lateral, and vertical accelerations. The road geometry is recorded using GPS and is computed with the aid of the Google map while the perceived bus safety is rated by the passengers in the bus in real time. A total of 13 variables were derived and used in the model development. The developed Bayesian Network model shows that the type of bus and the experience of the driver on the investigated route could have an influence on passenger's perception of their safety on buses. Road geometry is an indirect influencing factor through the driver's behavior. The findings of this model are useful for the authorities to structure an effective strategy to improve the level of perceived bus safety. A high level of bus safety will definitely boost passenger usage confidence which will subsequently increase ridership.

    BACKGROUND: The manufacturing industry is one of the catalysts for the Philippines. However, this sector is one of the most dangerous industries in the Philippines considering the frequency of occupational injuries.

    OBJECTIVE: To determine the primary and root causes of recorded accidents, demographics of the person involved, and solutions to prevent the recurrence of certain accidents.

    METHODS: This study analyzed 185 occupational injury cases in a food and beverage manufacturing company in the Philippines from January to December 2018. A comprehensive classification system was established to examine and code each case in terms of age, gender, working shift, employee type, tenure, department, category, activity during the accident, root cause of injury, injury classification, direct cause of injury, type of injury, part of body injured, agent of injury, and location of the accident. Cramer's V analysis and Phi coefficient analyses were employed on the subject cases to determine the significant factors and the corresponding extent of significance.

    RESULTS: The results showed that the majority of the occupational injuries were caused by stepping on, striking against, or stuck by objects (77 cases, 41.6%), caught in between (34 cases, 18.4%), fall (34 cases, 18.4%), and exposure or contact with extreme temperatures (24 cases, 13%). Interestingly, female workers who had accidents were more likely due to inadequate hazard information or lack of procedures whereas male workers were more likely due to failure to secure. The prevention measures such as passive safeguards and personal protective equipment, pictograms, and regular safety audits were derived from the results of these analyses.

    CONCLUSIONS: This study is the first comprehensive analysis of occupational injuries in the food and beverage industry in the Philippines. The findings can be applied to positively influence the effectiveness of prevention and rehabilitation programs mitigating workplace injuries and illnesses.

    It has been shown that road safety laws, such as motorcycle helmet and safety belt laws, have a significant effect in reducing road fatalities. Although an expanding body of literature has documented the effects of these laws on road safety, it remains unclear which factors influence the likelihood that these laws are enacted. This study attempts to identify the factors that influence the decision to enact safety belt and motorcycle helmet laws. Using panel data from 31 countries between 1963 and 2002, our results reveal that increased democracy, education level, per capita income, political stability, and more equitable income distribution within a country are associated with the enactment of road safety laws.

    Nighttime driving presents a critical challenge to road safety due to insufficient lighting and increased risk of driver fatigue. Existing methods for monitoring driver fatigue, mainly focusing on behavioral analysis and biometric monitoring, face significant challenges under low-light conditions. Their effectiveness, especially in dynamic lighting environments, is limited by their dependency on specific environmental conditions and active driver participation, leading to reduced accuracy and practicality in real-world scenarios. This study introduces a novel 'Illumination Intelligent Adaptation and Analysis Framework (IIAAF)', aimed at addressing these limitations and enhancing the accuracy and practicality of driver fatigue monitoring under nighttime low-light conditions. The IIAAF framework employs a multidimensional technology integration, including comprehensive body posture analysis and facial fatigue feature detection, per-pixel dynamic illumination adjustment technology, and a light variation feature learning system based on Convolutional Neural Networks (CNN) and time-series analysis. Through this integrated approach, the framework is capable of accurately capturing subtle fatigue signals in nighttime driving environments and adapting in real-time to rapid changes in lighting conditions. Experimental results on two independent datasets indicate that the IIAAF framework significantly improves the accuracy of fatigue detection under nighttime low-light conditions. This breakthrough not only enhances the effectiveness of driving assistance systems but also provides reliable scientific support for reducing the risk of accidents caused by fatigued driving. These research findings have significant theoretical and practical implications for advancing intelligent driving assistance technology and improving nighttime road safety.

    This study examines the risk characteristics of fatal motorcycle crashes in Cambodia over a 5-year period (2007-2011). Secondary data analyses were conducted using the Cambodia Road Crash and Victim Information System, the only comprehensive and integrated road crash surveillance system in the country. Researchers from the Centers for Disease Control and Prevention and Handicap International found that (1) males are dying in motorcycle crashes roughly seven times more frequently than females; (2) motorcyclist fatalities increased by about 30% from 2007 to 2011; (3) the motorcyclist death rates per 100,000 population increased from 7.4 to 8.7 deaths from 2007 to 2011; and (4) speed-related crashes and not wearing motorcycle helmet were commonly reported for motorcyclist fatalities at approximately 50% and over 80% through the study years, respectively. Additionally, this study highlights that Cambodia has the highest motorcycle death rate in South-East Asia, far surpassing Thailand, Malaysia, and Myanmar. By recognising the patterns of fatal motorcycle crashes in Cambodia, local road-safety champions and stakeholders can design targeted interventions and preventative measures to improve road safety among motorcyclists.

    Fatigue-related crashes have long been the topic of discussion and study worldwide. The relationship between fatigue-related crashes and time of day is well documented. In Malaysia, the possibility of banning express buses from operating during the early-hours of the morning has emerged as an important consideration for passenger safety. This paper highlights the findings of an impact assessment study. The study was conducted to determine all possible impacts prior to the government making any decision on the proposed banning. This study is an example of a simple and inexpensive approach that may influence future policy-making process. The impact assessment comprised two major steps. The first step involved profiling existing operation scenarios, gathering information on crashes involving public express buses and stakeholders' views. The second step involved a qualitative impact assessment analysis using all information gathered during the profiling stage to describe the possible impacts. Based on the assessment, the move to ban early-hour operations could possibly result in further negative impacts on the overall road safety agenda. These negative impacts may occur if the fundamental issues, such as driving and working hours, and the need for rest and sleep facilities for drivers, are not addressed. In addition, a safer and more accessible public transportation system as an alternative for those who choose to travel at night would be required. The proposed banning of early-hour operations is also not a feasible solution for sustainability of express bus operations in Malaysia, especially for those operating long journeys. The paper concludes by highlighting the need to design a more holistic approach for preventing fatigue-related crashes involving express buses in Malaysia.

    We report our experience of treating two patients with ankle amputation with different presentations. The first case was a clean-cut sharp amputation. The second case was an avulsion injury following a motor vehicle accident in a patient who arrived 8 hours after the injury. Replantation was successful in both cases. In avulsion injuries, a secondary operation for wound coverage is required at a later stage. With good strategy and a support team, encouraging limb survival outcomes are possible post-replantation.

    Like many other parts of the globe, in Malaysia, construction industry has a vital support to
    the national economy, even though statistics indicates that, at the same time, it is an extremely
    risky profession due to high accident rate. In addition, current evidences suggest that, in
    general, safety in Sarawak construction industries lack attention. As, human behavior is regarded
    as the principal component that contributes to accidents in this industry, this study
    intends to explore existing barriers in development and maintaining safety performance behavior
    as well as possible coping strategies. A qualitative analysis of in-depth interview (N=4)
    among different levels of professions within industries in Kuching, Sarawak reveals that,
    price sensitive subcontracting, short-term projects, irregular cash flow, dependency on larger
    companies, less managerial and supervisory engagement, and priority on progress alone create
    obstacles for safety behavior. On the other hand, possibly, the challenges can be managed
    through integration of safety into contracts as compliance, associated disbursement, and introducing
    incentives.

    Road accident is not stranger matter in Malaysia. Subsequently, often leads to a claim for personal injury by the persecuted party. In Malaysia, the method for calculating claims applies a multiplier-multiplicand approach. This approach is no longer relevant and unfair to the claimant as it excludes personal status in the quantum calculation of damages. Hence, this study uses the Ogden Table as introduced in the United Kingdom as benchmarking guidelines, by taking into account of all aspect of claimant's personal condition for the purpose of such calculation. This study is built upon a proposed framework of data modelling system known as Entity Relationship Diagram (ERD) and the created process modelling known as data flow diagram (DFD). Doing so, the claimants will insert their input data, run it through the first process, and store the information in the claim injury part database. They can also edit and store to claim injury part database on their own. This will generate a report with the information in claim injury part database and can be viewed by claimant, court and lawyer as target users. It is hoped that it will facilitate the calculation of injury claim which would serve justice and accuracy of personal injury in road accidents

    In recent years, driver drowsiness has been one of the major causes of road accidents and can lead to severe physical injuries, deaths and significant economic losses. Statistics indicate the need of a reliable driver drowsiness detection system which could alert the driver before a mishap happens. Researchers have attempted to determine driver drowsiness using the following measures: (1) vehicle-based measures; (2) behavioral measures and (3) physiological measures. A detailed review on these measures will provide insight on the present systems, issues associated with them and the enhancements that need to be done to make a robust system. In this paper, we review these three measures as to the sensors used and discuss the advantages and limitations of each. The various ways through which drowsiness has been experimentally manipulated is also discussed. We conclude that by designing a hybrid drowsiness detection system that combines non-intrusive physiological measures with other measures one would accurately determine the drowsiness level of a driver. A number of road accidents might then be avoided if an alert is sent to a driver that is deemed drowsy.

    Given the safety issues associated with flammability characteristics of alternative environmentally-friendly refrigerants, it is vital to establish measurement systems to accurately analyse the flammability of these mildly flammable refrigerants. In this study, we used a customised Hartmann bomb analogue to measure the minimum ignition energy (MIE) and laminar burning velocity (BV) for refrigerant/air mixtures of pure ammonia (R717), R32, R1234yf and mixtures of R32 and R1234yf with non-flammable refrigerants of R134a, R125 and carbon dioxide (R744). The MIEs of R717, R32, and R1234yf were measured at an ambient temperature of 24 °C to be (18.0 ± 1.4), (8.0 ± 1.5) and (510 ± 130) mJ at equivalence ratios of 0.9, 1.27 and 1.33, respectively. Adding the non-flammable refrigerants R134a, R125 and R744 along with R32 at volumetric concentrations of 5% each to R1234yf reduced the latter compound's flammability and increased its MIE by one order of magnitude. The laminar burning velocities of pure R717 and R32 were measured at an equivalence ratio of 1.1 using the flat flame method and found to be 8.4 and 7.4 cm/s, respectively. Adding 5% R1234yf to R32 decreased the laminar burning velocity by 11%, while a further 5% addition of R1234yf resulted in a decrease of over 30% in the laminar burning velocity.

    Maritime transportation is very important for coastal state country like Malaysia. However, as having the busiest straits in the world, Malaysia is continuously exposed to the risk of marine spill. The nation had experienced around 30 marine spill incidents since 1976 to the present. The main contributor of marine spill is ship’s accident and in term of category of accident, collision had contributed the most. In term of type of substance that mostly spilled by ships is highly persistence crude oil. The management of marine spill response of Malaysia is applying the three tiers system which is base on the area and scale of the spill. The basis of establishing the marine response service is in accordance with OPRC, which incorporated into the national environmental law. With the three tiers of response system, Malaysia is equipped with arsenal of marine spill response and control equipments. However, with the present magnitude of threat, the current capacities of equipments are insufficient. Nevertheless, the possibility of full magnitude of marine spill is unlikely due to the safety features incorporated into the design of the ships and FSO/FPSO. In term of claim and compensation for marine spill incident, Malaysia is applying the two layers of compensation under the CLC 1992 Protocol and FUND 1992 Protocol. Therefore, Malaysia is ready in various aspects of marine spill response and control.

    We present two cases of direct traumatic carotico-cavernous fistula following motor vehicle accident. Their main symptoms include loss of vision, progressive proptosis and recurrent epistaxis. Angiogram confirmed presence of carotico-cavernous fistula in both cases. One was successfully treated by open surgery and the other by embolization with Guglielmi detachable coils (GDC). In both cases, proptosis and epistaxis resolved completely.

    Individual factors have been considered to be a key issue to organizations’ performance and survival. Specifically, previous studies provide evidence that individual factors are closely associated with accidents in the workplace. Therefore, this study seeks to investigate the relationship between individual factors and workplace safety. 50 respondents have been randomly chosen from a manufacturing company in Johor Bahru, Malaysia. Data are collected using a survey and are analyzed using SPSS version 16.0. A descriptive statistic, cross tabulation, independent sample t-test and one-way between groups ANOVA are employed to interpret findings of the study. Findings show that employees’ perception on safety performance is at high level (Mean= 115, Standard Deviation= 17.73). Furthermore, the results of cross tabulation demonstrate that individual factors play a key role to determine the organization safety performance level. Another significant finding reveals that there is a significant difference of safety performance mean to two of individual factors namely gender and tenure of service. Individual factors namely gender, age, tenure of service and academic qualification give a significant impact to employees’ perception on safety performance.

    Work shift has been shown to correlate with accident rates. Understanding of such correlation is pertinent especially among emergency response personnel since the decisions that they make determines not only the outcome of their responses but also the risks of accidents to themselves. A questionnaire data derived study used together with a semi quantitative risk analysis method was adopted to estimate the levels of accident risks between firefighters working on two work shifts. Two hundred and forty eight Malaysia’s Fire and Rescue Department firemen from 24 fire stations working on shifts were selected as respondents. The accident rate among firefighters in year 2006 was 52.8 %. Results showed that the Accident Risk Index (ARI) among firefighters working the 24 hours shift was higher (ARI = 3.14) compared with those in the 12 hours shift (ARI = 2.98). However, there were no significant difference in overall severity of the accidents between the two shifts (p>0.05). The difference in risk levels was attributed to the difference in the likelihood of accident occurrence.
