    This study aims to report two cases with an uncommon, early manifestation of herpes zoster ophthalmicus which is keratouveitis. The first patient is a 61-year-old female who had presented with painful facial skin eruption and right eye redness without impairment of vision. She was treated initially as herpes zoster blepharoconjunctivitis; however, the disease had progressed to neurotrophic keratitis with severe anterior chamber reaction manifested by a mixture of hypopyon and hyphema. The second patient is a 74-year-old female who had presented after 2 weeks of facial skin eruption with blurring of vision and similar keratouveitic manifestations. Both patients had poor visual outcome due to severe ocular inflammation.

    Over the last decade, Egypt's aquaculture sector has expanded rapidly, which has contributed substantially to per capita fish supply, and the growth of domestic fish markets and employment across the aquaculture value chain. Despite the growing importance of aquaculture sector in Egyptian labour force, only a few studies have explored the livelihoods of Egypt's women and men fish retailers. Even fewer studies have examined gender-based market constraints experienced by these informal fish retailers. This study uses sex-disaggregated data collected in 2013 in three governorates of Lower Egypt to examine the economic and social constraints to scale of enterprises between women (n = 162) and men informal fish retailers (n = 183). Specifically, we employ linear regression method to determine the correlates of enterprise performance. We found that both women and men retailers in the informal fish market earn low profits and face livelihood insecurities. However, women's enterprise performance is significantly lower than that of men even after controlling for individual socio-economic and retailing characteristics. Specifically, the burden of unpaid household work and lack of support therein impedes women's ability to generate higher revenues. These findings strengthen the argument for investing in understanding how gender norms and attitudes affect livelihood options and outcomes. This leads to recommendations on gender-responsive interventions that engage with both men and women and enhance the bargaining power and collective voice of fish retailers.

    Atrial ectopic rhythm is one of the most common fetal arrhythmias that can present during the prenatal period. Detection of fetal arrhythmia can be made by auscultating fetal heart rate and rhythm using a fetal handheld Doppler, and this can be done even in a resource-limited setting. The finding of an abnormal fetal heart rate and rhythm should prompt early referral to a pediatric cardiologist, as this may improve clinical outcomes. We present a case of atrial ectopic rhythm detected in utero using a handheld Doppler.

    Chickenpox may lead to several neurological complications. Optic neuritis is one of the complications which has rarely been described, especially in immunocompetent individuals. We report a case of an 11-year-old immunocompetent girl who presented with sudden onset bilateral vision loss three weeks after varicella eruption. Ophthalmic examination revealed bilateral optic disc edema. Diagnosis of bilateral optic neuritis secondary to varicella was established based upon the preceding medical history, supported with clinical and radiological findings.

    OBJECTIVES: Periodontal disease and polycystic ovary syndrome (PCOS) share risk factors like obesity, insulin resistance, and dyslipidemia, along with evidence of chronic inflammation in the two conditions. Evaluating the influence of PCOS on periodontal health would, therefore, identify a possible association.

    MATERIALS AND METHODS: Sixty women, divided into equal groups of PCOS and healthy patients, were clinically examined for periodontal parameters like probing depth (PD), plaque index (PI), modified gingival index (mGI), and bleeding on probing (BOP). Fasting blood sugar (FBS), insulin (FI), triglycerides (TG), and free testosterone along with serum and gingival crevicular fluid (GCF) levels of malondialdehyde (MDA) and myeloperoxidase (MPO) were the biochemical parameters evaluated.

    The biological aspects of Channa limbata were studied between November 2013 and October 2014. A total of 346 fish specimens, 185 male and 161 female, were collected from Ta Bo, Huai Yai Wildlife Sanctuary, Phetchabun Province, Thailand. Specimens range from 7.3-17.2 cm in length with body weight 8-31 g; sex ratio between males and females was 1: 0.7. The length (L), weight (W) relationship for mixed sexes was W = 0.2064 L1.85 (R2=0.90). Gonadosomatic indices for males and females were measured monthly and varied from 0.21-0.65% and 1.96-3.74%, respectively. Condition factors for males and females ranged between 0.54 - 2.20 and 0.58 - 2.72, respectively, with fecundity range 956 to 4,652 eggs in females. Fecundity (F) to weight relationship was F = 189.53 W0.59 (R2 = 0.71) and fecundity to length relationship was F = 68.82 L1.15 (R2 = 0.77). The ratio between the intestine length and total length was 1:2, indicating that C. limbata was a carnivorous feeder. Analysis of the stomach contents gave 84% insects and 16% aquatic weed. These results can be applied to conserve efforts to prevent the extinction of C. limbata in protected areas.

    The sound produced by the anurans including toads can be a survival strategy for the selection of fittest males by their conspecific females and a form of effective prezygotic mechanism. Most of the published works on anuran sound production were on true frogs or ranids, yet little is known about sound characteristics of toads. Here we describe the properties of mating calls from six selected toad species from six study sites in Sarawak. Males' mating calls were recorded and analyzed using Sound Ruler Acoustic Analysis ver 0.9.6.0. Call characteristic were subsequently compared for toad species differentiation. The results showed that each species differ in call characteristics, with pulsating note, rate of note repetition and pitch being the most apparent characters. The highest number of pulse notes belongs to Duttaphrynus melanostictus while Ansonia spinulifer exhibited highest pitch and rate of note repetition. These species' differences in mating call characteristics provide an additional method for anuran species classification besides morphological and molecular DNA data. It is also a very useful guide during field survey as the sound is audible and identification can be made without the need of capturing the toads.

    In a 90-day feeding trial, twenty four (24) Saanen lactating does were assigned to four treatment group. The treatments
    were feeding: basal diet only as a control (T1), basal diet with urea molasses multi-nutrient block (UMMB) (T2), basal
    diet with medicated urea molasses multi-nutrient block (MUMB) (T3) and basal diet with commercial mineral block (CMB)
    (T4). There were significant differences (p<0.05) between T2, T3 and T4 on the fecal egg count (FEC). Animals in T2
    and T3 showed moderate level of parasite infestation i.e. at 750 epg and 950 epg, respectively, while animals in T1 and
    T4 showed severe parasite load at 4917 epg and 1850 epg, respectively. There were no significant differences (p>0.05)
    between treatments on WBC, LYM, MON, GRA, RBC, HCT, MCV, MCH, PLT, MPV and PCT. However, significant effects (p<0.05)
    on HBG, MCHC, RDW and PDW were observed in T2 and T3. This research showed that UMMB and MUMB were effective
    in controlling parasite infestation in Saanen lactating dairy goats apart from improving their blood hematological
    parameters. Comparison with CMB showed that it is practical to be used for parasite control.

    The Chêneau brace has proven its effectiveness in treating the adolescent idiopathic scoliosis patients. However, no studies reported on the analysis of interface pressure in double-curve adolescent idiopathic scoliosis patients. In this study, we evaluated the interface pressure of the Chêneau brace action in double-curve adolescent idiopathic scoliosis patient treatment. A total of 72 (60 girls and 12 boys) patients aged 10 years and above participated in the study. The F-Socket transducers (9811E) were used to evaluate the pressure on the right thoracic and left thoracolumbar curves between normal and maximum strap tension and variation in these interface pressures with other tasks. Each patient was asked to do nine different tasks corresponding to daily activities, and the interface pressures for each activity were recorded for both normal and maximum tension. The resultant mean peak pressure in double-curve adolescent idiopathic scoliosis was higher for right thoracic curves than left thoracolumbar curves in all tasks.     Although more Malaysians are taking part in International Multisport Games, these athletes well-being at such events have not been fully explored. The purpose of this study was to examine the pattern of injury and illness among Malaysian athletes during the XVII Asian Games 2014. Clinical and socio-demographic information of athletes diagnosed with injury or illness during the centralised training camp and at the Games were recorded in a standardised report form. Throughout the study period, 83 injuries and 64 illnesses were recorded from 276 athletes. Muscle strains and tears were the most common injury followed by ligamentous injury and soft tissues contusion. The number of injuries was highest among badminton players followed by hockey and rugby. Significantly higher incidence of injuries was observed among men than women hockey players. Athletes in individual events had higher proportion of more severe injury than those in team events. Respiratory tract infection was the most frequent illness diagnosed among athletes. Most injuries and illnesses diagnosed among athletes were minor and did not result in time away from participation. The incidence of injuries and illnesses among Malaysian athletes at the XVII Asian Games were comparable with those reported by previous authors. Injury and illness rate were influenced by gender and sports. Fortunately, majority of injuries and illnesses were minor and did not prevent athletes from participation.

    A comparative study was carried out to identify the prevalence of intestinal parasitic infestation among students in 3 different types of primary school localed in Ampang area, Kuala Lumpur. A survey of infection rate was done from April 10 June 2001. Stool samples were randomly collected from one·hundred and eleven schoolchildren aged 9-12 years were examined for intestinal parasites, using a light microscope (or direct smear and after concentration for formol-either method. It was found that an overall prevalence of intestinal parasite infection of schoolchildren was 51%. The infection rate of males and females was (60.3%) and (29.2%) respectively. Prevalence seems to be higher in the younger age groups. Also, the usual intestinal parasites were found more commonly in local and foreign schools than international schooL The local school had the highest prevalence (62%), followed by the foreign school (52%) and international school (38.9%).     INTRODUCTION: The high rate of recurrence in patellar dislocation treatment, requires a more comprehensive action, this is due to causes not only single but often multifactorial, including problems with static stability, dynamic stability or both.

    PRESENTATION OF CASE: 1st case: A-39-years-old male, complain of irreducible right patella dislocation with valgus knee and already done soft tissue procedure for patella dislocation. Long-leg radiographs of the right leg showed 18° valgus mechanical angle. 2nd case: A-26-years-old obese female, complain of dislocation of left patella and history of surgery for dislocation at 5 years old. Long-leg radiographs of the right leg showed 11° valgus mechanical angle.

    DISCUSSION: After knowing the cause of the patellar dislocation from history taking, physical and supporting examination, we performed lateral open wedge distal femoral osteotomy also MPFL and MCL reconstruction, and tibial tuberosity medialization osteotomy. There is improvement mean score in Tegner Lysholm Knee Scoring system and IKDC Scoring at 6 months after surgery.

    CONCLUSION: Lateral open wedge distal femur osteotomy combine with MPFL and MCL reconstruction and tibial tuberosity medialization realignment procedure can be successfully done for improve irreducible patellar dislocation in valgus knee, from clinical and radiological evaluation have good outcome after surgery.

    Distal phalanx fractures of the toes are common injuries. The majority of them are treated conservatively with good outcome. We present the case of a painful non-union fracture of the distal phalanx of the 4th toe in a 60-year-old female patient with symphalangism of the 4th and 5th toes. She underwent surgical fixation of the fracture with concomitant inter-phalangeal joint (IPJ) arthrodesis for better stability. A transverse dorsal incision was made just distal to the IPJ to allow preparation of both the fracture site and IPJ. Fibrous tissue at the fracture non-union site was removed and the opposing surfaces drilled with a 0.88mm K-wire. Cartilaginous tissue at the IPJ was removed and similarly drilled with the 0.88mm K-wire. Stabilisation was achieved with a percutaneous headless compression screw. Radiographic union was achieved and the patient had resolution of symptoms 16 weeks after the surgery. The patient continued to be symptom-free at one year follow-up. This is the first case report of a surgically treated symptomatic non-union of distal phalanx fracture of a lesser toe in the literature.

    Background and Objective: Clozapine is a second-generation antipsychotic drug that is considered the most effective treatment for refractory schizophrenia. Several clozapine population pharmacokinetic models have been introduced in the last decades. Thus, a systematic review was performed (i) to compare published pharmacokinetics models and (ii) to summarize and explore identified covariates influencing the clozapine pharmacokinetics models.

    Methods: A search of publications for population pharmacokinetic analyses of clozapine either in healthy volunteers or patients from inception to April 2019 was conducted in PubMed and SCOPUS databases. Reviews, methodology articles, in vitro and animal studies, and noncompartmental analysis were excluded.

    Results: Twelve studies were included in this review. Clozapine pharmacokinetics was described as one-compartment with first-order absorption and elimination in most of the studies. Significant interindividual variations of clozapine pharmacokinetic parameters were found in most of the included studies. Age, sex, smoking status, and cytochrome P450 1A2 were found to be the most common identified covariates affecting these parameters. External validation was only performed in one study to determine the predictive performance of the models.

    Conclusions: Large pharmacokinetic variability remains despite the inclusion of several covariates. This can be improved by including other potential factors such as genetic polymorphisms, metabolic factors, and significant drug-drug interactions in a well-designed population pharmacokinetic model in the future, taking into account the incorporation of larger sample size and more stringent sampling strategy. External validation should also be performed to the previously published models to compare their predictive performances.

    Gait analysis is a systematic study of human locomotion, which can be utilized in variousapplications, such as rehabilitation, clinical diagnostics and sports activities. The various limitationssuch as cost, non-portability, long setup time, post-processing time etc., of the current gait analysistechniques have made them unfeasible for individual use. This led to an increase in research interestin developing smart insoles where wearable sensors can be employed to detect vertical groundreaction forces (vGRF) and other gait variables. Smart insoles are flexible, portable and comfortablefor gait analysis, and can monitor plantar pressure frequently through embedded sensors thatconvert the applied pressure to an electrical signal that can be displayed and analyzed further.Several research teams are still working to improve the insoles' features such as size, sensitivity ofinsoles sensors, durability, and the intelligence of insoles to monitor and control subjects' gait bydetecting various complications providing recommendation to enhance walking performance. Eventhough systematic sensor calibration approaches have been followed by different teams to calibrateinsoles' sensor, expensive calibration devices were used for calibration such as universal testingmachines or infrared motion capture cameras equipped in motion analysis labs. This paper providesa systematic design and characterization procedure for three different pressure sensors: forcesensitiveresistors (FSRs), ceramic piezoelectric sensors, and flexible piezoelectric sensors that canbe used for detecting vGRF using a smart insole. A simple calibration method based on a load cellis presented as an alternative to the expensive calibration techniques. In addition, to evaluate theperformance of the different sensors as a component for the smart insole, the acquired vGRF fromdifferent insoles were used to compare them. The results showed that the FSR is the most effectivesensor among the three sensors for smart insole applications, whereas the piezoelectric sensors canbe utilized in detecting the start and end of the gait cycle. This study will be useful for any researchgroup in replicating the design of a customized smart insole for gait analysis.

    Post chemoradiation vocal cord immobility is a rare complication and this maybe life threatening when patients present with severe aspiration and recurrent pneumonia or even worse if they have an upper airway obstruction. We report a case of nasopharyngeal carcinoma patient whom after receiving curative concurrent chemoradiotherapy, presented with episodes of shortness of breath and aspiration pneumonia finally diagnosed with bilateral vocal cord immobility. She had no evidence of tumour recurrence.

    Hyperreflective dots (HRD) are activated retinal microglial cells induced by retinal inflammation in diabetic patients. This study was conducted to compare the HRD count of normal and diabetic subjects; to determine the correlation between hemoglobin A1c (HbA1c) levels and HRD count; to determine HbA1c cut-off levels for the appearance of HRD in diabetic patients. A cross-sectional study was conducted among normal and diabetic patients. Fundus photos, SD-OCT images and HbA1c levels were taken. A total of 25 normal subjects, 32 diabetics without retinopathy and 26 mild-to-moderate nonproliferative diabetic retinopathy (NPDR) diabetics were recruited. There was a statistically significant difference between the mean count of HRD among the normal group, the diabetic without retinopathy group and the mild-to-moderate NPRD group. The mean HRD count in the inner retina layer was significantly higher compared to the outer retina layer. There was a significant linear relationship between the HbA1c levels and HRD count. Using the receiver operating curve, the HbA1c level of 5.4% was chosen as the cut-off point for the appearance of HRD. The positive linear correlation between the HbA1c levels and the appearance of HRD may indicate that hyperglycemia could activate retina microglial cells in diabetic patients.

    BACKGROUND: The use of social media is widespread globally. It provides a quicker and faster means of efficient exchange of communications. The use of Twitter Applications to seek mental health advice is becoming popular.

    OBJECTIVES: This study aims to identify the determinants associated with Twitter use in psychiatric consultations and to assess the level of satisfaction in using the microblogging platform. In addition, the level of e-health literacy is also assessed among users.

    METHODS: The target population included Twitter users seeking psychiatric consultation. A leading psychiatrist's twitter account with 4.5 million followers was selected and consent obtained. A validated Patient Satisfaction Questionnaire was adopted to assess the level of satisfaction in Twitter use and e-health literacy. The questionnaire was tagged to the chosen Twitter account and reminders were sent until the sample size was reached. Data was analysed using SPSS version 22.0. The analysis included descriptive statistics tabulation, multi-response analysis, and cross-tabulation for satisfaction variables and the chi-square test was used to measure association between different variables.

    RESULTS: The study obtained 155 completed response sheets, of which 52 were Twitter users seeking psychiatric advice while the rest sought general health advice. Most of the study participants were females (71.6 %). Women, single status and income range between 4000-9000 Saudi riyal were found to be significantly associated with Twitter use for psychiatric consultation. Generally, most of the participants were satisfied with Twitter in seeking psychiatric consultation that reduced financial disbursement. Furthermore, concerns were expressed regarding the waiting period, word limitations and issues of privacy. The e-health literacy was higher among the participants.

    CONCLUSION: Psychiatric consultations via Twitter is more popular among women. By addressing privacy issues and reducing response time, Twitter may be used as a major platform to deliver mental health services to the population.

    We report a case of a 46-year-old woman who has presented to a peripheral hospital with progressive exertional dyspnoea and chest discomfort. The resting ECG showed features of left-sided ventricular hypertrophy. The initial chest radiograph was reported as cardiomegaly. Initial echocardiography revealed left atrial dilatation and 'left ventricular' hypertrophy with normal ejection fraction. She was treated as possible coronary artery disease and was subsequently referred to our centre for CT coronary angiography. Findings from the CT scan were consistent with congenitally corrected transposition of the great arteries (ccTGA). This report describes the radiological features of ccTGA, its associated cardiovascular anomalies, pathophysiology and potential complications.

    A residual bone defect at the distal aspect of the adjacent second molar may occur after total removal of the lower third molar. Lower third molar coronectomy has been proved to be a safe alternative to total removal, but the extent of bone regeneration at the adjacent tooth after coronectomy is not well reported. The aim of this prospective study was to investigate the long-term bone regeneration at the distal aspect of the adjacent second molar after lower third molar coronectomy. Preoperative and postoperative cone beam computed tomography scans were measured to assess bone regeneration at the distobuccal (DB), mid-distal (MD), and distolingual (DL) aspects of the lower second molar. Forty-eight coronectomies in 37 patients (23 female) with a mean±standard deviation age of 29.1±7.2 years were assessed. The mean follow-up was 93.2±8.7 months. The mean bone level increase at DB, MD, and DL aspects was 3.2±1.6mm, 3.5±1.5mm, and 3.2±1.6mm, respectively; the bone levels were significantly higher than the preoperative measurements (P<0.001). Age and impaction patterns were not factors affecting bone regeneration. Based on this study, it appears that coronectomy of the lower third molar brings favourable bone regeneration at the distal aspect of the adjacent second molar.
