This study examined the long-standing question of whether a person’s position among siblings has a lasting impact on that person’s personality randomly. A total of 120 participants collected data is based on a questionnaire which was answered by the respondents. Thirty firstborns, thirty middle children, thirty lastborn, and thirty only children were recruited into this study by random sampling. The data analysis use a Spearman’s correlation test and Chi- Square test. Their personality was assessed by Myers-Briggs Type Indicator Personality Test (MBTI). Results indicated that the personality types are vary among the participants. Participants of different birth orders did not differ significantly in personality overall. However, Spearman’s correlation showed that birth order was correlated positively only with Sensing/Intuition personality with r = -0.221. It showed that the lower the order of birth, the more intuitive the person will be.

The aim of the study is to evaluate the effects of contact and non-contact laser photocoagulation (LP) on ocular surface changes and Ocular Surface Disease Index (OSDI) score in patients with proliferative diabetic retinopathy (PDR). This was a single center, prospective, randomised, parallel-controlled trial of pilot study in Hospital Universiti Sains Malaysia between June 2013 and May 2014. Eye with PDR was selected and randomised into 2 groups (Contact LP group and Non-contact LP group) by using random sampling envelope method. Contact LP group was treated with contact LP via slit lamp laser delivery system. Non-contact LP group was treated with non-contact LP via binocular laser indirect ophthalmoscopy system. Main outcome measures were Schirmer test value, tear film break-up time (TBUT) and OSDI score at baseline and at 3 months post laser therapy. Statistical analyses were performed using SPSS version 22.0. A total of 60 eyes were recruited (30 eyes in Contact LP and 30 eyes in Non-contact LP). Contact LP showed significant reduction of TBUT (p = 0.038) and significant increase in mean OSDI score (p = 0.001) at 3 months post laser therapy. However, there was no significant difference of mean change of Schirmer test value and TBUT between the two groups except for OSDI score (p = 0.044). Both mode of laser deliveries (contact LP and non-contact LP) showed comparable effects on ocular surface disease in PDR patient that underwent laser pan retinal photocoagulation.

INTRODUCTION: Studies have shown that a compatible human leukocyte antigen (HLA) match can confer a favourable effect on graft outcomes. We examined the outcomes of HLA matching in renal transplant donors in Malaysia.

MATERIALS AND METHODS: A total of 140 patients who had compatible ABO blood type with negative T-cell lymphocytotoxicity crossmatch were included in the study and 25% of them were spousal transplant donors. No remarkable differences in acute rejection rate, graft survival, patient survival and serum creatinine level were observed between the spousal and living-related donor groups.

RESULTS: The spousal donor group had a higher degree of HLA mismatch than the living-related donor group. HLA-A mismatch was associated with increased rejection risk at 6 months (odds ratio [OR], 2.75; P = 0.04), 1 year (OR, 2.54; P = 0.03) and 3 years (OR, 3.69; P = 0.001). It was also observed in the deleterious effects of HLA-B and HLA-DQ loci when the number of antigen mismatches increased. The risk was 7 times higher in patients with ≥1 mismatch at HLA-A, HLA-B and HLA-DR loci than those who did not have a mismatch at these loci at 6 months (P = 0.01), 1 year (P = 0.03) and 3 years (P = 0.003).

CONCLUSION: A good match for HLA-A, HLA-B, HLA-DR and HLA-DQ can prevent acute rejection risk in renal transplant patients. Consequently, spousal donor transplants could be a safe intervention in renal patients.

The optimum combination of Musa paradisiaca (MP) and Trigona sp. Honey (TH) in formulating high antioxidant jelly was analysed for total carbohydrate content (CHO), antioxidant capacity (AC), and acceptability via the Response Surface Methodology. Central composite design was employed to optimise the combination effect of two independent variables; namely MP (X1: 20-100%) and TH (X2: 20-100%) on the recovery of three responses; total carbohydrate content (Y1), antioxidant capacity (Y2), and acceptability (Y3). A polynomial model generated a satisfactory fitting of the experimental data with regards to total carbohydrate content (R2
    = 0.8974, p < 0.0024), total antioxidant capacity (R2 = 0.9702, p < 0.0001), and acceptability (R2 = 0.9136, p < 0.0001). The optimum combination for maximum recovery of CHO, AC and acceptability were 20% of MP and 20% of TBH, with a predicted CHO of 33 Kcal/5 g, AC of
    0.34 nm and acceptability score of 6.16 (< 5: not accepted; > 5: accepted).

The effects of four marination methods (water, papaya leaves juice, papaya leaves powder, and commercial meat tenderizer) on the meat quality and sensory attributes of cooked (moist and/or grill) Pectoralis major muscle of spent chicken were examined. Pectoralis major muscles from 40 spent chickens were assigned to four marination methods: marinating with 100 mL distilled water, Control (T1) (n = 10); marinating with papaya leaves juice (50 g PLP + 100 mL distilled water), T2 (n = 10); marinating with 50 g papaya leaves powder, T3 (n = 10); and marinating with 50 g commercial meat tenderizer, T4 (n = 10). Results of meat quality revealed the lowest drip loss, cooking loss, and shear force values of samples from T3 as compared to the other treatments. Additionally, myofibril fragmentation index of marinated Pectoralis major muscle from T3 was significantly higher than the other treatments. Among the treatments within the moist cooking method, T3 presented significantly higher scores for tenderness and juiciness and significantly lower score for flavour as compared to T1 and T2. In the grill cooking method, the highest scores for tenderness and juiciness were significantly demonstrated by samples of T2 and T3. Furthermore, samples from T2 had significantly higher score for flavour. Results of the present work demonstrated that marinating spent chicken meat with 50 g papaya leaves powder improved its tenderness and water holding capacity. Furthermore, greater improvement in tenderness and juiciness were observed when meat samples marinated with papaya leaves powder (T3) were further subjected to moist cooking method.

Sexual practices among heterosexual men may differ between female sex workers (FSWs) and casual partners. We surveyed 203 heterosexual men and investigated the attributes associated with inconsistent condom use among them. Lower educational attainment was positively associated with inconsistent condom use with FSWs (adjusted prevalence ratio (aPR) 2.63; P = 0.018) and casual partners (aPR 1.55; P = 0.022), whereas early age of sexual debut (aPR 3.00; P = 0.012) and alcohol use during sex (aPR 7.95; P < 0.001) were positively associated with inconsistent condom use with FSWs. Socioecological factors may explain such differences.

Upon the discovery of RNA interference (RNAi), canonical small interfering RNA (siRNA) has been recognized to trigger sequence-specific gene silencing. Despite the benefits of siRNAs as potential new drugs, there are obstacles still to be overcome, including off-target effects and immune stimulation. More recently, Dicer substrate siRNA (DsiRNA) has been introduced as an alternative to siRNA. Similarly, it also is proving to be potent and target-specific, while rendering less immune stimulation. DsiRNA is 25-30 nucleotides in length, and is further cleaved and processed by the Dicer enzyme. As with siRNA, it is crucial to design and develop a stable, safe, and efficient system for the delivery of DsiRNA into the cytoplasm of targeted cells. Several polymeric nanoparticle systems have been well established to load DsiRNA for in vitro and in vivo delivery, thereby overcoming a major hurdle in the therapeutic uses of DsiRNA. The present review focuses on a comparison of siRNA and DsiRNA on the basis of their design, mechanism, in vitro and in vivo delivery, and therapeutics.

Inorganic nanoparticles are commonly employed as vectors for delivering drugs into cancer cells while decreasing undesired cytotoxicity in healthy tissues. Carbonate apatite is an attractive nonviral vector that releases drugs at acidic environment inside the cells following endocytosis. However, maintaining the smaller particle size is crucial for enhancing cellular uptake of drugs as well as prolonging their systemic circulation time. We aimed to modify carbonate apatite with citrate for reducing the growth kinetics of carbonate apatite particles and enhancing the cellular uptake of cyclophosphamide via endocytosis. Several concentrations of sodium citrate were used to fabricate citrate-modified carbonate apatite (CMCA) particle complexes in absence or presence of cyclophosphamide. The binding affinity of the drug towards the particles and its cellular uptake were measured by high-performance liquid chromatography (HPLC). The nanoparticles' average size and zeta potential were determined by Malvern Zetasizer. Fourier-transform infrared spectroscopy (FTIR) was performed to justify association of citrate with carbonate apatite. MTT assay was performed to evaluate the cell viability. CMCA exhibited 6% more binding efficiency for cyclophosphamide and promoted fast cellular uptake of cyclophosphamide with enhanced cytotoxicity in MCF-7 cells, compared to unmodified carbonate apatite. Therefore, CMCA nanoparticles have a high potential for intracellular delivery of anti-cancer drugs and demand for further investigated in animal models of cancer.

There has been no improvement in outcome for patients with unresectable locally advanced (stage III) non-small cell lung cancer (NSCLC) for more than 10 years. The standard treatment for these patients is definitive concurrent chemotherapy and radiation (CCRT). Although the goal of treatment in this setting is to achieve a cure, most patients progress and their prognosis is poor, with a 5-year survival rate of 15-30%. There is thus an urgent need for the development of novel anticancer treatments in this patient population. Recent advances in cancer immunotherapy have led to a marked improvement in clinical outcome for advanced NSCLC. Such immunotherapy mainly consists of the administration of immune checkpoint inhibitors (ICIs) such as antibodies to cytotoxic T lymphocyte-associated protein-4 (CTLA-4) or to either programmed cell death-1 (PD-1) or its ligand PD-L1. Durvalumab (MEDI4736) is a high-affinity human immunoglobulin G1 monoclonal antibody that blocks the binding of PD-L1 on tumor cells or antigen-presenting cells to PD-1 on T cells. The PACIFIC study recently evaluated consolidation immunotherapy with durvalumab versus placebo administered after concurrent chemoradiotherapy (CCRT) in patients with unresectable stage III NSCLC. It revealed a significant improvement in both progression-free and overall survival with durvalumab, and this improvement was associated with a favorable safety profile. This achievement has made durvalumab a standard of care for consolidation after CCRT in patients with unresectable stage III NSCLC, and it has now been approved in this setting by regulatory agencies in the United States, Canada, Japan, Australia, Switzerland, Malaysia, Singapore, India, and the United Arab Emirates. In this review, we briefly summarize the results of the PACIFIC trial, including those of post hoc analysis, and we address possible molecular mechanisms, perspectives, and remaining questions related to combined treatment with CCRT and ICIs in this patient population.

Background and Aim: Both virgin coconut oil (VCO) and tocotrienol-rich fraction (TRF) are rich in antioxidants and may protect the bone against bone loss induced by ovariectomy and high-fat diet. The study aimed to determine the protective effects of combined therapy of VCO and TRF on osteoporosis in ovariectomized (OVX) rat fed with high-fat diet.

Materials and Methods: Thirty-six female Sprague-Dawley rats were divided into six groups: Sham-operated (SHAM), OVX control, OVX and given Premarin at 64.5 µg/kg (OVX+E2), OVX and given VCO at 4.29 ml/kg (OVX+V), OVX and given TRF at 30 mg/kg (OVX+T), and OVX and given a combination of VCO at 4.29 ml/kg and TRF at 30 mg/kg (OVX+VT). Following 24 weeks of treatments, blood and femora samples were taken for analyses.

Results: There were no significant differences in serum osteocalcin levels between the groups (p>0.05), while serum C-terminal telopeptide of Type I collagen levels of the OVX+VT group were significantly lower than the other groups (p<0.05). The dynamic bone histomorphometry analysis of the femur showed that the double-labeled surface/bone surface (dLS/BS), mineral apposition rate, and bone formation rate/BS of the OVX+E2, OVX+T, and OVX+VT groups were significantly higher than the rest of the groups (p<0.05).

Conclusion: A combination of VCO and TRF has the potential as a therapeutic agent to restore bone loss induced by ovariectomy and high-fat diet.

Enteric protozoa infection among cattle may pose a threat to productivity and survival leading to negative impacts on the livestock industry. A number of these pathogens are also known to be zoonotic and are of public health concern. Despite the importance of these enteric protozoa to both animal and human health, there remains a paucity of published information on the epidemiological risk factors that may be associated with bovine cryptosporidiosis in Southeast Asia. The present study was undertaken to determine the molecular prevalence and associated risk factors for Cryptosporidium infection among beef and dairy cattle in Peninsular Malaysia. Faecal samples were collected from 824 cattle in 39 farms (526 beef and 298 dairy) situated in 33 locations throughout the country, and subjected to PCR detection for Cryptosporidium using primers targeting the 18S SSUrRNA gene. Epidemiological variables including host, environment and management factors were subjected to univariate and multivariate logistic regression analyses to determine the potential risk factors for infection. The prevalence of Cryptosporidium among the cattle was 12.5%, with significant difference in the infection rate among the various breeds. There was no significant effect of gender, and both the beef and dairy cattle were at similar odds for infection. The younger cattle had a significantly higher infection rate compared to the older animals. Multivariate analysis revealed that deworming practice, distance to human settlement, geographical location (zone) and farm management system were significant risk factors associated with Cryptosporidium infection. The cattle that were reared on farms located in the northeast of the country, closest (≤200 m) to human settlements, reared extensively, and dewormed every four months were at highest risk of infection. The present study constitutes the first attempt to analyze the multivariable epidemiological risk factors involved in bovine cryptosporidiosis in Malaysia and in Southeast Asia. It is envisaged that the data obtained will facilitate better control and prevention measures for Cryptosporidium infection among cattle in the region. Due to the potential zoonotic nature of the infection, serious steps should be instituted for animal treatment and biohazard waste management on local cattle farms.

Bakery industry in Malaysia is a very dynamic and fast-moving industry, where the use of bakery products has become common among society. The purpose of this study is to understand the behaviour of consumers as well as their purchase decision process. Two objectives of this research are to identify consumer’s behaviour towards bakery product in Section 13, Shah Alam, and to study the factors that influence consumers behaviour in their decision making. Researcher has developed a questionnaire to collect the primary data. A survey consisted of a quantitative method and the data were analysed by Statistical Package for Social Science using descriptive statistics. The data were derived from self-administrated questionnaires consisted of structured questions related. A survey is performed by involving 400 bakery consumers. The theoretical framework focusing on the developed marketing mix, consisting of product, price, place, and promotion that resulted positively, and significantly related to consumers’ behaviour in their decision making. Based on the results of this study, it show that the freshness of bakery products leads to the tendency of weekly purchase among consumers. Although consumers’ monthly income is ranging between RM1000-RM4000, the amount of RM51-RM100 as the cost of buying bakery products is considered as preferable and valuable. Moreover, consumers tend to spend their time in bakery within 20 to 30 minutes. Bagus Bakery has become the most visited bakery because of its status as the preferred convenient place with promotional offers during festive season. The most common reason of buying bakery products is determined because of the variety selection. As the study has achieved its objectives, it can be concluded that consumers are having their impression while making the decision towards bakery products during the pre-purchase and post purchase.
