    Urinary incontinence (UI) is common in women in postmenopausal age, but many women delay seeking treatment. The objective of this study is to determine health-seeking behavior of postmenopausal women with UI and its associated factors. This is a cross-sectional study involving 348 postmenopausal women from the outpatient clinic using personal data forms and a validated UI questionnaire. A total of 348 menopausal women were involved in the study. Only 13.17% of patients with UI sought treatment. Factors associated with seeking treatment were age of menopause and severity of UI. More health education needs to be done among these patients.

    Introduction: This study aimed to determine the relationship of maternal dietary
    intake with human milk nutritional composition, among Malay mothers during the
    postpartum period of exclusive breastfeeding. Methods: Human milk samples (20-
    30ml) were collected from mothers (n=32) at least once monthly for six months
    postpartum. Macronutrients and fatty acids contents were determined using
    proximate analysis and gas chromatography methods, respectively. Maternal
    dietary intakes were recorded using the multiple-pass diet recall method prior
    to each milk sampling and were analysed using the Nutritionist ProTM software.
    Associations between the milk composition and maternal diet were tested using
    Spearman correlation. Results: The energy content ranged between 49.6-59.2
    kcal/100ml, protein 1.3-1.4 g/100ml, carbohydrate 6.5-9.7 g/100ml and total
    fat 6.5-9.7 g/100ml. The polyunsaturated, monounsaturated, and saturated fatty
    acids concentrations were 10.5-19.1 %, 40.6-43.5 %, and 38.0-49.7 %, respectively.
    During confinement (first month postpartum), total energy and total fat content of
    human milk were the highest whereas total carbohydrate was the lowest, compared
    to the rest of the exclusive breastfeeding period. In contrast, intakes of total calorie
    and total fat were the lowest, whereas protein was the highest during this period.
    However, no associations were detected between human milk nutritional contents
    and maternal dietary intake. Conclusion: In our study population, the composition
    of maternal diet and nutritional content of human milk differed between confinement
    and post-confinement periods. However, the association between maternal diet and
    human milk composition itself warrants further investigation.

    Vitamin A deficiency is common in many countries where rice is the staple food. Food fortification is an important strategy to address this problem. As rice noodle is the second principal form of rice products widely consumed in Asia, rice noodles could be a potential vehicle for fortification of vitamin A. In this study, rice noodles were prepared from 0, 300, 600, 1,050, and 1,500 μg of vitamin A per 100 g of rice flour. Samples were analyzed for quality, sensory evaluation, and enhancement of vitamin A intakes. Increasing level of vitamin A fortification did not influence quality and sensory properties of the rice noodles, except for the ash content, color, and appearance of the noodles. Rice noodle that was fortified with the highest level of vitamin A was found to be the darkest in color. However, this sample received scores higher than 6 (like slightly) for appearance. Furthermore, sample fortified with the highest level of vitamin A produced rice noodles with the highest level of vitamin A retention suggesting that noodles were good vehicle for vitamin A fortification. Fortification of rice flour with 1,500 μg of vitamin A produced rice noodles with 24.88% of the RDI for vitamin A per serving and provided an effective means of enhancing vitamin A intake.

    Inhibition of Thymidine phosphorylase (TP) is continuously studied for the design and development of new drugs for the treatment of neoplastic diseases. As a part of our effort to identify TP inhibitors, we performed a structure-based virtual screening (SBVS) of our compound collection. Based on the insights gained from structures of virtual screening hits, a scaffold was designed using 1,3,4-oxadiazole as the basic structural feature and SAR studies were carried out for the optimization of this scaffold. Twenty-five novel bis-indole linked 1,3,4-oxadiazoles (7-31) were designed, synthesized and tested in vitro against E. coli TP (EcTP). Compound 7 emerged as potent TP inhibitor with an IC50 value of 3.50 ± 0.01 μM. Docking studies were carried out using GOLD software on thymidine phosphorylase from human (hTP) and E. coli (EcTP). Various hydrogen bonding, hydrophobic interactions, and π-π stacking were observed between designed molecules and the active site amino acid residues of the studied enzymes.

    Introduction: Nutritional requirements increase during lactation. However, maternal dietary intakes of Malaysian
    mothers are subjected to restrictions commonly included in traditional postpartum practices. This study aimed to
    assess the maternal dietary intake status during the recommended six month exclusive breastfeeding (part of which
    included the traditional confinement) period. Methods: Thirty-two Malay mothers aged 18-35 years, who had delivered full-term (at ≥37 weeks) singleton babies and were exclusively breastfeeding, were included in the study.
    Maternal dietary intake was assessed using multiple-pass diet recall on Days 10, 30, 60, 90, 120, 150, and 180,
    postpartum. The average total energy and macronutrient intakes were compared against recommended values. Results: The findings of this study demonstrated that maternal intakes of total energy ranged from 1,500-2,000kcal/day,
    carbohydrate 189-272g/day, protein 58-72g/day, and total fat 32-70g/day. Total calorie intake was the lowest during
    confinement period (Days 10 and 30) compared to the rest of the exclusive breastfeeding period. This is similar with
    total fat consumption. On the other hand, protein intake was the highest during confinement period whereas carbohydrate intake was consistent throughout the six-month period. Despite the increased requirements, intakes of total
    calories, protein, total fat, dietary fibre, and water, did not meet the recommended values throughout the exclusive
    breastfeeding period. Conclusion: Mothers’ inability to fulfil their nutritional requirements during exclusive breastfeeding period may be associated with traditional postpartum dietary practices. Dietary advice with consideration
    for cultural food taboos practiced by local mothers during confinement may help to improve maternal nutritional
    intakes during this crucial time.

    Malaysia is a tropical country that is highly dependent on surface water for its raw water supply. Unfortunately, surface water is vulnerable to pollution, especially in developed and dense urban catchments. Therefore, in this study, a methodology was developed for an extensive temporal water quality index (WQI) and classification analysis, simulations of various pollutant discharge scenarios using QUAL2K software, and maps with NH3-N as the core pollutant using an integrated QUAL2K-GIS. It was found that most of the water quality stations are categorized as Class III (slightly polluted to polluted). These stations are surrounded by residential areas, industries, workshops, restaurants and wet markets that contribute to the poor water quality levels. Additionally, low WQI values were reported in 2010 owing to development and agricultural activities. However, the WQI values improved during the wet season. High concentrations of NH3-N were found in the basin, especially during dry weather conditions. Three scenarios were simulated, i.e. 10%, 50% and 70% of pollution discharge into Skudai river using a calibrated and validated QUAL2K model. Model performance was evaluated using the relative percentage difference. An inclusive graph showing the current conditions and pollution reduction scenarios with respect to the distance of Skudai river and its tributaries is developed to determine the WQI classification. Comprehensive water quality maps based on NH3-N as the core pollutant are developed using integrated QUAL2K-GIS to illustrate the overall condition of the Skudai river. High NH3-N in the Skudai River affects water treatment plant operations. Pollution control of more than 90% is required to improve the water quality classification to Class II. The methodology and analysis developed in this study can assist various stakeholders and authorities in identifying problematic areas and determining the required percentage of pollution reduction to improve the Skudai River water quality.

    The subject of euthanasia has generated many controversial debates, particularly on its legality. This has been primarily due to the doctrine of sanctity of life which is a predominant principle in many religions and is embedded in moral values. The underlying precept is that human life is sacred and demands respect, as all life comes from God and life can only be taken away intentionally through due process of law. Thus, ending a person's life, even upon his or her request, is considered in many jurisdictions as a criminal offence. Irrespective of humanitarian motives, a doctor who aids a patient in this act will be in breach of his unequivocal duty as a doctor and considered to have committed an unlawful act, which will subject him to legal sanctions. Countries such as the United Kingdom, Australia, New Zealand and Turkey have explicit legal provisions banning, in particular, active euthanasia, while countries such as the Netherlands, Belgium, Switzerland and certain states in the U.S have taken positive steps to legalise euthanasia in certain aspects. The ethical codes and laws in Malaysia have yet to develop to the fullest extent in dealing with these issues. Although Malaysian statutory legislation contains provisions banning active euthanasia, the legal position on passive euthanasia remains implicit. In the absence of indicative legal provisions and judicial precedents governing certain matters arising, it has been the practice of the Malaysian courts to refer to relevant legal principles inherent in English cases. As Islam is proclaimed as the country's official religion, the Islamic perspective is also an important and influential factor in the development of the legal framework in Malaysia as a whole. Thus, there is a need for the existence of a clear and comprehensive regulatory framework governing the legality of euthanasia in Malaysia.

    The prime objective of the present research work was to evaluate the efficiency of bio-machine for the removal of Cadmium (Cd) from aquatic systems. Aspergillus niger fungus was used as bio-machine to remove Cd from aquatic systems. Twenty three different strains (IIB-1 to IIB-23) were isolated from industrial effluents and the Langmuir and Freundlich models were applied to the best Cadmium removal strain IIB-23 in order to obtain the adsorption parameters. Different parameters such as pH, temperature, contact time, initial metal concentratio, and biomass dosage on the biosorption of Cd were studied. The percent removal of Cd initially increased with an increase in pH ranging from 5.5-6.5 and then decreased by increasing pH from 7.0-7.5. An optimized pH used for Cd removal from aquatic systems was found to be 6.5. Additionally, an optimum amount of biomass was 1.33 g for the maximum removal of Cd from the aqueous solutions with initial metal concentration of 75 mg/L. The results obtained thus indicated that Langmuir model is the best suited for the removal of Cd from aquatic systems.

    Tuberculosis is an infection that can occur in every organ of the body, but it rarely affects the wrist joint. We report a rare case of a male patient with wrist tuberculosis with a subtle presentation. Our patient's left wrist had been swollen for four months and progressively worsened, becoming ulcerated one week prior to presentation to our center. He was asymptomatic, but a previous radiograph showed global destruction of the wrist joint. Clinical investigations, that is, polymerase chain reaction test for tuberculosis and histopathological examination, showed classic findings of tuberculosis, which lead to the initiation of anti-tuberculosis treatment.

    A young patient presented to our centre with swollen right hand following a motor vehicle accident. He was diagnosed with closed fractures of trapezoid, ulnar three metacarpal bones, radial styloid and ulnar styloid. The hand injuries were complicated with compartment syndrome. Emergent fasciotomy and application of external fixator of the hand were performed. Definitive fixation of the fractures was delayed due to the wound care post fasciotomy. During the definitive fixation of the hand, the trapezoid was found to be comminuted and completely extruded. Abundant callus was found at the fracture sites of the metacarpal bones. Anatomic fixation was not feasible. Principle-based intra-operative creativity and flexibility were of great significance in the unconventional fixation of the complex hand injuries described in this case report.

    In this paper, the Duckworth-Lewis-Stern (DLS) and Duckworth-Lewis-McHale-Asif (DLMA) methods of revising targets for a team batting in second innings in an interrupted Limited Overs International Cricket (LOI), are examined for fairness. The work discusses four significant points: flexibility, intuition, simplicity, and goodness-of-fit of the two mentioned methods. The research findings have shown that the DLMA method is better in every aspect than the DLS method. Further, the data of 1764 ODI matches played during 2004-2021 to investigate the compatibility of the DLMA for high run-scoring One-Day International matches. The results show that DLMA is compatible to the situation of the well-above run-scoring situation.

    BACKGROUND: This research aimed to model the outbreak of COVID-19 in Malaysia and develop a GUI-based model.

    DESIGN AND METHODS: The model is an improvement of the susceptible, infected, recovery, and death (SIRD) compartmental model.  The epidemiological parameters of the infection, recovery, and death rates were formulated as time dependent piecewise functions by incorporating the control measures of lockdown, social distancing, quarantine, lockdown lifting time and the percentage of people who abide by the rules. An improved SIRD model was solved via the 4th order Runge-Kutta (RK4) method and 14 unknown parameters were estimated by using Nelder-Mead algorithm and pattern-search technique. The publicly available data for COVID-19 outbreak in Malaysia was used to validate the performance of the model. The GUI-based SIRD model was developed to simulate the number of active cases of COVID-19 over time by considering movement control order (MCO) lifted date and the percentage of people who abide the rules.

    RESULTS: The simulator showed that the improved SIRD model adequately fitted Malaysia COVID-19 data indicated by low values of root mean square error (RMSE) as compared to other existing models. The higher the percentage of people following the SOP, the lower the spread of disease. Another key point is that the later the lifting time after the lockdown, the lower the spread of disease.

    CONCLUSIONS: These findings highlight the importance of the society to obey the intervention measures in preventing the spread of the COVID-19 disease.

    BACKGROUND: Perah seed is one of the most underutilized oilseeds, containing high nutritional values and high percentage of α-linoleneic acid, which may have a high potential in food and pharmaceutical applica- tions. The main objective of this study was to evaluate the influence of microwave (MW) cooking on the proximate composition and antioxidant activity of perah seeds.

    METHODS: In this study, the proximate composition and amygdalin concentration of MW ir- radiated perah seeds were determined. The total phenolic content (TPC), Maillard reaction products (MRPs) and antioxidant activity of methanol (PME), 70% methanol in water (PMW), ethanol (PEE), 70% ethanol  in water (PEW) extracts and methanol extract of oil (PMO) were evaluated during MW cooking. The anti- oxidant activity was evaluated using multiple assays, namely DPPH radical scavenging activity, β-Carotene bleaching assay, and reducing power.

    RESULTS: Microwave cooking did not significantly increase crude lipid and carbohydrate content, and the amounts of other nutrients such as ash, crude protein and fibre remained almost unchanged. As evaluated  by HPLC, the amygdalin concentration in the seeds was reduced by MW cooking. The TPC, MRP and anti- oxidant activity of the solvent extracts of perah seeds increased significantly with increasing roasting time. Of all the extracts, PMW at all MW cooking times displayed the highest antioxidant effectiveness. However, thermal treatment significantly reduced the antioxidant properties of PMO. The values for TPC, MRP and antioxidant effectiveness of the samples were ranked in the following order: PMW > PEW > PME > PEE > PMO, in both control and microwaved samples.

    CONCLUSIONS: In determining the overall quality of the products, MW cooking time was found to be a critical factor. Solubilization of phenolic compounds and formation of MRPs during MW cooking could have caused the increase in antioxidant activity of the perah seeds.

    INTRODUCTION: Digital device helps children enhance academic, cognitive and psychomotor skills. However, prolonged use causes physical inactivity, poor interpersonal skills and communication problems. Information on digital device use among young children in Malaysia is currently limited. Hence, this study aimed to determine the prevalence of digital device utilisation among preschool children in Kota Setar District, Kedah.

    METHOD: A cross-sectional study at government preschools in Kota Setar District was conducted from February to April 2020. Selection of preschools and students was done using multistage simple randomisation. A self-administered questionnaire containing demographic and digital device use details was filled by parents.

    RESULTS: The prevalence of digital device use among preschool children was 95.9% and mostly used smartphones (94.2%). Most children (95%) did not own the device, and usage was under supervision (95.7%). The reason for supervision was to prevent exposure to inappropriate content (70.5%). The common reasons for allowing digital device use were for educational (37.4%) and entertainment purposes (36%) through videos (30.9%) and games (30.2%). Approximately 21.5% and 50.3% of the children spent more than 1 and 2 hours on digital devices during weekdays and weekends, respectively.

    CONCLUSION: The prevalence of digital device use among the preschool children in Kota Setar District was very high. Most of them used digital devices for educational and entertainment purposes under parental supervision. However, some exceeded the recommended screen time on weekends. These findings could promote awareness of digital device use among young children and help design public health awareness programmes and future policies.

    Though a common symptom, epistaxis should never be overlooked as the implications may be consequential. Nasal leech infestation represents an unusual etiology of recurrent epistaxis. The leeches release enzymes to anaesthetize the attachment site to facilitate the feeding process. Given the inconspicuous area of infestation and the absence of pain, there is often a delayed treatment. A detailed history taking and physical examination remain paramount in the assessment of recurrent epistaxis.

    BACKGROUND: Caregivers are directly involved in managing the daily basic needs of children with autism spectrum disorder (ASD). The knowledge and attitudes of these caregivers are important to achieving success in their roles. Thus, this study aimed to determine what constitutes good knowledge, attitudes, and associated factors among caregivers toward children with ASD.

    METHODS: A cross-sectional study was conducted among 128 caregivers of children with ASD in Kota Bharu, Kelantan from May to August 2020, using convenience sampling. Validated questionnaires were used to assess the knowledge and attitudes toward children with ASD. The data were analyzed using SPSS version 24. Descriptive statistics and simple and multiple logistic regression analyses were then performed.

    RESULTS: The response rate was 100%. The proportion of good knowledge and attitudes toward children with ASD among caregivers was 85.1% and 88.3%, respectively. Factors significantly associated with good knowledge were being female (OR (95% CI) 2.79 [0.99-7.90]) and ASD children being non-first-born children (OR (95% CI) 0.41 [0.15-1.12]). Factors significantly associated with good attitudes were age of 30 years and older (OR (95% CI) 0.13 [0.03-0.62]) and caregiver having other children with other types of learning difficulties (OR (95% CI) 0.15 [0.04, 0.52]).

    CONCLUSIONS: The proportion of caregivers with good knowledge of ASD and good attitudes toward children with ASD was high. The caregiver's age and sex, the position of the ASD child among the siblings, and the presence of other types of learning disorders in the family need to be considered when managing children with ASD.

    BACKGROUND: A written asthma action plan (WAAP) is one of the treatment strategies to achieve good asthma control in children.

    METHODS: This randomized controlled trial was conducted to observe the effect of WAAP on asthma control and quality of life using the Asthma Control Questionnaire and Pediatric Asthma Quality of Life Questionnaire (PAQLQ) at baseline and after 3 months. A repeated measure analysis of variance was used to analyze the mean score difference between the two groups.

    RESULTS: There was no significant difference in mean score for asthma control at baseline between groups (F[degree of freedom (df)]=1.17 [1, 119], P=0.282). However, at 3 months, a significant difference in mean scores between groups was observed (F[df]=7.32 [1, 119], P=0.008). The mean±standard deviation (SD) scores in the intervention and control groups were 0.96±0.53 and 1.21±0.49, respectively. For the analysis of the PAQLQ, no significant difference was observed in the mean score for the quality of life baseline in both groups. There were significant mean score changes for the quality of life (F[df]=10.9 [1, 119], P=0.001) at 3 months follow-up, where those in the intervention group scored a mean±SD score of 6.19±0.45, and those in the control group scored 5.94±0.38. A time-group interaction analysis using repeated-measures analysis of variance showed significant differences in mean score changes (F[df]=5.03 [1, 116], P=0.027) and (F[df]=11.55 [1, 116], P=0.001) where a lower mean score was observed in the intervention group, indicating better asthma control and quality of life, respectively. A significant (P<0.001) negative Pearson correlation between asthma control and quality of life (-0.65) indicated a moderate correlation.

    CONCLUSION: WAAP, along with standard asthma treatment, improves asthma care.

    This study examines the impact of energy consumption, urbanization, and globalization on environmental degradation proxied by carbon emissions (CO2) in the South Asian Association for Regional Cooperation (SAARC) countries, namely Sri Lanka, Pakistan, Maldives, Nepal, Bhutan, Bangladesh, and India using data over the period 1990-2018. The cross-sectional autoregressive distributed lag (CS-ARDL), pooled mean group (PMG), and Dumitrescu and Hurlin (D-H) Granger causality techniques are employed for the empirical analysis. First and second-generation panel unit root tests are used to determine the stationary level of all data series which reveals mixed order of integration. The empirical findings show that urbanization, gross domestic product (GDP) per capita income, energy consumption, industrial growth, globalization, and financial development cause CO2 emissions, while the other variables, namely arable land and innovation, put negative effects on CO2 emissions. Moreover, the D-H heterogeneous test results exhibit that bi-directional relationship exists between CO2 and arable land, urbanization, industrial growth, and financial development, while a unidirectional causality exists between CO2 emissions and GDP per head income. These findings suggest that planned urbanization, investment in renewable energy sources, and effective strategies regarding the economic and financial integration with the global economies are required for a clean and green environment.
