    Giant lipomatous lesions of the thigh swelling with extension into retroperitoneum are rare. Lesions can be malignant or benign and can have similar clinical presentation. Treatments options differ and their prognosis varies with histology. We present two cases of liposarcoma and lipoma with the same clinical presentations that underwent surgical resection.

    Combined tendocutaneous defect of Achilles tendon remains a complex reconstructive challenge whereby both the soft tissue coverage and tendon reconstruction have to be considered to achieve a good functional and aesthetic outcome. A 14-year-old boy who sustained an open right calcaneum fracture (Gustilo-Anderson IIIB) with a transected Achilles tendon and huge skin defect from motorcycle wheel spoke injury was admitted. The Achilles tendon repair site broke down following initial surgical debridement and primary repair, resulting in a sizeable combined tendocutaneous defect. Simultaneous soft tissue coverage and tendon defect reconstruction using composite sensate free anterior lateral thigh (ALT) fasciocutaneous flap with vascularized fascia lata was performed subsequently. The vascularized fascia lata was tubularized to wrap the native proximal stump of Achilles tendon and secured using the modified Krakow suturing technique. The distal end of tubularized fascia lata was, then, secured by drilling across right calcaneum bone, passing the suture transosseously and screwed. He led an uneventful postoperative recovery with satisfactory functional and aesthetic outcomes at one year of follow-up. In conclusion, the present case demonstrates the reliability of this technique and its advantages over other flap choices in reconstruction of a huge combined tendocutaneous defect.

    The aim of this study was to determine the acute metabolic effects of different magnitudes of wearable resistance (WR) attached to the thigh during submaximal running. Twenty endurance-trained runners (40.8 ± 8.2 years, 1.77 ± 0.7 m, 75.4 ± 9.2 kg) completed six submaximal eight-minute running trials unloaded and with WRs of 1%, 2%, 3%, 4% and 5% body mass (BM), in a random order. The use of a WR resulted in a 1.6 ± 0.6% increase in oxygen consumption (VO2) for every 1% BM of additional load. Inferential based analysis found that the loading of ≥3% BM was needed to elicit any substantial responses in VO2, with an increase that was likely to be moderate in scale (effect size (ES) ± 90% confidential interval (CI): 0.24 ± 0.07). Using heart rate data, a training load score was extrapolated to quantify the amount of internal stress. For every 1% BM of WR, there is an extra 0.17 ± 0.06 estimated increase in training load. A WR ≥3% of BM was needed to elicit substantial responses in lactate production, with an increase which was very likely to be large in scale (ES ± 90% CI: 0.41 ± 0.18). A thigh-positioned WR provides a running-specific overload with loads ≥3% BM, resulting in substantial changes in metabolic responses.

    Fraktur avulsi tulang belakang iliac anterior (ASIS) jarang terjadi, dan lebih lumrah berlaku di kalangan remaja muda semasa bersukan. Ini disebabkan oleh rawan apophyseal yang lemah semasa remaja, apabila kontraksi otot yang kuat mengatasi ASIS semasa aktiviti olahraga. Seorang lelaki berusia 15 tahun dibawa ke Jabatan Kecemasan (ED) mengadu sakit pinggul kanan selepas perlumbaan semasa Hari Sukan sekolahnya. Ketika dia melintasi garisan penamat dan menang, dia tiba-tiba rasa sensasi “pop” di pinggul kanannya dan jatuh ke tanah dengan kesakitan. Di ED, dia baring meniarap dengan skor sakit 6/10. Intravena Ketorolac diberikan untuk melegakan kesakitannya. Pemeriksaan fizikal menunjukkan kesakitan setempat di kawasan hadapan pelvis kanan, tanpa bengkak atau kecacatan pada sendi pinggul kanan. Tiada perbezaan antara kepanjangan kaki kanan berbanding kaki kiri. Pergerakan pinggul kanan terhad akibat kesakitan. Pemeriksaan neurovaskular adalah normal. Radiografi pelvis menunjukkan fraktur avulsi ASIS kanan. Beliau telah dirujuk kepada pasukan Ortopedik dan telah dirancang untuk rawatan konservatif rehat di katil, analgesik, dan ambulansi tanpa berat. Dia dibenarkan pulang dan dirancang untuk temujanji di Klinik Ortopedik. Pemeriksaan semula selepas 2 bulan kecederaan menunjukkan pesakit sihat dan bebas kesakitan. ASIS mudah terdedah kepada kecederaan avulsi semasa menjalankan aktiviti sukan kerana otot sartorius menarik anteroinferior ASIS semasa lekukan maksimum pinggul dan lanjutan lutut. Walaupun fraktur avulsi apophyses pelvis jarang berlaku, ia harus dipertimbangkan di kalangan atlet remaja jika mekanisma kecederaan adalah sangat sugestif.

    Studies indicate that bacterial cross-contamination occurs during food preparation where bacteria can retent on the food contact surfaces and cause illness. The study evaluated the adherence of Campylobacter spp. to cutting boards, blades of knives and hands after cutting chilled, raw broiler parts (thighs + drumsticks, wings and livers). The adherence to cucumber cuts that were cut using the unwashed boards and knives was also analyzed. Generally, utensils have higher mean of Campylobacter spp. retained to them (1.4-223.3 MPN/ml rinse) than hands (0.7-43.4 MPN/ml rinse); however, Mann-Whitney U test showed no significant differences in the bacterial numbers found among the different surfaces. The transfer rates of Campylobacter spp. from utensils to cucumber cuts varied from 0% to more than 100%. The bacteria detected could be from the utensils and cucumber contamination before purchase or due to other factors where further investigation is required. The possibility is there for Campylobacter to spread to contact surfaces during chilled broiler handling; therefore, utensils and hands involved should be washed thoroughly especially before ready-to-eat food preparation.

    Samples of Pinna bicolor Gmelin and Pinna deltodes Menke were collected from the seagrass bed of Sungai Pulai, covering Merambong shoal, Tanjung Adang shoal and Merambong Island off south western coast of Johor, Malaysia for morphological study from August 2005 to June 2006. It was observed that P. deltodes is associated with hard substratum and P. bicolor is associated with soft substratum in the study area. Physically there was no difference between P. bicolor and P. deltodes. The species were identified on the basis of nine internal and external characteristics of the valves. The two species were found to be morphologically different on the basis of width of sulcus, distance between posterior adductor muscle to posterior dorsal nacreous layer, dorsal posterior margin length and shell width.

    INTRODUCTION: Stroke has emerged as the leading cause of disability globally. The provision of long-term rehabilitation to stroke survivors poses a health care burden to many countries. Robotic devices have created a major turning point in stroke rehabilitation program. Currently, the anthropometric evidence to support the benefit of robotic rehabilitation (RR) among stroke patients is scarce. Therefore, the aim of this study was to evaluate the impact of RR on the mid-thigh circumferences of the paretic limbs in stroke patients.

    METHODS: Twenty stroke patients from conventional rehabilitation (CR) (n = 10) and RR (n = 10) groups were recruited through a purposive sampling method. Patients in the CR group received a two-hour session of a five-day-a-week home-based CR program for 4 weeks. Patients in the RR group received a five-day-a-week of an hour combined physiotherapy and occupational therapy session and a one-hour robotic therapy session using the HAL® Cyberdyne lower-limb, for 4 weeks. The mid-thigh circumferences of both limbs were measured on day 1 (baseline), week 2 and week 4 of rehabilitation program.

    RESULTS: The results revealed no statistically significant difference in the mid-thigh circumferences between the paretic (F1.05,9.44 = 1.96, p = 0.20), and the normal (F1.05,9.44 = 1.96, p = 0.20) sides in the CR group (n = 10). For the comparison between the paretic and normal sides in the RR group (n = 10), the paretic mid-thigh circumferences revealed significant time effect results (F2,18 = 11.91, p = 0.001), which were due to changes between baseline and week 2, and baseline and week 4 measurements. Interestingly, the normal mid-thigh circumferences also revealed a significant time effect (F2,18 = 6.56, p = 0.007), which is due to changes between baseline and week 4. One-way analysis of variance was employed to compare the mean average between groups due to the difference in the baseline measurements of the mid-thigh circumferences between the paretic side of the CR and the RR groups. With this adjustment, the average means mid-thigh circumferences after 4 weeks of therapy were shown to be significantly different between the CR and RR groups (F1,18 = 12.49, p = 0.02).

    CONCLUSION: Significant increments in the mid-thigh circumferences following RR were seen in the paretic limbs of stroke patients. Hence, this study may provide some insights into further potential research related to the benefits of RR in stroke patients.

    We would like to highlight an unusual clinical presentation of Kimura's disease, a rare, benign, idiopathic condition, usually seen as swelling and lesions of the head and neck region and commonly affecting young men of Asian descent. It is rare for this disorder to present with lesions on other parts of the body, especially the lower limbs. We report a 27-year-old man who presented with a large mass located at the inguinal region and extending down to the upper thigh. Due to the large size of the mass (28 x 18 cm), our provisional diagnosis was a soft tissue sarcoma. Open biopsy showed that the lesion was benign.

    A traumatic iliacus hematoma is rare and usually occurs in patients after a fall involving a lower back injury. Although the hematoma may compress the femoral nerve causing femoral nerve palsy, the gold standard treatment for this condition has not been established. Here we report transcatheter arterial embolisation as a useful treatment strategy for a traumatic iliacus hematoma.

    Myxoid liposarcoma is the major subtype of liposarcoma and commonly presents in the extremities, particularly in the thigh. We introduce an unusual case of a myxoid liposarcoma presenting as a large perineal swelling occupying the para-rectal and para-anal spaces in a 49-year-old male patient. The diagnosis, management, and prognosis of myxoid liposarcoma are discussed. A literature review is performed for myxoid liposarcoma.

    There are reported cases of cortical reaction over the tension side of the normal femoral shafts in patients on long term treatment with alendronate, leading to subsequent femoral shaft fractures. We performed a retrospective review of patients with low-energy femoral shaft fracture on alendronate, admitted to our institution during the period 2004 to May 2009. The presence of radiological changes of cortical hypertrophy with or without Looser's zone over the tension side of the femoral bone (normal limb) was determined and correlated with clinical symptoms. Thirteen patients were identified. Average duration of alendronate use was 6.5 ± 3.3 years (ranges, two to 10 years). These radiological changes were noted in four patients. Average duration of alendronate usage in these four patients was 6.5 ± 2.4 years (ranges, 5 to10 years). Prodromal thigh pain was present in a patient, who had cortical hypertrophy with the presence of a Looser's zone traversing the cortex on the femoral shaft. One patient had Looser's zone limited at the lateral hypertrophied cortex without prodromal pain. The interobserver kappa coefficient was 0.96. A femoral radiograph should be performed in all patients who are on long-term alendronate therapy who present with thigh pain. We propose a new grading system based on our observation of the radiological features in these four cases. This new grading of the radiological spectrum of femoral shaft cortical pathology has the potential to stratify the risk of low energy femoral fracture for patients treated with long-term alendronate therapy.

    Epidermoid cyst is a common benign cutaneous swelling frequently encountered in surgical practice. It usually presents as a painless lump frequently occurring in hairbearing areas of the body particularly the scalp, scrotum, neck, shoulder and back. Giant epidermoid cysts commonly occur in hairy areas such as the scalp. We present here the case of a rare occurrence of a giant epidermoid cyst in the less hairy area of the right upper thigh mimicking a soft tissue sarcoma. Steps are highlighted for the management of this unusual cyst.

    Ossifying fibromyxoid tumor (OFMT) is a rare benign tumor, most of which occurs in adults with localization in the subcutaneous tissue or muscle of the extremities. A five-year-old girl presented with a mass in her right upper thigh. Due to the large size of the mass (10 x 7 cm), our provisional diagnosis was a soft tissue sarcoma. A tru-cut biopsy showed that the lesion was benign. The mass was excised and has not recurred since. To the best of our knowledge, this patient is the youngest case of OFMT reported in the English literature.

    Knowledge of normal as well as variant great saphenous vein is useful as it is the vein that can get varicosed; the vein that is used in bypass surgeries and the vein that is used for cannulation purpose. We observed almost complete duplication of the great saphenous vein in the left lower limb of an adult male cadaver. Both the great saphenous veins arose from the medial end of the dorsal venous arch and coursed parallel to each other throughout the limb. They united in the femoral triangle to form a short (1 inch long) common great saphenous vein. Common great saphenous vein terminated into the femoral vein. There were four communicating veins connecting the two great saphenous veins in the leg, giving the appearance of a venous ladder. Knowledge of this variation could be extremely useful in treatment of varicose veins of lower limb, in catheterizations and in various surgical procedures of the lower limb.

    Oral cavity malignancy reconstructive surgery has advanced from pedicle flaps to microvascular free tissue transfer. Previously, reconstruction of these defects were done using radial forearm free flaps in this centre. We report our experience using anterolateral thigh free flap in a 49 year old lady with squamous cell carcinoma of the tongue. Excision of tumour left a defect which was later reconstructed using microvascular free tissue transfer using the right anterolateral thigh free flap. The flap was well taken up at the recepient site and no evidence of flap failure noted post operatively and on completion of radiotherapy. She was able to maintain comprehensible speech and swallowing ability.

    The aim of the study was to assess the prevalence of Sarcocystis infection in wild and peri-urban rodents in some states in Peninsular Malaysia. The thigh muscle from these rodents were formalin preserved, sectioned and stained with hematoxylin and eosin and examined under light microscopy. Of the 146 muscle tissue examined only 73 were positive for Sarcocystis infection.
    Morphological identification showed the presence of some new morphological types to be present. Different species of Sarcocystis were seen in the sections but more extensive studies are needed to identify them to species level.
