      Disease outbreaks due to the consumption of contaminated food and feedstuff are a recurring problem worldwide. The major factor contributing to contamination are microorganisms, especially fungi, which produce low-molecular-weight compounds as secondary metabolites, with confirmed toxic properties referred to as mycotoxins. Several mycotoxins reported to date are cosmopolitan in distribution and incur severe health-associated risks (including cancer and neurological disorders). Hence, creating awareness among consumers, as well as developing new methods for detection and inactivation is of great importance for food safety. In this review, the focus is on the occurrence of various types of mycotoxins in food and feed associated with risks to humans and livestock, as well as legislation put forth by various authorities, and on presently practiced detoxification methods. Brief descriptions on recent developments in mycotoxin detection methodology are also inlcuded. This review is meant to be informative not only for health-conscious consumers but also for experts in the field to pave the way for future research to fill the existing gaps in our knowledge with regard to mycotoxins and food safety.

    OBJECTIVES: The objective of this study was to identify the factors influencing workarounds to the Hospital Information System (HIS) in Malaysian government hospitals.

    METHODS: Semi-structured interviews were conducted among 31 medical doctors in three Malaysian government hospitals on the implementation of the Total Hospital Information System (THIS) between March and May 2015. A thematic qualitative analysis was performed on the resultant data to deduce the relevant themes.

    RESULTS: Five themes emerged as the factors influencing workarounds to the HIS: (a) typing skills, (b) system usability, (c) computer resources, (d) workload, and (e) time.

    CONCLUSIONS: This study provided the key factors as to why doctors were involved in workarounds during the implementation of the HIS. It is important to understand these factors in order to help mitigate work practices that can pose a threat to patient safety.

    Introduction: Nursing documentation is the key to nursing care in hospitals. Nursing documentation contains ev- idences which demonstrate a significant association between the comprehensive level of nursing care and nurses’ professional practice. Therefore, nurses in Malaysia are trained to abide with the code of professional practice (1998) which required them to contrive a complete and comprehensive nursing documentation. Despite the importance of nursing documentation in the nursing professional practice, such study almost non-existent in Malaysia. Hence, there is a need to explore nurses understanding about existence of professionalism within their documentation, from a Malaysian context. Methods: The study utilized a qualitative approach which aimed to explore the perceptions among nurses in Malaysia on their documentation with relation to professionalism in nursing. Forty semi structured interviews were conducted in order to obtain an understanding of nurses’ views on their documentation and its influ- ence on their ways of preparing and completing their documentation. Thematic analysis was used to identify catego- ries and themes in nurses’ accounts of their documentation with relation to professionalism in nursing. Results: One of many profound findings from the study is the fear among nurses of “blaming culture” in that occurs their clinical setting. The nurses perceive “blaming culture” do not tolerate mistakes and they are more likely to be blamed for poor patient safety and insufficient quality of care. This is because nurses are directly involved in delivering care to patient or client in the hospital. “Blaming culture” however motivates nurses to ensure their documentation is com- plete, comprehensive and contemporaneous. Conclusion: Interestingly, the nurses perceive their documentation as an important evidence if there are possible future medical legal issues that they might have to involve with.

    RATIONALE, AIMS, AND OBJECTIVES: Clinical decision support (CDS) generates excessive alerts that disrupt the workflow of clinicians. Therefore, inefficient clinical processes that contribute to the misfit between CDS alert and workflow must be evaluated. This study evaluates the appropriateness of CDS alerts in supporting clinical workflow from a socio-technical perspective.

    METHOD: A qualitative case study evaluation was conducted at a 620-bed public teaching hospital in Malaysia using interview, observation, and document analysis to investigate the features and functions of alert appropriateness and workflow-related issues in cardiological and dermatological settings. The current state map for medication prescribing process was also modelled to identify problems pertinent to CDS alert appropriateness.

    RESULTS: The main findings showed that CDS was not well designed to fit into a clinician's workflow due to influencing factors such as technology (usability, alert content, and alert timing), human (training, perception, knowledge, and skills), organizational (rules and regulations, privacy, and security), and processes (documenting patient information, overriding default option, waste, and delay) impeding the use of CDS with its alert function. We illustrated how alert affect workflow in clinical processes using a Lean tool known as value stream mapping. This study also proposes how CDS alerts should be integrated into clinical workflows to optimize their potential to enhance patient safety.

    CONCLUSION: The design and implementation of CDS alerts should be aligned with and incorporate socio-technical factors. Process improvement methods such as Lean can be used to enhance the appropriateness of CDS alerts by identifying inefficient clinical processes that impede the fit of these alerts into clinical workflow.

    A CDSS generates a high number of inappropriate alerts that interrupt the clinical workflow. As a result, clinicians silence, disable, or ignore alerts, thereby undermining patient safety. Therefore, the effectiveness and appropriateness of CDSS alerts need to be evaluated. A systematic review was carried out to identify the factors that affect CDSS alert appropriateness in supporting clinical workflow. Seven electronic databases (PubMed, Scopus, ACM, Science Direct, IEEE, Ovid Medline, and Ebscohost) were searched for English language articles published between 1997 and 2018. Seventy six papers met the inclusion criteria, of which 26, 24, 15, and 11 papers are retrospective cohort, qualitative, quantitative, and mixed-method studies, respectively. The review highlights various factors influencing the appropriateness and efficiencies of CDSS alerts. These factors are categorized into technology, human, organization, and process aspects using a combination of approaches, including socio-technical framework, five rights of CDSS, and Lean. Most CDSS alerts were not properly designed based on human factor methods and principles, explaining high alert overrides in clinical practices. The identified factors and recommendations from the review may offer valuable insights into how CDSS alerts can be designed appropriately to support clinical workflow.

    The use of single-use items (SUDs) is now ubiquitous in medical practice. Because of the high costs of these items, the practice of reusing them after sterilisation is also widespread especially in resource-poor economies. However, the ethics of reusing disposable items remain unclear. There are several analogous conditions, which could shed light on the ethics of reuse of disposables. These include the use of restored kidney transplantation and the use of generic drugs etc. The ethical issues include the question of patient safety and the possibility of infection. It is also important to understand the role (or otherwise) of informed consent before reuse of disposables. The widespread practice of reuse may bring down high healthcare costs and also reduce the huge amount of hospital waste that is generated. The reuse of disposables can be justified on various grounds including the safety and the cost effectiveness of this practice.

    OBJECTIVE: This study aimed was to describe related factors of nurse practice in preventing fall risk in an inpatient ward at X General Local Hospital.

    METHOD: A descriptive, analytical quantitative with a cross-sectional approach was used in this study. The total of 95 nurses agreed to participate by using random sampling. Data collection using a structured questionnaire and observational form. The statistical model with a chi-square analysis was used in this study.

    RESULTS: The result showed a correlation between knowledge and nurse practice with OR 3.257 (1.375-7.715; p=0.012), attitude and nurse practice with OR 4.286 (1.775-10.345; p=0.002) training and nurse practicewith OR value 5.455 (2.233-13.322; p=0.000).

    CONCLUSIONS: Local authority in the hospital must apply patient safety standards to reduce injury rates, both nurses and patients. Nurses need to follow the current trend of nursing science focusing on patient safety.

    Crosswalks are critical locations in the urban transport network that need to be designed carefully as pedestrians are directly exposed to vehicular traffic. Although various methods are available to evaluate the level of service (LOS) at pedestrian crossings, pedestrian crossing facilities are frequently ignored in assessing crosswalk conditions. This study attempts to provide a comprehensive framework for evaluating crosswalks based on several essential indicators adopted from different guidelines. A new pedestrian crossing level of service (PCLOS) method is introduced in this research, with an aimto promote safe and sustainable operations at such locations. The new PCLOS employs an analytical point system to compare existing street crossing conditions to the guidelines' standards, taking into account the scores and coefficients of the indicators. The quantitative scores and coefficients of indicators are assigned based on field observations and respondent opinions. The method was tested to evaluate four pedestrian crosswalks in the city of Putrajaya, Malaysia. A total of 17 indicators were selected for the study after a comprehensive literature review. Survey results show that the provision of a zebra crossing was the most critical indicator at the pedestrian crossings, while drainage near crosswalks was regarded as the least important. Four indicators had a coefficient value above 4, indicating that these are very critical pedestrian crossing facilities and significantly impact the calculation of LOS for pedestrian crossings. Four crosswalks were evaluated using the proposed method in Putrajaya, Malaysia. The crosswalk at the Ministry of Domestic Trade Putrajaya got the "PCLOS A". In contrast, the midblock crossing in front of the Putrajaya Corporation was graded "PCLOS C". While the remaining two crosswalks were graded as "PCLOS B" crosswalks. Based on the assigned PCLOS grade, the proposed method could also assist in identifying current design and operation issues in existing pedestrian crossings and providing sound policy recommendations for improvements to ensure pedestrian safety.

    Although the Halal concept has not been a major element among non-Muslim consumers living in an Islamic country, whether the non-Muslim consumers are aware of the underlying advantages that come with Halal food products or their viewpoints arising from their religious belief, are some intriguing questions that need to be answered. Thus the objective of the study explore the underlying determinants that are likely to influence non-Muslim consumers’ perceptions and attitudes towards Halal concept and Halal food products in Malaysia in lieu of new paradigm in emerging global issues on sustainability, environmental, food safety and animal welfare. A survey was conducted in the Klang Valley where 400 non-Muslim respondents were interviewed via structured questionnaires to gather information on their awareness and attitude towards Halal food products in the Malaysian food market. Descriptive statistic was used to identify the socio-economic/ demographic characteristics and attitudes of the respondents toward the Halal food principles. The logit model
    was used to determine the extent to which selected socio-economic/demographic characteristics influenced
    the respondents’ attitude and understanding on Halal principles and Halal food products. The results of this
    study suggest that non-Muslim consumers are aware of the existence of Halal food, Halal principles and the
    advantages of Halal way in slaughtering the animals. This can be shown by their significant awareness that
    Halal is not only the way Muslim slaughter their animals but also relates to environmental, sustainability,
    animal welfare and food safety. In general, various socio-economic/demographic factors such as education
    level, older generation, those who are more religious and the urban dweller seem to more likely to be aware of the advantages of Halal principles.

    Background: The occupationally acquired accident and injuries in Malaysian medical laboratories are still largely unexplored prior to this survey. Some of these questions are attempted in this survey and act as source of reference for the number and accident injuries in medical laboratories in the area of Klang Valley and also in Malaysia.
    Methods : This survey was carried out based on recordable cases throughout the calendar year of 2001 to 2005 from 3 main medical laboratories of Hospital Kuala Lumpur (HKL), Hospital Universiti Kebangsaan Malaysia (HUKM) and Pusat Perubatan Universiti Malaya (PPUM).
    Results : The average annual incident rate for this three medical laboratories is 2.05/100 full time equivalent (FTE) employees. The annual incident rate in individual medical laboratory is 2.04/100 FTE (HKL), 2.07/100 FTE (HUKM) and 2.04/100 FTE (PPUM) employees, respectively. The most common injury that is 25.3% of the total cases reported was due to cuts by sharp objects and the second most common injury was exposure to biohazard and chemical substances which constitutes 19.9% of the total cases. . Needle prick injury (16.8%), fire (8.4%), fall/slip (6.3%) and gases leak and locked in cold room were reported as one case each.
    Conclusion : The average incident rate from this study is remarkably similar compared with the incident injury rate reported by BLS (2006) which is 2.1/100 FTE in the average size of medical laboratory and diagnostic. Besides this incident rate of injury and illness as a comparison, it also can be used as a benchmark to evaluate the safety performance among medical laboratories in Malaysia.

    Pyogenic granuloma is a benign lesion which is commonly found in the oral cavity. It is a reactive inflammatory process of the injured mucosa to trauma. It can appear as a sessile or pedunculated mass with smooth or lobulated surface, which sometimes can mimic malignant lesion. Excision biopsy of the lesion is the confirmatory investigation which also a treatment tool. We report a case of middle-aged lady who had pyogenic granuloma of the tongue, which was safely excised using ultrasonic scissors. This case highlights the new technique of using ultrasonic instruments for excision of benign tongue lesion, with marked reduction of blood loss and operation time.

    Since early 2000, Malaysian food-export industries have recognised the demand for food
    compliant with Islamic dietary law (halal), with primary consumer choices based on quality
    and safety. The lack of documented monitoring for health hazards and haram substances led
    to the withdrawal of certification. The HACCP-based halal quality-assurance standards were
    developed as a result, using the HACCP criteria for safety, religious dietary requirement and
    quality. Halal critical control points (HlCCP) are identified using HACCP criteria and a question
    tree on HALAL processing and storage. This approach harmonizes and unifies halal processing
    with the specific food industry via an HACCP–based halal quality-assurance system.

    The term ‘sustainable’ has become a buzz word in today’s business world. Consumption of green food is just one facet or ‘trip’ to the whole journey of sustainable development. This paper explores and synthesizes the findings of research on green food consumption in Malaysia. The already conducted studies in Malaysia have revealed various demographic and psychographic factors contributing to consumers’ intension to buy green food. This study revealed that majority of Malaysians consider food safety and health issue to be their prime reason for buying green food. To ensure the long term sustainability, the green food as well as the broad food industry in Malaysia must evidently understand the consumers’ buying motives of green food. Although mentionable limitation of the paper is the reliance on only published literature, this can be used as input for further large scale empirical research. The paper concludes with implications and suggestions for further research.

    Introduction: Behavioural-based safety (BBS) programme is an accident prevention programme emphasized on the aspect of unsafe behaviour, which considerably new in Malaysia. The oil and gas industry is among the industries in Malaysia that has successfully implemented and sustaining the programme. The aim of this study is to measure and evaluate the effectiveness of BBS towards the safety performance in the oil and gas industry in Malaysia that certainly has owned an outstanding safety performance.
    Methodology: A cross-sectional study using secondary data involving five indicators in the safety performance in the department of health, safety and environment (HSE) was performed at one of the oil and gas refinery companies in Malaysia. These indicators were chosen based on the frequency of its occurrence from January 2004 to December 2008.
    Result: The results of the study show the presence of a significant difference in the incidence rates of near misses (p=0.008) and medical treatment cases (p=0.035) before and after the implementation of BBS and a very strong negative relationship (r=-0.885) between the incidence rates of near-misses and the rates of BBS observations done. However, analysis on unsafe behaviour does not show a significant reduction from 2006 to 2008. Instead, a significant reduction was seen on equipments failure from year 2006 to 2007 (p=0.021) and 2006 to 2008 (p=0.032).
    Conclusion: The study has proven that the BBS is effective towards the reduction of the indicators in the safety performance, involving only the incidence rates of the nearmisses and the medical treatment cases. However, a very strong negative relationship only involves the incidence rate of near misses. A significant reduction was also seen on the occurrence of instrument failure, which is one of the root causes of near-misses. Although there is no significant reduction on thye unsafe behaviour, the reduction of equipment failure is expected to be related to the increase in the level of awareness and safe behaviour as a result of BBS implementation.

    Patient safety is a serious global healthcare issue. Harm can be caused by a range of errors or adverse events. Therefore, it is vital that the commissioning of a new operating theatre should comply to the highest standard before it is allowed to function. This paper accounts our experience in the commissioning of the University Malaya Medical Centre (UMMC) trauma centre operating theatre(OT) complex in July 2008. We highlighted the problems we faced in adhering to the international standard guidelines. Unanticipated events were handled professionally and solved. With this experience, we hope that the identified problems would provide suggestions for commissioning an operating theatre in the local setting in the future.

    The growing concern over the workers safety and health has lead many factories and organizations do the air monitoring to ensure the airborne at their workplace is safe for the worker’s health and complying the Occupational Safety and Health Act 1994 (Act 514). In this study, the monitoring covers an indoor air quality and chemical exposure to the workers in one of the power plant repair shop. A few workers from different sections namely blasting, welding, grinding, fitting and maintenance area were chosen to assist in the personal monitoring for 8 hours measurement. PM10 were measured at a few sampling points to collect dusts for 24 hours duration. The samples were brought back to the laboratory for gravimetric and SEM-EDAX analysis. The results were certainly exceed the limit for air quality, and many elements were detected such as Fe, Ni, Al, Si, Ca, K, Ba, S, Cr, Zn and Cl. The present of these elements shows that exposure to these particulate matters is quite risky and some measure needs to be taken for remedial action.

    Various types of Occupational Safety and Health Management Systems (OSH-MS) exist in the market. Basically, these systems have similar basic principles and even elements. However, the importance of these elements in terms of successful management of OSH differs according to the end-user. OSH Practitioners with different roles and responsibilities have different views on which element or elements contribute significantly to the overall success of OSH management. There are no standardization in terms quantifying the elements that qualifies an OSH-MS. A study was carried out to quantify the implementation of an OSH-MS through the determination of weighing factors for the different elements in an OSH-MS. Respondents for this study comprised of top safety and health management, safety and health officers (SHO) / radiation protection officers (RPO), DOSH officers, auditors and consultants who are very familiar with OSH-MS. Sample of this study was based on purposive sampling due to strict criteria and prerequisites to be met. Questionnaires were distributed to the identified organizations and personnel. Results from this study established that, auditor has the highest level of understanding of OSH-MS compared with top safety and health management, SHO / RPO, DOSH officers and OSH consultants. Among all the elements in the OSH-MS studied for their levels of importance in terms of the overall success of implementing an OSH-MS, OSH policy is the most critical element, followed by hazard identification, risk assessment and control, employee participation, responsibility and accountability and competence and training. The weighing factor for these top 5 elements are OSH policy (0.36); hazard identification, risk assessment and risk control (0.25); employee participation (0.15); responsibility and accountability (0.14); and competence and training (0.10). Application of the weighing factors of these elements allows the quantification of audit status (Audit Score) based on the equation: audit score = 0.36 [OSH Policy] + 0.25 [Hazard Identification, Risk Assessment and Risk Control] + 0.15 [Employee Participation] + 0.14 [Responsibility and Accountability] + 0.10 [Competence and Training]. Minimum tolerable target for each element of an OSH-MS was also determined. The minimum tolerable frequency of OSH policy is reviewed by top management is yearly; the minimum tolerable frequency of risk management is reviewed is yearly; the minimum tolerable percentage of employees who should be made known on relevant legal and other requirements is ≥90%; the minimum tolerable percentage of OSH objectives and programme(s) achieved / implemented is between 80 – 89%; the minimum tolerable percentage of money budgeted for OSH in a year compared to organization revenue is between 1 – 5%; the minimum tolerable number of hours for each employee needed to be trained on OSH per year is between 30 – 39 hours; the minimum tolerable percentage of employees who should be communicated on matters related to OSH is ≥90%; the minimum tolerable percentage of employees participation and involvement in hazard identification, risk assessment and determining controls is ≥90%; the minimum tolerable percentage of employees who should be involved in establishing OSH documents is

    Finite Element Analysis (FEA) and Computational Fluid Dynamics (CFD) analysis were performed in this work in order to obtain the best design for safety and aerodynamic performance of the bicycle cycling helmet. FEA analysis was computed on two different helmet designs to determine the critical area subjected to impact. A pressure load was applied on the helmets’ outer surface to simulate oblique loading. The critical areas of the helmets were then highlighted and identified, enabling design improvements to be made on both designs. CFD analysis was then executed in order to obtain the lowest drag coefficient number in reducing the air resistance induced by both of the helmet designs, inherently increasing cyclist performance and ensuring competition success.

    The Gamma Green House (GGH) is a chronic irradiation facility located at MINT Tech Park, Nuclear Malaysia, Jalan Dengkil. GGH is used for induction of mutation in plants and other biological samples with low dose radiation over period of time depending on the nature and sensitivity of the plant species. Gamma Greenhouse facility at Malaysian Nuclear Agency comprises an open topped
    irradiation area consisting of circular green house with 30 meters radius, control room and irradiator with interlock system. The irradiation source is a REVISS RSL6050 double encapsulated 800 Ci 137Cs (half-life 30.1 years for 137Cs) pencils and allowed to be exposed only when the entire 300 m diameter site is free from personnel. The irradiator system is secured by a sophisticated interlock system, which only allows the source to be exposed when all the prerequisite safety conditions are met, and automatically returns the source to the safe
    storage position if any safety device is compromised.

    The management of a chemical inventory is necessary for safety purposes as well as for fulfilling regulatory compliance. In most academic laboratories, the management of chemicals is still being done manually, which is time-consuming. As a result, data are not updated and expired chemicals are unintentionally used. This research proposes that UHF Class 1 Gen 2 Radio Frequency Identification (RFID) technology be used in the development of a chemical inventory information system to ease chemical tracking as well as to shorten the inventory process time. An information system integrating RFID data and a web-based rule identification interface was developed. WAMP 2.2.17, PHP 5.3.5 and MySQL 5.5.8 were downloaded and a programming language was written to check the expiration date of the chemicals as well as to produce alert notification status. Wireless technology through GSM modem helped in producing alert messages using the Short Message System (SMS) of the nearly expired chemicals to the handphone of the person in charge in real time.
