    Phenothiazines have been associated with various ocular adverse effects, in particular cataract and corneal deposits. Several effects are vision threatening. Awareness of these effects, in particular by Psychiatrists and Ophthalmologist, is essential for prompt diagnosis and appropriate treatment. We report two patients presented to us with corneal deposits and lenticular opacities following long term use of Phenothiazines.

    INTRODUCTION: The aim of this study was to compare the visual outcomes of phacoemulsification with intraocular lens implantation (IOL) surgery in patients with and without diabetes mellitus (DM) in Malaysia over a 12-year period and to identify factors that may contribute to poor visual outcome.

    MATERIALS AND METHODS: Data was retrieved from the webbased Malaysian Cataract Surgery Registry (CSR). Perioperative data for cataract surgery performed from 2007- 2018 were analysed. Inclusion criteria were age ≥40 years, phacoemulsification and IOL and senile cataract. Combined surgeries, surgeries performed by trainees and ocular comorbidities were excluded. Post-operative Best-Corrected Visual Acuity (BCVA) were compared. Factors affecting poor visual outcomes among those with DM were analysed using multivariate logistic regression to produce adjusted odds ratio (OR) for variables of interest.

    RESULTS: Total number of cases between 2007-2018 was 442,858, of whom 179,210 qualified for our analysis. DM group consisted of 72,087 cases (40.2%). There were 94.5% cases in DM group and 95.0Ź from non-DM group who achieved BCVA ≥6/12 (p<0.001). Among patients with DM, advanced age (70-79 years old, OR: 2.54, 95% Confidence Interva, 95%CI: 1.91, 3.40; 80-89 years old, OR: 5.50, 95%CI: 4.02, 7.51), ≥90 years, OR: 9.77, 95%CI: 4.18, 22.81), poor preoperative presenting visual acuity [<6/18-6/60] (OR: 2.40, 95%CI: 1.84, 3.14) and <6/60-3/60 (OR: 3.00, 95%CI: 2.24, 4.02), <3/60 (OR 3.63, 95%CI: 2.77, 4.74)], presence of intraoperative complication (OR 2.24, 95%CI: 1.86, 2.71) and presence of postoperative complication (OR 5.21, 95%CI: 2.97, 9.16) were significant factors for poor visual outcome.

    CONCLUSIONS: Visual outcomes following phacoemulsification with IOL implantation surgery among cases with DM were poorer compared to cases without DM. Risk factors for poor visual outcomes among cases with DM were identified.

    OBJECTIVE: Cataract is the leading cause of blindness in Malaysia. There is an alarming backlog of cataract extraction surgery as the majority believes they did not require surgery. This study aimed to explore the barriers at the primary care level to cataract surgery from the perspective of patients with severe cataract blindness.

    METHODS: Eleven participants were involved in this qualitative research which utilised the interpretative phenomenological analysis approach more renowned in health psychology research. All interviews conducted at their home. The interviews were recorded, typed verbatim, and the transcripts were analysed using NVivo software version 8.0.

    RESULTS: The main barriers identified at the primary care level were 1) nondisclosure of their visual problems originated from their belated needs for better sight, delayed awareness of their visual status and social stigma and 2) patient-provider-related issues namely miscommunication and delayed referral. The first main theme explains their belief for not requiring surgery. This has led to their delayed awareness and impeded disclosure of their visual problems to family members or primary care providers. The second main theme reflects the provider-patient-related issues which retarded cataract detection and referral process required for earlier cataract extraction surgery.

    CONCLUSION: Thus, the appropriate approach targeting these specific barriers at primary care level will be able to detect, motivate and assist patients for early uptake of cataract extraction surgery to improve their vision and prevent severe blindness.

    Reports of work change and transitions are common amongst individuals with chronic illnesses such as multiple sclerosis (MS). However, there is little research on the lived experience of these work transitions. The scarcity of this research is particularly evident within low-and-middle-income countries, where protection laws and resources such as anti-discrimination laws and reasonable work modifications may not exist or be well enforced. In this paper, we explore how and why individuals with MS seek and achieve work transitions in the structural context of Malaysia. We interviewed ten working individuals with MS (July-december 2015) using a joint hermeneutic phenomenology and constructivist grounded theory approach. Using a broad conceptual lens of 'sustainable careers', we examine their careers as a series of experiences, decisions, and events, paying attention to the influences of context, time, their personal levels of agency and sense of meaning. Participants described work transitions as early as within the first year of diagnosis, that were prompted by voluntary, involuntary and semi-voluntary reasons. Key aspects of the process of seeking new roles included an exploration of alternative roles and paths, and then acquiring, trialing/adapting and remaining engaged in their new roles. Participants identified the perception and experience of 'being unemployable', based on how their diagnosis and short-term symptoms were responded to by employers. Nevertheless, participants used various strategies and career resources to obtain and maintain meaningful work roles. However, success in obtaining or maintaining new roles were not equally achieved. This research draws attention to the cumulative economic disadvantage of a chronic illness diagnosis, even at milder and episodic stages. Furthermore, it reiterates the need for cohesive structural protection in low-and-middle-income countries to facilitate a more equal ability to remain economically resilient and capable of engaging in meaningful long-term careers when living with a chronic illness.

    Narrow beam width, higher gain and multibeam characteristics are demanded in 5G technology. Array antennas that are utilized in the existing mobile base stations have many drawbacks when operating at upper 5G frequency bands. For example, due to the high frequency operation, the antenna elements become smaller and thus, in order to provide higher gain, more antenna elements and arrays are required, which will cause the feeding network design to be more complex. The lens antenna is one of the potential candidates to replace the current structure in mobile base station. Therefore, a negative refractive index shaped lens is proposed to provide high gain and narrow beamwidth using energy conservation and Abbe's sine principle. The aim of this study is to investigate the multibeam characteristics of a negative refractive index shaped lens in mobile base station applications. In this paper, the feed positions for the multibeam are selected on the circle from the center of the lens and the accuracy of the feed position is validated through Electromagnetic (EM) simulation. Based on the analysis performed in this study, a negative refractive index shaped lens with a smaller radius and slender lens than the conventional lens is designed, with the additional capability of performing wide-angle beam scanning.

    In the 5G mobile system, new features such as millimetre wave operation, small cell size and multi beam are requested at base stations. At millimetre wave, the base station antennas become very small in size, which is about 30 cm; thus, dielectric lens antennas that have excellent multi beam radiation pattern performance are suitable candidates. For base station application, the lens antennas with small thickness and small curvature are requested for light weight and ease of installation. In this paper, a new lens shaping method for thin and small lens curvature is proposed. In order to develop the thin lens antenna, comparisons of antenna structures with conventional aperture distribution lens and Abbe's sine lens are made. Moreover, multi beam radiation pattern of three types of lenses are compared. As a result, the thin and small curvature of the proposed lens and an excellent multi beam radiation pattern are ensured.

    There is growing interest in the use of plant bioresources for managing type 2 diabetes. In this study, Rhodamnia cinerea, which is used traditionally to manage diseases in Malaysia, was explored for its antidiabetic effects. Type 2 diabetic rats were managed for 4 weeks using aqueous extract of R. cinerea or quercetin. Weights and fasting glucose were measured weekly, while serum lipid profiles, insulin, antioxidant status, urea, creatinine and liver enzymes were assayed at the end. Sorbitol contents, antioxidant capacities and aldose reductase activities of the kidney, lens and sciatic nerve were also assessed. The results showed that the aqueous extract of R. Cinerea mainly contained Myricitrin and it reduced glycemia (p>0.05), lipid profiles (p<0.05), F2-isoprostanes (p<0.05) and overall metabolic condition of type 2 diabetic rats. R. cinerea also attenuated sorbitol contents of the nerve (p<0.05) and kidney (p<0.05), partly through regulating the activity of aldose reductase (p<0.05 for nerve) and sorbitol dehydrogenase (p<0.05 for kidney) in comparison with diabetic untreated group. Quercetin is a known aldose reductase inhibitor and can improve several metabolic indices related to Type 2 diabetes. In this study, the results of R. cinerea were comparable to or better than those of quercetin, suggesting that R. cinerea extract can be a good candidate for managing Type 2 diabetes and its complications related to sorbitol accumulation.

    A cross-sectional study was undertaken to determine the refractive and biometric status of premature children without Retinopathy of Prematurity (ROP) and full term children. Fifty eight children between the ages of 3 and 7 years (32 children born premature without ROP and another 26 children born full term and normal) were examined. Refractive error, corneal curvature, axial length, anterior chamber depth and crystalline lens thickness were determined. The results revealed that children between the age of 3 and 7 years were emmetropic, irrespective of whether they were born premature without ROP or full term. However, children born premature without ROP had significantly steeper corneas (t = 3.14, p = 0.0349), shorter axial lengths (t = 3.18, p = 0.0313) and thicker crystalline lens (t = 3.31, p = 0.0256) compared to children born full term within the same age group. This study suggests that compensation in ocular parameters can occur to maintain emmetropia, mainly by adjustment of axial length and corneal curvature.

    The objective of this study is to estimate the annual effective dose for cardiologists and nurses by measuring Hp(10) and Hp(0.07) during cardiac catheterization procedures. A total of 16 staffs members were working in interventional cardiology during 1 year at a tertiary hospital. The occupational dose was measured using calibrated thermo-luminescent dosemeters (TLD-100, LiF:Mg,Ti). The overall mean and range of the annual Hp(10) and Hp(0.07) (mSv) for cardiologists were 3.7 (0.13-14.5) and 3.2 (0.21-14.7), respectively. Cardiologists were frequently exposed to higher doses compared with nurses and technologists. The exposure showed wide variations, which depend on occupation and workload. Staff is adhered to radiation protection guidelines regarding shielding the trunk, thyroid shield, thus appropriately protected. Lens dose measurement is recommended to ensure that dose limit is not exceeded.

    Cataract, a leading cause of blindness, is of special concern in diabetics as it occurs at earlier onset. Polyol accumulation and increased oxidative-nitrosative stress in cataractogenesis are associated with NFκB activation, iNOS expression, ATP depletion, loss of ATPase functions, calpain activation and proteolysis of soluble to insoluble proteins. Tocotrienol was previously shown to reduce lens oxidative stress and inhibit cataractogenesis in galactose-fed rats. In current study, we investigated anticataract effects of topical tocotrienol and possible mechanisms involved in streptozotocin-induced diabetic rats. Diabetes was induced in Sprague Dawley rats by intraperitoneal injection of streptozotocin. Diabetic rats were treated with vehicle (DV) or tocotrienol (DT). A third group consists of normal, non-diabetic rats were treated with vehicle (NV). All treatments were given topically, bilaterally, twice daily for 8 weeks with weekly slit lamp monitoring. Subsequently, rats were euthanized and lenses were subjected to estimation of polyol accumulation, oxidative-nitrosative stress, NFκB activation, iNOS expression, ATP levels, ATPase activities, calpain activity and total protein levels. Cataract progression was delayed from the fifth week onwards in DT with lower mean of cataract stages compared to DV group (p<0.01) despite persistent hyperglycemia. Reduced cataractogenesis in DT group was accompanied with lower aldose reductase activity and sorbitol level compared to DV group (p<0.01). DT group also showed reduced NFκB activation, lower iNOS expression and reduced oxidative-nitrosative stress compared to DV group. Lenticular ATP and ATPase and calpain 2 activities in DT group were restored to normal. Consequently, soluble to insoluble protein ratio in DT group was higher compared to DV (p<0.05). In conclusion, preventive effect of topical tocotrienol on development of cataract in STZ-induced diabetic rats could be attributed to reduced lens aldose reductase activity, polyol levels and oxidative-nitrosative stress. These effects of tocotrienol invlove reduced NFκB activation, lower iNOS expression, restoration of ATP level, ATPase activities, calpain activity and lens protein levels.

    OBJECTIVE: This study measured the radiation exposure of the eye lens of medical personnel performing fluoroscopy and interventional procedures at the Sarawak General Hospital in Kuching, Sarawak, Malaysia. This study was the first in Malaysia to utilise in vivo radiation measurement relatively near the eye lens.

    METHODS: 41 medical personnel performing 79 procedures were monitored for their eye lens exposure using the NanoDot™ optically-stimulated luminescence dosimeters (OSLD) taped to the outer canthus of their eyes. The air-kerma area product (KAP), fluoroscopy time (FT) and number of procedure runs were also recorded.

    RESULTS: KAP, FT and number of runs were strongly correlated. However, only weak to moderate correlations were observed between these parameters with the measured eye lens doses. The average median equivalent eye lens dose was 0.052 mSv (ranging from 0.0155 to 0.672 mSv). The eye lens doses of primary operators were found to be significantly higher than their assistants due to the closer proximity to the patient and X-ray tube. The left eye lens of the operators received the highest amount of radiation due to their habitual positioning towards the radiation source.

    CONCLUSION: KAP and FT were not useful in predicting the equivalent eye lens dose exposure in interventional radiological procedures. Direct in vivo measurements were needed to provide a better estimate of the eye lens doses received by medical personnel during these procedures. This study highlights the importance of using direct measurement, such as OSLDs, instead of just indirect factors to monitor dose in the eye lens in radiological procedures.

    The present conventional (destructive) method used in determining LAI is laborious, difficult and time consuming. Thus, an image-based measurement using camera system with fish eye lens offers an alternative means for an accurate indirect measurement of LAI in oil palm. In this study, a methodology was developed to improve the leaf area index of the oil palm determination using hemispherical photography as an indirect method. A set of true LAI data, collected using the destructive method, were used as a reference to calibrate the LAI measurements obtained by the hemispherical photography. A good relationship (r = 0.85) was found between age of palm and hemispherical photographic LAI. However, the estimated LAI obtained by the hemispherical photographic method was underestimated as compared to the destructive method. Some means of calibration was necessary to determine the relationship between the actual LAI and the hemispherical photographic LAI. It was necessary to multiply the LAI value from 5 years to 16 years, by a clumping factor of 2.14 for 5 to 9 year old palms, 2.33 for 10 to 14-year old palms and 2.37 for above 15- year old palms to calculate the accurate LAI values. For palms which are less than 5 year old (i.e. 2 to 3 years in this study), the photography LAI value was equal to the calculated LAI value. This proposed that correction factors would solve this underestimation effect. In addition, two equations were also proposed to estimate the true LAI from the Photographic LAI for immature and mature oil palm plantation.

    PURPOSE: Increased lenticular oxidative stress and altered calcium/magnesium (Ca/Mg) homeostasis underlie cataractogenesis. We developed a liposomal formulation of magnesium taurate (MgT) and studied its effects on Ca/Mg homeostasis and lenticular oxidative and nitrosative stress in galactose-fed rats.

    METHODS: The galactose-fed rats were topically treated with liposomal MgT (LMgT), liposomal taurine (LTau), or corresponding vehicles twice daily for 28 days with weekly anterior segment imaging. At the end of the experimental period, the lenses were removed and subjected to analysis for oxidative and nitrosative stress, Ca and Mg levels, ATP content, Ca(2+)-ATPase, Na(+),K(+)-ATPase, and calpain II activities.

    RESULTS: The LTau and LMgT groups showed significantly lower opacity index values at all time points compared to the corresponding vehicle groups (p<0.001). However, the opacity index in the LMgT group was lower than that in the LTau group (p<0.05). Significantly reduced oxidative and nitrosative stress was observed in the LTau and LMgT groups. The lens Ca/Mg ratio in LMgT group was decreased by 1.15 times compared to that in the LVh group. Calpain II activity in the LMgT group was decreased by 13% compared to the LVh group. The ATP level and Na(+),K(+)-ATPase and Ca(2+)-ATPase activities were significantly increased in the LMgT group compared to the LVh group (p<0.05).

    CONCLUSIONS: Topical liposomal MgT delays cataractogenesis in galactose-fed rats by maintaining the lens mineral homeostasis and reducing lenticular oxidative and nitrosative stress.

    Oxidative and nitrosative stress underlies cataractogenesis, and therefore, various antioxidants have been investigated for anticataract properties. Several vitamin E analogs have also been studied for anticataract effects due to their antioxidant properties; however, the anticataract properties of tocotrienols have not been investigated. In this study, we investigated the effects of topically applied tocotrienol on the onset and progression of cataract and lenticular oxidative and nitrosative stress in galactosemic rats.

    We report the case series of phacoemulsification-related Descemet membrane detachment (DMD) encountered at a tertiary hospital in Kuala Lumpur. Case 1 was an iatrogenic DMD which was detected intraoperatively and managed early with good outcome. Case 2 and case 3 described unusual presentation of DMD which was initially undiagnosed. This report highlights the use of anterior segment optical coherence tomography (ASOCT) in detecting and confirming the correct diagnosis for DMD. With the aid of an experienced corneal specialist, the higher threshold for suspicious occurrence of DMD was confirmed using ASOCT. Treatment was tailored accordingly, with successful clearance of corneal oedema and visual recovery. This case series highlighted the importance of proper operative documentation and high threshold for suspicion for DMD in focal corneal oedema following an otherwise uneventful cataract surgery. It is concluded that ASOCT is an excellent tool to confirm diagnosis of DMD and success of treatment.

    Background: A randomised single blinded clinical trial comparing the effectiveness of two methods of cataract surgery with intraocular lens implantation: extracapsular cataract extraction (ECCE) and phacoemulsification (PEA) was carried out at Hospital Universiti Kebangsaan Malaysia (HUKM) between March 2000 and August 2001. Methods: The effectiveness of cataract surgery was assessed from the quality of life specifically for vision via Visual Function 14 (VF-14) preoperatively, one week, two months and six months after surgery. Results: The result showed that there was a significant increased in VF-14 scores after a week, two months and six months postoperation compared to the score before surgery for both techniques. However there was no significant difference in VF-14 scores when compared between ECCE and PEA. Conclusions: This study indicated that both techniques give equal benefit to cataract patients. Since effectiveness of cataract surgery with intraocular lens implantation is unrelated to operative procedures, less costly technique should be promoted.

    Senile cataract is the most common cause of bilateral blindness and results from the loss of transparency of the lens. Maintenance of the unique tissue architecture of the lens is vital for keeping the lens transparent. Membrane transport mechanisms utilizing several magnesium (Mg)-dependent ATPases, play an important role in maintaining lens homeostasis. Therefore, in Mg-deficiency states, ATPase dysfunctions lead to intracellular depletion of K(+) and accumulation of Na(+) and Ca(2+). High intracellular Ca(2+) causes activation of the enzyme calpain II, which leads to the denaturation of crystallin, the soluble lens protein required for maintaining the transparency of the lens. Mg deficiency also interferes with ATPase functions by causing cellular ATP depletion. Furthermore, Mg deficiency enhances lenticular oxidative stress by increased production of free radicals and depletion of antioxidant defenses. Therefore, Mg supplementation may be of therapeutic value in preventing the onset and progression of cataracts in conditions associated with Mg deficiency.

    PURPOSE: The purpose of the study was to establish baseline data for amplitude of accommodation (AA) measured using both subjective and objective techniques in students at a private Malaysian university.

    METHODS: This cross-sectional study was conducted including 34 healthy participants with a mean age of 22.26 ± 1.88 years. AA was measured using dynamic retinoscopy and the push-up, pull-away, modified push-up, and minus-lens techniques.

    RESULTS: The mean AA scores for the push-up, pull-away, minus-lens, and modified push-up techniques and dynamic retinoscopy were 11.38 ± 2.03, 10.35 ± 1.64, 9.24 ± 1.18, 8.26 ± 1.44, and 7.2 ± 1.0 diopters, respectively. No AA measurements showed significant difference among ethnicities (Chinese, Malay, and Indian). This study suggested that AA obtained using push-up (p = 0.005) and pull-away (p = 0.017) methods and dynamic retinoscopy (p = 0.041) were significantly different according to sex. No significant difference was observed in AA for the minus-lens (p = 0.051) and modified push-up (p = 0.216) techniques by sex. A moderately negative correlation was found between AA and age for the push-up (r = -0.434, p = 0.010), pull-away (r = -0.412, p = 0.016), and minus-lens (r = -0.509, p = 0.002) techniques and dynamic retinoscopy (r = -0.497, p = 0.003). A weak negative correlation was found between age and AA measured using a modified push-up technique (r = -0.393, p = 0.022).

    CONCLUSIONS: Mean AA was highest for the push-up technique, followed by the pull-away technique, the minus-lens technique, the modified push up technique, and dynamic retinoscopy. The push-up and pull-away methods and dynamic retinoscopy showed a significant difference in measurement of AA between sexes.
