    Plunging epidermoid cyst of the floor of mouth is indeed an uncommon entity. A 34-year old Malay
    gentleman had presented to our centre with a floor of mouth lesion that extended into the submental
    region. Clinical findings and imaging studies pointed towards the impression of a plunging ranula.
    Histopathological examination of a completely excised mass via intraoral approach, confirmed the
    diagnosis of an epidermoid cyst. He was well on follow up with no subsequent recurrence. We discuss
    the nature of epidermoid cyst and its surgical management.

    Ergonomics is generally to design for human use and optimizing the working and living conditions. The comfort, safety and ease of use of systems, products and machines require the principles of ergonomics to be incorporated into the design. Anthropometry is a part of ergonomics which deals with the measurement of body dimensions, its capability and limitations of the human ability. Studies have indicated that body dimensions differ for various populations, depending on ethnic group, sex and age. The measurement of physical characteristics and abilities of people provides information that is essential for the appropriate design of occupational and non- occupational environments, as well as for the design of consumer products, clothing, tools and equipment. It has been considered as the very basic core of ergonomics in an attempt to resolve the dilemma of 'fitting people to machines'. It has also been regarded as a branch of anthropology which deals with physical characteristics of the man in time and space, particularly with individual variation, ontogenesis and generic development. Knowledge about man from the point of view of physical anthropometry is very useful for the purpose of ergonomics and design. Thus it is important that an anthropometric database for each nation is developed to ensure design of products and systems are compatible with the user population. In Malaysia, many researchers have conducted anthropometric studies on various groups of people, focused on a small sample size and used for specific designs. Unfortunately, the measurements are not representatives of the population. The work has been individually based, confined to certain group of researchers in various universities and there is no concerted effort towards developing a national anthropometric database.This paper discusses some of the anthropometry work conducted in Malaysia, the problems and challenges in developing the national database, the importance of anthropometry and the roles that the government has to play in developing the database.

    Objective: To determine the validity and reliability of the Chinese parent proxy and child self-report
    health related quality of life measure for children with epilepsy (CHEQOL-25) in Malaysia. Methods:
    Face and content validity of the Chinese parent proxy and child self-report CHEQOL-25 was verified
    by an expert panel, and piloted in five children with epilepsy (CWE). The Chinese CHEQOL-25 was
    then administered to 40 parent proxies and their CWE (aged 8-18 years), from two tertiary hospitals,
    at baseline and 2 weeks later. Results: Forty parents and their CWE were recruited. Cronbach’s alpha
    for each subscale ranged from 0.56-0.83. At test-retest, the interclass correlation for all items ranged
    from 0.68-0.97. Items 8 and 25 were removed as their corrected item-total correlation values were

    This study aims to identify the relationship between dietary intakes of β-carotene with risk of oral cancer.
    Methods: A hospital-based, case-control study was conducted on 306 Malaysians who seek treatment at participating centres/hospitals. Subjects selected from the Malaysian Oral Cancer Data and Tissue Banking System (MOCDTBS) consisted of 153 cases and 153 controls that were matched for gender, age (±5 years) and ethnicity. Food consumption was measured using Food Frequency Questionnaire (FFQ). NutrieMart Version 2.0.0 software was used to estimate daily nutrient of each subject from the FFQ. Logistic Regression analysis was conducted to compute the odds ratio (OR) for intakes of β-carotene and oral cancer risk.
    Results: Intake of β-carotene was found to be not associated with risk of oral cancer (OR 0.83, 95%CI: 0.42-1.66, p>0.05).
    Conclusion: No significant association was found between dietary intakes of β-carotene with oral cancer risk in this study population.

    Teachers face one of the highest demands of any professional group to use their voices at work. Thus, they are at
    higher risk of developing voice disorder than the general population. The consequences of voice disorder may have
    impact on teacher’s social and professional life as well as their mental, physical and emotional state and their
    ability to communicate. Objectives of this study are to determine the prevalence of voice disorder and the
    relationship between voice disorder with associated risk factors such as teaching activities and lifestyle factors
    among primary school teachers in Bintulu, Sarawak. A cross sectional study was conducted based on random sample
    of 4 primary schools in Bintulu, Sarawak between January-March 2014. A total of 100 full-time primary school
    teachers were invited to participate in the study. Data were collected through a self-administered questionnaire
    addressing the prevalence of voice disorder and potential risk factors. Descriptive analysis and chi-square test was
    used to measure the relationship between voice disorder and associated risk factors. The response rate for this study
    was 78% (78/100). The study found that the prevalence of voice disorder among primary school teachers in Bintulu,

    Posture is one of the most important factor that need to be considered in any postural analysis. Awkward, extreme,
    and repetitive postures can increase the risk of musculoskeletal disorders (MSDs). As observational methods are more
    widely used than instrumentation-based methods to assess postural problems, this study reviews and assesses the
    scientific literature of observational methods and focused on pen and paper based specifically. In order to identify the
    published methods, a list of English or Malay articles dating as far back as 1990 was compiled from PubMed, Science
    Direct and Google Scholar. The keywords were ergo*, posture*, method*, observational*, postural problems*, pen and
    paper*, posture analysis*, indirect* and macro-ergo*. In addition, a secondary search was also performed using
    bibliography of retrieved articles so that additional papers for conducting review and evaluations can be collected. A
    total of 121 articles that assessed postural problems in working activities were found. However, after intensive
    screening process only 6 articles were selected to be further analyzed. Posture of upper arms/shoulder, lower arms/
    elbow, wrist, neck, back/trunk and leg were highlighted in this study. The limitations and the strengths of the
    published pen and paper based observational method focusing on those postures were also discussed. The finding of
    this review will benefit researchers in the process of understanding unsafe posture in workplace. It could also provide
    to researcher on how to improve the current pen and paper based observational method for assessing postural
    problems.

    The number of people above 60 years of age will increases due to the growth of the population in the first half of the 21st century, particularly in Malaysia. This has led to the growing number of older drivers which is often unavoidable as driving is a necessary task that not only achieves mobility but also shows a sign of independence and improves self-esteem, which are essential towards effecting the lifestyle. This paper briefly looks at common elderly related visual impairment, reviews design approaches and tools that tackle or include these issues in the design process and further outlines new research needs. It is the intention of this paper to steer the research direction of the development of future design approach and tools that would enhance the capabilities of designers to be well prepared to cater for inclusive design for the elderly, more specifically for visual impairments issues.

    The contribution of microbial depolymerase has received much attention because of its potential in biopolymer degradation. In this study, the P(3HB) depolymerase enzyme of a newly isolated Burkholderia cepacia DP1 from soil in Penang, Malaysia, was optimized using response surface methodology (RSM). The factors affecting P(3HB) depolymerase enzyme production were studied using one-variable-at-a-time approach prior to optimization. Preliminary experiments revealed that the concentration of nitrogen source, concentration of carbon source, initial pH and incubation time were among the main factors influencing the enzyme productivity. An increase of 9.4 folds in enzyme production with an activity of 5.66 U/mL was obtained using optimal medium containing 0.028% N of di-ammonium hydrogen phosphate and 0.31% P(3HB-co-21%4HB) as carbon source at the initial pH of 6.8 for 38 h of incubation. Moreover, the RSM model showed great similarity between predicted and actual enzyme production indicating a successful model validation. This study warrants the ability of P(3HB) degradation by B. cepacia DP1 in producing higher enzyme activity as compared to other P(3HB) degraders being reported. Interestingly, the production of P(3HB) depolymerase was rarely reported within genus Burkholderia. Therefore, this is considered to be a new discovery in the field of P(3HB) depolymerase production.

    Seventy-three articles related to metabolic syndrome were found in a search through databases dedicated to indexing all literature with original data involving the Malaysian population between years 2000 and 2015. Metabolic syndrome affects 25 to 40% of adult population of Malaysia with the risk increasing with age. Obese children are also at risk. Indian ethnicity has the highest rates, followed by Malay and chinese. It was found that socioeconomics determinants such as living in urban areas, unemployment, lower income, lower education level and shift workers had higher prevalence of metabolic syndrome. Metabolic syndrome is associated with other medical conditions like cardiovascular diseases, psychiatric disorders, erectile dysfunction, polycystic ovarian syndrome and colorectal cancer. Several biomarkers have been determined to be relevant to our local population but their usage in clinical setting needs further research. Literature into effectiveness of management of metabolic syndrome in Malaysia is lacking and the results were only modest. there are several diagnostic criteria available for metabolic syndrome internationally and their individual significant to our local population is not clear. It also makes it difficult to compare results between studies using different criteria. Finally, we could not identify any local study to look at the health economic burden of metabolic syndrome locally.

    BACKGROUND: Malnutrition is common among patients with cancer, but little attention is given to its risks and consequences.
    OBJECTIVES: The aim of this study is to assess the nutritional status and identify the factors associated with malnutrition among newly diagnosed patients with cancer.
    METHODS: Patients admitted with newly diagnosed cancer at a teaching hospital in Malaysia were recruited from January to April 2015. Nutritional status was assessed before treatment initiation, and patients were classified into three categories.
    FINDINGS: A total of 132 pretreatment patients were recruited into the study. About half were severely malnourished. Patients with stage III cancer had the highest prevalence of severe malnourishment. Clinical parameters and disease characteristics were significantly associated with nutritional status. Demographic variables were also statistically significantly associated with severe nutritional status.

    Protection of medium voltage (MV) overhead lines against the indirect effects of lightning is an important issue in Malaysia and other tropical countries. Protection of these lines against the indirect effects of lightning is a major concern and can be improved by several ways. The choice of insulator to be used for instance, between the glass, ceramic or polymer, can help to improve the line performance from the perspective of increasing the breakdown strength. In this paper, the electrical performance of a 10 kV polymer insulator under different conditions for impulse, weather and insulator angle with respect to a cross-arm were studied (both experimental and modelling) and the results were discussed accordingly. Results show that the weather and insulator angle (with respect to the cross-arm) are surprisingly influenced the values of breakdown voltage and leakage current for both negative and positive impulses. Therefore, in order to select a proper protection system for MV lines against lightning induced voltage, consideration of the local information concerning the weather and also the insulator angles with respect to the cross-arm are very useful for line stability and performance.

    Lagaropsylla signata (Wahlgren, 1903), previously known only from the Island of Java, Indonesia is redescribed and reported for the first time in Deer Cave, Gunung Mulu National Park, Sarawak, Malaysia (west coast of Borneo). Many were found clinging to the earwig Arixenia esau Jordan, 1909. A similar account of a phoretic flea (Lagaropsylla turba Smit, 1958) on the same species of cave-dwelling earwig has been reported in peninsular Malaysia in a well-documented association with the hairless naked bulldog bat, Cheiromeles torquatus Horsfield, 1824. The association of Lagaropsylla signata with Arixenia esau is parallel to the evolution and co-existence with bats in Deer Cave just as in the case of Lagaropsylla turba, Arixenia esau, and Cheiromeles torquatus. The evidence suggests that Lagaropsylla turba and Lagaropsylla signata are obligate phoretic parasites whose survival depends on Arixenia esau to access a bat host. Arixenia esau is reported for the first time in Deer Cave and the occurrence of Lagaropsylla signata on the island of Borneo represented a new record, previously being found only on the island of Java. Images of Lagaropsylla signata attached to Arixenia esau are provided. Xeniaria jacobsoni (Burr, 1912), often associated with Arixenia esau in other geographical areas, was not present in the material examined from Deer Cave. The natural history of the earwig genera Arixenia Jordan, 1909 and Xeniaria Maa, 1974 are discussed and summarized relative to their associations with phoretic fleas and their bat hosts.

    Introduction: Despite general acknowledgement of the importance in assessing family needs in critical care
    patients, there is no psychometric instrument to measure the family needs within Malaysian settings. This
    study aimed to perform factorial validation and establish psychometric properties of Malay translated
    Critical Care Family Need Inventory (CCFNI-M) for Malaysians. Methods: This study consisted of four
    protocols: Forward-Backward translation, validity, internal reliability and inter domain correlations phases.
    The factorial validation of the CCFNI-M was based on its administration to 109 family members of critical
    care patients admitted to the Intensive Care Unit of Hospital Universiti Sains Malaysia, Kelantan, Malaysia.
    At validity phase, factorial validation was performed using Exploratory Factor Analysis using Principal
    Component Analysis with Varimax rotation. The internal consistency and inter domain correlations were
    calculated using Cronbach’s alpha and Pearson correlation coefficient respectively. Results: Preliminary
    analyses reported the suitability of data for factorial validation. With reference to the original CCFNI, five
    factors were extracted which explained 49.4% of the total variance. After removal of several items for
    different reasons, the final items in CCFNI-M were 42. The internal consistency values for five dimensions
    ranged from 0.72 to 0.87 with inter domain correlation values (r) among the dimensions ranged between
    0.36 and 0.61. Conclusion: The high measures of factorial validity, internal consistency and inter domain
    correlations values of the CCFNI-M make it suitable measure for assessing the family needs of critical care
    patients.

    Background: Apart from reducing occupational exposure to cytotoxic hazards, the PhaSeal® closed-system transfer device (CSTD) can extend the beyond-use dates (BUDs) of unfinished vials of antineoplastic drugs for up to 168 hours (seven days). In this study, the total material cost incurred by its use in a Malaysian government-funded hospital was calculated. Methods: A list of vial stability following initial needle punctures of 29 commonly-used antineoplastic drugs was compiled. The amount of the materials used, including drugs, infusion bottles, the PhaSeal® CSTD and other consumables, was recorded on a daily basis for three months in 2015. The total cost was calculated based on the actual acquisition costs, and was compared with that of a hypothetical scenario, whereby conventional syringe-needle sets were used for the same amounts of preparations. Results: The use of the PhaSeal® CSTD incurred a cost of MYR 383,634.52 (USD 92,072.28) in three months, representing an average of MYR 170.5 (USD 40.92) per preparation or an estimated annual cost of MYR 1,534,538.08 (USD 368,289.14). Compared with conventional syringe-needle approach, it is estimated to lead to an additional spending of MYR 148,627.68 (USD 35,670.64) yearly. Conclusion: Although there was a reduction of drug wastage achieved by extending BUDs of unfinished vials using the PhaSeal® CSTD, cost saving was not observed, likely attributable to the wide use of lower-priced generic drugs in Malaysia. Future studies should further evaluate the possibility of cost saving, especially in health settings where branded and high-cost antineoplastic drugs are more commonly used.

    Mosquitoes are vectors of various human diseases in the tropics including yellow fever, dengue, malaria and West Nile virus. Mosquitoes can act as vectors between wildlife and humans, which is particularly important for diseases where wild animals serve as reservoirs of parasites in the absence of human infections. Research has mainly focused on the medical impacts of Anopheles, Aedes, Mansonia and Culex, however, very little attention has been directed towards other mosquito genera, especially those which act as vectors of diseases of wildlife. We have observed adults of Mimomyia (Etorleptiomyia) luzonensis (Ludlow, 1905) feeding on a toad, Ingerophrynus parvus, near an oil palm plantation settlement in Setia Alam, Selangor state, Peninsular Malaysia. Mimomyia is known to feed on reptiles and amphibians, and is a documented vector of several arboviruses, including West Nile virus. The observation of Mimomyia feeding on a common toad near a human settlement highlights a need to understand the relationships between mosquitoes, toads and humans from an ecological perspective. We report on-site observations of the feeding habit of Mimomyia; the first records from Malaysia.

    Job satisfaction is defined as pleasurable or positive emotional state which results from the appraisal of one’s job or job experience. It is often determined by how well outcome meet or exceed expectations. There are many factors which are related to job satisfaction among family physicians. Data on satisfaction among family physicians varies from country to country. This study aimed to determine the level of job satisfaction among family physicians in Malaysia and its associated factors. A cross sectional study was performed among 117 family physicians in Malaysia between July 2012 and December 2012. A questionnaire consists of socio-demographic characteristic, professional and health clinic characteristics and Warr-Cook-Wall job satisfaction scale was used. The results showed that 85% of Malaysian family physicians are satisfied with their job. They are mostly satisfied with hours of work, colleagues and fellow workers, and freedom to choose own method of working. They are least satisfied with physical working condition, rate of pay and recognition. Female gender and less number of health clinics in-charged were associated with increased in overall job satisfaction. Most of the family physicians in Malaysia are satisfied with their job. However there are certain areas that should be looked into which are physical working condition, rate of pay and recognition. Malaysian family physicians should receive equal career opportunity, promotion and salary scale like other specialties

    The development of genomic markers is described for Nile tilapia, Oreochromis niloticus, using the Diversity Arrays Technology (DArT) genotype-by-sequencing platform. A total of 13 215 single nucleotide polymorphism (SNP) markers and 12 490 silicoDArT (dominant) markers were identified from broodstock of two selective breeding programs [Genetically Improved Farmed Tilapia (GIFT) strain from Malaysia and the Abbassa strain from Egypt]. Over 10 000 SNPs were polymorphic in either strain, and 2985 and 3087 showed strain-specific polymorphisms for the GIFT and Abbassa strains respectively. We demonstrate the potential utility of these markers for rapid genomic screening and use in breeding programs.

    In developing ergonomically sound furniture, there is a need in evaluating anthropometrics from various aspect such as gender differences. The objectives of this study was to evaluate anthropometrics differences between genders of children in Malaysia in order to determine the need in having specified chairs for each gender. Represented by four states in Malaysia, a total of 2400 children were involved in the study. A custom made measurer and an anthropometer were used to measure the subjects. Six body measurements, which are, sitting shoulder height, sitting subscapular height, shoulder breadth, buttock popliteal length, popliteal height and hip width were documented for evaluation. From this study, there were some significant differences on some body parts while other body parts were similar to male and female. Therefore, chairs are needed to be made according to the suitability of both gender.

    Motorcyclists are particularly vulnerable when compared to large vehicles on the road. This study was conducted to gather feedbacks from motorcycle deliveryman regarding their riding conditions. A self-rated questionnaire was used to determine prevalence of riding symptom and its association with Whole Body Vibration and other associated factors among motorcycle deliveryman in Malaysia. A set of questionnaire was developed, pilot tested for its reliability and validity and distributed to 100 respondents. An interview was conducted after gathering the data from the respondents through the questionnaire. Survey findings show majority of the respondents preferred to ride according to the standard riding posture. Large majority of them agreed handlebar and foot peg/rest exposed them directly to the vibration. In addition, majority of them felt discomfort at the lower back and felt fatigue after long hours of riding. These findings are in-line with past studies regarding the road users.

    This study on anthropometrics of Primary School children from grade 1 to 5 in Peninsular Malaysia involves 2310 students aged seven to eleven years old. The objectives were to analyze the differentiation of anthropometrics between children of grades 1 to 5 and grouping them to suitable levels in which they are appropriate to propose chair dimensions. A multi-stage sampling method was used, and rural and urban areas were also included in providing anthropometric database that represents the whole Peninsular Malaysia population. There were six dimensions measured in this study, which are sitting shoulder, sitting subscapular height, sitting elbow height, hip width, buttock-popliteal length and popliteal height.
