    The Ephemeroptera, Plecoptera and Trichoptera (EPT) community structure and the specific sensitivity of certain EPT genera were found to be influenced by water parameters in the rivers of Gunung Jerai Forest Reserve (GJFR) in the north of peninsular Malaysia. The scores of EPT taxa richness of >10 in all rivers indicated all rivers' habitats were non-impacted, having good water quality coinciding with Class I and Class II of Malaysian water quality index (WQI) classification of potable water. The abundance of EPT was very high in Teroi River (9,661 individuals) but diversity was lower (22 genera) than Tupah River which was highly diverse (28 genera) but lower in abundance (4,263 individuals). The lowest abundance and moderate diversity was recorded from Batu Hampar River (25 genera). Baetis spp. and Thalerosphyrus spp., Neoperla spp. and Cheumatopsyche spp. were the most common genera found. Classification for all rivers using EPT taxa Richness Index and WQI gave different category of water quality, respectively. The WQI classified Tupah and Batu Hampar rivers into Class II and Teroi River (Class I) was two classes above the classification of the EPT taxa Richness Index.

    Forward osmosis (FO) is an important desalination method to produce potable water. It was also used to treat different wastewater streams, including industrial as well as municipal wastewater. Though FO is environmentally benign, energy intensive, and highly efficient; it still suffers from four types of fouling namely: organic fouling, inorganic scaling, biofouling and colloidal fouling or a combination of these types of fouling. Membrane fouling may require simple shear force and physical cleaning for sufficient recovery of membrane performance. Severe fouling may need chemical cleaning, especially when a slimy biofilm or severe microbial colony is formed. Modification of FO membrane through introducing zwitterionic moieties on the membrane surface has been proven to enhance antifouling property. In addition, it could also significantly improve the separation efficiency and longevity of the membrane. Zwitterion moieties can also incorporate in draw solution as electrolytes in FO process. It could be in a form of a monomer or a polymer. Hence, this review comprehensively discussed several methods of inclusion of zwitterionic moieties in FO membrane. These methods include atom transfer radical polymerization (ATRP); second interfacial polymerization (SIP); coating and in situ formation. Furthermore, an attempt was made to understand the mechanism of improvement in FO performance by zwitterionic moieties. Finally, the future prospective of the application of zwitterions in FO has been discussed.

    Arsenic is a carcinogen element that occurs naturally in our environment. Humans can be exposed to arsenic through ingestion, inhalation, and dermal absorption. However, the most significant exposure pathway is via oral ingestion. Therefore, a comparative cross-sectional study was conducted to determine the local arsenic concentration in drinking water and hair. Then, the prevalence of arsenicosis was evaluated to assess the presence of the disease in the community. The study was conducted in two villages, namely Village AG and Village P, in Perak, Malaysia. Socio-demographic data, water consumption patterns, medical history, and signs and symptoms of arsenic poisoning were obtained using questionnaires. In addition, physical examinations by medical doctors were performed to confirm the signs reported by the respondents. A total of 395 drinking water samples and 639 hair samples were collected from both villages. The samples were analyzed using Inductively Coupled Plasma-Mass Spectrometry (ICP-MS) to determine arsenic concentration. The results showed that 41% of water samples from Village AG contained arsenic concentrations of more than 0.01 mg/L. In contrast, none of the water samples from Village P exceeded this level. Whilst, for hair samples, 85 (13.5%) of total respondents had arsenic levels above 1 μg/g. A total of 18 respondents in Village AG had at least one sign of arsenicosis and hair arsenic levels of more than 1 μg/g. Factors significantly associated with increased arsenic levels in hair were female, increasing age, living in Village AG and smoking. The prevalence of arsenicosis in the exposed village indicates chronic arsenic exposure, and immediate mitigation action needs to be taken to ensure the wellbeing of the residents in the exposed village.

    Although toxic Cd (cadmium) and Cr (chromium) in the aquatic environment are mainly from natural sources, human activities have increased their concentrations. Several studies have reported higher concentrations of Cd and Cr in the aquatic environment of Malaysia; however, the association between metal ingestion via drinking water and human health risk has not been established. This study collected water samples from four stages of the drinking water supply chain at Langat River Basin, Malaysia in 2015 to analyze the samples by inductivity coupled plasma mass spectrometry. Mean concentrations of Cd and Cr and the time-series river data (2004-2014) of these metals were significantly within the safe limit of drinking water quality standard proposed by the Ministry of Health Malaysia and the World Health Organization. Hazard quotient (HQ) and lifetime cancer risk (LCR) values of Cd and Cr in 2015 and 2020 also indicate no significant human health risk of its ingestion via drinking water. Additionally, management of pollution sources in the Langat Basin from 2004 to 2015 decreased Cr concentration in 2020 on the basis of autoregression moving average. Although Cd and Cr concentrations were found to be within the safe limits at Langat Basin, high concentrations of these metals have been found in household tap water, especially due to the contamination in the water distribution pipeline. Therefore, a two-layer water filtration system should be introduced in the basin to achieve the United Nations Sustainable Development Goals (SDGs) 2030 agenda of a better and more sustainable future for all, especially via SDG 6 of supplying safe drinking water at the household level.

    BACKGROUND:
    Oral cancer is a foremost health dilemma in several regions of the world. General dental practitioners and general medical practitioners play a major role in recognition of oral mucosal changes that may lead to malignancy. Their knowledge in oral cancer itself and the risk factors associated with the disease need to be sufficient.

    OBJECTIVE:
    The objective of the present study was to investigate awareness and knowledge of undergraduate dental and medical students in early detection and prevention of oral cancer.

    MATERIALS AND METHODS:
    Dental and medical students were invited to participate by answering a questionnaire on their habits of the oral mucosa examination and history taking, knowledge on risk factors and changes related with oral cancer, referral of patients as well as their desire to receive further information on oral cancer. Chi-square test was carried out to analyze knowledge and awareness between undergraduate dental and medical students.

    RESULTS:
    Undergraduate dental students were more likely to examine oral mucosa (96.7%) and advice risk habits to patients (93.9%) compared to medical students (60.6% and 79.8% respectively). Significantly more dental students considered smoking (84.4%), betel quid chewing (76.1%), and alcohol drinking (35%) as risk factors. Clinical changes of oral cancer were better identified by dental students (leukoplakia-52.8%, erythroplakia-45%, and non-healing ulcer-40%) compared to medical students (leukoplakia-12.9%, erythroplakia-4.6%, and non-healing ulcer-10.3%). Both dental and medicals students reported the desire to receive further information in relation to oral cancer.

    CONCLUSION:
    Dental students have better knowledge and awareness in prevention and early detection of oral cancer compared to medical students.

    Groundwater hazard assessments involve many activities dealing with the impacts of pollution on groundwater, such as human health studies and environment modelling. Nitrate contamination is considered a hazard to human health, environment and ecosystem. In groundwater management, the hazard should be assessed before any action can be taken, particularly for groundwater pollution and water quality. Thus, pollution due to the presence of nitrate poses considerable hazard to drinking water, and excessive nutrient loads deteriorate the ecosystem. The parametric IPNOA model is one of the well-known methods used for evaluating nitrate content. However, it cannot predict the effect of soil and land use/land cover (LULC) types on calculations relying on parametric well samples. Therefore, in this study, the parametric model was trained and integrated with the multivariate data-driven model with different levels of information to assess groundwater nitrate contamination in Saladin, Iraq. The IPNOA model was developed with 185 different well samples and contributing parameters. Then, the IPNOA model was integrated with the logistic regression (LR) model to predict the nitrate contamination levels. Geographic information system techniques were also used to assess the spatial prediction of nitrate contamination. High-resolution SPOT-5 satellite images with 5 m spatial resolution were processed by object-based image analysis and support vector machine algorithm to extract LULC. Mapping of potential areas of nitrate contamination was examined using receiver operating characteristic assessment. Results indicated that the optimised LR-IPNOA model was more accurate in determining and analysing the nitrate hazard concentration than the standalone IPNOA model. This method can be easily replicated in other areas that have similar climatic condition. Therefore, stakeholders in planning and environmental decision makers could benefit immensely from the proposed method of this research, which can be potentially used for a sustainable management of urban, industrialised and agricultural sectors.

    Ground water contain natural radioactivity associated with uranium and thorium that present naturally in rocks and soils. Humans may be exposed to the emission of energetic alpha particle from supported radon decaying process in this water when it is inhaled or ingested. Assessment of supported radon in ground water was carried out using fourteen ground water samples from Cameron Highlands. The measurement was accomplished by degassing the water samples using pump and then allowing the gas to flow into specially constructed 0.0191 m8 metal chamber. The activity concentration of supported radon in water sample was measured using continuous radon monitor inside the radon chamber. Measurement was carried out at one hour interval for twenty four hours. The hourly supported radon concentration was found to stabilize after about 8 hours. The stabilized concentration was used to determine supported radon activity concentration in the water samples. Results of the study show that depending on the sampling location, the activity concentrations of supported radon are in the range from 0.09 - 0.48 Bq/L which is lower than the activity concentration of radon in drinking water as proposed by USEPA (11 Bq/L).

    Malnutrition is associated with childhood diseases such as diarrhea or other severe illness. Children who suffer from
    repeated episodes of diarrhea are more likely to suffer from malnutrition. In addition, the source of drinking water is
    a very important factor affecting the nutritional status. The objective of this study was to find out the prevalence of
    malnutrition condition among children aged 3 to 5 years old and the factors associated with it. A cross sectional study
    using simple random sampling was conducted among 220 children aged 3 to 5 years old from four kindergartens in
    Baghdad city, Iraq. Nutritional assessment was carried out using the indicator weight-for-age z-score based on World
    Health Organization cut off points. Acute malnutrition was found in 18.2 % of the respondents. In this study 65 % of
    the respondents used tap water to drink as compared to 35 % who used sterilized bottles as source of drinking water.
    The environmental factors mainly the source of drinking water was significantly associated with child nutritional

    INTRODUCTION: Oral cancer is a life-threatening disease. Lack of public awareness is a potent barrier for the early detection of oral cancer, especially for high-risk populations.

    OBJECTIVE: This study aimed to determine the awareness and knowledge of the signs, symptoms and risk factors of oral cancer among a Siamese ethnic group in Tumpat, Kelantan.

    METHODS: A cross-sectional study was conducted, using a guided questionnaire on sociodemography, habits, awareness and knowledge of the signs, symptoms and risk factors of oral cancer. Individuals under 18 years old and who had been diagnosed with oral cancer were excluded from this study.

    RESULTS: A total of 195 respondents participated, 61.5% were female and the mean age was 46 (1.64). About 41% of the respondents had received secondary education and 35.4% were illiterate. Most respondents were self-employed (21.5%), followed by farmers (19.5%) and housewives (20%). The majority of them had a monthly income that fell below the poverty level of RM 830 (76.9%). Among the respondents, 22.6% had the habit of smoking, 25.6% consumed alcohol, 8.2% were betel quid chewers and 2.6% chewed tobacco. Out of 195 respondents, only 6.7% were aware of oral cancer. About 16.9% of the respondents correctly answered all of the questions regarding the signs and symptoms of oral cancer and only 4.1% knew the risk factors of oral cancer.

    CONCLUSION: The awareness and knowledge of oral cancer in this targeted population were unsatisfactory. Future effective health promotion programs and education should be emphasised.

    BACKGROUND: Results from several cohort and case-control studies suggest a protective association between current alcohol intake and risk of thyroid carcinoma, but the epidemiological evidence is not completely consistent and several questions remain unanswered.

    METHODS: The association between alcohol consumption at recruitment and over the lifetime and risk of differentiated thyroid carcinoma was examined in the European Prospective Investigation into Cancer and Nutrition. Among 477 263 eligible participants (70% women), 556 (90% women) were diagnosed with differentiated thyroid carcinoma over a mean follow-up of 11 years. Hazard ratios (HRs) and 95% confidence intervals (CIs) were estimated using multivariable Cox proportional hazards models.

    RESULTS: Compared with participants consuming 0.1-4.9 g of alcohol per day at recruitment, participants consuming 15 or more grams (approximately 1-1.5 drinks) had a 23% lower risk of differentiated thyroid carcinoma (HR=0.77; 95% CI=0.60-0.98). These findings did not differ greatly when analyses were conducted for lifetime alcohol consumption, although the risk estimates were attenuated and not statistically significant anymore. Similar results were observed by type of alcoholic beverage, by differentiated thyroid carcinoma histology or according to age, sex, smoking status, body mass index and diabetes.

    CONCLUSIONS: Our study provides some support to the hypothesis that moderate alcohol consumption may be associated with a lower risk of papillary and follicular thyroid carcinomas.

    The previous research showed that slow sand filtration (SSF) can remove the total coli by approximately 99% because of the schmutzecke layer in the filter. The presented study aimed to complete the previous research on SSF, especially on the schmuztdecke layer mechanism, to remove total coli. Total coli is a parameter of water quality standard in Indonesia, and the behavior of schmutzdecke affects the total coli removal. In the present study, the raw water from Amprong River was treated using horizontal roughing filter (HRF) and SSF. The variations in SSF rate used were 0.2 and 0.4 m/h. Total coliforms were analyzed using the most probable number test, and schmutzdecke visualization was conducted through scanning electron microscopy-energy-dispersive X-ray spectroscopy (SEM-EDX). The best coliform concentration in water treated by the combination of HRF and SSF was 4,386 colonies per 100 mL of sample using the filtration rate of 0.2 m/h, and its removal efficiency was 99.60%. However, the quality of water treated by the combination of HRF and SSF did not meet the drinking water quality standard because the removal of total coli must be 100%. The SEM-EDX visualization results in schmutzdecke showed that the average bacteria in the schmutzdecke layer were small, white, opaque, and circular, with entire edge and flat elevation. The Gram test results showed that the schmutzdecke bacteria consisted of Gram-positive and Gram-negative bacteria with basil as the common cell form.

    The contribution and impact of beverage intake to total nutrient and energy intake may be substantial. Given the link between lifestyle, diet, and the risk of pregnancy complications, this study investigated the association between the quantity and types of beverages with gestational diabetes mellitus (GDM) risk. The study included 452 women from the Seremban Cohort Study (SECOST). The mean energy by beverage intake was 273 ± 23.83 kcal/day (pre-pregnancy), 349 ± 69.46 kcal/day (first trimester) and 361 ± 64.24 kcal/day (second trimester). Women significantly increased intake of maternal milks and malted drinks, but significantly reduced the intake of carbonated drinks and other drinks from before until the second trimester of pregnancy. For chocolate drinks, carbonated drinks, and soy milk, women increased intake from pre-conception to the first trimester, but reduced their intake from the first to the second trimester. While higher intake of cultured-milk drinks was associated with an increased risk of GDM, higher fruit juice intake was associated with a lower risk of GDM. However, these associations were only observed for intake prior to pregnancy and during the first trimester. Further research is needed to corroborate these findings and investigate the contributions of different beverages to overall diet quality as well as adverse health outcomes during pregnancy.

    Epidemiological studies have reported inconsistent findings for the association between B vitamins and breast cancer (BC) risk. We investigated the relationship between biomarkers of folate and vitamin B12 and the risk of BC in the European Prospective Investigation into Cancer and Nutrition (EPIC) cohort. Plasma concentrations of folate and vitamin B12 were determined in 2,491 BC cases individually matched to 2,521 controls among women who provided baseline blood samples. Multivariable logistic regression models were used to estimate odds ratios by quartiles of either plasma B vitamin. Subgroup analyses by menopausal status, hormone receptor status of breast tumors (estrogen receptor [ER], progesterone receptor [PR] and human epidermal growth factor receptor 2 [HER2]), alcohol intake and MTHFR polymorphisms (677C > T and 1298A > C) were also performed. Plasma levels of folate and vitamin B12 were not significantly associated with the overall risk of BC or by hormone receptor status. A marginally positive association was found between vitamin B12 status and BC risk in women consuming above the median level of alcohol (ORQ4-Q1  = 1.26; 95% CI 1.00-1.58; Ptrend  = 0.05). Vitamin B12 status was also positively associated with BC risk in women with plasma folate levels below the median value (ORQ4-Q1  = 1.29; 95% CI 1.02-1.62; Ptrend  = 0.03). Overall, folate and vitamin B12 status was not clearly associated with BC risk in this prospective cohort study. However, potential interactions between vitamin B12 and alcohol or folate on the risk of BC deserve further investigation.

    Water treatment is a worldwide issue. This review aims to present current problems and future challenges in water treatments with the existing methodologies. Carbon nanotube production, characterization, and prospective uses have been the subject of considerable and rigorous research around the world. They have a large number of technical uses because of their distinct physical characteristics. Various catalyst materials are used to make carbon nanotubes. This review's primary focus is on integrated and single-treatment technologies for all kinds of drinking water resources, including ground and surface water. Inorganic non-metallic matter, heavy metals, natural organic matter, endocrine-disrupting chemicals, disinfection by-products and microbiological pollutants are among the contaminants that these treatment systems can remediate in polluted drinking water resources. Significant advances in the antibacterial and adsorption capabilities of carbon-based nanomaterials have opened up new options for excluding organic/inorganic and biological contaminants from drinking water in recent years. The advancements in multifunctional nanocomposites synthesis pave the possibility for their use in enhanced wastewater purification system design. The adsorptive and antibacterial characteristics of six main kinds of carbon nanomaterials are single-walled carbon nanotubes, multi-walled carbon nanotubes, graphene, graphene oxide, fullerene and single-walled carbon nanohorns. This review potentially addressed the essential metallic and polymeric nanocomposites, are described and compared. Barriers to use these nanoparticles in long-term water treatment are also discussed.

    The prolonged persistence of toxic arsenic (As) in environment is due to its non-biodegradable characteristic. Meanwhile, several studies have reported higher concentrations of As in Langat River. However, it is the first study in Langat River Basin, Malaysia, that As concentrations in drinking water supply chain were determined simultaneously to predict the health risks of As ingestion. Water samples collected in 2015 from the four stages of drinking water supply chain were analysed for As concentration by inductively coupled plasma mass spectrometry. Determined As concentrations along with the time series data (2004-2015) were significantly within the maximum limit 0.01 mg/L of drinking water quality standard set by World Health Organization. The predicted As concentration by auto-regression moving average was 3.45E-03 mg/L in 2020 at 95% level based on time series data including climatic control variables. Long-term As ingestion via household filtration water at Langat Basin showed no potential lifetime cancer risk (LCR) 9.7E-06 (t = 6.68; p = 3.37E-08) as well as non-carcinogenic hazard quotient (HQ) 4.8E-02 (t = 6.68; p = 3.37E-08) risk at 95% level. However, the changing landscape, ex-mining ponds and extensive use of pesticides for palm oil plantation at Langat Basin are considered as the major sources of increased As concentration in Langat River. Therefore, a two-layer water filtration system at Langat Basin should be introduced to accelerate the achievement of sustainable development goal of getting safe drinking water supply.

    Cancer causes approximately 12% of all deaths throughout the world and is the third leading cause of death in developing countries. In Malaysia, Indians have the highest incidence of mouth cancer compared to other races, and females are more affected compared to males.
    Objective: The main objective of this study was to analyze the cases of oral cancer treated in the dental department of Penang hospital, Malaysia and to determine the risk factors associated with oral cancer.

    Methodology: We reviewed the medical reports of all the patients with oral cancer treated in the dental department of Penang General Hospital from 1994 to 2004.

    Results: There were 46 cases of oral cancer treated by the dental department of Penang General Hospital during this time period. 22 were males and 24 females. The mean age of the patients was 61.2 years old. Indians comprised the majority of the cases (n=23; 50%) followed by Malays (n=12; 26.1%) and Chinese (n=11; 23.9%).     The utilization of metal-based conventional coagulants/flocculants to remove suspended solids from drinking water and wastewater is currently leading to new concerns. Alarming issues related to the prolonged effects on human health and further pollution to aquatic environments from the generated nonbiodegradable sludge are becoming trending topics. The utilization of biocoagulants/bioflocculants does not produce chemical residue in the effluent and creates nonharmful, biodegradable sludge. The conventional coagulation-flocculation processes in drinking water and wastewater treatment, including the health and environmental issues related to the utilization of metal-based coagulants/flocculants during the processes, are discussed in this paper. As a counterpoint, the development of biocoagulants/bioflocculants for drinking water and wastewater treatment is intensively reviewed. The characterization, origin, potential sources, and application of this green technology are critically reviewed. This review paper also provides a thorough discussion on the challenges and opportunities regarding the further utilization and application of biocoagulants/bioflocculants in water and wastewater treatment, including the importance of the selection of raw materials, the simplification of extraction processes, the application to different water and wastewater characteristics, the scaling up of this technology to a real industrial scale, and also the potential for sludge recovery by utilizing biocoagulants/bioflocculants in water/wastewater treatment.

    Characterisation of the leaching behaviour of coal fly ash from Tenaga Nasional Berhad (TNB) by using tank leaching test method has been reported. The leachability of the constituents such as major elements and toxic metals in the coal fly ash was studied. Eight renewed leachant solutions after 6 hours, 1, 2, 5, 8, 21, 36 and 64 days were investigated after filtration. The parameters namely pH, cumulative release regarding the major elements and toxic metals to duration were presented. The results showed that the pH solutions increased from pH 4 to neutral and remained stable during the test. It might have resulted from the large buffering capacity of the coal fly ashes. Five major elements namely Al, Ca, K, Mg and Na were detected with Ca concentration in the leachant solutions was the highest for all samples. Toxic metals such as As, Ba, Co, Cr, Mn, Ni, Pb, Se and Zn were found and the test showed consistent results on the As, Ba, Mn, Se and Zn in leachant solutions. The findings also showed that some of the toxic metal concentrations namely As, Ba, Cr, Pb and Se exceeded the maximum allowance of the guideline of drinking water quality in Malaysia and WHO. Obviously, proper waste management has to be applied in this scenario.
