    The rapid urban expansion in East-Asian cities has increased the need for comfortable public spaces. This study presents field measurements and parametric simulations to evaluate the microclimatic characteristics in a university campus in the tropical climate of Kuala Lumpur, Malaysia. The study attempts to identify the thermally uncomfortable areas and their physical and design characteristics while debating on the circumstances of enhancing the outdoor comfort conditions for the campus users. Simulations in Envi-met and IES-VE are used to investigate the current outdoor thermal conditions, using classic thermal metric indices. Findings show high levels of thermal discomfort in most of the studied spaces. As a result, suggestions to improve the design quality of outdoor areas optimizing their thermal comfort conditions are proposed. The study concludes that effective re-design of outdoor spaces in the tropics, through adequate attention to the significant impacts of shading and vegetation, can result in achieving outdoor spaces with high frequency of use and improved comfort level.

    Significant changes have occurred in relation to how chronic asthma is being treated. Emphasis has now shifted from viewing asthma as a condition of smooth muscle dysfunction to one of chronic inflammation. As such, anti-inflammatory therapy forming the cornerstone of treatment represents the first important milestone in the evolution of asthma treatment. For this purpose, inhaled corticosteroid (ICS) is by far the most effective anti-inflammatory therapy. Another important milestone is the recognition of the superiority of adding long-acting beta2-agonist (LABA) to ICS over escalating ICS dose alone or other forms of add-on therapies in treating asthmatic patients not responding to regular ICS alone. The effectiveness of adding LABA to ICS in treating asthma logically led to combining the two drugs into one single inhaler (salmeterol/fluticasone and budesonide/formoterol) that has the attractiveness of being user-friendly and ensuring that ICS is not missed out. The unique property of formoterol that allows for repetitive flexible dosing paved way to the concept of using Symbicort for both regular maintenance dosing and as required rescue medication. This revolutionary approach has been recently shown to provide improved asthma outcome, achieved at an overall lower or at least comparable corticosteroid intake, and may represent another evolutionary step in the treatment strategy of chronic asthma.     A study of 50 healthy nurses from the University Hospital showed that 72% had chronic headache. Among those who had chronic headache, 33% were due to migraine. Another 30% were probable migraine subjects and 33% suffered from tension headache. Twenty two nurses had headache more than once a month and 18 nurses described the headache as moderate to severe. The common precipitating factors mentioned were tension, exposure to the sun, lack of sleep and menstruation.

    Various pharmacy services are offered in public health facilities, ranging from distributive activities (dispensing) to patient-oriented services (pharmaceutical care). These activities are monitored through indicators established at the national level. In Malaysia, the indicators have not been transformed into a measurement of hospital pharmacy service efficiency. The main objectives of this study were to assess the relative performance of hospital pharmacy services and to investigate the factors that may affect the performance levels. Double-bootstrap data envelopment analysis was applied to measure the technical efficiency levels of 124 public hospital pharmacies in 2014. An input-oriented variable returns to scale model was adopted in the study, while bootstrap truncated regression was conducted to identify the factors that may explain the differences in the efficiency levels. The average bias-corrected technical efficiency score varies according to the hospital size (0.84, 0.78 and 0.82 in small, medium and large hospitals, respectively). The hospital size, hospital age, urban location and information technology are important determinants of the efficiency levels. The study contributes to establishing baseline technical efficiency information for public hospital pharmacy services in Malaysia. The measurement of hospital pharmacy efficiency can guide future policy making to improve performance and ensure the optimum level of available resources.

    BACKGROUND: In Malaysia, the domestic water buffaloes (Bubalus bubalis) are classified into the swamp and the murrah buffaloes. Identification of these buffaloes is usually made via their phenotypic appearances. This study characterizes the subspecies of water buffaloes using karyotype, molecular and phylogenetic analyses. Blood of 105 buffaloes, phenotypically identified as swamp, murrah and crossbred buffaloes were cultured, terminated and harvested using conventional karyotype protocol to determine the number of chromosomes. Then, the D-loop of mitochondrial DNA of 10 swamp, 6 crossbred and 4 murrah buffaloes which were identified earlier by karyotyping were used to construct a phylogenetic tree was constructed.

    RESULTS: Karyotypic analysis confirmed that all 93 animals phenotypically identified as swamp buffaloes with 48 chromosomes, all 7 as crossbreds with 49 chromosomes, and all 5 as murrah buffaloes with 50 chromosomes. The D-loop of mitochondrial DNA analysis showed that 10 haplotypes were observed with haplotype diversity of 0.8000 ± 0.089. Sequence characterization revealed 72 variables sites in which 67 were parsimony informative sites with sequence diversity of 0.01906. The swamp and murrah buffaloes clearly formed 2 different clades in the phylogenetic tree, indicating clear maternal divergence from each other. The crossbreds were grouped within the swamp buffalo clade, indicating the dominant maternal swamp buffalo gene in the crossbreds.

    CONCLUSION: Thus, the karyotyping could be used to differentiate the water buffaloes while genotypic analysis could be used to characterize the water buffaloes and their crossbreds.

    To identify the potential zones for gold mineralization at the Felda Chiku 3, Gua Musang, Kelantan, East coast Malaysia, twenty-one (21) geophysical survey lines were conducted at the proposed mineral exploration site using the pole - dipole of the electrical resistivity and induced polarization arrays to get the maximum depth of 150 m with 400 m survey length. From the resistivity and chargeability concentration maps, the potential mineralized zones as delineated, was observed to be dominantly concentrated towards the southwest and northern part of the area. The 3D resistivity and chargeability slice model present low resistivity values and high chargeability values that are well correlated which is palpable especially at the depths of 25 m and 50 m respectively. The data showed that the potential mineralized zones are trending approximately north-south directions. Forty (40) drilling locations were proposed for follow-up drilling based on the resistivity and chargeability models.

    The knowledge of 16 medical officers and 21 house officers from the Department of Medicine regarding the technique of using the pressurised metered dose inhaler (MDI) was evaluated. The doctors were interviewed individually and each was requested to demonstrate the use of a placebo inhaler. Only 21.6% of the participants correctly performed all the ten steps considered to constitute correct inhalation technique. The most common mistake was failure to actuate the inhaler at the start of breathing in through the mouth. Although inhaler technique was better among the medical officers, all of whom claimed to have instructed patients on the use of pressurised MDIs, house officers who were also involved in patient instruction performed equally well. Unfortunately, doctors with inadequate technique had also instructed patients and this could be one of the causes of incorrect technique among patients.     In Malaysia, chest radiograph is a part of compulsory investigations during routine medical examination. Majority of these chest radiographs are interpreted by medical officers at the outpatient clinic. This study was to determine the concordance of chest radiograph interpretations between medical officers and radiologist. Medical officers' report of routine chest radiographs at the outpatient clinic Hospital Universiti Sains Malaysia over a 6-month period were compared to that of a radiologist. Of 408 chest radiographs reported, the concordance of interpretation between medical officers and radiologist was 93.6% (382 of 408). Medical officers correctly interpret 98.2% (378/385) of normal chest radiograph compared to 17.4% of abnormal chest radiographs (4/23). Therefore, routine chest radiograph interpretations of normal radiographs by medical officers were generally accurate. However, they were weak in interpreting abnormal chest radiographs. Medical officers need to be trained on detecting abnormalities on chest X-ray since most routine chest X-ray reporting is done based on their interpretation.

    Investment funds are growing in Malaysia since people are more knowledgeable about
    investments and aware of investment opportunities in order to secure good savings for the
    future. These investments include unit trusts, gold, fixed deposits, stock prices and property
    investments. It is essential for individuals or organisations to know the value of future share
    prices of their investment portfolio in order to predict the profit or loss in the future. The
    purpose of study is to identify the best duration of historical data and forecast days in order
    to accurately forecast share prices. The study uses Geometric Brownian Motion model in
    forecasting share prices of companies in Bursa Malaysia. This study focused on 40 listed
    companies in Bursa Malaysia from the top gainers list. It was found that 65 historical days
    could forecast the share prices for 21 days accurately.

    The aim of this study was first to analyse the prescribing habits of primary care doctors with a view to providing feedback which may help them to rationalise their prescribing. This analysis was helped by comparing the prescribing practices in two different settings and thus highlighting anomalous differences. The second aim of this study was to obtain data on the diagnoses being made in primary care settings in Malaysia as this information, though available from other countries, is limited here. Lists of the most commonly prescribed drugs and most common diagnoses made are provided, together with tables showing the most commonly prescribed drugs for the ten most common diagnoses. Differences in prescribing habits between the two settings are discussed and possible reasons are suggested.

    Ultimately, the majority of our medical graduates ends up in primary health care either in private practice or in the government service. It would be appropriate that their education and training should meet not only the requirements of their eventual vocation, but just as importantly, the expectations of a more discerning community at large. Rising pressures on the profession to provide more cost-effective and affordable health services of good quality would put an increasing emphasis on the development and promotionof primary health care to higher standards. Primary health care workers would be hard-pressed to provide more health information and to actively participate in disease prevention and control as part of their professional duties. As medical specialisation and technology contibute towards more fragmented, complex and dehumanising forms of practice, the greater will be the need for the integrative skills of the primary physician providing personal and continuing care. The future nature of medical care will have to respond to this community demand.

    Cogeneration systems are extensively used in Malaysia to produce power as a primary
    source. However, in the event of cogeneration system failure, the customer or the client are
    forced to use a redundancy to avoid power interruptions. There are two methods commonly
    used as a backup in the cogeneration systems which are Generator set and public utility. In
    order to choose the best redundancy for a particular cogeneration system, it is essential to
    evaluate the economic benefit analysis by considering several factors such as Maximum
    demand charge, installation cost and Discount interest. In the evaluation of economic
    benefit, this study identifies the number of failure and associated downtime using reliability
    and availability approach, and then present value method was applied. The result shows
    that the usage of public utility as redundancy is beneficial if the cogeneration system
    operates within five years period. However, if the cogeneration system operates more than
    five years, generator set option would be a better option to minimize the total cost. This
    research also addresses the effect of various factors such as installation cost, maximum
    demand charge, fuel cost, discount interest rate and production capacity. In general, the
    output of the research would be beneficial for the plant operator to select the appropriate
    redundancy option based on the economic advantages.

    : Concrete failure will lead to serious safety concerns in the performance of a building structure. It is one of the biggest challenges for engineers to inspect and maintain the quality of concrete throughout the service years in order to prevent structural deterioration. To date, a lot of research is ongoing to develop different instruments to inspect concrete quality. Detection of moisture ingress is important in the structural monitoring of concrete. This paper presents a novel sensing technique using a smart antenna for the non-destructive evaluation of moisture content and deterioration inspection in concrete blocks. Two different standard concrete samples (United Kingdom and Malaysia) were investigated in this research. An electromagnetic (EM) sensor was designed and embedded inside the concrete to detect the moisture content within the structure. In addition, CST microwave studio was used to validate the theoretical model of the EM sensor against the test data. The results demonstrated that the EM sensor at 2.45 GHz is capable of detecting the moisture content in the concrete with linear regression of R² = 0.9752. Furthermore, identification of different mix ratios of concrete were successfully demonstrated in this paper. In conclusion, the EM sensor is capable of detecting moisture content non-destructively and could be a potential technique for maintenance and quality control of the building performance.

    History A 58-year-old woman was seen in the rheumatology clinic for bilateral wrist and knee pain that was unresponsive to physiotherapy and intra-articular steroid injections. Remote fracture of the left tibia from a motor vehicle collision was reported and was previously treated with conservative management. Serologic work-up for inflammatory disease was negative. The patient reported no prior surgical or medical history. Social history revealed remote immigration from Malaysia. Radiographs of the hands and knees were obtained.

    Blood alcohol levels from 155 UKM forensic postmortem cases of various causes of death from August 1988 to mid-September 1989 were studied. 59 cases (38%) were related to fatal road traffic accidents. 13 of these 59 cases (22%) showed blood alcohol levels of more than 50 mg/dl. 11 of these 13 cases (84.6%) revealed blood alcohol levels of more than 100 mg/dl after correction. Further analysis showed 53.8% were Indian, 76.9% aged between 20 and 40 years, 84.6% of the accidents occurred between 8.00 p.m. and 4.00 a.m. and all the alcohol related fatal road traffic accident victims studied in this series were males. This study provides helpful information.
