    BACKGROUND: The emergence of personalized medicine (PM) has raised some tensions in healthcare systems. PM is expensive and health budgets are constrained - efficient healthcare delivery is therefore critical. Notwithstanding the cost, many countries have started to adopt this novel technology, including resource-limited Southeast Asia (SEA) countries. This study aimed to describe the status of PM adoption in SEA, highlight the challenges and to propose strategies for future development.

    METHODS: The study included scoping review and key stakeholder interviews in four focus countries - Indonesia, Malaysia, Singapore, and Thailand. The current landscape of PM adoption was evaluated based on an assessment framework of six key themes - healthcare system, governance, access, awareness, implementation, and data. Six PM programs were evaluated for their financing and implementation mechanisms.

    RESULTS: The findings revealed SEA has progressed in adopting PM especially Singapore and Thailand. A regional pharmacogenomics research network has been established. However, PM policies and programs vary significantly. As most PM programs are champion-driven and the available funding is limited, the current PM distribution has the potential to widen existing health disparities. Low PM awareness in the society and the absence of political support with financial investment are fundamental barriers. There is a clear need to broaden opportunities for critical discourse about PM especially for policymakers. Multi-stakeholder, multi-country strategies need to be prioritized in order to leverage resources and expertise.

    CONCLUSIONS: Adopting PM remains in its infancy in SEA. To achieve an effective PM adoption, it is imperative to balance equity issues across diverse populations while improving efficiency in healthcare.

    Improving organizational performance is the main objective of any organization. Six
    Sigma is one of the most important approaches to improve performance and sustain
    competitive advantage. This article reviewed the literature related to organizational
    performance and explains the potential impact of Six Sigma and innovation culture on
    organizational performance. Given that previous studies have reported inconclusive
    results, this paper tries to establish a mechanism to explain the role of Six Sigma in
    improving organizational performance. Therefore, based on the theoretical foundation
    and comprehensive review of previous literature, a research framework is proposed.
    This proposed framework is grounded in the implementation of Six Sigma projects; it
    also establishes that innovation culture can help organizations to achieve success in a
    turbulent business environment.

    The advancement of technology has allowed a variety of tools for employees and
    managers, or knowledge workers, to choose as communication tools, especially for
    managing project teams. Instead of communicating and updating project progress
    through emails and short messaging system, social messaging (SM) applications, such
    as WhatsAppTM, are the current daily choice among them. The over-utilisation of the
    SM applications has given an opportunity for a research on project knowledge
    retention within project team, since it becomes an addiction to create new groups each
    time a new project team is formed. It is believed that project knowledge resides within
    these virtual groups, and this paper looks into how project knowledge is managed by
    analysing the communication patterns against a measurement metrics based on
    personal knowledge management (PKM) theory called GUSC Model. The
    communication patterns found existed in the SM application are qualitatively
    identified and mapped against the PKM processes within, with weighted score
    assigned to each process to quantitatively measure the overall project knowledge
    management participation. The differences among the organisational communication
    levels, namely upper and lower level communication levels, are analysed and the final
    result of participation expectation on member of SM application groups is presented.
    The outcome of this research recommends the future intervention on SM application
    usage and the application of knowledge management concept in current
    communication tool.

    The aim of this study is to predict the next day PM10 concentration using Bayesian Regression with noninformative
    prior and conjugate prior models. The descriptive analysis of PM10, temperature, relative humidity,
    nitrogen dioxide (NO2), sulphur dioxide (SO2), carbon monoxide (CO) and ozone (O3) are also included. A case
    study used two-years of air quality monitoring data at three (3) monitoring stations to predict the future PM10
    concentration with seven parameters (PM10, temperature, relative humidity, NO2, SO2, CO, and O3). The descriptive
    analysis showed that the highest mean PM10 concentration occurred at Klang station in 2011 (71.30 µg/m3
    ) followed
    by 2012 (68.82 µg/m3
    ). The highest mean PM10 concentration was at Nilai in 2012 (68.86 µg/m3
    ) followed by 2011
    (66.29µg/m3
    ) respectively. The results showed that the Bayesian regression model used a conjugate prior with a
    normal-gamma prior which was a good model to predict the PM10 concentration for most study stations with (R2 =
    0.67 at Jerantut station), (R2 = 0.61 at Nilai station) and (R2 = 0.66 at Klang station) respectively compared to a
    non-informative prior.

    The aluminium-silicon (Al-Si) based on Metal Matrix Composites (MMCs) is widely used in lightweight
    constructions and transport applications requiring a combination of high strength and ductility. A grain
    refinement plays a crucial role in improving characteristics and properties of Al alloys. In this investigation,
    titanium diboride (TiB2) and scandium (Sc) inoculants were added to the Al-Si alloys for grain refinement of
    an alloy. In this investigation, the corrosion resistance rate of Al-Si cast alloy reinforced by TiB2 and Sc were
    measured by potentiostat (AUTOLAB) instrument. The aim of this research is to investigate the corrosion
    rate for Al-Si-TiB2-Sc composites that immersed in different concentration of acidic solutions. Besides, the
    immersion time of acidic solutions also was investigated. All the samples were prepared accordingly for
    ASTM standard by the composition of 6.0 wt% TiB2 and 0.6wt% Sc. All the samples undergo cold mounting
    technique for easy handling on corrosion tests. Then the samples were immersed in two different
    concentrations acidic medium solutions, which were 0.1.and 1.0 M hydrochloric acids (HCl). The corrosion
    rate also was investigated for immersion samples of 1.0 M HCl for 21 days. From the results obtained, added
    TiB2 and Sc onto Al-Si alloy gave the better properties in corrosion resistance. Corrosion rates to reduce when
    the samples were immersed in a lower concentration of acidic medium, 0.1 HCl. However, there are some
    significant on the result but it still following the corrosion rates trend. Thus, improvements to reinforcement
    content need to be done in further research to cover the lack of this corrosion rates trend.

    There are various approaches to enhancing the catalytic properties of TiO₂, including modifying its morphology by altering the surface reactivity and surface area of the catalyst. In this study, the primary aim is to enhance the photocatalytic activity by changing the TiO₂ nanotubes' architecture. The highly ordered infrastructure is favorable for a better charge carrier transfer. It is well known that anodization affects TiO₂ nanotubes' structure by increasing the anodization duration which in turn influence the photocatalytic activity. The characterizations were conducted by FE-SEM (fiend emission scanning electron microscopy), XRD (X-ray diffraction), RAMAN (Raman spectroscopy), EDX (Energy dispersive X-ray spectroscopy), UV-Vis (Ultraviolet visible spectroscopy) and LCMS/MS/MS (liquid chromatography mass spectroscopy). We found that the morphological structure is affected by the anodization duration according to FE-SEM. The photocatalytic degradation shows a photodegradation rate of k = 0.0104 min-1. It is also found that a mineralization of Simazine by our prepared TiO₂ nanotubes leads to the formation of cyanuric acid. We propose three Simazine photodegradation pathways with several intermediates identified.

    BACKGROUND: Pinnatane A from the bark of Walsura pinnata was investigated for its anti-cancer properties by analyzing the cytotoxic activities and cell cycle arrest mechanism induced in two different liver cancer cell lines.

    METHODS: A 3-(4,5-Dimethyl-2-thiazolyl)-2,5-diphenyl-2H-tetrazolium bromide (MTT) assay was used to analyze the pinnatane A selectivity in inducing cell death in cancer and normal cells. Various biological assays were carried out to analyze the anti-cancer properties of pinnatane A, such as a live/dead assay for cell death microscopic visualization, cell cycle analysis using propidium iodide (PI) to identify the cell cycle arrest phase, annexin V-fluorescein isothiocyanate (annexin V-FITC)/PI flow cytometry assay to measure percentage of cell populations at different stages of apoptosis and necrosis, and DNA fragmentation assay to verify the late stage of apoptosis.

    RESULTS: The MTT assay identified pinnatane A prominent dose- and time-dependent cytotoxicity effects in Hep3B and HepG2 cells, with minimal effect on normal cells. The live/dead assay showed significant cell death, while cell cycle analysis showed arrest at the G₀/G₁ phase in both cell lines. Annexin V-FITC/PI flow cytometry and DNA fragmentation assays identified apoptotic cell death in Hep3B and necrotic cell death in HepG2 cell lines.

    CONCLUSIONS: Pinnatane A has the potential for further development as a chemotherapeutic agent prominently against human liver cells.

    The present study aimed to report the prevalence of anaemia and iron deficiency (ID) and to explore the associations among socio-demographic characteristics, nutritional status and inflammation status in the occurrence of anaemia and ID in a nationally representative sample of Malaysian primary schoolchildren. Using data from the South East Asian Nutrition Surveys (SEANUTS), 544 Malaysian children aged 7 to 12 years were included in this secondary analysis. Blood samples were drawn for haemoglobin and serum ferritin analysis while C-reactive protein (CRP) and α-1-acid glycoprotein (AGP) were measured to detect inflammation. Prevalence of anaemia and ID were 4.0% and 5.2%, respectively. There were significantly more anaemic indigenous bumiputra children (9.9%) than Chinese children (0.6%). Correction for inflammation did not change the prevalence of ID. More overweight/obese children than thin/normal weight children were found to have elevated acute phase protein (APP). Children with elevated inflammatory markers had significantly higher ferritin level than children without inflammation. Periodic health assessments of anaemia and ID at the population level to monitor and clarify the epidemiology of health problems are required to inform public health policies and strategies.

    Background: The irrational use, "over the counter supply", and unregulated supply chains of antimicrobials are contributing toward antimicrobial resistance. Antimicrobial stewardship programs regulate antimicrobials usage to prevent resistance and reduce health care burden. Objective: To assess the knowledge and practice of pharmacists' working in various healthcare settings toward antimicrobial stewardship in Pakistan. Method: A cross-sectional study was conducted among pharmacists working in different sectors between March to June 2017. Results: A total of 181 pharmacists participated, of whom (n = 145, 80.1%) were males. The majority of participants were in the 20⁻30 age group (n = 147, 81.2%) and hold Doctor of Pharmacy degrees. More than 80% of pharmacists agreed that "antimicrobial stewardship is essential to improve patient care"; while (n = 159, 87.8%) pharmacists agreed that "pharmacist should be trained on the use of antimicrobial". Close to 90% of pharmacists agreed that "adequate training should be provided to pharmacists on antimicrobial use". Regarding the practice of antimicrobial stewardship, (n = 72, 39.8%) pharmacists often/always "make efforts to prevent or reduce the transmission of infections within the community"; (n = 58, 32%) pharmacists never "dispense antimicrobials without a prescription"; and (n = 60, 32%) pharmacist often/always "communicate with prescribers if unsure about the appropriateness of an antibiotic prescription". Conclusions: Increased antimicrobial stewardship efforts can both optimize the treatment of infections and reduce adverse events associated with antibiotic use. Pharmacists in Pakistan have good knowledge and adopt positive practices toward antimicrobial stewardship. Pharmacist and other health care professionals should collaborate within multi-disciplinary teams to reduce the problem of antimicrobial resistance and improve the quality of life of patients.

    Magnetic iron oxide nanoparticles are among the most useful metal nanoparticles in biomedical applications. A previous study had confirmed that phytic acid-chitosan-iron oxide nanocomposite (Phy-CS-MNP) exhibited antiproliferative activity towards human colorectal cancer (HT-29) cells. Hence, in this work, we explored the in vitro cytotoxicity activity and mechanistic action of Phy-CS-MNP nanocomposite in modulating gene and protein expression profiles in HT-29 cell lines. Cell cycle arrest and apoptosis were evaluated by NovoCyte Flow Cytometer. The mRNA changes (cyclin-dependent kinase 4 (Cdk4), vascular endothelial growth factor A (VEGFA), c-Jun N-terminal kinase 1 (JNK1), inducible nitric oxide synthase (iNOS), and matrix metallopeptidase 9 (MMP9)) and protein expression (nuclear factor-kappa B (NF-κB) and cytochrome c) were assessed by quantitative real-time polymerase chain reaction (PCR) and western blotting, respectively. The data from our study demonstrated that treatment with Phy-CS-MNP nanocomposite triggered apoptosis and G₀/G₁ cell cycle arrest. The transcriptional activity of JNK1 and iNOS was upregulated after treatment with 90 μg/mL Phy-CS-MNP nanocomposite. Our results suggested that Phy-CS-MNP nanocomposite induced apoptosis and cell cycle arrest via an intrinsic mitochondrial pathway through modulation of Bax and Bcl-2 and the release of cytochrome c from the mitochondria into the cytosol.

    Objectives: The effects of water and 50% ethanolic-water extracts of Orthosiphon stamineus Benth (OS) on cell proliferation and apoptotic activity against uterine leiomyosarcoma (SK-UT-1) cells were investigated. Methods: Anti-proliferation effect was evaluated through cell cycle analysis whereas apoptotic activity was determined via screening and quantifying using fluorescence microscopy and flow cytometric analysis, respectively. The effect of extracts on molecular mechanism was studied using real-time reverse transcription polymerase chain reaction and Western blotting. Results: Cell cycle flow cytometric analysis showed the induction of cell cycle arrests were behaves in a p53-independent manner. The examination using fluorescence microscopy and Annexin V flow cytometry revealed the presence of morphological features of apoptotic bodies. Downregulation of anti-apoptotic gene (Bcl-2) supports the apoptotic activity of OS extracts although poorly induce PARP-1 cleavage in Western blot analysis. The extracts also inhibit the SK-UT-1 growth by suppressing VEGF-A, TGF-β1 and PCNA genes, which involved in angiogenesis and cell proliferation. Conclusion: This study demonstrates that O. stamineus extracts are able to inhibit proliferation and induced apoptosis of uterine fibroid cells and is worth further investigation.

    Background: Cervical cancer is currently the third most common female cancer in Malaysia , with the human
    papillomavirus (HPV) considered as one of the important contributory factors. This study was conducted to determine
    HPV prevalence, its genotype distribution, and other potential risk factors among women in Kota Kinabalu, Sabah
    in order to evaluate the likely efficacy of current HPV vaccines in the local population. Methods: A total of 240
    cervical samples were collected and subjected to DNA extraction, PCR amplification using the MY09/MY11 primer
    pair, and restriction fragment length polymorphism (RFLP) for HPV detection and genotyping. Sociodemographic,
    clinical, and behavioural data were also collected via questionnaires. Results: The prevalence of HPV infection was
    9.6%. The most common HPVs among 13 genotypes were high-risk HPV-56 (16.7%) and probable high-risk HPV-70
    (16.7%) followed by HPV-16, -58, -53, -61, -33, -59, and -66 (in decreasing order of prevalence) including the rare
    genotypes: HPV-62, -81, -82 and -84. Statistical analyses using logistic regression models showed that HPV infection
    was significantly associated with employment (OR 4.94; CI 1.58-15.40) and education at secondary/high school level
    (OR 0.13; CI 0.03-0.62). Conclusion: Distribution of HPV genotypes in Sabah indicated a high prevalence of HPV-56
    and -70 which are among the rare HPV types in West Malaysia and merit consideration in future strategies for HPV
    vaccination specifically for local Sabahan women.

    The incidence of diabetes mellitus (DM) is increasing worldwide. One of the main complications in DM is delayed wound healing which often requires amputation. Various drugs have been used to treat DM but they present with various complications and patients often do not comply with such treatment. This opens the door for complementary and alternative medicine. In the present review, we explore the molecular concept of wound healing occurring in different stages with special emphasis to DM. We also highlight potential herbal products such as NF3 (Chinese 2-Herb Formula), Zicao, Jing Wan Hong ointment, mixture of Adiantum capillus-veneris, Commiphora molmol, Aloe Vera, and henna, Aleo vera, Phenol-rich compound sweet gel, Jinchuang ointment, San-huang-sheng-fu (S) oil, Yi Bu A Jie extract, Astragali Radix (AR) and Rehmanniae Radix (RR), Yiqi Huayu, Tangzu yuyang ointment, Shengji Huayu recipe, Angelica sinensis, Lithospermun erythrorhison, Hippophae rhamnoides L., Curcuma longa, and Momordica charantia that could be effectively used to treat DM wounds. Future clinical trials are needed for designing potential drugs which may be effective in treating DM wounds.

    Objective: The aim of this study was to evaluate the psychometric properties of the Malay translated version of the Theory of Planned Behavior (TPB) intention to quit smoking questionnaire.
    Methods: A cross-sectional study was performed involving 185 male smokers. The forward-backward translation procedure was adopted to translate the questionnaire from English to Malay. The internal consistency and stability were assessed using Cronbach’s alpha and a correlation analysis and Exploratory Factor Analysis was conducted.
    Result: The translated questionnaire showed good internal consistency with Cronbach’s alpha values of 0.86, 0.64, 0.74 and 0.90 for each of the four respective factors. The test-retest reliability revealed acceptable stability, with Spearman’s correlation coefficients ranging from low to moderate (r>0.30-0.50) and a satisfactory inter class correlation coefficient (ICCs). The construct validity achieved an acceptable factor loading for each construct which ranged from 0.40 to 0.90.
    Conclusion: The current study provided
    psychometric evidence for an appropriate, reliable and valid tool of TPB Malay version. This questionnaire could be applied in evaluating smoking cessation interventions in Malaysia.

    Acute bacterial meningitis is not an uncommon central nervous system infection. In severe cases, it can be associated with various neurological or systemic complications. However, acute spinal cord dysfunction rarely occurs. We report a case of bacterial meningitis complicated with spinal cord infarction despite adequate treatment with antibiotics and corticosteroid therapy. He had residual paraplegia and was fully dependent in the activity of daily living.

    Nanoparticle science is rapidly changing the landscape of various scientific fields and defining new technological platforms. This is perhaps even more evident in the field of nanomedicine whereby nanoparticles have been used as a tool for the treatment and diagnosis of many diseases. However, despite the tremendous benefit conferred, common pitfalls of this technology is its potential short and long-term effects on the human body. To understand these issues, many scientific studies have been carried out. This review attempts to shed light on some of these studies and its outcomes. The topics that were examined in this review include the different possible uptake pathways of nanoparticles and intracellular trafficking routes. Additionally, the effect of physicochemical properties of nanoparticle such as size, shape, charge and surface chemistry in determining the mechanism of uptake and biological function of nanoparticles are also addressed.

    Background: The aim of this study was to explore reasons for delayed health-seeking for late stage oral cancer patients. Methods: Semi-structured in-depth interviews were conducted with 35 oral cancer patients with TNM stage III to IV disease, who were treated at six tertiary regional centres managing oral cancer throughout Malaysia. Interviews were audio-recorded, transcribed verbatim, coded using NVivo (version 10.0) qualitative software and analysed using framework analysis. Results: Participants interpreted their early symptoms as a minor condition and did not consider it as requiring immediate attention. Four types of coping strategies causing delayed help-seeking emerged: 1) self-remedy 2) self-medication 3) seeking traditional healers and 4) consulting general medical practitioners (GPs) instead of dentists. Socio-economic factors, cultural beliefs and religious practices have some influence on diagnostic delay. Conclusion: Low levels of public knowledge and awareness regarding early signs and symptoms of oral cancer as well as GPs’ misdiagnosis of early lesions results in delayed diagnosis.

    Intentions form the basis of behavioral action to improve animal welfare; however an intention-behavior gap has been previously identified. Livestock stakeholders in China, Malaysia, Vietnam and Thailand (n = 1041) involved in slaughter and transport completed a survey in which they were asked their level of intention to improve animal welfare, and their level of confidence in their ability to do this. Chinese respondents had the most confidence in their ability to improve animal welfare, and veterinarians showed more confidence than livestock team leaders. Those with high or low intentions, and either high or low confidences were compared for key influencing factors to identify the circumstances that may be most conducive to behavior change. Respondents with high intentions and low confidence in their ability to improve animal welfare identified extrinsic factors associated with their immediate workplace and different company priorities, and the intrinsic factor of lack of personal knowledge. It is concluded that targeting these areas to improve confidence in stakeholders in livestock transport and slaughter could bring the most improvements in animal welfare initiatives.

    Three new bisindole alkaloids, bisleuconothines B-D (1-3), were isolated from the bark of Leuconotis griffithii. Their structures were elucidated by 1D and 2D NMR spectroscopy and DFT calculations. Bisleuconothine B (1) is the first monoterpene indole alkaloid dimer featuring bridges between both C-16-C-10' and C-2-O-C-9'. All compounds were deemed noncytotoxic (IC50 > 10 μM) when tested against A549 human lung adenocarcinoma cells.

    BACKGROUND: Bioactive proteins from milk fat globule membrane (MFGM) play extensive roles in cellular processes and defense mechanisms in infants. The aims of this study were to identify differences in protein compositions in human and caprine MFGM using proteomics and evaluate possible nutritional benefits of caprine milk toward an infant's growth, as an alternative when breastfeeding or human milk administration is not possible or inadequate.

    MATERIALS AND METHODS: Human and caprine MFGM proteins were isolated and analyzed, initially by polyacrylamide gel electrophoresis, and subsequently by quadrupole time-of-flight liquid chromatography-mass spectrometry. This was then followed by database search and gene ontology analysis. In general, this method selectively analyzed the abundantly expressed proteins in milk MFGM.

    RESULTS: Human MFGM contains relatively more abundant bioactive proteins compared with caprine. While a total of 128 abundant proteins were detected in the human MFGM, only 42 were found in that of the caprine. Seven of the bioactive proteins were apparently found to coexist in both human and caprine MFGM.

    RESULTS/DISCUSSION: Among the commonly detected MFGM proteins, lactotransferrin, beta-casein, lipoprotein lipase, fatty acid synthase, and butyrophilin subfamily 1 member A1 were highly expressed in human MFGM. On the other hand, alpha-S1-casein and EGF factor 8 protein, which are also nutritionally beneficial, were found in abundance in caprine MFGM. The large number of human MFGM abundant proteins that were generally lacking in caprine appeared to mainly support human metabolic and developmental processes.

    CONCLUSION: Our data demonstrated superiority of human MFGM by having more than one hundred nutritionally beneficial and abundantly expressed proteins, which are clearly lacking in caprine MFGM. The minor similarity in the abundantly expressed bioactive proteins in caprine MFGM, which was detected further, suggests that it is still nutritionally beneficial, and therefore should be included when caprine milk-based formula is used as an alternative.
