    INTRODUCTION: Incidental fish bone ingestion may penetrate the upper aerodigestive tract and cause extraluminal migration due to late presentation or missed diagnosis. The migrated fish bone exhibits a wide spectrum of clinical manifestations, ranging from mild symptoms to potentially fatal complications.

    PRESENTATION OF CASE: We report three cases of extraluminal fish bone migration with diverse clinical presentations and complications. The first patient had mild throat symptoms and a fish bone that travelled through the neck and migrated towards the subcutaneous tissue without causing complications. The second patient developed deep neck abscess and thoracic complications as a result of the migrated foreign body, but recovered after surgical exploration and foreign body removal. The third patient presented late in sepsis and upper airway obstruction, subsequently succumbed to multiorgan failure before any surgical intervention.

    DISCUSSION: Thorough physical and endoscopy examinations are essential in patients with fish bone ingestion. Normal endoscopic findings in a symptomatic patient should always raise the suspicion of a migrated fish bone. A radiographic imaging study is often helpful in locating the foreign body and potential complications. The migrated fish bone that acts as the source of infection in the neck should be traced and removed surgically. The resulting abscess, if present, must be drained. The management of a migrated fish bone can be challenging and often require multi-discipline collaboration.

    CONCLUSION: The migration of the ingested fish bone outside the upper aerodigestive tract can cause serious complications and death in some cases. Clinicians should always maintain a high level of suspicion towards extraluminal migration in a patient with a history of fish bone ingestions but normal endoscopic findings. We emphasize the importance of early recognition and prompt surgical intervention to remove the migrated fish bone to minimise the potential morbidity and mortality.

    Currently, the world is facing the emergence of a virus that causes pneumonia in humans, which has a higher probability of causing complications that include respiratory distress syndrome and death. The new coronavirus 2019 (2019-nCoV), which is currently known as SARS-CoV-2, is the cause of the Coronavirus disease 2019 (COVID-19) . This virus was first detected in Wuhan, Hubei Province of China, and appears to have been a zoonotic infection that has now adapted to humans. On March 11 2020, COVID-19 was announced as a pandemic by the World Health Organisation (WHO), causing widespread panic worldwide. SARS-CoV-2 is genetically similar to the 2003 Severe Acute Respiratory Syndrome-related (SARS) and shares many similarities with the disease features of influenza virus infection. Scientists around the world are racing towards the development of vaccines and antiviral for COVID-19. This review will provide an update of COVID-19, a brief review of the symptoms and route of transmission, SARS-CoV-2 virus, and why it is highly infectious as well as the currently available treatments and comorbidities.

    BACKGROUND: Current assays for detection of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) rely on time consuming, costly and laboratory based methods for virus isolation, purification and removing inhibitors. To address this limitation, we propose a simple method for testing RNA from nasopharyngeal swab samples that bypasses the RNA purification step.

    METHODS: In the current project, we have described two extraction-free reverse transcription loop-mediated isothermal amplification (RT-LAMP) assays for the detection of SARS-CoV-2 by using E gene and RdRp gene as the targets.

    RESULTS: Here, results showed that reverse transcription loop-mediated isothermal amplification assays with 88.4% sensitive (95% CI: 74.9-96.1%) and 67.4% sensitive (95% CI: 51.5-80.9%) for E gene and RdRp gene, respectively.

    CONCLUSION: Without the need of RNA purification, our developed RT-LAMP assays for direct detection of SARS-CoV-2 from nasopharyngeal swab samples could be turned into alternatives to qRT-PCR for rapid screening.

    Lithium is a medication with a variety of medical usage for various diseases including bipolar mood disorder. As the therapeutic window of lithium is narrow, its usage is commonly associated with toxicity. Lithium toxicity affects multiple systems especially the central nervous system, leading to neuropsychiatric complications. Haemodialysis is an effective method for lithium removal especially in severe lithium toxicity such as neurotoxicity with electroencephalogram changes. We describe a case of lithium neurotoxicity with electroencephalographic abnormalities which was reversed following haemodialysis.

    Menopause is the leading cause of osteoporosis for elderly women due to imbalanced bone remodelling in the absence of oestrogen. The ability of tocotrienol in reversing established bone loss due to oestrogen deficiency remains unclear despite the plenitude of evidence showcasing its preventive effects. This study aimed to investigate the effects of self-emulsified annatto tocotrienol (SEAT) on bone histomorphometry and remodelling in ovariectomised rats. Female Sprague Dawley rats (n=36) were randomly assigned into baseline, sham, ovariectomised (OVX) control, OVX-treated with annatto tocotrienol (AT) (60 mg/kg), SEAT (60 mg/kg) and raloxifene (1 mg/kg). Daily treatment given through oral gavage was started two months after castration. The rats were euthanised after eight weeks of treatment. Blood was collected for bone biomarkers. Femur and lumbar bones were collected for histomorphometry and remodelling markers. The results showed that AT and SEAT improved osteoblast numbers and trabecular mineralisation rate (p<0.05 vs untreated OVX). AT also decreased skeletal sclerostin expression in OVX rats (p<0.05 vs untreated OVX). Similar effects were observed in the raloxifene-treated group. Only SEAT significantly increased bone formation rate and reduced RANKL/OPG ratio (p<0.05 vs untreated OVX). However, no changes in osteoclast-related parameters were observed among the groups (p>0.05). In conclusion, SEAT exerts potential skeletal anabolic properties by increasing bone formation, suppressing sclerostin expression and reducing RANKL/OPG ratio in rats with oestrogen deficiency.

    Cancer is one of the leading causes of death in both developed and developing countries. Due to its heterogenous nature, it occurs in various regions of the body and often goes undetected until later stages of disease progression. Feasible treatment options are limited because of the invasive nature of cancer and often result in detrimental side-effects and poor survival rates. Therefore, recent studies have attempted to identify aberrant expression levels of previously undiscovered proteins in cancer, with the hope of developing better diagnostic tools and pharmaceutical options. One class of such targets is the π-subunit-containing γ-aminobutyric acid type A receptors. Although these receptors were discovered more than 20 years ago, there is limited information available. They possess atypical functional properties and are expressed in several non-neuronal tissues. Prior studies have highlighted the role of these receptors in the female reproductive system. New research focusing on the higher expression levels of these receptors in ovarian, breast, gastric, cervical, and pancreatic cancers, their physiological function in healthy individuals, and their pro-tumorigenic effects in these cancer types is reviewed here.

    Craniopharyngioma is a rare and benign sellar and suprasellar region tumour. It rarely manifests during pregnancy. We report a 32-year-old primigravida at 19 weeks of gestation with a craniopharyngioma, presenting with visual disturbances during pregnancy. Her vision was 6/9 OD and counting fingers OS with a relative afferent pupillary defect in the left eye. Fundi were normal bilaterally. Visual field testing showed a nasal field defect OD and generalized depression OS. Brain magnetic resonance imaging revealed a suprasellar tumour with chiasmatic compression. Craniotomy and excision of the tumour were done at 20 weeks of gestation. Histopathological examination was consistent with craniopharyngioma. Postoperatively, mother and foetus were stable. Vision improved from counting fingers to 6/6 OS and remained at 6/9 OD. Subsequently, she delivered a healthy baby at term. Such rare and difficult cases warrant close multidisciplinary cooperation pre- and post-operatively to attain optimal outcomes for both mother and baby. By optimizing the patient's medical condition, risks of complications may be reduced. A poor pre-operative vision should also not deter surgical intervention as a proven good visual outcome is achievable.

    Endogenous Klebsiella pneumoniae panophthalmitis commonly affects those with compromised immunity. The occurrence of this infection in healthy individuals is rare. We describe the case of a healthy adult who presented with endogenous Klebsiella pneumoniae panophthalmitis from an asymptomatic liver abscess. A 64-year-old, previously healthy gentleman presented with rapidly progressive left eye periorbital swelling and blurring of vision. He had a low-grade fever three days prior to the development of ocular symptoms, but otherwise no other systemic complaints. Visual acuity was light perception in the left eye, and ocular motility was restricted in all directions of gaze. Ocular examination revealed proptosis, severe conjunctival chemosis, hazy cornea, and fibrin deposition in the anterior chamber. The posterior segment could not be visualized. Ultrasound of the hepatobiliary system revealed an abscess in the right lobe of his liver. His blood cultures grew Klebsiella pneumoniae. Despite being treated with high-dose intravenous antibiotics, his eye condition deteriorated. Evisceration was performed when he developed scleral melting and globe perforation. We highlight the importance of a high index of suspicion of endogenous Klebsiella panophthalmitis as it can be easily missed in healthy adults. Early diagnosis and prompt management are needed to prevent morbidity and mortality from this devastating infection.

    Lannea coromandelica (Houtt.) Merr. is a deciduous tree in the family Anacardiaceae, which grows in lowland and hill forests; 100-1800 m. SW Guangdong, S Guangxi, S Yunnan [Bhutan, India, Myanmar, Nepal, Sri Lanka; cultivated elsewhere in continental SE Asia, such as in Cambodia, Laos, Malaysia, Thailand, Vietnam, where it is probably naturalized]. The length of the complete plastome is 162,460 bp, including 130 genes consisting of 85 protein-coding genes, 37 tRNA genes and 8 rRNA genes. The assembled plastome has the typical structure and gene content of angiosperms plastome, which includes two inverted repeats (IRs) regions of 26,877 bp, a large single copy (LSC) region of 89,599 bp and a small single-copy (SSC) region of 19,107 bp. The total G/C content in the plastome of L. coromandelica is 37.7%. The complete plastome sequence of L. coromandelica will provide contributions to the conservation genetics of this species as well as to phylogenetic studies in Anacardiaceae.

    Background: he number of patients on warfarin therapy is rising steadily. Although warfarin is beneficial, it carries a high risk of bleeding, especially if the international normalized ratio (INR) values exceed 3.0. Currently, no warfarin initiation regimens have been developed for the Asian population, especially for Malaysians. Objective: This article describes the efficacy and safety of a new initiation regimen for warfarin among warfarin-naive patients. Method: Data were retrospectively collected from the ambulatory and inpatient settings. Results: A total of 165 patients who each had a target INR of 2.0 to 3.0 were included in the study. The mean age was 57.2 years and 94 patients were male. A total of 108 patients used Regimen 1 (5 mg/5 mg/3mg) and the rest of the patients used Regimen 2 (5 mg/3 mg/3 mg). Most patients used warfarin either for atrial fibrillation (52.1%) or for venous thromboembolism (29.7%). Overall, 88 of the patients had INR values above 50% from the baseline on Day 4. Additionally, 13 patients had INR values of >3.2, which required withholding and lower dose of warfarin. The predicted weekly maintenance warfarin dose (23 ± 0.5 mg/week) was found to have correlated closely with the actual maintenance dose (22.8 ± 0.5 mg/week; r 2 = 0.75). Nearly two thirds (70.3%) of the patients achieved the target INR on Day 11. Conclusion: The warfarin initiation regimens in this study was simple, safe, and suitable to be used in both ambulatory and inpatient settings for managing warfarin therapy.

    The recent advances in sustainable supply chain management are integrated with Industry 4.0 concepts. This study develops a new integrated model to consider the sustainability and Industry 4.0 criteria for the supplier selection management. The proposed approach consists of the fuzzy best worst method (FBWM) and the two-stage fuzzy inference system (FIS) to assess the selection of suppliers. Firstly, this study determines a comprehensive list of Industry 4.0 and sustainability criteria along with their definitions. Then, the importance weight of each criterion is computed by the FBWM. Subsequently, a two-stage FIS is devoted to nominate the suppliers' performance with regard to the sustainability and Industry 4.0 criteria. To show the applicability of our integrated model, a case study for a textile company in Iran is provided. Finally, some sensitivity analyses are done to assess the efficiency of the proposed integrated approach. One finding is to establish a decision-making framework to evaluate suppliers separately, rather than relatively in a fuzzy environment using Industry 4.0 and sustainability criteria.

    Coronavirus Disease 2019 (COVID-19), during the second wave in early 2021, has caused devastating chaos in India. As daily infection rates rise alarmingly, the number of severe cases has increased dramatically. The country has encountered health infrastructure inadequacy and excessive demand for hospital beds, drugs, vaccines, and oxygen. Adding more burden to such a challenging situation, mucormycosis, an invasive fungal infection, has seen a sudden surge in patients with COVID-19. The rhino-orbital-cerebral form is the most common type observed. In particular, approximately three-fourths of them had diabetes as predisposing comorbidity and received corticosteroids to treat COVID-19. Possible mechanisms may involve immune and inflammatory processes. Diabetes, when coupled with COVID-19-induced systemic immune change, tends to cause decreased immunity and an increased risk of secondary infections. Since comprehensive data on this fatal opportunistic infection are evolving against the backdrop of a major pandemic, prevention strategies primarily involve managing comorbid conditions in high-risk groups. The recommended treatment strategies primarily included surgical debridement and antifungal therapy using Amphotericin B and selected azoles. Several India-centric clinical guidelines have emerged to rightly diagnose the infection, characterise the clinical presentation, understand the pathogenesis involved, and track the disease course. Code Mucor is the most comprehensive one, which proposes a simple but reliable staging system for the rhino-orbital-cerebral form. A staging system has recently been proposed, and a dedicated registry has been started. In this critical review, we extensively analyse recent evidence and guidance on COVID-19-associated mucormycosis in India.

    A combination of searches for top squark pair production using proton-proton collision data at a center-of-mass energy of 13 Te at the CERN LHC, corresponding to an integrated luminosity of 137 fb - 1 collected by the CMS experiment, is presented. Signatures with at least 2 jets and large missing transverse momentum are categorized into events with 0, 1, or 2 leptons. New results for regions of parameter space where the kinematical properties of top squark pair production and top quark pair production are very similar are presented. Depending on the model, the combined result excludes a top squark mass up to 1325 Ge for a massless neutralino, and a neutralino mass up to 700 Ge for a top squark mass of 1150 Ge . Top squarks with masses from 145 to 295 Ge , for neutralino masses from 0 to 100 Ge , with a mass difference between the top squark and the neutralino in a window of 30 Ge around the mass of the top quark, are excluded for the first time with CMS data. The results of theses searches are also interpreted in an alternative signal model of dark matter production via a spin-0 mediator in association with a top quark pair. Upper limits are set on the cross section for mediator particle masses of up to 420 Ge .

    The application of plant-based coagulants in wastewater treatment has increasingly progressed in the coagulation-flocculation process toward green economy and cleaner production. Plant-based coagulants have a potential as essential substitutes for commercially used chemical coagulants because of their natural characteristics and biodegradability. Chemical coagulants leave residues in treated water and generated sludge, which cause harm to human health and the ecosystem. Thus, the exploration of plant-based coagulants in wastewater treatment could reduce and eliminate the potential damage of chemical coagulants and promote the alternative approach for sustainable environment. The general processing steps of the end-to-end plant-based coagulant production, which includes primary, secondary, and tertiary stages, are discussed. However, this review focuses more on the extraction process using different solutions and compares the performance of different coagulants in removal activities after effluent treatment. Discussion on the arising challenges is elaborated, and approaches for plant-based coagulant research in the near future are suggested.

    Wetlands in the Global South are under increasing pressure due to multiple stressors associated with global change. Water and sediment quality assessments, as well as biomonitoring using macroinvertebrates communities, are fundamental tools for informing wetland condition and management strategies. Here, we examine water and sediment parameters affecting aquatic macroinvertebrates in Nlyslvey Wetland, Limpopo Province, South Africa. Water quality, sediments, fish and macroinvertebrate community data were collected across three seasons (hot-dry, hot-wet, cool-dry) from five sites. Baetidae, Corixidae, Coenogrionidae, Dytiscidae and Physidae were the most abundant and dominant families, with functional feeding group (FFG) ratios indicating that all sites were strongly autotrophic, had high predator-prey ratios, few shredders and had a stable substrate across seasons. Fish abundances increased significantly towards the cool-dry season. Based on redundancy analysis, P, Ca, pH, Cu and Na were strongly positively associated with macroinvertebrates, including Physidae, Corixidae, Planorbidae, Ostracoda, Potamonautidae and Hydropyschidae; turbidity and sulphur were associated with Dytiscidae, Oligochaetae, Libellulidae, Gerridae and Dixidae, and fish abundance, Fe, oxygen reduction potential and total dissolved solids were negatively associated with Baetidae, Belostomatidae, Hydrophilidae and Leptoceridae. Therefore, these variables accounted for large levels of variation in macroinvertebrate families, with the cool-dry season clearly distinguished from the hot-wet and hot-dry seasons according to functional feeding groups. Being a protected area, this information could provide a useful baseline for further studies into wetlands in the region subject to greater anthropogenic stresses, as well as future studies in this Ramsar site under global changes. Further studies are required to assess the importance of environmental factors influencing the richness and distribution of macroinvertebrate communities in wetlands under growing anthropogenic pressures.

    Agrobacterium tumefaciens is considered a prominent phytopathogen, though most isolates are nonpathogenic. Agrobacteria can inhabit plant tissues interacting with other microorganisms. Yeasts are likewise part of these communities. We analyzed the quorum sensing (QS) systems of A. tumefaciens strain 6N2, and its relevance for the interaction with the yeast Meyerozyma guilliermondii, both sugarcane endophytes. We show that strain 6N2 is nonpathogenic, produces OHC8-HSL, OHC10-HSL, OC12-HSL and OHC12-HSL as QS signals, and possesses a complex QS architecture, with one truncated, two complete systems, and three additional QS-signal receptors. A proteomic approach showed differences in QS-regulated proteins between pure (64 proteins) and dual (33 proteins) cultures. Seven proteins were consistently regulated by quorum sensing in pure and dual cultures. M. guilliermondii proteins influenced by QS activity were also evaluated. Several up- and down- regulated proteins differed depending on the bacterial QS. These results show the QS regulation in the bacteria-yeast interactions.

    Asian green mussel is a marine animal that is used as food by most Indonesians. The mussels are widely cultivated in tropical countries such as Indonesia, Malaysia, Thailand, and other Asian countries. The mussel, known as perna viridis, is marine biota that is a filter feeder in the waters. Therefore, the quality of its meat is greatly influenced by the quality of the sea in its habitat. It is a food that is quite popular with the community but can endanger public health due to the accumulation of heavy metals. This study used a literature review by collecting data related to heavy metal concentrations in green mussel tissue in Indonesia. The results showed that the mussels from several sampling locations still exceeded the maximum acceptable limits of lead (Pb), mercury (Hg), and cadmium (Cd) concentration according to the standards of the food and drug administration of the Republic of Indonesia. Consumption of green mussels can increase health risks if you frequently consume them from cultivating or catching locations that have been contaminated with heavy metals.

    A divergent synthesis of skeletally distinct arboridinine and arborisidine was achieved. The central divergent strategy was inspired by the divergent biosynthetic cyclization mode of arboridinine and arborisidine and their hidden topological connection. The branch point was reached through a Michael and Mannich cascade process. A site-selective intramolecular Mannich reaction was developed to construct the tetracyclic core of arboridinine, while a site-selective intramolecular α-amination of ketone was used to access the tetracyclic core of arborisidine. A strategic Peterson olefination through intramolecular nucleophile delivery was able to set up the exocyclic olefin of arboridinine.

    OBJECTIVE: The purpose was to increase use of alcoholic hand rub (AHR) in specialised newborn care unit (SNCU) to improve hand hygiene in order to reduce neonatal sepsis and mortality at Netaji Subhash Chandra Bose Medical College and Hospital, Jabalpur.

    DESIGN: A prospective interventional and observational study.

    METHODOLOGY: We formed a quality improvement (QI) team in our SNCU consisting of doctors, nurses, auxiliary staff and parents (a floating member) to improve proper use of AHR. To identify the barriers to the problem, we used fishbone analysis tool. The barriers which were not allowing the health providers to use AHR properly identified were amount of AHR in millilitres to be used per day per baby, how much and when the amount of AHR to be indented from the main store and what is the proper site to place the bottle. We used plan-do-study-act cycles to test and adapt solutions to these problems. Within 5-6 weeks of starting our project, AHR use increased from 44 mL to 92 mL per baby per day and this is sustained around 100 mL per baby per day for over 2 years now.

    RESULTS: Significant decrease in neonatal mortality was observed (reduced from median of 41.0 between August 2016 and April 2018 to 24.0 between May 2018 and December 2019). The neonates discharged alive improved from 41.2 to 52.3 as a median percentage value. The percentage of babies who were referred out and went Left Against Medical Advice (LAMA) deceased too.

    CONCLUSION: Multiple factors can lead to neonatal deaths, but the important factors are always contextual to facilities. QI methodology provides health workers with the skills to identify the major factors contributing to mortality and develop strategies to deal with them. Improving processes of care can lead to improved hand hygiene and saves lives.

    This paper presents the preparation and measurement of tissue-mimicking head phantom and its validation with the iteratively corrected coherence factor delay-multiply-and-sum (IC-CF-DMAS) algorithm for brain stroke detection. The phantom elements are fabricated by using different chemical mixtures that imitate the electrical properties of real head tissues (CSF, dura, gray matter, white matter, and blood/stroke) over the frequency band of 1-4 GHz. The electrical properties are measured using the open-ended dielectric coaxial probe connected to a vector network analyzer. Individual phantom elements are placed step by step in a three-dimensional skull. The IC-CF-DMAS image reconstruction algorithm is later applied to the phantom to evaluate the effectiveness of detecting stroke. The phantom elements are preserved and measured multiple times in a week to validate the overall performance over time. The electrical properties of the developed phantom emulate the similar properties of real head tissue. Moreover, the system can also effectively detect the stroke from the developed phantom. The experimental results demonstrate that the developed tissue-mimicking head phantom is time-stable, and it shows a good agreement with the theoretical results in detecting and reconstructing the stroke images that could be used in investigating as a supplement to the real head tissue.
