    BACKGROUND AND OBJECTIVES: Globally, there is a high prevalence of postpartum depression (17.7%) reported in a recent study among mothers during the postpartum period. It contributes to poor health and well-being among newly delivered women. We reviewed the published effect of nutrition and physical activity interventions on improving and treating postpartum depression.

    METHODS AND STUDY DESIGN: The scoping review was performed using Arksey and O'Malley's methodological framework. The systematic search was conducted using Scopus, Pubmed, EBSCOHost and Google Scholar in April 2020, updated in March 2021. Only literature published between January 2010 until February 2021 was searched.

    RESULTS: A total of 25 articles were included, of which 23 were randomised controlled trials , and 2 were quasi-experimental studies. Some of studies found improvements in depression (76% out of all studies). On this basis, nutrition or physical activity intervention probably improves postpartum depression. Moreover, the integration of nutrition and physical activity appears to improve depression in the more thorough follow-up of participants . Active involvement of the participant in the interventions was contributory to effectiveness.

    CONCLUSIONS: Nutrition and physical activity interventions with appropriate strategy and delivery are promising options for the management of postpartum maternal mental health. More definitive investigation of non-pharmacological interventions to ameliorate depression among postpartum women is warranted.

    BACKGROUND: Malay medical manuscripts have deciphered the medicinal value of Piper betle (sirih) enormously. In this review, an effort was made to explore the medicinal use of P. betle and correlate this information with the scientific evidence.

    METHODS: The information regarding the use of P. betle was retrieved from the books consisting of a Malay medical manuscript with an identification number MSS 2219 from the National Library of Malaysia. PubMed, ScienceDirect and Scopus databases were used to collect information regarding the scientific evidence for the medicinal use of P. betle. This review was written following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The keywords used for searching the articles included P. betle, antimicrobial, analgaesic, haepatic and gastric.

    RESULTS: MSS 2219 showed that P. betle has varied medicinal uses and based on that, it can be grouped into six categories. P. betle application method was different in different conditions. In terms of the literature search, 226 articles were found, 75 articles were extracted for detailed analysis and only 23 met the inclusion criteria. The information was related to the chemical assays, in vivo and in vitro studies.

    CONCLUSION: In summary, P. betle has the potential to treat medical conditions in various types of categories as recorded in the Malay medical manuscripts and also based on scientific publications. For clinical purposes, more information is required, such as the specific mechanism involved, the best extraction method and the best dosage for treatment.

    The Malaysian Journal of Medical Sciences (MJMS) has conducted a simple analysis of its scholarly publication, based on the auto-generated data compiled from ScholarOne Manuscripts(™), an innovative, web-based, submission and peer-review workflow solution for scholarly publishers. The performance of the MJMS from 2014-2015 is reported on in this editorial, with a focus on the pattern of manuscript submission, geographical contributors and the acceptance-rejection rate. The total number of manuscript submissions has increased from 264 in 2014, to 272 in 2015. Malaysians are the main contributors to the MJMS. The total number of manuscript rejections following the review process was 79 (29.9%) in 2014, increasing to 92 (33.8%) the following year, in accordance with the exacting quality control criteria applied by the journal's editor to the submitted manuscripts.

    Nasal septum abscesses caused by uncontrolled diabetes mellitus are rare. We report 3 cases. Very few cases have been published concerning non-traumatic nasal septum abscesses. The development of the condition, possible complications, and treatment are discussed.

    Background: Obesity is major contributor to diseases and mortality. Constant weight loss significantly reverses diseases related to obesity and helps prolong human life expectancy. To date, there has been limited scientific evidence comparing the effectiveness of the Aqua Zumba® Fitness program and conventional aqua jog among obese women. Thus, the purpose of this study is to compare the effects between 12 weeks of Aqua Zumba® Fitness and aqua jog on health parameters among middle-age women. Methods: A total of sixty sedentary obese women (BMI >30 kg/m2), aged 40-59 years old were randomized to either Aqua Zumba® Fitness (AZ, n=20), aqua jog (AJ, n=20) or control group (C, n=20). Exercise groups were involved in different 12-week aqua exercise programs, 3 times per week, 60 minutes per session, with an intensity of 50-75% of maximum heart rate.     Thermally driven electrochemical cells (thermocells) are able to convert thermal gradient applied across redox electrolyte
    into electricity. The performance of the thermocells heavily depends on the magnitude and integrity of the applied thermal
    gradient. Herein, we study the iodide/triiodide (I–/I3
    –) based 1-Ethyl-3-methyl-imidazolium Ethylsulfate ([EMIM][EtSO4])
    solutions in a thermocell. In order to comprehend the role of fluidity of the electrolyte, we prepared set of solutions by
    diluting [EMIM][EtSO4] with 0.002, 0.004, and 0.010 mol of Acetonitrile (ACN). We realized a significant improvement
    in ionic conductivity (σ) and electrochemical Seebeck (Se) of diluted electrolytes as compared to base [EMIM][EtSO4]
    owing to the solvent organization. However, the infra-red thermography indicated faster heat flow in ACN-diluted-[EMIM]
    [EtSO4] as compared to the base [EMIM][EtSO4]. Therefore, the maximum power density of base [EMIM][EtSO4] (i.e.
    118.5 µW.m-2) is 3 times higher than the ACN-diluted-[EMIM][EtSO4] (i.e. 36.1 µW.m-2) because of the lower thermal
    conductivity. Hence this paper illustrates the compromise between the fast mass/flow transfer due to fluidity (of diluted
    samples) and the low thermal conductivity (of the pure [EMIM][EtSO4]).

    Introduction: Traditionally, Mallotus paniculatus (Balik Angin) plant is used in the treatment of various
    diseases in rural areas such as remedy after childbirth, wound healing and fever. In this present study, four
    medicinal properties of the plant were investigated which included antibacterial, antifungal, anticancer and
    antioxidant activities. Materials and Methods: Potential medicinal compounds were extracted from the plant
    leaves by sonication with 3 different solvents namely ethanol, ethyl acetate and hexane respectively. The
    antibacterial and antifungal properties were determined using disc diffusion agar and broth dilution assay,
    the antioxidant activity by DPPH scavenging assay and the anticancer effect by MTT assay. Results: From the
    screening of the medicinal properties, M. paniculatus leave extracts were shown to possess antibacterial,
    antioxidant and anticancer properties but not antifungal properties. Ethanolic and ethyl acetate extracts of
    the leave were active against gram positive bacteria (Staphylococcus aureus and Bacillus subtilis) but not
    gram negative bacteria (Pseudomonas aeruginosa and Escherichia coli). The antioxidant activity of the
    ethanolic crude extract was high; with IC50 of 30 μg/ml comparable with the positive controls; ascorbic acid
    and butylated hydroxytoluene (BHT). Both ethanolic and ethyl acetate extracts were cytotoxic against breast
    cancer (MCF7), colon cancer (HT-29), cervix cancer (Hela) cell lines. Conclusion: M. paniculatus leave
    extract has many potential medicinal values for further studies.

    Acute lymphoblastic leukemia (ALL) is the most common leukemia subtypes among paediatrics in Malaysia. Although treatment options are available but some patients remain incurable, some undergo relapse and many experiences adverse effects by the conventional therapies. Thus, we aim to investigate possible treatment alternative by studying the antileukemogenesis properties of concentrated Nypa fruticans sap called nisaan by focusing on adiponectin expression.
    Our study model was CCRF-CEM, an acute lymphoblastic leukemia cell lines. The cells were treated with nisaan at a range of concentration and treated for 24, 48 and 72 hours followed by determination of the leukemic cells viability using tryphan blue method. Effective nisaan concentrations that significantly reduced the cells viability were again treated to the cells followed by determination of the cell proliferation using BrdU colorimetric kit and adiponectin level using adiponectin ELISA kit.
    The results showed that, increase concentration of nisaan treatment reduced the cells viability and cells proliferation and enhance the adiponectin level in the leukemic cells.
    This preliminary data suggest that Nypa fruticans might has the antileukemogenesis effect on acute lymphoblastic cells by regulating the adiponectin expression.

    Neurofibromatosis Type 1 (NF1), also known as von Recklinghausen’s disease is a multisystem genetic disorder that occurs with an incidence of 1 in 4000 live births. Plexiform nmeurofibroma (PN) is a rare important variant of NF1, seen in 5 to 15% of cases. We report a 7-year-old boy, without family history of neurofibromatosis presented with left neck swelling associated with multiple café au lait spots for one-year duration. Computed tomography of head and neck revealed an extensive soft tissue lesion involving the left parapharyngeal, carotid space and parotid space encasing the left internal and common carotid artery until its origin at the arch of aorta with erosion of the basiocciput. He underwent transcervical excision of the lesion, which was confirmed histopathologically as PN.

    We report a rare case of homozygous familial hypercholesterolemia (HoFH), a 22-year-old Malay woman who presented initially with minor soft tissue injury due to a cycling accident. She was then incidentally found to have severe xanthelasma and hypercholesterolemia (serum TC 15.3 mmol/L and LDL-C 13.9 mmol/L). She was referred to the Specialized Lipid Clinic and was diagnosed with familial hypercholesterolemia (FH) based on the Simon Broome (SB) diagnostic criteria. There was a family history of premature coronary heart disease (CHD) in that three siblings had sudden cardiac death, and of consanguineous marriage in that her parents are cousins. DNA screening of LDLR and APOB genes was done by Polymerase Chain Reaction (PCR), followed by Denaturing High Performance Liquid Chromatography (DHPLC). Homozygous mutation C255S in Exon 5 of her LDLR gene was found. There was no mutation was found in Exon 26 and Exon 29 of the APOB gene. This report is to emphasize the importance of identifying patients with FH and cascade screening through established diagnostic criteria and genetic studies in order to ensure early detection and early treatment intervention to minimize the risk of developing CHD and related complications.

    Osteoporosis is a proven complication of long-term glucocorticoid therapy. Concern on glucocorticoid induced osteoporosis has increased dramatically in recent years with the widespread use of synthetic glucocorticoids. Glucocorticoid action in bone depends upon the activity of 11βhydroxysteroid dehydrogenase type 1 enzyme (11βHSD1). This enzyme plays an important role in regulating corticosteroids by locally interconverting cortisone into active cortisol. This has been demonstrated in primary cultures of human, mouse or rat osteoblasts. Therefore, inhibition of this enzyme may reduce bone resorption markers. Piper sarmentosum (Ps) is a potent inhibitor of 11βHSD1 in liver and adipose tissue. In this study we determined the effect of Ps on 11βHSD1 activity in bones of glucocorticoid-induced osteoporotic rats.

    Visceral obesity may be due to the dysregulation of cortisol production or metabolism that lead to metabolic disease. In adipose tissue, the enzyme 11beta-hydroxysteroid dehydrogenase type 1 regulates cortisol metabolism (11beta-HSD1). A previous study showed an increase in the visceral fat deposition in adrenalectomised rats given intramuscular dexamethasone. Glycyrrhizic acid (GCA) has been shown to reduce fat deposition because it is a known potent inhibitor of the 11beta-HSD1 enzyme. Piper sarmentosum (PS) is an edible medicinal plant commonly used in Asia as traditional medicine for treating diabetes, hypertension and joint pains. In this study, we determined the effects of PS extract on the disposition and morphology of perirenal adipocytes of adrenalectomised rats given intramuscular dexamethasone.

    Aim: The aim of this study was to analyze and identify differences in the palatal rugae patterns and genderwise distributions in three different populations (Kerala, Mahe and Puducherry).

    Materials and Methods: Ninety maxillary study models (30 from each group including males and females) were examined in the age group ranging from 18 to 25 years. The palatal rugae pattern was analyzed for shape.

    Results: After analyzing the rugae patterns among the groups, the most common pattern was the wavy pattern (53.57%) followed by curved (18.22%) and straight (13.66%). The least was circular (1.3%). When compared between sex, the most common pattern was found to be wavy (male - 54.3% and female - 53.09%), while the curved pattern was more common among the females (21.09%) than males (13.97%). The straight pattern was more common among the males (18.8%) than females (10.18%). The least common pattern was found to be the circular in both sexes which accounted for around 1%.

    Conclusion: This study shows no two palates are identical in terms of their rugae pattern. Palatal rugae possess unique characteristics as they are absolutely individualistic. Study also confirms that the "wavy" type of palatal rugae pattern was the most predominant among these three populations.

    Glucocorticoids cause osteoporosis by decreasing bone formation and increasing bone resorption activity. Glucocorticoid action in bones depends on the activity of 11-beta-hydroxysteroid dehydrogenase type 1 (11β-HSD1) enzyme, which plays an important role in regulating corticosteroids. 11β-HSD1 is expressed by human and rat osteoblasts. We aimed to investigate the relationship between 11β-HSD1 dehydrogenase activity and bone histomorphometric changes in glucocorticoid-induced osteoporotic bone in rats.

    BACKGROUND: Long-term glucocorticoid therapy causes secondary osteoporosis leading to pathological fractures. Glucocorticoid action in bone is dependant upon the activity of 11β-hydroxysteroid dehydrogenase type 1 enzyme (11β-HSD1). Piper sarmentosum is a local herb that possesses the ability to inhibit 11-βHSD1 enzyme activity. We aimed to determine the effects of Piper sarmentosum water extract on 11-βHSD1 expressions and activity in the bones of glucocorticoid-treated adrenalectomized rats.

    METHODS: Forty male Sprague-Dawley rats (200-250 g) were used. Twenty-four animals were adrenalectomized and received intramuscular injection of dexamethasone (120 μg/kg/day). They were simultaneously administered with either Piper sarmentosum water extract (125 mg/kg/day), GCA (120 mg/kg/day) or distilled water as vehicle by oral gavage for two months. Eight animals were sham-operated and given vehicle daily, i.e. intramuscular olive oil and oral distilled water.

    RESULTS: Following two months treatment, dexamethasone-treated adrenalectomized rats had significantly lower 11β-HSD1 dehydrogenase activity and higher 11β-HSD1 expression in the femoral bones compared to the sham-operated and baseline group. The rats supplemented with Piper sarmentosum water extract had significantly higher 11β-HSD1 dehydrogenase activity and lower 11β-HSD1 expression in the bones.

    CONCLUSION: The results showed that Piper sarmentosum water extract had the ability to prevent glucocorcoticoid excess in the bones of glucocorticoid-treated adrenalectomized rats through the local modulation of 11β-HSD1 expression and activity, and may be used as prophylaxis for osteoporosis in patients on long-term glucocorticoid treatment.

    Introduction: Awareness of the importance of dietary fibre (DF) in health among adolescents is seldom reported in Malaysia. This study aimed to compare the knowledge, attitude and practice (KAP) of DF intake between Malaysian rural and urban school-going adolescents. Pulau Pangkor in Perak and Damansara in Selangor were randomly selected as rural and urban schools, respectively. Methods: A total of 305 school adolescents with a mean age of 13.5±0.6 years were randomly selected from rural (72 Malay, 85 Chinese) and urban (86 Malay, 62 Chinese) schools completed socio-demographic, validated KAP on DF intake questionnaire, as well as a 24-hour dietary recall. Results: Adolescents from both areas have moderate knowledge (54.4±11.3%), positive attitude (78.7±13.1%) and good practice (65.8±19.9%) scores towards DF intake. Although rural adolescents had significantly (p=0.022) higher DF intake (7.8±3.5 g) compared to urban adolescents (6.9±3.5 g), their DF intake was still lower than the Malaysian recommendation of 20-30 g/day. The attitude of DF intake of rural (r=0.390) and urban (r=0.370) adolescents showed significant positive correlations with practice score of DF intake. While a significant correlation was found between the practice score and DF intake (r=0.191, p=0.017), no significant correlation was found between knowledge and attitude scores with DF intake. Conclusion: Public health authorities and schools should raise awareness on health benefit of consuming DF to promote an increase in DF consumption among school adolescents.

    Layered double hydroxide (LDH)-based nanocomposite, created by interacting LDH with another nanoparticles usually applied in the area of environmental, catalytic, industrial and biomedical field. In this study, a new herbicide delivery system was developed through the intercalation of Magnesium Aluminiumlayered double hydroxide (MAN host) with 4-chlorophenoxyacetic acid (4-CPA). This intercalation was done by using co-precipitation method at pH 10 with different concentration of 4-CPA which were 0.5 M and 0.7 M. The successful intercalation was obtained at 0.7 M Magnesium Aluminium-4-chlorophenoxyacetic acid (MAC) and had been confirmed through several analysis. Firstly, the XRD pattern shows expansion of basal spacing shifted from 8.9 Å to 9.3 Å. This result supported by the FTIR-ATR spectrum that shows the disappearance nitrate peak (NO3 - ) and appearance of carboxylate ion (COO- ) peak at 1596 cm-1 . Furthermore, this intercalation was validated with the result of surface morphology by FESEM. The controlled release study of 4-CPA from 0.7 M MAC shown sodium carbonate solution released the percentage of 4-CPA higher than tap water. This study has signifies MAC as a safer agent of agrochemicals by reducing the dosage of herbicide in the agriculture field and protect the herbicide through encapsulation system.

    Chlorella vulgaris (ChV), sejenis alga hijau unisel telah dilaporkan mempunyai khasiat kesihatan pada penyakit tertentu termasuk kanser. Objektif utama kajian ialah untuk mengukur dan menilai kesan antioksidan dan antitumor ekstrak air panas ChV ke atas sel kanser hepar yang dijalankan secara in vivo dan in vitro. Asai DPPH yang dijalankan menunjukkan peratus pengautan ChV yang tinggi. Dalam kajian in vivo, tikus Wistar jantan (200-250 g) dibahagikan kepada lapan kumpulan: tikus kawalan (diet normal), tikus diaruh kanser hepar (diet kurang kolin + 0.1% etionin dalam air minuman) atau singkatannya CDE, tikus diberi rawatan ChV pada tiga dos berbeza (50, 150 dan 300 mg/kg berat badan) dan tikus CDE diberi rawatan ChV pada tiga dos berbeza. Sampel darah dan tisu diambil dari semua kumpulan tikus pada minggu 0, 4, 8 dan 12 untuk penentuan kadar proliferasi dan apoptosis sel untuk melihat kesan antitumor ChV. Peratus pembentukan nodul praneoplasia adalah tinggi pada tikus diaruh kanser hepar (CDE) tetapi ChV pada semua dos berjaya mengurangkannya. Pertambahan jumlah sel kanser semasa hepatokarsinogenesis ditunjukkan dengan peningkatan proliferasi hepatosit yang signifikan (p<0.05) pada tikus CDE berbanding kawalan tetapi ChV pada semua dos berjaya mengurangkan proliferasi secara signifikan (p<0.05). Peratus apoptosis sel didapati meningkat secara signifikan (p<0.05) pada tikus CDE, tetapi peningkatan yang lebih ketara berlaku pada tikus CDE diberi ChV (300 mg/kg berat badan). Dalam kajian in vitro pula, aktiviti antitumor ekstrak air panas ChV telah ditentukan dengan melihat perubahan dalam proliferasi dan apoptosis sel kanser hepar HepG2 yang dikultur di makmal. Ekstrak air panas ChV berjaya menurunkan kadar proliferasi sel HepG2 dengan signifikan secara berkadar terus dengan dos yang digunakan dengan nilai IC50 1.6 mg/ml. Hasil analisis TUNEL pula menunjukkan ekstrak air panas ChV berjaya mengaruh apoptosis dalam sel HepG2. Keputusan ini disokong oleh hasil pemblotan Western dengan peningkatan pengekspresan protein P53 dan protein proapoptosis BAX dan Kaspase-3. Daripada hasil-hasil kajian, dapatlah dicadangkan bahawa ChV berpotensi tinggi sebagai antioksidan serta berupaya memberi kesan antitumor kepada kanser hepar pada kajian in vivo dan in vitro.
