    The close relationship between dogs and humans has led to concerns regarding the transmission of zoonotic pathogens through ectoparasites such as ticks. This study aimed to investigate the occurrence of ticks and the intensity on stray dogs with specific risk factors (size, sex, neutering status, body part). Additionally, identifying the predilection sites of ticks on stray dogs was crucial for developing an effective tick control program in Malaysia. A cross-sectional study was conducted among 64 stray dogs from Kelantan and Selangor States. These dogs were subjected to integumentary examinations, collecting 431 ticks comprising Rhipicephalus sanguineus s.l. "tropical lineage" and Haemaphysalis bispinosa from 53 infested dogs. The overall occurrence of tick infestation was 82.81% (53/64), with an average intensity of 8.13 ticks (range: 1-17) per stray dog. All the potential risk factors considered in this study showed no statistically significant result (P value >0.05). The head, ear, and neck were the most preferred attachment sites for ticks. These findings underscore the importance of implementing tick control programs for stray dogs, which serve as reservoirs of ticks and tick-borne pathogens for owned dogs and humans.

    INTRODUCTION: Little is known regarding the educational needs and perspectives of people living with Parkinson's disease (PD), particularly in Asia.

    OBJECTIVE: To assess knowledge and perceptions regarding PD in a large multiethnic urban Asian cohort of patients and caregivers.

    METHODS: We conducted a survey at a university hospital neurology clinic, using a novel Knowledge and Perception of Parkinson's Disease Questionnaire (KPPDQ).

    RESULTS: The KPPDQ had satisfactory psychometric properties among patients and caregivers. Five hundred subjects were recruited with a 97% response rate (211 patients, 273 caregivers). Non-motor symptoms such as urinary problems, visual hallucinations and pain were relatively poorly recognized. Many (≈ 50-80%) respondents incorrectly believed that all PD patients experience tremor, that PD is usually familial, and that there is a cure for PD. About one-half perceived PD to be caused by something the patient had done in the past, and that PD medications were likely to cause internal organ damage. Issues of stigma/shame were relevant to one-third of patients, and 70% of patients perceived themselves to be a burden to others. Two-thirds of participants felt that PD imposed a heavy financial toll. Participants were about equally divided as to whether they would consider treatment with deep brain stimulation, tube feeding or invasive ventilation. Over three-quarters of patients expressed a preference to die at home.

    CONCLUSIONS: Important knowledge gaps, misperceptions and perspectives on PD were identified, highlighting the need for further efforts to raise awareness and provide accurate information regarding PD, and to address patient's and caregivers' needs and preferences.

    INTRODUCTION: Next-generation sequencing (NGS) diagnostics have shown clinical utility in predicting survival benefits in patients with certain cancer types who are undergoing targeted drug therapies. Currently, there are no guidelines or recommendations for the use of NGS in patients with metastatic cancer from an Asian perspective. In this article, we present the Asia-Pacific Oncology Drug Development Consortium (APODDC) recommendations for the clinical use of NGS in metastatic cancers.

    METHODS: The APODDC set up a group of experts in the field of clinical cancer genomics to (i) understand the current NGS landscape for metastatic cancers in the Asia-Pacific (APAC) region; (ii) discuss key challenges in the adoption of NGS testing in clinical practice; and (iii) adapt/modify the European Society for Medical Oncology guidelines for local use. Nine cancer types [breast cancer (BC), gastric cancer (GC), nasopharyngeal cancer (NPC), ovarian cancer (OC), prostate cancer, lung cancer, and colorectal cancer (CRC) as well as cholangiocarcinoma and hepatocellular carcinoma (HCC)] were identified, and the applicability of NGS was evaluated in daily practice and/or clinical research. Asian ethnicity, accessibility of NGS testing, reimbursement, and socioeconomic and local practice characteristics were taken into consideration.

    RESULTS: The APODDC recommends NGS testing in metastatic non-small-cell lung cancer (NSCLC). Routine NGS testing is not recommended in metastatic BC, GC, and NPC as well as cholangiocarcinoma and HCC. The group suggested that patients with epithelial OC may be offered germline and/or somatic genetic testing for BReast CAncer gene 1 (BRCA1), BRCA2, and other OC susceptibility genes. Access to poly (ADP-ribose) polymerase inhibitors is required for NGS to be of clinical utility in prostate cancer. Allele-specific PCR or a small-panel multiplex-gene NGS was suggested to identify key alterations in CRC.

    CONCLUSION: This document offers practical guidance on the clinical utility of NGS in specific cancer indications from an Asian perspective.

    Vibrio cholerae is a Gram-negative bacterium that causes diarrheal disease. V. cholerae O1 and O139 serogroups are toxigenic and are known to cause epidemic cholera. These serogroups produce cholera toxin and other accessory toxins such as accessory cholera enterotoxin, zonula occludens toxin, and multifunctional, autoprocessing repeat in toxin (MARTX). In the present study, we incorporated mutated rtxA and rtxC genes that encode MARTX toxin into the existing aminolevulinic acid (ALA) auxotrophic vaccine candidate VCUSM2 of V. cholerae O139 serogroup. The rtxC mutant was named VCUSM9 and the rtxC/rtxA mutant was named VCUSM10. VCUSM9 and VCUSM10 were able to colonize intestinal cells well, compared with the parent vaccine strain, and produced no fluid accumulation in a rabbit ileal loop model. Cell rounding and western blotting assays indicated that mutation of the rtxC gene alone (VCUSM9 strain) did not abolish MARTX toxicity. However mutation of both the rtxA and rtxC genes (VCUSM10) completely abolished MARTX toxicity. Thus we have produced a new, less reactogenic, auxotrophic rtxC/rtxA mutated vaccine candidate against O139 V. cholerae.

    Introduction: Ascariasis is a parasitic infection, which commonly affects immunocompromised patients. Most pa-tients remained asymptomatic during the early larval migration stage and respond well with conventional anti-hel-minthic drugs. Previous literature had reported symptomatic Ascaris infection mimicking bacterial pneumonia and the typical eosinophilia found in Loeffler syndrome was absent in patients on corticosteroids. Thus, a high index of suspicion for ascariasis is needed for immunosuppressed patients presented with infection. We present here a case of severe ascariasis infection in a systemic lupus erythematosus patient. Case description: A 16-year-old boy presented with fever, generalized maculopapular rash associated with neutropenia and thrombocytopenia. He was treated initially as Dengue Fever initially. However his symptoms did not resolve at even day 14 of admission. On further assessment, we were convinced he has SLE based upon presence of malar rash, oral ulcers, urinary protein-uria, persistent leucopenia, thrombocytopenia with low complements and ANA positive. He was promptly started on IV hydrocortisone. He showed a good progress in the first few days. On day 5 of admission, he coughed out a round worm which later identified as Ascarisis lumbricoides. He was started on Albendazole. Unfortunately he developed hemoptysis and respiratory compromisation where he required intubation. Post intubation he went into cardiac arrest, which required CPR. Following that event, his condition further deteriorated with multi organ failure. He succumbed to his illness three days later. Conclusion: Immunocompromised patients are prone to opportunistic infections including parasitic infections. we present here a case of ascariasis in an SLE patient who unfortunately succumbed to the illness. Due to the variable clinical symptoms that mimic other infections, screening for parasitic infections needs to be considered especially if the patients do not respond to antibiotics and routine treatments.

    Introduction: Universal HBV and HCV screening among pregnant women is not a current practice in Malaysia. It is aimed to conduct a systematic review on available evidences in an effort to strengthen the national hepatitis screen-ing programs. Methods: Systematic search was performed from databases; Medline, Cochrane, PubMed and IN-AHTA. Relevant studies according to inclusion/exclusion criteria were critically appraised and evidence graded. Re-sults: From 782 titles identified, two systematic reviews, two retrospective cohort studies, two cross-sectional studies, one cost-utility analysis and one cost-effectiveness analysis were included. Universal antenatal HBV screening was associated with almost complete vaccination coverage for newborns. Replacing targeted screening with universal HBV screening was associated with increased identification of newborns indicated for HBV-immunization from 50% to 96%. Universal antenatal HBV screening had incremental cost-effectiveness ratio (ICER)s ranged from €2,032 to €26,181 per life year (LY) gained. As for HCV, targeted antenatal screening was associated with low HCV prevalence. Universal screening did not detect significantly more women with HCV infection than did targeted screening. One cost-effectiveness analysis found that universal antenatal HCV screening had ICER of €52,473 per LY gained and one cost-utility analysis reported ICER of £9,139 per QALY gained. Conclusion: Based on the above review, universal HBV screening in pregnant women is effective in increasing vaccination coverage for newborns. However, the ICERs had wide range. Therefore, local economic evaluation is needed to estimate cost implications before considering addition into national screening program. While for HCV, both universal and targeted screening in pregnant women had low detection rate thus high-risk approach screening is appropriate in Malaysia.

    PR-10 is a member of pathogenesis-related (PR) genes elicited by the plant’s defense mechanism during pathogen attack.
    Elevated expression of PR-10 upon different pathogen invasions has been observed in many plant species suggesting
    its role as an anti-bacterial, anti-viral and anti-fungal gene. However, the effect of PR-10 in mitigating the infection of
    Fusarium oxysporum f. sp. cubense (Foc), the causal agent of Fusarium wilt in banana has not been reported. In this
    study, the coding sequences of PR-10 gene isolated from Foc resistant Musa acuminata ssp. malaccensis (MaPR-10)
    were integrated into a local Foc susceptible commercial banana cultivar, Berangan via co-cultivation of embryogenic
    cell suspension and Agrobacterium tumefaciens. Out of 17 putative transgenic lines established, 11 of them positively
    harbored MaPR-10. Among these, Line-19 plantlets showed the most rapid in-vitro propagation and successfully overexpressed the transgene. Following a nursery challenge experiment with a virulent Foc race 4 (CI HIR) isolate, about 30%
    of Line-19 plants showed a one-week delay in disease progression when compared to the untransformed controls. From
    the final evaluation performed in the 5th week-post-inoculation, the leaf symptoms index (LSI) and rhizome discoloration
    index (RDI) of Line-19 was 3.4 and 6.1, respectively, indicating the disease had progressed. The findings of this study
    enrich the current existing knowledge on the roles of PR-10 in combating fungal disease in plants.

    Background: Depression and erectile dysfunction (ED) are common in aging and the two conditions often co-exist. These conditions have been shown to be associated with hormonal changes in men. This paper examines the association between depression, ED, and hormonal status of men aged above 50 years in the Klang Valley, Malaysia.
    Methods: Five hundred men aged 50 years and above were randomly selected via the electoral roll and invited to participate in a community-based study on men's health: 351 men responded. Respondents were interviewed individually based on a self-developed questionnaire, which included information on socio-demographic data. Erectile function was measured using the International Index for Erectile Function-5 (IIEF-5) and depression was measured using the 15 item Geriatric Depression Scale (GDS-15).
    Results: Sixty-nine percent of the men were diagnosed with ED. Mean GDS score was 3.33 (SD = 3.29). Nineteen percent (n = 67) of the men had abnormal levels of testosterone (≤11 nmol/l) and this comprised 73% of men with ED (n = 49) and 27% of men without ED (n = 18). There was no significant association between testosterone level and ED (χ2 = 0.68, p = 0.41). Significant association was found between depression (GDS ≥ 5) and men with ED (χ2 = 6.07, p = 0.014). Sex hormone binding globulin and luteinising hormone were negatively correlated with erectile function. Results of the multiple linear regression showed that age and depression are predictors of erectile function.
    Conclusion: Depression and ED should be screened for when either exists in the male patient and treatment directed accordingly. © 2006 WPMH GmbH.

    Background: Men's major concern (MMC) is a common quality of life issue affecting the aging male population. This paper examines the association between MMC and cardiometabolic diseases in Malaysian men in an urban setting.
    Methods: A cross-sectional community based study was conducted in an urban area involving 1046 men, aged over 40 years and above, who were randomly selected using the electoral roll in Selangor Malaysia. Participants were assessed by trained doctors using structured questionnaire which included medical history, IIEF-5 to assess men's erectile dysfunction and IPSS score to assess prostatic symptoms. Clinical assessments including blood pressure, weight, height and waist circumference were performed. In addition, biochemical assessments for fasting blood glucose and lipid profile were done.
    Results: The response rate was 62.8%. The mean age of the participants was 55.8±8.4 (range: 41-93) years. Around two fifth of the participants (39.8%) had no MMC, 28.4% were diagnosed with 1 MMC, 9.9% with 2MMCs and 1.2% with 3MMCs. Increasing number of MMCs were significantly associated with increasing rate of cardiometabolic diseases (No MMC = 26.2%, 1 MMC = 43.4%, 2 MMCs = 63.5% and 3 MMCs = 69.2%) (P < 0.01) and metabolic syndrome based on Asian criteria (No MMC = 21.5%, 1 MMC = 39.0%, 2 MMCs = 49.1%, 3 MMCs = 62.5%) (P < 0.01). Participants with one or more MMC significantly showed a higher proportion of undiagnosed diabetes as well as reported diabetes (No MMC = 1.7%, 1 or more MMCs = 3.6%) when compared to participants without MMC (P ≤ 0.01).
    Conclusions: Men's Major Concerns (e.g., ED, LUTS and TD) were associated with cardiometabolic diseases and collectively, they could be used as an important portal to men's health. © 2011 WPMH GmbH.

    It has been recognized extensively that studies of pharmacogenetics provide massive examples of causal relationship between genotypes and drug effectiveness to account for interindividual phenotypic variations in drug therapy. In most cases, cytochrome P450 (CYP) polymorphisms are one of the major variables that affecting those drug plasma concentration, drug detoxification and drug activation in humans. Thus, understanding of CYP polymorphisms can be crucially valuable in order to allow early and more accurate drug dosage prediction and improve the drug response accordingly. Despite the high level of homologous amino acid sequences, CYP2C9 and CYP2C19 genes are among the most important CYP genes which metabolize a wide range of clinically therapeutic drugs. Several critical reviews have been published relating to the aforementioned genes. However, this minireview aims to systematically merge reported studies on the SNPs frequencies of both genes concentrating only on Malaysian population. It is hoped that, the minireview can be an opener for new opportunities to reevaluate the evidence on the prevalence of CYP2C genes as a potential genetic factor influencing a particular drug efficacy and safety among Malaysian. Such evaluation can be developed to the next level of early prediction of better and specific drug treatment, thereby improving the drug response while helping the government in minimising the drug expenditures.

    Sugarcane juice deteriorates rapidly as enzymatic browning and microbial spoilage take place soon after juice extraction. Therefore, the effects of pasteurization treatment (70ºC for 10 minutes) and addition of calamansi juice at different concentrations (0, 1.0, 1.5, and 3.0%, v/v) on the physicochemical (color, pH, total soluble solid, titratable acidity, and peroxidase enzymatic activity), microbiological (total plate count, yeast and mold), and sensory characteristics of black stem sugarcane juice were investigated. Quantitative descriptive analysis (QDA®) was conducted by eight trained panelists to determine the intensity of the sensory attributes of sugarcane juice, such as brownish color, grassy aroma, citrus aroma, sweetness, sourness, sweet aftertaste, and overall acceptability using a 15 cm unstructured line scale. The results showed that the pasteurization treatment significantly (p ˂ 0.05) increased the L* value (lightness) of sugarcane juice at 0% (v/v) of calamansi juice. Increasing the concentration of calamansi juice decreased the pH value and increased the titratable acidity of the sugarcane juice at 5% significance level. The relative effectiveness in reducing the peroxidase (POD) enzymatic activity and microbial load of sugarcane juice was shown after the addition of calamansi juice and pasteurization treatment, respectively. Significant (p ˂ 0.05) changes in the intensity of brownish color, grassy aroma, and sweet aftertaste of sugarcane juice were observed after the pasteurization treatment at 0% (v/v) calamansi juice; whereas, the addition of calamansi juice at different concentrations increased the intensity of citrus aroma and sourness, but decreased the intensity of grassy aroma and sweet aftertaste of sugarcane juice at 5% significance level. Hence, pasteurization treatment coupled with the addition of 1.5% (v/v) calamansi juice was effective in achieving microbial stability and consumer’s acceptability on sugarcane juice.

    We are aware of the shortage of psychiatrists in Malaysia.
    However, there is no formal report on the distribution and ratio of
    psychiatrists in each state in Malaysia. We aim to do a detailed count of the
    number of psychiatrists in the country. Methods: We obtained the figures for
    the psychiatrists practicing in the government, private and university settings
    by accessing the Ministry of Health database, information from the
    Malaysian Psychiatric Association, Malaysian Mental Health Association,
    National Specialist Register and websites of the respective Universities. The
    total number and ratio of psychiatrists per 100,000 population are calculated.
    Results: There was a total of 410 registered psychiatrists in Malaysia in the
    private universities, private clinics, public universities and government
    hospitals. The state with the highest number of psychiatrists is Wilayah
    Persekutuan Kuala Lumpur with a total of 94 psychiatrists which has a ratio
    of 5.24 per 100, 000 population followed by Wilayah Persekutuan Putrajaya
    with 3.38 per 100,000 population. The states with the least ratio of
    psychiatrists are Sabah with 0.54 and Kedah with 0.55 per 100, 000
    population. Conclusion: There is a discrepancy in the geographical
    distribution of psychiatrists in Malaysia. People living in larger, urban states
    have better access to mental health care whereas the smaller states face a
    serious lack of psychiatrists. More effort should be taken to improve mental
    healthcare in Malaysia as recommended by WHO with one psychiatrist for
    every 10, 000 population.

    Alzheimer’s disease (AD) is a progressive neurodegenerative disorder characterized by deterioration of the brain functions
    that result in impairment of memory, cognition and behavioural functions. Oxidative stress is well known to be one of the
    causative factors for AD. Thus this disease is potentially modulated by natural antioxidants such as vitamin E. The aim
    of this study was to evaluate the effect of tocotrienol-rich fraction (TRF) supplementation on antioxidant enzymes and
    DNA damage using APPswe/PS1dE9 transgenic mouse model of AD. Animals were supplemented with TRF (200 mg/kg)
    or alpha-tocopherol (αT) (200 mg/kg) for six months starting from nine months old. We found that superoxide dismutase
    (SOD) activity in AD mouse was decreased by supplementation of TRF and αT as compared with AD control mouse with no
    significant differences in glutathione peroxidise (GPx) activity in all groups. TRF supplementation significantly increased
    catalase (CAT) activity. The level of DNA damage of AD mouse shows significant decrease with supplementation of TRF
    and αT. In conclusion, TRF was able to modulate antioxidant enzymes activity and decreased the level of DNA damage
    of AD transgenic mouse model.

    Agrobacterium-mediated transformation of indica rice is undoubtedly a challenging task due to the rice recalcitrant nature to transformation process. Therefore, optimization of the transformation protocol is important for specific indica rice cultivar to ensure effectiveness of the transformation. In this study, crucial parameters affecting Agrobacteriummediated transformation were optimized to obtain transgenic rice of local rice cultivar (indica MR219). Embryogenic calli were chosen for inoculation with Agrobacterium tumefaciens strain LBA4404 harbouring a binary vector pH2GW7ABP57 containing gene of interest, Auxin binding protein 57 (Abp57). The parameters that have been optimized were the immersion time, co-cultivation period, acetosyringone concentration and co-cultivation temperature. A total of four days co-cultivation period and 30 min immersion of embryogenic callus are optimum for the transformation of MR219 with transformation efficiency of 26.4% and 16.0%, respectively. Acetosyringone at 200 μM and co-cultivation at 28°C also gave the highest transformation efficiency (14.4 and 18.4%, respectively). Meanwhile, inclusion of 20 g/L maltose+20 g/L sorbitol into the regeneration media has significantly improve the transformed somatic embryos growth and increase the regeneration efficiency up to 40.0%. The results of polymerase chain reaction (PCR) and reverse transcription-polymerase chain reaction (RT-PCR) indicated that the transgene was successfully integrated and overexpressed in transgenic rice of MR219. In conclusion, significant improvement in transformation efficiency for rice cv. MR219 has been obtained by using the optimised protocol for transformation and regeneration developed in this study.

    In this study, we analysed the genetic polymorphisms present in the third intron region of Interleukin-4 gene in Malaysian patients with Systemic Lupus Erythematosus. Overall, the RP I and II alleles were found evenly distributed in both the SLE patients and control individuals. There was no significant association observed in the distribution of allelic and genotypic frequencies between SLE patients and healthy controls. The result obtained is similar to a previous study carried out on SLE Chinese patients in Taiwan.
