Summary of Paper: This study aimed to assess the knowledge on basic sciences and clinical psychiatry (psychopathology and clinical syndromes) of the part I candidates on the mock MCQ paper. Methodology and results: The study is a descriptive and cross-sectional study and was carried out in a group of postgraduate students in psychiatry from Faculty of Medicine, Universiti Kebangsaan Malaysia (UKM) and Faculty of Medicine, Universiti Malaya (UM) attending a series of revision course organized by the Department of Psychiatry, UKM. Revision course in psychiatry is an annual intensive course organized by the Department of Psychiatry, UKM to help young candidates preparing postgraduate psychiatry examination to refresh and consolidate their knowledge on basic sciences and clinical psychiatry. Candidates from various universities sitting for part 1 exam and completed their lectures and training in area of basic sciences are invited to attend this course which is held in The Department of Psychiatry, Hospital UKM.
Symptoms presented by patients suffering from Schizophrenia can be influenced by cultural factors. A case of Schizophrenia presenting with a common cultural medical complaint as a delusion, is described here. The appropriate assessment, identification and treatment have led to an improved out.
This is a personal experience with MERCY Malaysia’s Team 7 during its sixday sojourn in Gaza immediately after the 22-day war. It was a mission cut short. We were asked to evacuate as Israel intensified its shelling. This paper is divided into two main parts: First, my observations as a psychiatrist and humanitarian volunteer; and second, a plan for a psychosocial programme in
Gaza.
Spatial and temporal variations in concentrations of several metals and isotopes in sediment cores from around Penang Island, an area with economically important biological resources off the northwest coast of peninsular Malaysia, are reported. Because of a typical, monazite rich mineralogy in surrounding drainage basins, sedimentary metal enrichment factors relative to global average materials, enrichment factors (EFs) of ˃1.0 do not always indicate significant anthropogenic metal inputs. Because of extensive metal solubilization in the hot, organic carbon rich area, EFs of < 1.0 may be observed for several metals despite significant anthropogenic contributions. Comparison of metal-Al relationships in Penang area surface sediments with those in nearby and presumed uncontaminated Strait of Malacca sediments more accurately correct for atypical regional solubilization and mineralogical effects than comparison to global average materials. Such comparisons show concentrations of Cd, Cu, Pb, Ni, Cr, As, Sb, Zn and V have changed by less than a factor of two by anthropogenic discharges. Sedimentary concentration profiles of Pb, Zn and Cu, ratioed to Sc to normalize for variations in grain size and mineralogy, have subsurface maxima suggestive of modest and recently reduced anthropogenic inputs. Mn, U, As and Sb have Sc-normalized concentration profiles clearly affected by diagenetic processes. Sc-normalized profiles of Cr, Th, Ce and Sm show only small changes with depth, confirming insignificant anthropogenic inputs and undetectable postdepositional diagenetic mobility.
Excess ²¹⁰Pb activities and fluxes in Penang area sediments are limited by supply of this radionuclide, in contrast to sediments of both the northwestern U.S.A. and Amazon continental shelves, where they are limited by particle scavenging reactions.²¹⁰Pb activities in sediments of
the shallow, dynamic Penang area often show erratic or unconvincing changes with depth that
cannot be reliably modeled by assuming steady state, constant deposition rate of particles of
uniform chemistry, mineralogy and initial unsupported ²¹⁰Pb, and that mixing is limited to a
recognizable surface layer and resemble a diffusive process.
A trial study had been conducted to determine the particulate form of 137 Cs in seawater surrounding East Malaysia. Large volume of seawater was filtered at a flow rate of 15 liters/min through the copper hexacyanoferrates (II) impregnated filters. These filters were ashed and counted using the gamma spectrometry system to determine the dissolved 137 Cs activity. It was found that the particulate form of 137 Cs consists of 20 to 49 % of the total 137 Cs activity concentrations. Some reasonable explanations for higher particulate percentage such as sampling locations, high water flow-rate, and large volume of seawaters were further discussed. It is hope that the result of this study will help to build a better understanding about the usage of impregnated filters to study dissolved 137 Cs activity concentrations.
Nuclear power is considered as one of the best option for future energy development in Malaysia. Since Malaysia has no experience in nuclear energy generation, commissioning the first nuclear power plant needs tremendous effort in various aspects. Site selection is one of important step in nuclear power plant commissioning process. This paper proposes candidate sites for nuclear power plant in Mersing, Kota Tinggi, Muar and Batu Pahat district in Johor, Malaysia. The candidate selection process uses the IAEA document and AELB guideline as main reference, supported by site selection procedure by various countries. MapInfo Professional software was used to simulate the selection process for candidate areas for the nuclear power plant. This paper concluded that Tenggaroh and Jemaluang area are the most suitable area for nuclear power plant facilities in Johor, Malaysia.
Present work shows the development of nuclear technology in Malaysia and highlights its
applications that have been developed by using the instrumental neutron activation analysis
(INAA) method. In addition, present study exhibits a comprehensive review of INAA for
calculation of neutron flux parameters and concentration of elements. The INAA is a
powerful method to analyse the sample which identifies qualitative and quantitative of
elements present in a sample. The INAA is a working instrument with advantages of
experimental simplicity, high accuracy, excellent flexibility with respect to irradiation and
counting conditions, and suitability for computerization. In INAA, sample is irradiated and
measured directly. In practical. INAA is based on an absolute, relative and single-comparator
standardisation method. The INAA has been developed since 1982 when the
TRIGA Mark II reactor of Malaysia has commissioned. The absolute method was less
utilised, the relative method has been used since 1982, and the ko-INAA method is derived
from single-comparator standardization method has been developed since 1996 in Malaysia.
The relative method, because of its advantages, such as high accuracy, easy for using, has
many applications in Malaysia. Currently, local universities and Malaysian Nuclear Agency
(MNA) research reactor use INAA method in Malaysia.
This study aims to examine the decision making by rugby sevens referees, and its relationship with the referees' performance. The instruments used in this study are the Rugby Referee Decision Making Test (α=.74) and the Referee Sevens Field Performance Evaluation (α=.94). It was administered to 132 rugby sevens referees (mean age 33.4 + 1.5 years; 132 males) from the Malaysian Rugby Union (MRU), which have been refereeing in 10 rugby sevens tournaments in Malaysia. Descriptive and Inferential statistics (one way ANOVA and Pearson's Correlation) were employed to analyse the data. Decision Making ( ̅= 24.13, SD=5.24) and performance ( ̅= 136.45, SD = 4.47) were identified at a moderate level. The findings indicated no significant differences [F= (3, 128) =.246, p>0.05] in the decision making across age level, but there were significant differences [F= (3, 128) =63.159, p
Microwave Remote sensing data have been widely used in land cover and land use classification. The objective of this research paper is to investigate the feasibility of the multi-polarized ALOS-PALSAR data for land cover mapping. This paper presents the methodology and preliminary result including data acquisitions, data processing and data analysis. Standard supervised classification techniques such as the maximum likelihood, minimum distance-to-mean, and parallelepiped were applied to the ALOS-PALSAR images in the land cover mapping analysis. The PALSAR data training areas were chosen based on the information obtained from
optical satellite imagery. The best supervise classifier was selected based on the highest overall accuracy and
kappa coefficient. This study indicated that the land cover of Butterworth, Malaysia can be mapped accurately
using ALOS PALSAR data.
Background: A randomised clinical trial was carried out to study the cost-effectiveness of continuous venovenous hemofiltration using high volume and standard volume.
Methods: Study was done through interviews involving patients or their relatives and document review on patients’ treatment and progress note during the hemofiltration therapy in the Intensive Care Unit, Hospital University Kebangsaan Malaysia. Study also involved secondary data analysis and a structured questionnaire survey to assess the treatment and medical cost incurred by the hospital during the continuous venovenous hemofiltration therapy.
Results: The result of this study showed that the continuous venovenous hemofiltration given at high volume 4-6 litres/hour is more cost effective than standard volume of 2 litres/hour. The Sequential Organ Failure Assessment (SOFA) score reduction in the high volume hemofiltration is 3.0 units over 24 hours. This reduction is higher than the standard volume hemofiltration which is only 0.5 unit over 24 hours.
Conclusions: High volume hemofiltration is more cost effective than standard volume therapy, where only RM 5,552 compared to RM 23,512 is needed for every one unit of SOFA score reduction respectively.
Quercus infectoria gall, which is known as manjakani in Malaysia, was traditionally used in treating diseases. The bioactive compounds from the galls can be extracted using various extraction methods. In this study, supercritical carbon dioxide (SC-CO2) extraction was used to study the effects of CO2 flow rate on the yield, total phenolic content and antioxidant activity of Q. infectoria extract by fixing the pressure and temperature at the highest density (P: 30 MPa, T: 40°C). The results were compared with those acquired from the Soxhlet extraction method. The results showed that the Soxhlet extraction had a higher percentage of extraction yield than SC-CO2 extraction. The selectivity of Q. infectoria extracts using SC-CO2 extraction was better than the Soxhlet extraction method. Meanwhile, the extraction efficiency using the SC-CO2 extraction ranged from 46% to 53%. The SC-CO2 extraction also yielded higher total phenolic content than using the Soxhlet extraction method when 2 mL/min of CO2 flow rate was applied (203.53 mg GA/g sample). This study also revealed that the extracts from the SC-CO2 extraction showed a better radical scavenging activity compared to the Soxhlet extraction when analysed using DPPH (2,2-diphenyl-1-picryl hydrazyl) radical scavenging activity assays.radical scavenging activity compared to the Soxhlet extraction when analysed using DPPH (2,2-diphenyl-1-picryl hydrazyl) radical scavenging activity assays.
The objective of this study was to explore the geographic distribution and temporal patterns of violent crime cases in Peninsular Malaysia by using the tools and techniques for spatial analysis. This study will also provide a general picture of violent crime patterns in Malaysia. The unit of analysis is district and the violent crime data from the year 2000 until 2009 were used in this study. In order to obtain the optimum number of components of crime in the space-time period, the space-time Normal Mixture Models were used. Based on the results of this model, the mapping of the crime occurrences was made. This map displays the spatial distribution of crime occurrence in 82 districts of Peninsular Malaysia. From this analysis, more violent crimes were shown to have occurred in developed states such as Selangor, Wilayah Persekutuan Kuala Lumpur and Johor. The findings of this study could be used by policy makers or responsible agencies to take any relevant actions in terms of crime prevention, human resource allocation and law enforcement so as to overcome this important issue in future.
The practice of quack dentistry worldwide is a challenge to the profession of dentistry as it undermines the trust of the general public and places the patients at unnecessary risks through their illegal practices. Although the ratio of patients to dentists is rapidly decreasing in Malaysia due to the increased output of qualified dentists from both local and foreign institutions, quack dentistry is still practiced in many areas. This report intends to highlight a popular treatment prescribed by these quack dentists which is removable partial dentures that is fixed to the adjacent teeth by cold cure acrylic. We also aim to discuss the design of the denture and the means employed to remove this ’fixed partial denture’. It is hoped that this case report will highlight the prevalence of the practice of quack dentistry especially in geographical area of this case report and allow the authorities to devise means to stop this illegal practice.
This paper aims to describe a Malay version of the Boston Naming Test (MBNT) and its normative data. The M-BNT follows closely the general administration procedures of the original Boston Naming Test (BNT) but is different in terms of item content. A total of 29 items from the original 60 items on the test were deemed culturally and linguistically valid for the Malay population and were thus retained. A total of 41 additional items were added to make a total list of 70 items for pilot testing. These items were first vetted by a panel of experts and then trialed on a sample of 40 Malay adults. Based on the item analysis from the pilot study, the M-BNT was reduced to a 50 item test. This was administered to 230 normal Malay subjects in five age groups (20 - 29 years, 30 - 39 years, 40 - 49 years, 50 - 59 years, and 60 - 69 years), split into two main educational levels (i.e. < 12 years of education, and 12 years or more) and across gender. The Malay subjects were chosen representative of the four major geographical regions in West Malaysia. Initial normative data was computed according to the five age groups and two educational levels. It is hoped that the M-BNT will become a test useful in the identification of patients with an expressive language word-finding disorder.
This study was carried out to gauge the level of job satisfaction among pharmacists in Malaysia. Surveys forms developed after referring to published studies, were mailed to 1700 pharmacist. Analysis of responses from 405 respondents revealed significant differences in job satisfaction with age, position held and monthly salary. Pharmacist within the age group of 26 to 35 were less satisfied with their work as compared to the other pharmacist. A monthly salary of more than RM8000 and prospects of promotion within an organization contributed to a high level of satisfaction among pharmacist. Sector of work and length of service did not appear to influence job satisfaction. Further analysis indicated that motivator factors and hygiene factors had a significant influence on job satisfaction, whereas job stress contributed toward dissatisfaction at work. The results of the study supported Herzberg’s theory, which states the motivator factors were far more important than hygiene factors in providing job satisfaction. From this study, it appeared that more than 50% of the pharmacists who responded had at least a moderate level of job satisfaction. However, efforts can be taken by employers to further improve job satisfaction among Malaysian pharmacists.
Basal cell carcinoma is the commonest skin malignancy diagnosed in Malaysia. Surgery is the recommended treatment of choice with the lowest failure rate provided a clear margin is obtained. However, the defect may be too large to be repaired primarily. Formal reconstruction using grafts and flaps should be done to achieve the optimal aim of maintaining the integrity, function and cosmetic patient outcome. Three reconstructive methods are described in this series to restore the facial defect following the wide local excision. The procedures described were peri-alar crescentic advancement flap, nasolabial rotational flap and full thickness skin graft using supraclavicular skin. This series highlights the usage of the procedures based on solid foundation and principles, without compromising the desired outcomes for the patient.
The state of Selangor, Malaysia, is facing a significant rise in the number of malaria cases with the incidence varying from 20 to 90 per 100,000 persons in a population. A study was carried out to describe the distribution of malaria cases in relation to the growing number of foreign workers in Selangor. Data were reviewed using the return forms "PBV (M) 101", a summary of malaria cases in Selangor for 2006 and Annual Reports of Vector-Borne Disease Control Unit, Selangor State Department for 2001- 2005. The malaria cases in Selangor varied between 172 cases in 2001 and 90 cases in 2006. Most of the cases were contributed by foreign workers, who were predominantly male of economically active age group and came from malaria endemic countries. Most of these cases were concentrated in the urban districts. Malaria is still endemic in Selangor. Malarial infection has the potential of contributing to an urban outbreak of malaria.
The aim of this descriotive study is to determine the incidence of injuries among recreational badminton players. We evaluated 86 recreational badminton players in the city of Malacca; 35 were excluded for fitting the exclusion criteria. The average recreational badminton player was 36.13 years old and had been playing badminton for the past 17.84 years at a frequency of 2.11 times per week. 39.21% of the recreational badminton players complained of recent injuries in relation to playing badminton. Our data showed that the most common injury sustained by recreational badminton players was pain and stiffness at the shoulder joint. None of the injuries sustained by the players in our data were serious enough to warrant any form of surgical intervention. We conclude that badminton is a sport of relatively low risk and that the majority of related injuries were chronic overuse injuries.
The global environment is in a dynamic flux due to rapid development. As a result of this, new
diseases are emerging and old diseases are re-emerging in many parts of the world. Therefore there is a constant need for appropriate data for formulation of effective policies to mitigate the adverse effects of environmental degradation on human health. The Kyoto Protocol of 1997 is a milestone that sets the direction for good environmental management initiatives and the success
of this depends on good data. Malaysia currently has the Environmental Quality Act 1974 in place to
control environment related problems. However good guidelines must be developed to keep the initiatives for good environmental management on course, for this we need good data. The Centre for Environmental and Population Health at IMU, coordinates research activities in specific thrust areas in an endeavour to produce important data that is required for developing appropriate guidelines for environmental health.
Known as ‘belangkas’ in Malaysia, horseshoe crabs have been used by locals for food, bait and as fertilizer. Currently, these ancient mariners are gaining a lot of attention as the amoebocyte from their blood are harvested by the biomedical industry for use to detect human pathogens in patients, injectable drugs and intravenous devices.
