    BACKGROUND: The Delphi method has been widely applied in many study areas to systematically gather experts' input on particular topic. Recently, it has become increasingly well known in health related research. This paper applied the Fuzzy Delphi method to enhance the validation of a questionnaire pertaining chronic obstructive pulmonary disease (COPD) risk factors among metal industry workers.

    MATERIALS AND METHODS: A detailed, predefined list of possible risk factors for COPD among metal industry workers was created through a comprehensive and exhaustive review of literature from 1995 to 2015. The COPD questionnaire were distributed among people identified as occupational, environmental, and hygiene experts. Linguistic variable using Likert scale was used by the expert to indicate their expert judgment of each item. Subsequently, the linguistic variable was converted into a triangular fuzzy number. The average score of the fuzzy number will be used to determine whether the item will be removed or retained.

    RESULTS: Ten experts were involved in evaluating 26 items. The experts were in agreement with most of the items, with an average fuzzy number range between 0.429 and 0.800. Two items were removed and three items were added, leaving a total 26 items selected for the COPD risk factors questionnaire. The experts were in disagreement with each other for items F10 and F11 where most of the experts claimed that the question is too subjective and based on self-perception only.

    CONCLUSION: The fuzzy Delphi method enhanced the accuracy of the questionnaire pertaining to COPD risk factors, and decreased the length of the established tools.

    Ergonomics can be viewed as an approach to reduce injury and illness rates to improve the overall working
    conditions for employees by addressing risk factor exposure that may occur during manual tasks. The objective
    of this research was to analyze ergonomics risk factors by associating the perception of employer and employees
    towards their workplace condition in quarry and mining industry. A Questionnaire on Ergonomics Risk Assessment
    was used to determine the comparison level awareness and perception analysis among quarry and mining industry in
    Malaysia. The findings of this research prove that the exposure of ergonomics risk factors towards the workers is in
    a moderate level with a mean of 3.59 for the overall respondent review about the ergonomics risk among workers at
    their workplace. Besides, the most concern in ergonomics is about the awkward posture at work. Some 8.8% of them
    agreed and 6.6% of them totally agreed that they were in awkward posture while doing their work. As a conclusion,
    assessment of ergonomics in quarry and mining industry will be a platform to provide a safe and healthy working
    environment.

    This paper assessed the relationship between team competency and design risk
    management among Kuantan construction industries. Similarly, a survey was
    conducted among 10 local and national construction industries in Kuantan with a
    stratified random sampling. The returned questionnaires yielded 10 responses which
    was used for data analysis. Five point Likert scale categories of risk management from
    the previous studies was used and statistical analysis affirmed that there is positive
    relationship between team competency and design risk management among Kuantan
    construction industries.

    In this paper, the influence of effective communication in construction risk
    management among Kuantan Malaysian construction industries was assessed. Then a
    survey was conducted on pilot test among 10 local construction industries in Kuantan
    Malaysian, using stratified random sampling, out of which 10 questionnaires were
    collected for data analysis. Methodologically, this research is perhaps the first to
    study the influence of effective communication on construction risk management
    among Kuantan Malaysian construction industries. Using the five point Likert scale
    categories of risk management from previous studies. Pearson Correlation anaylysis
    output results affirmed that there is a strong positive relationship between effective
    communication and construction risk management among Kuantan construction
    industries.

    Global risk concerns spurred a debate regarding empowering an organization's risk management system. Risk assessment, management, and planning are vital for running a firm, but they are not universal. This research investigates how operational risk management is institutionalized in an organization and reveals best practices from a Malaysian government-linked enterprise (GLC). This phenomenological study interviewed 39 risk management officers, executives, and employees. Data are thematically analyzed. Burawoy's Social Theory uses a case study to integrate micro- and macro-organizational elements. The case company's risk management officers, executives, and employees are involved in seven processual factors, according to the analysis. Strong leadership and external consultants, setting up the apparatus and assigning the task to the person in charge, risk framework, risk diagnostic, monitor and measure, developing and nurturing risk management culture, and consistent risk management enforcement and monitoring could explain the institutionalization process of risk management in the organization. Global and local entities have institutionalized risk management. This phenomenological study helps comprehend the role of risk management institutionalization in corporate risk management. This study contributes to a practical implication such as to the GLC. It suggests that top management support and a standard risk framework are necessary for risk management homogeneity. Leaders and frameworks must address organizational processes and capabilities to ensure risk management consistency. This study contributes to the literature on risk management practices in developing nations. The paper concludes with limitations and research recommendations.

    Businesses are becoming more conscious of operational risk management practices due to the COVID-19 pandemic. However, some firms practice risk management without fully comprehending how it might help them and their needs. As a result, companies that practice risk management without realizing it are being controlled by the discipline itself. The goal of this study is to look into the epistemic process of risk management practice in the workplace. This phenomenological study interviewed 39 risk management officers, executives, and employees. Data are thematically analyzed. This study discovered five epistemic processes of risk mapping using Foucault's governmentality paradigm. This phenomenological study, interestingly, revealed the black box of risk management practices, as well as the behavior of risk management officers, executives, and risk owners who preferred to monitor the compliance aspects of risk management practices rather than comprehend the capabilities of risk management that could be used within their strategic planning process. Unaware of this black box, organizational actors were blanketed by the organization's culture of fear, which created the impression that the authority was always watching every word said and every action taken. Practically, this study contributes an improved understanding of the real function of risk management that helps them justify the practice and reduce unnecessary fear. The paper concludes with limitations and research recommendations.

    The domain of risk analysis is expanded to consider strategic interactions among multiple participants in the management of extreme risk in a system of systems. These risks are fraught with complexity, ambiguity, and uncertainty, which pose challenges in how participants perceive, understand, and manage risk of extreme events. In the case of extreme events affecting a system of systems, cause-and-effect relationships among initiating events and losses may be difficult to ascertain due to interactions of multiple systems and participants (complexity). Moreover, selection of threats, hazards, and consequences on which to focus may be unclear or contentious to participants within multiple interacting systems (ambiguity). Finally, all types of risk, by definition, involve potential losses due to uncertain events (uncertainty). Therefore, risk analysis of extreme events affecting a system of systems should address complex, ambiguous, and uncertain aspects of extreme risk. To accomplish this, a system of systems engineering methodology for risk analysis is proposed as a general approach to address extreme risk in a system of systems. Our contribution is an integrative and adaptive systems methodology to analyze risk such that strategic interactions among multiple participants are considered. A practical application of the system of systems engineering methodology is demonstrated in part by a case study of a maritime infrastructure system of systems interface, namely, the Straits of Malacca and Singapore.

    This study was the first assessment of a nationwide risk approach system to antenatal management introduced to Malaysia in 1989. Three rapid, record-based surveys on three different study groups were conducted to determine risk factor prevalence, accuracy of risk assignment, action after risk assignment and the relationship of risk level and place of delivery. The most frequent risk factors were short birth interval, high parity and first pregnancy. Accuracy of risk assignment was highest at the lowest levels of risk and poorest at the highest levels. Women at the lowest levels of risk were more likely to be seen by a doctor than women at highest risk. These was a trend to deliver in hospital, rather than at home, as level of risk increased; but many women at high risk still delivered at home. Recommendations are made on modifications to the system prior to future evaluation.

It is considered as a part of an ancient medicine with interest in it becomes rapidly escalating in this modern era. Expansive and prescribed medicine is not the only true path to treat various illnesses. Without disclaiming the modern medicine, herbs can be used as an alternative or a combined therapy with conventional medication. Phytomedicine or ethnomedicine is not a new
    field in the world of research. It has been started long ago since the ancient times. As we realized the expensive modern medication is not the only mean to treat illness, interest in the discovery of remedy through natural or conventional products has become more escalating.     The aim of this research is to apply the variance and conditional value at risk (CVaR) as risk measures in portfolio selection problem. Consequently, we are motivated to compare the behavior of two different type of risk measures (variance and CVaR) when the expected returns of a portfolio vary from a low return to a higher return. To obtain an optimum portfolio of the assets, we minimize the risks using mean variance and mean CVaR models. Dataset with stocks for FBMKLCI is used to generate our scenario returns. Both models and dataset are coded and implemented in AMPL software. We compared the performance of both optimized portfolios constructed from the models in term of risk measure and realized returns. The optimal portfolios are evaluated across three different target returns that represent the low risk low returns, medium risk medium returns and high risk high returns portfolios. Numerical results show that the composition of portfolios for mean variance are generally more diversified compared to mean CVaR portfolios. The in sample results show that the seven optimal mean CVaR0:05 portfolios have lower CVaR0:05 values as compared to their optimal mean variance counterparts. Consequently, the standard deviation for mean variance optimal portfolios are lower than the standard deviation of its mean CVaR0:05 counterparts. For the out of sample analysis, we can conclude that mean variance portfolio only minimizes standard deviation at low target return. While, mean CVaR portfolios are favorable in minimizing risks at high target return.

    PURPOSE: Leptin and adiponectin are adipokines which have been commonly implicated in carcinogenesis. As such, many studies have investigated the association of circulating leptin and adiponectin levels with colorectal cancer (CRC) risk. However, the results remained inconsistent.

    METHODS: In this work, we performed a systematic review and meta-analysis to precisely examine the association between circulating levels of leptin and adiponectin and CRC risk. A systematic literature search was performed in PubMed/MEDLINE, Scopus, Web of Science, and EMBASE databases from inception until October 2020. The pooled effect size was then estimated by calculating the odds ratio (OR).

    RESULTS: A total of 23 records (comprising 26 studies) were included in the meta-analysis. The overall analysis found that circulating levels of leptin and adiponectin were not significantly associated with CRC risk (P > 0.05). Interestingly, subgroup analysis revealed that a higher level of adiponectin was significantly associated with an increased CRC risk among overweight individuals (OR = 1.16; 95 % CI: 1.02, 1.32), and a decreased CRC risk among normal weight individuals (OR = 0.76; 95 % CI: 0.62, 0.92). Besides, a higher level of adiponectin was also significantly associated with a decreased risk of CRC in men (OR = 0.76; 95 % CI: 0.59, 0.98).

    CONCLUSIONS: In conclusion, circulating leptin level was not associated with CRC risk, but that of adiponectin was associated with CRC risk only in specific subgroups.

    Cardiovascular disease (CVD) is the leading cause of death worldwide and is a global health issue. Traditionally, statistical models are used commonly in the risk prediction and assessment of CVD. However, the adoption of artificial intelligent (AI) approach is rapidly taking hold in the current era of technology to evaluate patient risks and predict the outcome of CVD. In this review, we outline various conventional risk scores and prediction models and do a comparison with the AI approach. The strengths and limitations of both conventional and AI approaches are discussed. Besides that, biomarker discovery related to CVD are also elucidated as the biomarkers can be used in the risk stratification as well as early detection of the disease. Moreover, problems and challenges involved in current CVD studies are explored. Lastly, future prospects of CVD risk prediction and assessment in the multi-modality of big data integrative approaches are proposed.

    Catheter-related bloodstream infection (CRBSI) is an important healthcare-associated infection caused by various nosocomial pathogens. Candida parapsilosis has emerged as a crucial causative agent for the CRBSI in the last two decades. Many factors have been associated with the development of CRBSI including, demography, pre-maturity, comorbidities (diabetes mellitus, hypertension, heart diseases, neuropathy, respiratory diseases, renal dysfunction, hematological and solid organ malignancies, and intestinal dysfunction), intensive care unit (ICU) admission, mechanical ventilation (MV), total parenteral nutrition (TPN), prior antibiotic and/or antifungal therapy, neutropenia, prior surgery, immunosuppressant, and type, site, number, and duration of catheters. This study aims to determine C. parapsilosis CRBSI risk factors. A retrospective study has been performed in an 853-bedded tertiary-care hospital in north-eastern Malaysia. All inpatients with C. parapsilosis positive blood cultures from January 2006 to December 2018 were included, and their medical records were reviewed using a standardized checklist. Out of 208 candidemia episodes, 177 had at least one catheter during admission, and 31 cases had not been catheterized and were excluded. Among the 177 cases, 30 CRBSI cases were compared to 147 non-CRBSI cases [81 bloodstream infections (BSIs), 66 catheter colonizers]. The significance of different risk factors was calculated using multivariate analysis. Multivariate analysis of potential risk factors shows that ICU admission was significantly associated with non-CRBSI as compared to CRBSI [OR, 0.242; 95% CI (0.080-0.734); p = 0.012], and TPN was significantly positively associated with CRBSI than non-CRBSI [OR, 3.079; 95%CI (1.125-8.429); p = 0.029], while other risk factors were not associated significantly. Patients admitted in ICU were less likely to develop C. parapsilosis CRBSI while patients receiving TPN were more likely to have C. parapsilosis CRBSI when compared to the non-CRBSI group.

    How many people are affected by tinnitus? Is the risk of developing tinnitus on the rise or has it been declining over time? What modifiable lifestyle factors could help to prevent tinnitus? These population-based questions can be addressed through epidemiological research. Epidemiology refers to the underlying and basic science of public health. It describes the study of the distribution and determinants of health-related states or events in specified populations and the application of this study to the control of health problems. There are two key concepts in epidemiology: (1) measures of frequency and (2) measures of effect. In this chapter, we introduce the two main measures of frequency, prevalence and incidence. We also introduce the notion of risk factors, critical for understanding measures of effect concerning the risk of developing a health condition. In both sections, we provide illustrative examples from the published literature on tinnitus. We end by offering a critical evaluation of the current status of epidemiological research on tinnitus and point to some promising future directions.

    The aim of this paper is to identify, review and carry out comparison to seven guidelines where the main focus is on its risk
    assessment elements related to confined space activities. This is due to lack of the specific approach of the Legislation for confined
    space such as entry procedure requirements, risk assessment method, existing practice of risk assessment is too general and additional
    references are required for conducting risk assessment which would lead to poor response from industries. The selected guidelines will
    be summarized on its requirements, followed by a review of each guideline and finally the comparison of Legislations will be carried
    out. The results from the study would be used as a contribution factor to promote an ideal method in developing an effective risk
    assessment tools for working safely in confined space. Furthermore, several literatures which have been referred could give additional
    input for developing the risk assessment tools.

    This study investigates the value-at-risk (VaR) using nonlinear time-varying volatility (ARCH model) and extreme-value-theory (EVT) methodologies. Similar VaR estimation and prediction are observes under the EVT and heavy-tailed long-memory ARCH approaches. The empirical results evidence the EVT-based VaR are more accurate but only at higher quantiles. It is also found that EVT approach is able to provide a convenient framework for asymmetric properties in both the lower and upper tails which implies that the risk and reward are not equally likely for the short- and long-trading positions in Malaysian stock market.

    A common practice in psychiatry when treating patients is the concurrent administration of anticholinergics along with antipsychotics, either to prevent or treat extrapyramidal syndrome reactions from occurring. However, most antipsychotics have inherent anticholinergic properties themselves. Therefore, this subtype of these patients have a higher than usual risk of developing anticholinergic side-effects, of which the central nervous manifestations can mimic psychosis, and may cloud judgement on patients' progress towards their treatment.     Despite growing knowledge of the adverse effects of cigarette smoking on general health, smoking is one of the most widely prevalent addictions around the world. Globally, about 1.1 billion smokers and over 8 million people die each year because of cigarette smoking. Smoking acts as a source for a variety of oral and systemic diseases. Various periodontal issues such as increased pocket depth, loss of alveolar bone, tooth mobility, oral lesions, ulcerations, halitosis, and stained teeth are more common among smokers. This systematic review was conducted according to the guidelines from PRISMA, and research articles were retrieved from the Web database sources on 31 May 2021. The quality of research articles was ensured by the type of evidence from combined schema incorporating as schema-13 evidence type description, Cochrane health promotion and public health field (CHPPHF), and the health gains notation framework-14 screening question for quality assessment of qualitative and quantitative studies. Smokers have been found to have bleeding on probing, periodontal pockets, and clinical attachment loss compared to nonsmokers. Oral and respiratory cancers are among the most lethal known diseases caused by cigarette smoking and other commonly occurring sequelae such as stained teeth, periodontal diseases, etc.
