    CONTEXT: No universal waist circumference (WC) percentile cutoffs used have been proposed for screening central obesity in children and adolescents.

    OBJECTIVE: To develop international WC percentile cutoffs for children and adolescents with normal weight based on data from 8 countries in different global regions and to examine the relation with cardiovascular risk.

    DESIGN AND SETTING: We used pooled data on WC in 113,453 children and adolescents (males 50.2%) aged 4 to 20 years from 8 countries in different regions (Bulgaria, China, Iran, Korea, Malaysia, Poland, Seychelles, and Switzerland). We calculated WC percentile cutoffs in samples including or excluding children with obesity, overweight, or underweight. WC percentiles were generated using the general additive model for location, scale, and shape (GAMLSS). We also estimated the predictive power of the WC 90th percentile cutoffs to predict cardiovascular risk using receiver operator characteristics curve analysis based on data from 3 countries that had available data (China, Iran, and Korea). We also examined which WC percentiles linked with WC cutoffs for central obesity in adults (at age of 18 years).

    MAIN OUTCOME MEASURE: WC measured based on recommendation by the World Health Organization.

    RESULTS: We validated the performance of the age- and sex-specific 90th percentile WC cutoffs calculated in children and adolescents (6-18 years of age) with normal weight (excluding youth with obesity, overweight, or underweight) by linking the percentile with cardiovascular risk (area under the curve [AUC]: 0.69 for boys; 0.63 for girls). In addition, WC percentile among normal weight children linked relatively well with established WC cutoffs for central obesity in adults (eg, AUC in US adolescents: 0.71 for boys; 0.68 for girls).

    CONCLUSION: The international WC cutoffs developed in this study could be useful to screen central obesity in children and adolescents aged 6 to 18 years and allow direct comparison of WC distributions between populations and over time.

    This paper reports on the presence of dual burden households in Orang Asli (OA, indigenous people) communities and its associated factors. A total of 182 OA households in two districts in Selangor with the required criteria (182 non-pregnant women of child bearing age and 284 children aged 2-9 years old) participated in the study. Height and weight of both women and children were measured. Energy intake and food variety score (FVS) were determined using three 24-hour diet recalls. While 58% were underweight and 64% of the children were stunted, the prevalence of overweight and obesity in women were 31% and 20% respectively. The percentage of dual burden households (overweight mother/underweight child) was 25.8% while 14.8% households had normal weight mother/normal weight child. The mean food variety score (FVS) was similar for women (7.0+/-2.1) and children (6.9+/-1.9). Dual burden households were associated with women's employment status (OR: 3.18, 95% CI: 2.65-5.66), FVS of children (OR: 0.71, 95% CI: 0.51-0.95) and FVS of women (OR: 1.39, 95% CI: 1.02- 1.89). The FVS of children (OR: 0.49, 95% CI: 0.25-0.89) and women (OR: 1.92, 95% CI: 1.64-2.77) remained significant even when dual burden households were compared to only households with normal weight mother/normal weight child. In these OA communities, food variety may predict a healthier diet in children, but may increase the risk of overweight and obesity in adults. Efforts to address households with dual burden malnutrition should consider promotion of healthy diets and lifestyle for all members.

    OBJECTIVE: This study was performed to determine the efficacy and tolerability/safety of IQP-AE-103 on body weight reduction in overweight to moderately obese adults.

    METHODS: A double-blind, randomized, placebo-controlled trial involved one hundred and eight subjects (BMI between 25 and 35 kg/m2) that were randomly assigned to either the low-dose or the high-dose IQP-AE-103 group, or the placebo group. Following a 2-week run-in period, subjects received two capsules of investigational product after three daily main meals for 12 weeks. Subjects were instructed to maintain a nutritionally balanced hypocaloric diet according to the individual's energy requirement. Body weight, body fat, and waist and hip circumference were measured at baseline, and after 2, 4, 8, and 12 weeks. Subjects also rated their feelings of hunger and fullness using visual analogue scales, and food craving on a 5-point scale at the same time intervals. Blood samplings for safety laboratory parameters were taken before and at the end of the study.

    RESULTS: After 12 weeks of intake, the high-dose IQP-AE-103 group had a significantly greater weight loss compared with the placebo (5.03 ± 2.50 kg vs. 0.98 ± 2.06 kg, respectively; p < 0.001) and the low-dose group (3.01 ± 2.19 kg; p=0.001). The high-dose group experienced a decrease in body fat of 3.15 ± 2.41 kg compared with a decrease of 0.23 ± 2.74 kg for the placebo group (p < 0.001). High-dose IQP-AE-103 also decreased the feeling of hunger in 66% subjects. A beneficial effect of IQP-AE-103 on the lipid metabolism was also demonstrated in the subgroup of subjects with baseline total cholesterol levels above 6.2 mmol/L. No side effects related to the intake of IQP-AE-103 were reported.

    CONCLUSIONS: These findings indicate that IQP-AE-103 could be an effective and safe weight loss intervention. This trial is registered with NCT03058367.

    Cardiovascular disease (CVD) risk is a continuum across blood pressure. The term prehypertension was introduced because it is now recognized that blood pressure readings between what is deemed optimal and hypertension is associated with increased CVD risk. The prevalence of prehypertension is high and the progression to hypertension is also high. Prehypertension is also commonly associated with other CVD risk factors namely dyslipidaemia, dysgylcaemia and overweight/ obesity. Eighty-five percent of prehypertensives have one other or more CVD risk factor compared to normotensives. A recent study has shown a reduction in the development of hypertension from prehypertension with the use of an angiotensin receptor blocker. Unfortunately to date, the impact of treatment of prehypertension on CVD outcome is still unknown except in those with high CVD risk like diabetes or established CVD. However this does not mean nothing can be done for those with prehypertension. The aim of managing prehypertension is to lower the BP, prevent progression to hypertension and to prevent BP related CVD deaths. Lifestyle changes can reduce BP and this by itself can lower CVD risk. Until more evidence about other modalities of treatment become available this is a sensible and cost-effective way to manage prehypertension.

    Aim: This study evaluated the relationship between missing posterior teeth and body mass index with regard to age and socioeconomic state in a sample of the suburban south Indian population.

    Materials and Methods: The 500 individuals of both males and females aged 40 years and older with missing posterior teeth and not rehabilitated with any prosthesis were gone through a clinical history, intraoral examination, and anthropometric measurement to get information regarding age, sex, socioeconomic status, missing posterior teeth, and body mass index (BMI). Subjects were divided into five groups according to BMI (underweight > 18.5 kg/m2, normal weight 18.5-23 kg/m2, overweight 23-25 kg/m2, obese without surgery 25-32.5 kg/m2, obese with surgery < 32.5 kg/m2). Multivariate logistic regression was used to adjust data according to age, sex, number of missing posterior teeth, and socioeconomic status.

    Results: People with a higher number of tooth loss were more obese. Females with high tooth loss were found to be more obese than male. Low socioeconomic group obese female had significantly higher tooth loss than any other group. No significant relation between age and obesity was found with regard to tooth loss.

    Conclusion: The BMI and tooth loss are interrelated. Management of obesity and tooth loss can help to maintain the overall health status.

    Local profiles on the use of weight loss products are scarce. The study aims to address this together with concerns on the users’ misperception of the safety of these products, and the absence of high-quality evidence to support such use. Methods: This was a cross-sectional study conducted in overweight and obese patients attending a public primary care clinic in Penang. Selected patients were given a set of self-administered questionnaire that assessed types of weight loss products used, factors that influenced the usage and the users’ perception of their own body weight and the diet products they are taking. Results: From 332 participants of this study, 18.7% were users. Mean age of users were 44.6 (SD 11.9) years. The majority (66.1%) only used dietary supplements, 11.3% used weight loss medications and the rest (22.6%) used both products. Reasons for its use were for health, a faster result to lose weight and failing dieting and exercise regimes. The average amount spent on this was RM100 per month. Commonest source of weight loss products were friends. Majority (80.6%) did not discuss the use of the products with doctors. Factors associated with the use of weight loss products were being female (AOR=5.59), had tertiary level education (AOR=2.27), being employed (AOR=3.42), self-perceived of overweight (AOR=3.61) and perception that weight loss products as safe (AOR 2.48). Conclusion: Users of weight loss products are among highly educated working females who perceived themselves as being overweight and assume the products are safe.

    Overweight and obesity have become a serious health issue in worldwide and Malaysia is no exception. In view that obesity is one of the leading causes of non-communicable diseases, this systematic review aimed to explore whether Malaysian are getting fatter (overweight or obesity) and rounder (abdominal obesity). Searches of the published literature were conducted using 3 databases including EMBASE, MEDLINE, and PubMed. These were limited to cross-sectional or cohort studies in Malaysia. The databases provided the prevalence of overweight, obesity, and abdominal obesity among 15 years old and above Malaysian population from 2009 to 2015. In total, 47 articles were included in this review. Findings exhibit that the prevalence rates of overweight, obesity, and abdominal obesity range from 9.5% to 43.5%, 1.77% to 26.4% and 11.4% to 57.4%, respectively. Overweight is more predominant among males. However, obesity and abdominal obesity are more predominant among females. This means that more females are getting fatter and “rounder” as compared to males in Malaysia. The findings imply a lack of intervention programmes based on proper randomized controlled trials which are needed to adequately address the problem of obesity in Malaysia.

    Objectives: Awareness of body image perception among children and adolescents has been rising during recent years. The objectives of this study were to identify body image dissatisfaction and its associated factors among primary-school students.

    Methods: A cross-sectional study was conducted in seven randomly selected primary schools, involving students aged 11 to 12.     In Malaysia, breakfast is the most frequently skipped meal. Skipping breakfast has been
    associated with an increased risk of childhood obesity. However, this relationship has not been
    investigated widely among preschoolers in Malaysia. Hence, this cross-sectional study aimed to
    determine the association between breakfast intake and Body Mass Index (BMI) among
    preschoolers in Taska Permata Keluarga (TPK), Kuala Nerus. A total of 131 Malays preschoolers
    aged four to six years old were recruited from nine TPK using convenience sampling method.
    Information on sociodemographic, breakfast intake pattern and anthropometric measurements
    (weight and height) were obtained. Respondents consisted of 74 (56.5%) boys and 57 (43.5%)
    girls. Anthropometric results showed that 8.4% preschoolers were overweight and obese. Among
    the preschoolers, 22.1% of them were breakfast skippers. In terms of gender breakdown, 20.3%
    boys and 24.6% girls skipped breakfast. There was a significant association between breakfast
    intake and BMI status among preschoolers (p = 0.003). This indicated that preschoolers that
    skipped breakfast were associated with overweight or obese compared to those who consumed
    breakfast daily. Breakfast consumption is a marker of a healthy lifestyle which can reduce the risk
    of childhood obesity. This habit should be inculcated during this critical period of life in which
    parents play a vital role in promoting breakfast consumption among preschoolers.

    Loss of teeth can affect masticatory efficiency in older adults. This may result in avoidance or modifications in food
    choices and lead to lower intake of important nutrients among older individuals. The aim of this study was to determine
    the association between tooth loss and body mass index, BMI, among older adults in Kuala Pilah, Negeri Sembilan. A
    cross-sectional study was carried out on 428 older adults aged 50 years and above from selected villages in district of
    Kuala Pilah. Respondents were interviewed to collect information on their demographic characteristics. Number of tooth
    loss was determined through oral assessment, followed by anthropometric assessment to calculate the BMI of respondents.
    Findings showed that majority of the respondents were overweight and obese, 40.4% and 19.9% respectively, while only
    a small proportion was underweight, 3.9%. The proportion of edentulism (total tooth loss) was 18.3% and majority of
    the older adults had lost more than 12 teeth (77.1%) and less than 4 pairs of occluding posterior teeth (86.0%). Total
    and partial tooth loss was found to be not significantly associated with BMI. In relation to the arrangement of teeth,
    older adults with reduced number of posterior occluding pairs of teeth were more likely to experience unsatisfactory
    BMI (OR = 3.61, 95% CI: 1.48, 8.76). This may suggest that functional arrangement of the tooth in the oral cavity is more
    important than the number of tooth loss alone. Thus, maintaining an optimum number of teeth for chewing is essential
    for maintenance of ideal BMI.

    Objectives: Overweight and obesity are known to cause various patterns of alteration to the pulmonary function test (PFT) parameters. We sought to investigate gender differences in PFT parameters and examine the relationship between body mass index (BMI) and PFT parameters.
    Methods: We conducted a retrospective study of 126 patients referred for a PFT by various medical specialties between January and December 2015. PFT was measured using spirometry, and BMI was calculated using Quetelet's index.
    Results: Female patients exhibited lower mean values for all PFT parameters compared to male patients. The forced vital capacity (FVC)% predicted was less than 80% for all patients while the ratio of forced expiratory volume in 1 second (FEV1)/FVC was higher with increased BMI. BMI was positively correlated with peak expiratory flow in all patients, and with FEV1/FVC ratio in males but not in females.
    Conclusions: In our studied population, males exhibited higher mean values of PFT parameters than females. Increased BMI may be associated with a restrictive pattern on spirometry.

    BACKGROUND: Offspring of type 2 diabetes patients have an absolute risk of 20-40% of developing the condition. Type 2 diabetes patients should be encouraged to speak to their offspring regarding diabetes risk and prevention strategies. The Health Belief Model conceptualises that the higher the perceived risk, the more likely an individual will modify their behaviour. The objectives of this study were to i) determine the distribution of type 2 diabetes patients regarding their willingness to accept training to speak to their offspring, ii) determine the distribution of type 2 diabetes patients regarding their willingness to accept training based on the HBM and iii) to determine the factors associated with their willingness to accept training.

    METHODS: This was a cross-sectional study amongst type 2 diabetes patients attending two primary care clinics in Malaysia. Sociodemographic data and knowledge of diabetes risk factors were collected. The adapted, translated and validated Diabetes Mellitus in the Offspring Questionnaire-Malay version (DMOQ-Malay) was self-administered. Statistical analysis included descriptive statistics, univariate and multiple logistic regression (MLogR).

    RESULTS: A total of 425 participants were recruited. Of these, 61.6% were willing to accept training. In MLogR, six variables were found to be significantly associated with willingness to accept training. These were i) positive family history [Adj. OR 2.06 (95% CI: 1.27, 3.35)], ii) having the correct knowledge that being overweight is a risk factor [Adj. OR 1.49 (95%CI: 1.01, 2.29)], iii) correctly identifying age ≥ 40 years old as a risk factor [Adj. OR 1.88 (95%CI: 1.22, 2.90)], iv) agreeing that speaking to their offspring would help them to prevent type 2 diabetes [Adj. OR 4.34 (95%: 1.07, 17.73)], v) being neutral with the statement 'I do not have much contact with my offspring' [Adj. OR: 0.31 (95% CI: 0.12, 0.810] and vi) being neutral with the statement 'my offspring are not open to advice from me' [Adj. OR: 0.63 (95% CI: 0.31, 0.84].

    CONCLUSION: The majority of type 2 diabetes patients were willing to accept training to speak to their offspring to prevent diabetes. A training module should be designed to enhance their knowledge, attitude and skills to become family health educators.

    This study aims to examine the level of knowledge, attitude and practices (KAP) of adolescents towards sugar-sweetened beverages (SSB), together with the associated factors that determine their KAP. Data were collected using self-administered questionnaires that consisted of sociodemographic, the KAP for the SSB questionnaire, and the Beverage Intake Questionnaire (BEVQ). The respondents' heights, weights, waist circumferences and body fat percentages were measured. This study involved 439 adolescents aged between 13 and 17 years old, in public secondary schools in Selangor, Malaysia. The results reveal that 35% of the adolescents were overweight, 26% had a high waist circumference, and 45% had a high body fat percentage. Caffeinated drinks and full cream milk were the most frequently consumed SSBs. The KAP score revealed a good attitude (88.4%), a moderate knowledge (51.8%) and a poor practice (40.5%). Those with a higher body fat percentage showed significantly good attitude scores (p < 0.05). Low household income groups, females, adolescents aged 16-17 years old and being from an urban area demonstrated a significant (p < 0.05) positive determinant towards the KAP score. In conclusion, high awareness of negative health outcomes associated with SSBs among adolescents was not in accordance with the level of their lifestyle choices.

    Childhood obesity is a serious public health problem worldwide. Perturbations in the gut microbiota composition have been associated with the development of obesity in both children and adults. Probiotics, on the other hand, are proven to restore the composition of the gut microbiome which helps reduce the development of obesity. However, data on the effect of probiotics on gut microbiota and its association with childhood obesity is limited. This study aims to determine the effect of probiotics supplement intervention on gut microbiota profiles in obese and normal-weight children. A total of 37 children, 17 normal weight, and 20 overweight school children from a government school in Selangor were selected to participate in this study. Participants were further divided into intervention and control groups. The intervention groups received daily probiotic drinks while the control groups continued eating their typical diet. Fecal samples were collected from the participants for DNA extraction. The hypervariable V3 and V4 regions of 16S rRNA gene were amplified and sequenced using the Illumina MiSeq platform. No significant differences in alpha diversity were observed between normal weight and obese children in terms of the Shannon Index for evenness or species richness. However, a higher intervention effect on alpha diversity was observed among normal-weight participants compared to obese. The participants' microbiome was found to fluctuate throughout the study. Analysis of the taxa at species level showed an increase in Bacteroides ovatus among the normal weight cohort. Genus-level comparison revealed a rise in genus Lachnospira and Ruminococcus in the overweight participants after intervention, compared to the normal-weight participants. The probiotics intervention causes an alteration in gut microbiota composition in both normal and overweight children. Though the association could not be defined statistically, this study has provided an improved understanding of the intervention effect of probiotics on gut microbiome dysbiosis in an underrepresented population.

    Introduction: Anaemia is a significant public health problem among adolescents globally but there is limited data in many countries, including Malaysia. This study aims to investigate the 5-years incidence of anemia among Malaysian adolescents by gender, ethnicity, locality of schools, Body Mass Index, stature and diet intake. Methods: A secondary analysis of existing data from MyHeART study was conducted within a closed cohort of 528 adolescents (aged 13 years) attended 15 public secondary schools from Kuala Lumpur, Selangor and Perak. The adolescents who were followed up at 15 and 17 years old had completed haemoglobin assessment, anthropometric measurements and -days diet history. The data was cleaned and missingness was handled accordingly with multiple imputation. SPSS Software version 21 was used to analyse the data, with Generalised Estimating Equation (GEE) showing the effect of time on the trajectory of prevalence of anaemia over the 5 years. Results: The prevalence or incidence? of anaemia in 2012, 2014 and 2016 was 7.9% (95%CI: 5.0-12.3), 13.9% (95%CI: 10.0-19.0) and 15.8% (95%CI:11.3-21.7). In females, anaemia increased from 11.1% (95%CI:6.7-18.7) to 15.7% (11.4-21.3) and 23.1%(95%CI: 16.8-31.0); the change was significant from 13 to 15 years old. Similar trend was noticed in those who are stunted, overweight/obese, in both urban/rural schools and didn’t meet their daily recommended nutrient intake for total calorie, protein and iron. Conclusion: Anaemia is increasing in trend among the adolescents over the years and deems attention from the relevant stakeholders to create a robust anemia prevention program.

    Introduction: The aim of this study was to determine the effects of pedometer-based intervention and the relationship between daily walking steps and nutritional status among overweight/obese university students in Kuala Terengganu. Materials and methods: The study was an 8-week intervention study that included 23 overweight/obese university students. After 7 days of baseline activity, anthropometric data such as weight, height and waist circumference and a one-day dietary record were taken. The participants were divided into a control and case group. The control group (n=15) was instructed to continue with their normal activities and habits throughout the 8 weeks. The case group (n=8) was instructed to walk based on the targeted number of steps. Data such as body weight, waist circumference and a one-day dietary record were taken after the intervention. Results: Both case and control group show significant differences in body mass index (BMI) with p=0.017 and p=0.047 respectively. However, there is a large and significant difference between walking steps and BMI before and after intervention for the case group. There were no differences in any other study parameters during the 8-week study. Conclusion: In conclusion, short-term intervention with a pedometer increased physical activity and positively affected the BMI of overweight/obese participants. The use of a pedometer was efficient in measuring daily physical activity (r= 0.756, p< 0.05).

    University students tend to be more concerned on their body image and body shape concerns because of the surrounding The present study was conducted among undergraduate students at
    the International Islamic University Malaysia (IIUM) Kuantan with the aim to identify the body image
    perceptions, body shape concerns and weight loss behaviors. Methods: A total of 135 participants
    comprising of 50 male and 85 female undergraduate students were recruited from six Kulliyyah in IIUM Kuantan. The Contour Drawing Rating Scale, Body Shape Questionnaire (BSQ-16) and Weight Loss Behavior Scales (WLBS) were used as assessment tools. Results: The result showed 73.3% of students had normal BMI status, 6.7% were underweight and the remaining were overweight or obese. More than 50% of the participants were somewhat concern about their body shape. In addition, more than half of the participants (54.8%) were dissatisfied with their body image and had desired to be thinner. The most preferred weight loss behavior among the participants was exercise compared to other weight loss approaches. The result shows that concerns to dieting and weight have a significant association with BMI status. Meanwhile, body shape concerns are found to be significantly associated with concern to dieting and weight, as well as avoidance of fatty and sweet foods. However, there was no association between body shape concerns and exercise. Conclusion: Based on the findings, more tailored intervention programs that lead to healthy body image and weight loss behavior are needed among young adults.

    Introduction: Obesity is a growing public health concern. Poor diet and lifestyle choices are the main contributors to its development. Lifestyle modifications should be aggressively promoted in the community. Recent studies found that worksite obesity prevention and control programs are effective in increasing physical activity and weight reduction among employees. In Malaysia, there is insufficient data on prevalence studies of workplaces. This study investigated the proportion of overweight and obesity among workers in a private university and hope to serve as a baseline for future healthy workplace programs. Methods: This cross-sectional study investigated overweight and obesity and examined its association with occupational stress, sleeping quality, dietary behaviour and physical inactivity among employees in an academic institution. Weight and height were measured, and BMI was calculated and coded as underweight, normal, overweight and obese according to the cut-off points for the Asian population. Results: Out of ninety-five employees, 55 participated in the survey. Among the 55 employees, 4 (7.3%) were underweight, 10 (18.2%) were normal, 25 (45.5%) were overweight, and 16 (29.1%) were obese. The obese and overweight proportion was similar to the national prevalence. Poor sleep quality and high Job stress scores were reported by 40% of the employees, while physical inactivity of more than 6 hours a day was reported by 50%, and the majority (90%) do not consume enough fibre. Among the risk factors investigated, none was associated with obesity. Conclusion: The proportion of overweight and obesity in this community was like the general population. While not significant in this study, the association between obesity and the four risk factors should be further investigated using a larger sample size. Based on the results, community-based health promotion intervention is suggested to reduce obesity among the employees.

    Introduction: This paper discussed the effectiveness of “Healthy Kids Programme (HKP)”, specifically in improving the body weight status as well as knowledge, attitude and practice (KAP) of the children. Methods: In this longi- tudinal intervention study, a total of 12 primary schools in Klang Valley were randomly selected and divided into intervention and control groups. There were 386 standard three children participated at baseline. All children in the intervention group participated in the HKP that was conducted by nutritionists. The evaluations included the pre- and post-Immediate Impact survey (IIS) (every topic), and changes in nutrition KAP survey and anthropometric assess- ment of the children at the beginning and end of every year for three years. Results: School children in intervention group reported with an increased in the IIS score for each topic and achieved a significantly higher mean nutrition knowledge score than those in the control group over time. There were no significant differences in mean attitude and practice scores between two groups after the intervention. Children in the intervention group had lower rates of overweight and obesity (pre-test: 27.3%; post-test: 32.5%) than their counterpart (pre-test: 26.6%; post-test: 44.0%). Conclusions: The HKP is effective in improving nutrition knowledge of the children. A lower rate of overweight and obesity was observed in intervention group than control group. This approach will contribute towards inculcating healthy eating and active lifestyle habits among Malaysian children that bring about long-term health benefits.
