This paper focuses on the detection of retinal blood vessels which play a vital role in reducing the proliferative diabetic retinopathy and for preventing the loss of visual capability. The proposed algorithm which takes advantage of the powerful preprocessing techniques such as the contrast enhancement and thresholding offers an automated segmentation procedure for retinal blood vessels. To evaluate the performance of the new algorithm, experiments are conducted on 40 images collected from DRIVE database. The results show that the proposed algorithm performs better than the other known algorithms in terms of accuracy. Furthermore, the proposed algorithm being simple and easy to implement, is best suited for fast processing applications.
Rock filters are an established technology for polishing waste stabilization pond effluents. However, they rapidly become anoxic and consequently do not remove ammonium-nitrogen. Horizontal-flow aerated rock filters (HFARF), developed to permit nitrification and hence ammonium-N removal, were compared with a novel vertical-flow aerated rock filter (VFARF). There were no differences in the removals of BOD5, TSS and TKN, but the VFARF consistently produced effluents with lower ammonium-N concentrations (<0.3 mg N/L) than the HFARF (0.8-1.5 mg N/L) and higher nitrate-N concentrations (24-29 mg N/L vs. 17-24 mg N/L).

The success of dental implant treatment depends on the healing of both hard and soft tissues. While osseointegration provides initial success, the biological seal of the peri-implant soft tissue is crucial for maintaining the long term success of implants. Most studies of the biological seal of peri-implant tissues are based on animal or monolayer cell culture models. To understand the mechanisms of soft tissue attachment and the factors affecting the integrity of the soft tissue around the implants, it is essential to obtain good quality histological sections for microscopic examination. The nature of the specimens, however, which consist of both metal implant and soft peri-implant tissues, poses difficulties in preparing the specimens for histomorphometric analysis of the implant-soft tissue interface. We review various methods that have been used for the implant-tissue interface investigation with particular focus on the soft tissue. The different methods are classified and the advantages and limitations of the different techniques are highlighted.
The performance of a digital mammography system (Siemens Mammomat Novation) using different target/filter combinations and tube voltage has been assessed. The objective of this study is to optimize beam quality selection based on contrast-to-noise ratio (CNR) and mean glandular dose (MGD). Three composition of breast were studied with composition of glandular/adipose of 30/70, 50/50, and 70/30. CNR was measured using 2, 4 and 6 cm-thick simulated breast phantoms with an aluminium sheet of 0.1 mm thickness placed on top of the phantom. Three target/filter combinations, namely molybdenum/molybdenum (Mo/Mo), molybdenum/rhodium (Mo/Rh) and tungsten/rhodium (W/Rh) with various tube voltage and mAs were tested. MGD was measured for each exposure. For 50/50 breast composition, Mo/Rh combination with tube voltage 26 kVp is optimal for 2 cm-thick breast. W/Rh combination with tube voltage 27 and 28 kVp are optimal for 4 and 6 cm-thick breast, respectively. For both 30/70 and 70/30 breast composition, W/Rh combination is optimal with tube voltage 25, 26 and 27 kVp, respectively. From our study it was shown that there are potential of dose reduction up to 11% for a set CNR of 3.0 by using beam quality other than that are determined by AEC selection. Under the constraint of lowest MGD, for a particular breast composition, calcification detection is optimized by using a softer X-ray beam for thin breast and harder X-ray beam for thick breast. These experimental results also indicate that for breast with high fibroglandular tissues (70/30), the use of higher beam quality does not always increase calcification detection due to additional structured noise caused by the fibroglandular tissues itself.
Palm oil industry is one of the leading agricultural industries in Malaysia with average crude palm oil production of more than 13 million tonne per year. However, production of such huge amount of crude palm oil has consequently resulted to even larger amount of palm oil mill effluent (POME). POME is a highly polluting wastewater with high chemical oxygen demand (COD) and biochemical oxygen demand (BOD) in which can caused severe pollution to the environment, typically pollution to water resources. On the other hand, POME was identified as a potential source to generate renewable bioenergies such as biomethane and biohydrogen through anaerobic digestion. In other words, a combination of wastewater treatment and renewable bioenergies production would be an added advantage to the palm oil industry. In line with the world's focus on sustainability concept, such strategy should be implemented immediately to ensure palm oil is produced in an environmental friendly and sustainable manner. This review aims to discuss various technologies to convert POME to biomethane and biohydrogen in a commercial scale. Furthermore, discussion on using POME to culture microalgae for biodiesel and bioethanol production was included in the present paper as a new remedy to utilize POME with a greater beneficial return.

Landfill leachate is one of the major contamination sources. In this study, the ability of synthetic ion exchange resins which carry different mobile ion for removing color, chemical oxygen demand (COD), and ammonia nitrogen (NH(3)-N) from stabilized leachate was investigated. The synthetic resin INDION 225 Na as a cationic exchanger and INDION FFIP MB as an anionic exchanger were used in this study. INDION 225 Na was used in hydrogen form (H(+)) and in sodium form (Na(+)), while INDION FFIP MB resin was used in hydroxide form (OH(-)) and in calcium form (Cl(-)) form. The results indicated better removal of color, COD and NH(3)-N by using INDION 225 Na in H(+) as compared with Na(+) form, while no performance differences were observed by using INDION FFIP MB in OH(-) or Cl(-) form. Applying cationic resin followed by anionic resin achieved 97, 88 and 94, percent removal of color, COD and NH(3)-N. The residual amounts were 160 Pt-Co, 290 mg/L and 110 mg/L of color, COD and NH(3)-N respectively.

This research investigates the use of charge coupled device (abbreviated as CCD) linear image sensors in an optical tomographic instrumentation system used for sizing particles. The measurement system, consisting of four CCD linear image sensors are configured around an octagonal shaped flow pipe for a four projections system is explained. The four linear image sensors provide 2,048 pixel imaging with a pixel size of 14 micron × 14 micron, hence constituting a high-resolution system. Image reconstruction for a four-projection optical tomography system is also discussed, where a simple optical model is used to relate attenuation due to variations in optical density, [R], within the measurement section. Expressed in matrix form this represents the forward problem in tomography [S] [R] = [M]. In practice, measurements [M] are used to estimate the optical density distribution by solving the inverse problem [R] = [S](-1)[M]. Direct inversion of the sensitivity matrix, [S], is not possible and two approximations are considered and compared-the transpose and the pseudo inverse sensitivity matrices.
Mature landfill leachate is typically non-biodegradable and contains high concentration of refractory organics. The aim of this research was to optimize operating parameters in electro-Fenton process, for the removal of recalcitrant organics from semi-aerobic landfill leachate using response surface methodology (RSM). Effectiveness of important process parameters H(2)O(2)/Fe(2+) molar ratio, current density, pH and reaction time were determined, optimized and modeled successfully. Significant quadratic polynomial models were obtained (R(2)=0.9972 and 0.9984 for COD and color removals, respectively). Numerical optimization based on desirability function were employed; in a 43 min trial 94.07% of COD and 95.83% of color were removed at pH 3 and H(2)O(2)/Fe(2+) molar ratio 1, while current density was 49 mA/cm(2). The results indicate that E-Fenton process was an effective technology for semi-aerobic landfill leachate treatment.

The motivation of this paper is to analyse the efficiency and reliability of our proposed algorithm of femur length (FL) measurement for the estimation of gestational age. The automated methods are divided into the following components: threshold, segmentation and extraction. Each component is examined, and improvements are made with the objective of finding the optimal result for FL measurement. The methods are tested with a total of 200 different digitized ultrasound images from our database collection. Overall, the study shows that the watershed-based segmentation method combined with enhanced femur extraction algorithm and a 12 x 12 block averaging seed-point threshold method perform identically well with the expert measurements for every image tested and superior as compared to a previous method.
We studied the efficacy of two surgical methods used for the treatment of intracranial subdural empyema (ISDE) at our centre. A cross-sectional study (1999-2005) of 90 patients with non-traumatic supratentorial ISDE revealed that the two surgical methods used for empyema removal were burr hole/s and drainage (50 patients, 55.6%) and a cranial bone opening procedure (CBOP) (40 patients, 44.4%). Patients in the CBOP group had a better result in terms of clinical improvement (chi-squared analysis, p=0.006) and clearance of empyema on brain CT scan (chi-squared analysis, p<0.001). Reoperation was more frequent among patients who had undergone burr hole surgery (multiple logistic regression, p<0.001). The outcome and morbidity of ISDE survivors were not related to the surgical method used (p>0.05). The only factor that significantly affected the morbidity of ISDE was level of consciousness at the time of surgery (multiple logistic regression, p<0.001). We conclude that CBOP and evacuation of the empyema is a better surgical method for ISDE than burr hole/s and drainage. Wide cranial opening and empyema evacuation improves neurological status, gives better clearance of the empyema and reduces the need for reoperation. Level of consciousness at the time of presentation is a predictor of the morbidity of ISDE. Thus, aggressive surgical treatment should occur as early as possible, before the patient deteriorates.
This paper presents an approach for breast cancer diagnosis in digital mammogram using curvelet transform. After decomposing the mammogram images in curvelet basis, a special set of the biggest coefficients is extracted as feature vector. The Euclidean distance is then used to construct a supervised classifier. The experimental results gave a 98.59% classification accuracy rate, which indicate that curvelet transformation is a promising tool for analysis and classification of digital mammograms.
We report on two patients with intra-operative rupture of cerebral aneurysms that were managed by microsuturing. This is one of only a few reports of successful direct repair using suturing. We found that stitching remains an option to repair a tear of a saccular part of an aneurysm and a torn neck of a blister-like aneurysm, and thus this technique can be considered before sacrificing the artery.
An intelligent prediction system has been developed to discriminate drug-like and non drug-like molecules pattern. The system is constructed by using the application of advanced version of standard multilayer perceptron (MLP) neural network called Hybrid Multilayer Perceptron (HMLP) neural network and trained using Modified Recursive Prediction Error (MRPE) training algorithm. In this work, a well understood and easy excess Rule of Five + Veber filter properties are selected as the topological descriptor. The main idea behind the selection of this simple descriptor is to assure that the system could be used widely, beneficial and more advantageous regardless at all user level within a drug discovery organization.
A headspace single-drop microextraction (HS-SDME) procedure is optimized for the analysis of organochlorine and organophosphorous pesticide residues in food matrices, namely cucumbers and strawberries by gas chromatography with an electron capture detector. The parameters affecting the HS-SDME performance, such as selection of the extraction solvent, solvent drop volume, extraction time, temperature, stirring rate, and ionic strength, were studied and optimized. Extraction was achieved by exposing 1.5 microL toluene drop to the headspace of a 5 mL aqueous solution in a 15-mL vial and stirred at 800 rpm. The analytical parameters, such as linearity, correlation coefficients, precision, limits of detection (LOD), limits of quantification (LOQ), and recovery, were compared with those obtained from headspace solid-phase microextraction (HS-SPME) and solid-phase extraction. The mean recoveries for all three methods were all above 70% and below 104%. HS-SPME was the best method with the lowest LOD and LOQ values. Overall, the proposed HS-SDME method is acceptable in the analysis of pesticide residues in food matrices.
Skin colour is vital information in dermatological diagnosis as it reflects the pathological condition beneath the skin. It is commonly used to indicate the extent of diseases such as psoriasis, which is indicated by the appearance of red plaques. Although there is no cure for psoriasis, there are many treatment modalities to help control the disease. To evaluate treatment efficacy, the current gold standard method, PASI (Psoriasis Area and Severity Index), is used to determine severity of psoriasis lesion. Erythema (redness) is one parameter in PASI and this condition is assessed visually, thus leading to subjective and inconsistent results. Current methods or instruments that assess erythema have limitations, such as being able to measure erythema well for low pigmented skin (fair skin) but not for highly pigmented skin (dark skin) or vice versa. In this work, we proposed an objective assessment of psoriasis erythema for PASI scoring for different (low to highly pigmented) skin types. The colour of psoriasis lesions are initially obtained by using a chromameter giving the values L*, a*, and b* of CIELAB colour space. The L* value is used to classify skin into three categories: low, medium and highly pigmented skin. The lightness difference (DeltaL*), hue difference (Deltah(ab)), chroma (DeltaC*(ab)) between lesions and the surrounding normal skin are calculated and analysed. It is found that the erythema score of a lesion can be distinguished by their Deltah(ab) value within a particular skin type group. References of lesion with different scores are obtained from the selected lesions by two dermatologists. Results based on 38 lesions from 22 patients with various level of skin pigmentation show that PASI erythema score for different skin types i.e. low (fair skin) to highly pigmented (dark skin) skin types can be determined objectively and consistent with dermatology scoring.
Transnasal endoscopic placement of nasoenteric tubes (NETs) has been demonstrated to be useful in the critical care setting, with limited data on its role in non-critically ill patients. The authors collected data on consecutive patients from a non-critical care setting undergoing transnasal endoscopic NET placement. All NETs were endoscopically placed using a standard over-the-guidewire technique, and positions were confirmed with fluoroscopy. Patients were monitored until the removal of NETs or death. Twenty-two patients (median age = 62.5 years, 36.4% female) were referred for postpyloric feeding, with main indications of persistent gastrocutaneous fistula (n = 6), gastroparesis or gastric outlet obstruction (n = 5), duodenal stenosis (n = 6), acute pancreatitis (n = 4), and gastroesophageal reflux after surgery (n = 1). Postpyloric placement of NET was achieved in 19 of 22 (86.3%) patients, with 36.8% tube positions in the jejunum, 47.4% in the distal duodenum, and 15.8% in the second part of the duodenum. NET placement was least successful in cases with duodenal stenosis. NETs remained in situ for a median of 24 days (range, 2-94), with tube dislodgement (n = 3) and clogging (n = 5) as the main complications. NET feeding resulted in complete healing of gastrocutaneous fistulae in 5 of 6 patients and provision of total enteral nutrition in 3 of 4 cases of acute pancreatitis and 9 of 11 cases of gastroparesis or proximal duodenal obstruction. Transnasal endoscopy has a role in the placement of NET in non-critically ill patients requiring postpyloric feeding. However, there are some limitations, particularly in cases with altered duodenal anatomy.
The liquid-phase adsorption of phenol onto coconut shell-based activated carbon, CS850A was investigated for its equilibrium studies and kinetic modeling. Coconut shell was converted into high quality activated carbon through physiochemical activation at 850 degrees C under the influence of CO(2) flow. Beforehand, the coconut shell was carbonized at 700 degrees C and the resulted char was impregnated with KOH at 1:1 weight ratio. In order to evaluate the performance of CS850A, a series of batch adsorption experiments were conducted with initial phenol concentrations ranging from 100 to 500 mg l(-1), adsorbent loading of 0.2g and the adsorption process was maintained at 30+/-1 degrees C. The adsorption isotherms were in conformation to both Langmuir and Freundlich isotherm models. Chemical reaction was found to be a rate-controlling parameter to this phenol-CS850A batch adsorption system due to strong agreement with the pseudo-second-order kinetic model. Adsorption capacity for CS850A was found to be 205.8 mg g(-1).

The realization of Health Information Systems (HIS) requires rigorous evaluation that addresses technology, human and organization issues. Our review indicates that current evaluation methods evaluate different aspects of HIS and they can be improved upon. A new evaluation framework, human, organization and technology-fit (HOT-fit) was developed after having conducted a critical appraisal of the findings of existing HIS evaluation studies. HOT-fit builds on previous models of IS evaluation--in particular, the IS Success Model and the IT-Organization Fit Model. This paper introduces the new framework for HIS evaluation that incorporates comprehensive dimensions and measures of HIS and provides a technological, human and organizational fit.
