The present study explored for the first time the blood-based proteomic signature that could potentially distinguish older adults with and without cognitive frailty (CF). The participants were recruited under the Malaysian Elders Longitudinal Research (MELoR) study. Cognition and physical frailty were determined using the Montreal Cognitive Assessment (MoCA) and Fried's criteria, respectively. The differential protein expression in the blood samples (38 CF vs 40 robust) were then determined using the Sequential Window Acquisition of All Theoretical Mass Spectra (SWATH) analysis. A total of 294 proteins were found to be differentially expressed in the CF group as opposed to the robust group. Considering proteins with fold change (FC) ≥  ± 2 and p-values  78%, specificity > 75%, accuracy > 80% and area under the curve (AUC) > 0.8. The major biological pathways that could be potentially dysregulated by the nine proteins were associated with lipid metabolism and the retinoid system. The present findings warrant further validation in future studies that involve a larger cohort.

Recent fieldwork in eastern Sabah led to the discovery of new little-known crickets from the subfamily Pteroplistinae (Orthoptera, Grylloidea). Three new species are described: Tembelingiola biaculeata sp. nov. and Tembelingiola kabili sp. nov. from Sepilok; Pteroplistes silam sp. nov. from Mount Silam. Previously, the calling song of only one species of Pteroplistinae was known. Here, the calling songs of the three new species, as well as that of Pteroplistes lagrecai Gorochov, 2004, are also described for the first time. Therefore, distinctive calling songs for five species and three genera of Pteroplistinae are presently known.

INTRODUCTION: Intracranial large artery atherosclerosis (ICAS) is a common cause of stroke in Asia. Prior epidemiological publications on ICAS have largely been single-country reports. This collaborative study involving several Asian countries aims to investigate the characteristics and outcomes of patients with stroke attributable to ICAS.

METHODS: This is an international, multicenter, observational, prospective cohort study of patients admitted with acute ischemic stroke secondary to ICAS to stroke centers in six Asian countries. Stroke due to ICAS was diagnosed when there was a ≥50% intracranial large artery stenosis ipslateral to a non-lacunar infarct, without significant ipsilateral extracranial stenosis, cardiac cause or other mechanism found for the stroke. Data were collected on patient demographics, vascular risk factors, stroke location, and severity. Outcomes of interest were stroke recurrence and mortality at 12-month follow-up.

RESULTS: A total of 356 patients were recruited. Mean age was 62.7 ± 13.8 years, and 39.9% were females. Mean NIHSS on admission was 9 ± 8, with majority of patients having mild (39.3%) or moderate (37.9%) strokes. Stroke recurrence was 6.7% (95% CI: 4.4-9.9%) while mortality rate was 13.2% (95% CI: 9.9-17.2%) within 1 year. The risk of stroke recurrence was associated with increasing age (odds ratio [OR]: 1.04, 95% CI: 1-1.06, p = 0.05) and hypertension (OR: 3.23, 95% CI: 1.09-9.61, p = 0.035). Mortality was associated with age (OR: 1.05, 95% CI: 1.01-1.08, p = 0.006) and NIHSS (OR: 1.12, 95% CI: 1.07-1.17, p < 0.001).

CONCLUSIONS: This multicenter Asian study demonstrates a high risk of stroke recurrence and mortality among patients with acute stroke due to ICAS. They are associated with age (both), as well as hypertension (for recurrence) and NIHSS (for mortality). Better treatment modalities are needed to reduce the frequency of adverse outcomes in symptomatic ICAS.

Wastewater treatment has been regarded as an effective solution in lowering the potential environmental hazards caused by palm oil mill effluent (POME). To ensure the efficient remediation of POME, the implementation of a promising strategy is necessary to overcome the limitations of conventional water treatment methods for the treatment of this pollutant. In this study, the synthesis of carbon, nitrogen codoped titanium dioxide nanoparticles (C, N-TiO2 NPs) was successfully performed by a sol-gel approach for the treatment of POME as a model pollutant under solar light irradiation. The synthesized C, N-TiO2 NPs displayed unique characteristics including an anatase phase with an average crystallite size of 11.35 nm and irregular spherical structures. Additionally, C, N-TiO2 possessed a lower band gap energy of 2.95 eV than 3.2 V of bulk anatase TiO2 and slower electron-hole (e--h+) pair recombination rate as evidenced by photoluminescence (PL) studies. The adsorption isotherm study of POME was most compatible with the Langmuir isotherm model, and the POME degradation kinetics proceeded according to first-order kinetics. Accordingly, the photocatalytic degradation of POME displayed a maximum degradation efficiency of 100% under the optimum condition of pH 7 in the presence of 0.12 g of the C, N-TiO2 photocatalyst within 150 min. The scavenging study showed that the superoxide radical (•O2 -) was the primary active species in the POME photodegradation. Finally, the reusability analysis confirmed that the C, N-TiO2 NPs could be reused for a maximum of five cycles, making them promising photocatalysts for wastewater treatment.

BACKGROUND AND AIMS: Despite the known association of hip dysplasia and Charcot Marie Tooth disease (CMT), evidence is limited regarding its exact prevalence. Available studies pre-date genetic confirmation of CMT subtypes and current hip reconstruction surgical options. This study examined the prevalence of hip dysplasia in CMT in a tertiary neuromuscular center.

METHODS: This was a retrospective study of children with CMT who had at least one pelvic radiograph between 2000 and 2020. Reimer's migration percentage, acetabular index and lateral center edge angle were used to identify hip dysplasia.

RESULTS: A total of 178 children were included with a median age of 6.4 (IQR 3.4-11.3) years at CMT diagnosis. First pelvic radiographs were performed at a median age of 8.0 (IQR 4.6-12.2) years and 64 (35.8%) had hip dysplasia, of which 20 normalized over time. Repeat radiographs were done in 96/178 children (53.9%), and six children with originally normal radiographs developed later radiographic hip dysplasia. At the time of last follow up, 50/178 children (28.1%) had hip dysplasia and 17/178 children (9.6%) required surgical intervention. The frequency of hip dysplasia in specific CMT subtypes was: 28/100 in CMT1A, 5/7 in Dejerine-Sottas disease, 3/10 in CMT2A, and 4/4 in TRPV4-related CMT.

INTERPRETATION: The prevalence of hip dysplasia in children with CMT in this cohort was estimated to be between 9.6% and 28.1%. Serial imaging is important to monitor outcomes into adulthood. Specific CMT subtypes were more likely to be associated with hip dysplasia.

BACKGROUND: Various barriers lead to a shortage of organs for transplantation in Malaysia. One drive to improve the organ donation rate operates through future healthcare practitioners and practitioner advocacy. This scoping review was carried out to establish and summarise findings about organ donation-related articles among the public, health sciences students and health personnel. A further aim was to synthesise the latest data on knowledge and attitudes towards organ donation in the Malaysian population.

METHODS: PubMed, Scopus, Google Scholar and the Malaysian Medical Repository (MyMedR) were used for a search conducted up to May 2022. Relevant search terms included 'Organ donation' and 'Malaysia'. Journal articles related to knowledge, attitudes and intention were grouped under the general public and health science. Students and health personnel were included. Eligible studies were reviewed by two independent reviewers. Any disagreements were resolved by consensus with a third reviewer.

RESULTS: The 31 included articles revealed an increased level of awareness among the public regarding organ donation. The analysis identified that nonrecognition of brainstem death (38.5%), no knowledge of how to contact the Organ Transplant Coordinator (82.3%) and never approaching the families of a potential donor (63.9%) led to a lack of confidence among healthcare practitioners to promote organ donation.

CONCLUSION: The shortage of organ donors is the result of the failure to identify the expected donor, obtain consent and procure the organs due to the passivity of Malaysian health professionals in promoting the organ donation process.

Oroxylum indicum possesses promising flavonoid secondary metabolites. However, translation of these compounds into clinical practice for neurological disease treatment is halted as the toxicity and safety profile of the plant extracts are yet to be determined. This study was conducted to assess the acute oral toxicity and subacute neurotoxicity that could be imposed by the flavonoid-enriched fraction (FEF) extracted from O. indicum leaves, by strictly following the procedures set in Organization for Economic Co-operation and Development (OECD) Guidelines No.420 and 424. It was found that at the highest dosage (2000 mg/kg b.wt), no death or toxicity-related behavioral changes were observed. No significant alteration in hematological and serum biochemical parameters beyond the standard laboratory range was observed. Detailed histopathological examination, as verified by clinical pathologist, revealed absence of detectable inflammation, changes in any macroscopic and microscopic tissue abnormalities in all vital organ of the treated rats. Moreover, neurological functional test of rats treated with repeated doses of FEF for 28 d also showed absence of neurological abnormality, suggesting negative long term side effects of this fraction on the animal. In conclusion, this study presented valuable pre-clinical safety validation of a high-quality herbal medicinal product, setting the foundation for its future application in clinical setting.

Recently, the phenomenon of population ageing and its impact on the insurance industry has garnered increasing global attention. However, a notable gap in scholarly research persists in understanding the nuanced effects of ageing on consumer behaviour and insurance purchase intentions. This study maps the current academic evidence on how ageing influences individual consumers' insurance decisions. Using a scoping review methodology aligned with the Preferred Reporting Items for Systematic Reviews and Meta-Analysis extension for scoping reviews and Joanna Briggs Institute guidelines, 44 articles out of 1082 from four databases-Web of Science, Scopus, ScienceDirect, and Emerald Insight-are reviewed. The results reveal a rising interest in this research area, with China emerging as a significant contributor. The focus is predominantly on Theory of Planned Behavior, quantitative methods, questionnaire survey, regression analysis, older population, and general health insurance. Variables capturing the impact of ageing, beyond demographic information, include family-related, risk-related, and expectation-related factors. This study highlights the current state of research on ageing's effect on insurance purchase intentions and offers valuable insights and directions for future research.

Wound healing is a natural process to restore damaged tissues due to loss of tissue integrity. Moringa oleifera (locally known as merunggai in Malaysia) has been traditionally used in various ailments, including for wound management. To evaluate the wound healing properties in M. oleifera, publications were searched and selected following the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement with predetermined inclusion criteria. The databases searched for primary studies include PubMed, Google Scholar, Science Direct, LILACS, ClinicalTrials.gov, and CENTRAL. In total, 18 in vivo studies were included, which involved the leaves, while the remaining 5 studies involved other plant parts tested on excision, incision, dead space, abrasion, and burn-induced wound models. All studies reported significant wound healing abilities. Most studies used different topical formulations of aqueous leaves extract. The accumulation of collagen content and underlying wound healing mechanism through antimicrobial, antioxidant, and anti-inflammatory activities may be contributed by its bioactive phytochemical content, which has the potential to accelerate the wound contraction, increase the rate of epithelialization, and protect tissues against oxidative damage. In conclusion, M. oleifera showed wound healing potential but further studies are warranted to determine the main bioactive phytocompounds and safety.

Liquid oils are typically used to dilute solid fat in aerated emulsions, yet the structure of lipid components determines their functional properties. This study investigates the mechanism of liquid diacylglycerol (DAG) and triacylglycerol (TAG) on the whipping capabilities of aerated emulsions from the perspective of fat crystal- membrane interactions. Although there were no significant differences in thermodynamic properties, DAG significantly delayed the reduction in lamella thickness of fat crystals compared to TAG, thereby maintaining the density of the fat crystal network at high liquid oil levels. Additionally, the extra hydroxyl group in DAG, compared to TAG, enabled DAG-rich fat globules to occupy the air-liquid interface more rapidly, thereby significantly enhancing the occurrence and development of partial coalescence during whipping. Therefore, the whipping capabilities of aerated emulsions rich in DAG were greatly improved. This study enhances understanding of structural lipids in aerated emulsions and offers new insights into improving whipping capabilities.

Diacylglycerol (DAG), a functional lipid with two isomers, exhibits distinct physical properties that affect oil systems. This study investigated the crystallization behaviors and physical property stabilities in binary systems composed of sn-1,3 DAG and its migration equilibrium product (ME-DAG, with an sn-1,3 DAG:sn-1,2 DAG ratio of 65:35) at mass ratios of 20 %, 50 %, and 80 % with palm kernel oil (PKO). The results revealed that the sn-1,3 DAG/PKO systems exhibited faster crystallization rates and more pronounced eutectic effects than the ME-DAG/PKO systems. In addition, sn-1,3 DAG inhibited the polymorphic transformation (α → β' → β) in the DAG/PKO binary systems. Higher DAG concentrations enhanced the crystallization and nucleation rates. Furthermore, sn-1,3 DAG at 50 % and 80 % notably reduced post-hardening and moderated the slope of the solid fat content curve. Microscopic analysis revealed the presence of Maltese cross crystals and disordered acicular crystals in the DAG/PKO binary systems. These findings underscore the critical role of DAG acyl migration in compatibility, crystallization kinetics, polymorphism, and texture, offering insights for developing functional plastic lipids with tailored properties.

Antimicrobial resistance (AMR) poses a formidable challenge in global healthcare, driving the exploration of natural products for novel antimicrobials. Among these, essential oils (EOs) derived from medicinal plants are rich sources of diverse bioactive compounds. Monoterpenes and monoterpenoids, critical constituents of EOs, have emerged as promising agents in combating multidrugresistant (MDR) pathogens. This review analyzed recent literature on the efficacy of monoterpenes against AMR, highlighting their broad-spectrum activity and potential as alternative therapeutic options for MDR infections. Mechanistic insights reveal their ability to disrupt cell membranes, inhibit biofilm formation, and modulate gene expression linked to virulence and resistance, thereby reducing microbial viability through alterations in membrane potential, enzymatic activity, and genetic regulation. Synergistic interactions between monoterpenes and conventional antibiotics are also elucidated. Innovative approaches in monoterpene research are explored, although challenges such as resistance, limited solubility, volatility, and potential toxicity are acknowledged, emphasizing the need for advanced formulation strategies and interdisciplinary research. The synergy observed with conventional antibiotics, coupled with their ability to target specific microbial resistance mechanisms, underscores the potential of monoterpenes in combating antibioticresistant infections. Future investigations should prioritize optimizing monoterpenes' therapeutic properties and assessing their safety profiles to fully exploit their potential in addressing AMR.

Cold argon plasma (CAP) effectively modifies casein (CN) structures by cleaving peptide chains and altering allergenic epitopes. This study assessed the allergenicity of CAP-treated CN in KU812 cells and BALB/c mouse models, supported by a multiomics approach integrating 16S rDNA sequencing, serum metabolomics, and jejunal transcriptomics. CAP treatment reduced CN allergenicity, evidenced by decreased KU812 cell degranulation, alleviated allergic responses in mice, and a Th1/Th2 balance shift toward Th1 dominance. Furthermore, CAP-treated CN restored the gut microbiota equilibrium, increasing the number of beneficial bacteria. Multiomics analysis highlighted its impact on lipid metabolism pathways, with Zbp1 and Hbb-bt identified as potential regulators of allergic responses. These findings underscore the potential of cold argon plasma as an innovative strategy to reduce food allergenicity through multifaceted physiological mechanisms, offering promising therapeutic applications in food allergy management.

Introduction. Tumor-related epilepsy is a prevalent condition in patients with gliomas. Accurate prediction of epilepsy is crucial for early treatment. This study aimed to evaluate the novel application of the eXtreme Gradient Boost (XGBoost) machine learning (ML) algorithm into a radiomics model predicting preoperative tumor-related epilepsy (PTRE). Its performance was compared with 4 conventional ML algorithms, including the least absolute shrinkage and selection operator (LASSO), elastic net, random forest, and support vector machine.Methods.This study used four magnetic resonance imaging (MRI) images consisting of four sequences (T1-weighted [T1W], T1-weighted contrast [T1WC], T2-weighted [T2W], and T2-weighted fluid-attenuated inversion recovery [T2W FLAIR]) acquired from 74 glioma patients, 30 with PTRE and 44 without PTRE. 394 radiomics features were extracted from the MRI scans usingPyradiomics, alongside 12 clinical features from the medical records. The ML algorithms were mixed and matched to create 20 radiomics models with two stages for: (1) feature selection and (2) prediction of PTRE. Nested cross-validation was used to tune the algorithms and select the stable features.Results.The XGBoost radiomics model demonstrated the second-highest balanced accuracy and F1-score of 0.81 ± 0.01 and 0.80 ± 0.01 respectively. It also achieved the highest recall of 0.81 ± 0.02. It used mostly textural radiomics features from the T1W, T2W and T2W FLAIR sequences to make the predictions.Conclusion.This study demonstrates that XGBoost is a viable alternative to conventional ML algorithms for developing a radiomics model to predict PTRE, as the model produced from XGBoost had among the highest metrics. XGBoost selected features with a higher predictive value than other models. The features selected by XGBoost were more stable, which is a useful property for radiomics analysis. Features selected from multiple MRI sequences were important in the model's decision.

This study aims to validate the accuracy of low-cost fitness smartwatches by comparing their data with gold-standard measurements for cardiovascular and physical activity parameters. The study enrolled 50 subjects, 26 undergoing validation testing for heart rate, blood oxygen saturation (SpO2), and sleep data against polysomnography (PSG). Additionally, 24 subjects participated in the 3-Minute Walk Test (3MWT) and Stairs Climbing (SC), with step counts validated against manual video calculations. Results showed no significant difference between the device's measurements and gold standard values for shallow sleep, deep sleep, REM time, mean heart rate, minimum heart rate, and SpO2. However, the device significantly underestimated manually counted steps (p = 0.009 (3MWT); p = 0.012 (SC)), total sleep duration (p = 0.004), and wake time (p = 8.94 × 10-8) while overestimating maximum heart rate (p = 0.011). These findings highlight the importance of accurate validation and interpretation of wearable device data in clinical contexts. Given these limitations, excluding the device's readings in future analyses is recommended to maintain data reliability and research integrity. This study underscores the need for ongoing validation and improvement of wearable technology to ensure its reliability and effectiveness in healthcare.

The investigation of microRNAs (miRNAs) for the purpose of identifying biomarkers and new treatments for breast cancer has been gaining traction from scientists in recent years. Of all the miRNAs, miR-155 has been reportedly involved in breast cancer development as it regulates various cellular processes such as glucose uptake, proliferation, metastasis, and migration. Various efforts have been done towards researching miR-155 as a biomarker in breast cancer; however, the results were varied. The objective of the current systematic review is to compile and summarize information regarding miR-155 as a potential diagnostic biomarker for breast cancer. All eligible studies were found from SCOPUS and PubMed databases. Out of the 376 potential eligible records, only 26 original articles were selected for further assessment according to inclusion and exclusion criteria. The expressions of miR-155 in serum, plasma, biopsy, urine, nipple aspirate fluid, serum exosomes, and peripheral blood mononuclear cells were recorded and analyzed. Besides that, the expression of miR-155 was also correlated to clinicopathological features in breast cancer patients. The area under the curve (AUC) values from receiver operating characteristic (ROC) analysis used to evaluate diagnostic sensitivity and specificity of miR-155 as a diagnostic biomarker were also recorded. The limitations such as the small sampling size, the unemployment of internal controls for quantitative real-time polymerase chain reaction (RT-qPCR), and inconsistency of sensitivity as well as specificity values of miR-155 as a biomarker have been discussed. The present study proposed that miR-155 is a good diagnostic biomarker for breast cancer; however, further clinical research is required to assess the validity of miR-155 as a potential biomarker to translate the research outcomes into clinical practice.

BACKGROUND: Intention toward infidelity or extramarital affairs could be caused by sex life dissatisfaction and low relationship satisfaction. Although sex is just a tiny part of a marriage's relationship, the importance of sex life in a relationship cannot be easily underestimated. Past studies have found some important associations between sex life and relationship satisfaction in determining whether a person would decide to engage in marital infidelity. Nonetheless, the underlying mechanism of the association between sex life satisfaction and intention toward infidelity is unexplored in the Malaysian context. Therefore, this study aimed to examine the mediating role of relationship satisfaction in its direct association between sex life satisfaction and intention toward infidelity among Malaysian married individuals.

METHOD: A total of 292 married individuals (Mage=42.98; SDage=9.82; 66.4% women; 65.4% Chinese Malaysians, 15.8% Indians Malaysians, 14.7% Others Ethnic Malaysians, and 4.1% Malay Malaysians) participated in the cross-sectional quantitative online survey.

RESULTS: A significant positive association was found between sex life and relationship satisfaction. However, sex life and relationship satisfaction were negatively associated with intention toward infidelity. Meanwhile, relationship satisfaction significantly mediates the link between sex life satisfaction and intention toward infidelity after controlling for the effects of age and gender. These findings support the hypothetical mediating role of relationship satisfaction in the association between sex life satisfaction and intention toward infidelity.

CONCLUSIONS: This study may fill a knowledge gap in Malaysia about the underlying association among these variables as well as the practical implication of the importance of sex life and relationship satisfaction in determining whether one would engage in extramarital affairs.
