    Hypoadiponectinemia is characterized by low plasma adiponectin levels that can be caused by genetic factors, such as single nucleotide polymorphisms (SNPs) and mutations in the adiponectin gene or by visceral fat deposition/obesity. Reports have suggested that hypoadiponectinemia is associated with dyslipidemia, hypertension, hyperuricemia, metabolic syndrome, atherosclerosis, type 2 diabetes mellitus and various cardiovascular diseases. Previous studies have highlighted several potential strategies to up-regulate adiponectin secretion and function, including visceral fat reduction through diet therapy and exercise, administration of exogenous adiponectin, treatment with peroxisome proliferator-activating receptor gamma (PPARγ) agonists (e.g., thiazolidinediones (TZDs)) and ligands (e.g., bezafibrate and fenofibrate) or the blocking of the renin-angiotensin system. Likewise, the up-regulation of the expression and stimulation of adiponectin receptors by using adiponectin receptor agonists would be an effective method to treat obesity-related conditions. Notably, adiponectin is an abundantly expressed bioactive protein that also exhibits a wide spectrum of biological properties, such as insulin-sensitizing, anti-diabetic, anti-inflammatory and anti-atherosclerotic activities. Although targeting adiponectin and its receptors has been useful for treating diabetes and other metabolic-related diseases in experimental studies, current drug development based on adiponectin/adiponectin receptors for clinical applications is scarce, and there is a lack of available clinical trial data. This comprehensive review discusses the strategies that are presently being pursued to harness the potential of adiponectin up-regulation. In addition, we examined the current status of drug development and its potential for clinical applications.

    This study was conducted to determine the seasonal prevalence of allergies to house dust, D. pteronyssinus, D. farinae, cat fur, dog hair, mixed moulds, mixed grass pollens and American cockroach. A total of 314 patients with clinically suspected allergic rhinitis was examined by prick test using commercial preparations of the above allergens. Total serum IgE of the patients was determined by a Sandwich ELISA. Ninety-six percent of the patients tested positive to more than one allergen. Most were positive to a combination of 4 allergens. More than 70% of the patients were positive to house dust, D. pteronyssinus, D. farinae and cat fur. Analysis indicates that for an individual who tests positive for house dust, there is a very high risk of the person being allergic to the dust mites and cat fur too. Most of the allergens had 2 peak period of high positive PT rates; mixed moulds and mixed grass pollens had 3 peaks. There was significant positive correlation between the monthly positive PT rates against mixed moulds and mixed grass pollens with maximum daily mean temperature and mean temperature at 14.00 hours.

    Objective: This study aimed to assess the effectiveness of a psycho-education intervention programme in improving the coping strategies of Jordanian breast cancer patients.

    Methods: A double-blinded randomised control trial involving 200 participants between the ages of 20 to 65 years old breast cancer patients was performed. Apart from those who refused participation, patients with chronic diseases and extreme baseline depression scores were also excluded. The control group received standard care twice a week from the social welfare services team facilitator compared to the intervention group that received additional psycho-education intervention programme (PEIP). The coping strategies were measured using the Brief-COPE inventory consisting of 28 items. It was administered on the second and 12th week of trial. The primary end point was compared between pre- and post-intervention. The effect of the intervention between groups, time, and covariates was measured using the generalised linear mixed model (GLMM) analysis.

    Results: The mean (SD) of adaptive coping score among the intervention group increased from 5.63 (1.3) at baseline to 6.42 (1.3) at post-intervention. The mean avoidant coping score was 3.87 (1.1) at baseline but reduced to 3.69 (0.8) post-intervention. GLMM showed that women who received the intervention reported significantly higher usage of the adaptive coping strategies after attending the programme (B=0.921, p <0.001).

    Conclusion: PEIP significantly improved knowledge of breast cancer patients. Thus, this programme may be considered as a part of the healthcare services in Jordan towards improving the adaptive coping strategies among breast cancer patients, which may point towards the potential for these services to increase adaptive coping strategies among patients in Jordan.

    Implications for Public Health: PEIP may be considered as psychosocial intervention in public health and healthcare setting to address rising concerns on quality of care among breast cancer patients.

    We extend the operational matrices technique to design a spectral solution of nonlinear fractional differential equations (FDEs). The derivative is considered in the Caputo sense. The coupled system of two FDEs is considered, subjected to more generalized integral type conditions. The basis of our approach is the most simple orthogonal polynomials. Several new matrices are derived that have strong applications in the development of computational scheme. The scheme presented in this article is able to convert nonlinear coupled system of FDEs to an equivalent S-lvester type algebraic equation. The solution of the algebraic structure is constructed by converting the system into a complex Schur form. After conversion, the solution of the resultant triangular system is obtained and transformed back to construct the solution of algebraic structure. The solution of the matrix equation is used to construct the solution of the related nonlinear system of FDEs. The convergence of the proposed method is investigated analytically and verified experimentally through a wide variety of test problems.

    AIMS: Mesenchymal stem/stromal cells (MSCs) hold promises for the treatment of diverse diseases and regeneration of injured tissues. Genetic modification of MSCs through gene delivery might enhance their therapeutic potential. Adiponectin has been appeared as a potential biomarker for predicting various diseases. Plasma adiponectin levels are negatively correlated with various metabolic and vascular diseases and supplementation of exogenous adiponectin ameliorates the diseases. This study aims to develop adiponectin secreting genetically modified MSCs (GM-MSCs) as a potent strategic tool to complement endogenous adiponectin for the treatment of adiponectin deficiency diseases.

    MAIN METHODS: Human bone marrow derived MSCs were isolated, expanded in vitro and transfected with adiponectin gene containing plasmid vector. Total RNA was extracted and cDNA was prepared by reverse transcription polymerase chain reaction (RT-PCR). The expression of adiponectin gene and protein in GM-MSCs was analyzed by PCR and Western blotting respectively. The secretion of adiponectin protein from GM-MSCs was analyzed by enzyme-linked immunosorbent assay.

    KEY FINDINGS: The expression of adiponectin gene and plasmid DNA was detected in GM-MSCs but not in control group of MSCs. Adiponectin gene expression was detected in GM-MSCs at 2, 7, 14, 21 and 28days after transfection. Western blotting analysis revealed the expression of adiponectin protein only in GM-MSCs. The GM-MSCs stably secreted adiponectin protein into culture media at least for 4weeks.

    SIGNIFICANCE: GM-MSCs express and secret adiponectin protein. Therefore, these adiponectin secreting GM-MSCs could be instrumental for the supplementation of adiponectin in the treatment of adiponectin deficiency related diseases.

    BACKGROUND: Immune thrombocytopenic purpura (ITP) is a hemorrhagic diathesis, characterized by platelets destruction alongside impaired production. Patients from Asian regions often exhibit distinctive characteristics in comparison to the western patients. We accomplished this study to evaluate the prevalence of primary versus secondary ITP along with the comparative analysis between them. The secondary objective was to determine the etiological spectrum of secondary ITP.

    METHODS: We illustrate the results of a large cohort of newly diagnosed adults ITP from southern Pakistan. The study extended from January 2009-December 2013. Complete blood counts, HbsAg, Anti-HCV, ANA, stool for Helicobacterpylori were done on all. HIV, TSH, anti-dsDNA, RA factor, APLA and direct coombs test were evaluated in cases where indicated.

    RESULTS: A total of 417 patients were included with a mean age of 40.95±14.82 years. Primarily disease was observed in the 3rd decade of life. Male to female ratio was 1:1.5. Mean platelets count was 46.21±27.45x109/l. At diagnosis 43.16% (n=180) patients had hemorrhagic manifestations whilst 56.8% (n=237) were asymptomatic. None of the patient presented with visceral, retropharyngeal or intracranial bleed. The prevalence of secondary ITP was substantially higher (64.8%) as compared to primary ITP (35.2%). Secondary ITP was predominantly seen in HCV reactive patients (24.4%) followed by helicobacter-pylori infection (11%). Nevertheless 16.4% patients had underlying autoimmune disorders. Providentially no study subject was found to be HIV reactive.

    CONCLUSIONS: Our study revealed predominance of secondary ITP. However bleeding manifestations and degree of thrombocytopenia were high in primary-ITP. Infectious etiology followed by autoimmune disorders is mainly implicated for secondary ITP in our setting.

    BACKGROUND: Breastfeeding offers incredible health benefits to both child and mother. It is suggested by World Health Organization that an able mother should practice and maintain exclusive breastfeeding for first six months of her infant's life. The objective of this study was to determine the prevalence and factors associated with exclusive breastfeeding for first six months of an infant's life in Bangladesh.

    METHODS: Data was extracted from Bangladesh Demographic and Health Survey (BDHS-2014). BDHS-2014 collected data from 17,863 Bangladeshi married women in reproductive age from the entire country using two stages stratified cluster sampling. We included only mothers having at least one child currently aged not less than 6 months. Mothers who did not have child to breastfeed, some incomplete information and missing samples were excluded from the data set and consequently 3541 mothers were considered in the present study. Chi-square test, binary logistic regression models were used in this study.

    RESULTS: The prevalence of exclusive breastfeeding (EBF) for first six months of an infant's life in Bangladesh was 35.90%. Binary multivariable logistic regression model demonstrated that relatively less educated mothers were more likely to exclusively breastfeed their children than higher educated mothers.     Introduction: The mortality rate of glioma patients particularly with glioblastoma multiforme (GBM) remains high even with advancements in the multimodality treatment. This is partly due to chemoresistance of the glioma cells towards drug treatment which finally reduced the survival of GBM patients. In this study, we determined the chemosensitisation and oncogenic characteristics of ZFP36L2 in LN18 GBM cells using RNA interference (RNAi). Methods: Meta-analysis of microarray datasets was used to identify the druggable genes responsive to GBM chemosensitivity. Subsequently, the genes were validated using RNAi screening [pooled small interference RNA (siRNA)]. Temozolomide- resistant LN18 cells were used to evaluate the effects of gene silencing on chemosensitisation to the sub-lethal dose (1/10 of IC50) of temozolomide. Assays such as cell viability, proliferation, migration and invasion and apoptosis assays were carried out. The apoptosis pathway underlying chemosensitisation by ZFP36L2 siRNA was determined using a human apoptosis array kit. Statistical analyses were performed using one-way analysis of variance. Results: ZFP36L2 was identified as a potential marker of GBM based on the meta-analysis and RNAi screening. ZFP36L2 knockdown lead to 1) Apoptosis induction (p < 0.05) 2) Reduced cell migration (p < 0.05) 3) Reduced up to 82% of cell invasion (p < 0.01) and 4) Decreased cellular proliferation in siZFP36L2-treated LN18 cells. Downstream analysis showed that the sub-lethal dose of temozolomide caused major upregulation of BCL2-associated X, apoptosis regulator (BAX). Conclusion: ZFP36L2 is oncogenic and chemosensitive thus may contribute to gliomagenesis through cell proliferation, migration, invasion and apoptosis. RNAi therapy in combination with chemotherapy treatment such as temozolomide may serve as potential therapeutic approach in the future.

    Glioblastoma multiforme (GBM) is a high-grade brain tumor of which the survival patients remain poor.
    Tousled-like kinase 1 (TLK1), a serine-threonine kinase, was identified to be overexpressed in cancers such
    as GBM. TLK1 plays an important role in controlling survival pathways. To date, there is no structure
    available for TLK1 as well as its inhibitors. We aimed to create a homology model of TLK1 and to identify
    suitable molecular inhibitors that are likely to bind and inhibit TLK1 activity via in silico high-throughput
    virtual screening (HTVS) protein-ligand docking. The 3D homology models of TLK1 were derived from
    various servers. All models were evaluated using Swiss Model QMEAN server. Validation was performed
    using multiple tools. Energy minimization was performed using YASARA. Subsequently, HTVS was
    performed using Molegro Virtual Docker 6.0 and ligands derived from ligand.info database. Drug-like
    molecules were filtered using ADME-Tox filtering program. Best homology model was obtained from the
    Aurora B kinase (PDB ID:4B8M) derived from Xenopus levias structure that share sequence similarity with
    human TLK1. Two compounds were identified from HTVS to be the potential inhibitors as it did not violate
    the Lipinski rule of five and the CNS-based filter as a potential drug-like molecule for GBM

    Introduction: To investigate the questions that Muslim patients, embryologists, paramedics, clinical counsellor and doctors may have pertaining to the compliance of Assisted Reproductive Technique (ART) to Shari’aa.

    Methods: As a preliminary study, an open-ended survey was administered to participants. Participants for the survey comprised of 21 patients, 2 embryologists, 2 doctors, 4 paramedics and 1 counsellor.

    Results: Data analysis showed that most of the participants posed questions that fall into six classifications which are: Munakahat (Family Law), social Hukm, Fiqh Ibadah, Fiqh Qadha (Islamic Hukm), General Fiqh and Usul Fiqh.

    Conclusion: It is clear that even though the participants were rather familiar with some of the existing Fatwas pertaining to ART and IVF in Malaysia, there are still remaining doubts which need to be clarified. This indicates that there are challenges faced in seeking and providing ART treatment to Muslim patients. This needs to be addressed perhaps in the form of Shari’aa-compliant guidelines to provide a clear explanation of each process and procedure in fertility treatment.

    Similarity measurement is a critical component in any case-based reasoning (CBR) system. CBR is
    a superior technique for solving new problems based on previous experiences. Main assumption in
    CBR relies on the hypothesis that states similar problems should have similar solutions. This paper
    describes a comparative analysis on several commonly used similarity measures (Canberra, Clark, and Normalized Euclidean distance) in retrieving phase of the case-based reasoning approach to facilitate supplier selection. In addition, the proposed agent-based supplier selection framework was designed to use customer’s defined weights to evaluate the price, volume, quality grade, and delivery date of supply materials, and also provide them with alternative products which are closest to their first order if it was out of stock. Finally, based on the proposed framework, a numerical example of the used approach is illustrated.

    Introduction: To analyze the ART outcome and factors associated with successful ART treatment for infertile couples in a newly set up medical university IVF centre. Methods: A six-month cohort study of patients undergoing their first assisted reproductive technique cycle was conducted at IIUM fertility centre, Kuantan (from 15th June until 31st Dec 2009). Outcome measures include clinical pregnancy rate, ongoing pregnancy rate, take home baby rate and complications rate. Results: Fifty-five first cycles of IVF/ICSI were studied. Mean embryo per transfer was 2.1±0.8. Four patients were pregnant (23.5%) from a single embryo transfer (SET). Clinical pregnancy rate was 30.9% (17/55). Ongoing pregnancy rate was 23.6% (13/55) and take home baby rate was 21.8% (12/55). There was only one twin pregnancy. Nine patients (16.4%) had OHSS whereby 77.8% were mild and 22.2% were moderate. Conclusion: Take home baby rate at the IIUM fertility centre is comparable to most established ART centres in Malaysia considering that this is only the first year of operation.

    In the electrodeposition system, adding saccharin alters the properties of the metal deposits by changing the electrode kinetics of the deposited surface. In this study, nanocrystalline cobalt-iron (CoFe) coating was synthesised using the electrodeposition technique with different saccharin concentrations. The results obtained showed that the coating thickness increased while the grain size decreased from 51 nm to 40 nm when the saccharin concentration increased from 0 to 2 g/L. The nanocrystalline CoFe coating produced with 2 g/L of saccharin concentration resulted in the smallest particle size of 71.22 nm and the highest microhardness of 251.86 HV. From the salt spray test (24 and 48 hours) it was found that the use of saccharin at higher concentration of 2 g/L improves the corrosion resistance of the nanocrystalline CoFe coating significantly due to the change of surface morphology as well as the decrease in grain size.

    Safety culture has been shown to be related to patient outcomes and Safety Attitude Questionnaire (SAQ) is one of the measures of safety culture that has good psychometric properties. The present study attempts to adapt the short version of the Generic SAQ for use in Malaysian healthcare setting. The process of adaptation included forward translation and backward translation method, followed by content validity analysis by seven subject matter experts. All 36 items of the SAQ was retained for the field test. The Malaysian SAQ (MSAQ) was distributed to 400 healthcare workers in a hospital in Kuala Lumpur. There were 126 returned and usable questionnaires (31.5% return rate). The internal consistency indices of the MSAQ is acceptable but two items were revised due to low corrected itemtotal correlation. The revised MSAQ and the Barriers to Medication Administration Error Reporting scale was administered to nurses (n=175, with 76.1% response rate) of two public hospitals in East Peninsula of Malaysia. Internal consistency of the dimensions improved to .71 to .91. Dimensions of MSAQ correlated negatively with the barriers to error reporting, providing evidence of convergent validity. Thus, the revised MSAQ is suggested to be used for research and interventions in Malaysian healthcare organizations.

    Effective and rapid methods for RNA extraction from conidia, germinating conidia and appressoria of the fungal plant pathogen, Colletotrichum gloeosporioides is reported in this study. The procedure for the RNA extraction from conidia and germinating conidia was carried out using TRI REAGENT® solution (Molecular Research Center, USA) and can be completed in less than one and a half hours. The procedure for RNA extraction from appressoria was carried out using a modified protocol employing guanidine isothiocyanate and mechanical cell disruption by glass beads. The efficiency of the RNA extraction procedures was evaluated by several measures to determine RNA integrity, purity and applicability in RT-PCR. RNA integrity was assessed by observing the integrity of the major RNA species (18S and 28S rRNA) on denaturing agarose gel electrophoresis. The ethidium bromide-staining pattern of intact total RNA extracted from the three fungal morphogenetic cells showed clearly defined 28S and 18S rRNA bands and no genomic DNA contamination. Spectrophotometric assessment of RNA from each sample indicated relatively high purity and absence of carbohydrate contamination. Finally, we have demonstrated that the methods used for RNA extraction of conidia, germinating conidia and appressoria produced RNA of sufficient quality suitable for RT-PCR in detecting the expression of protein kinase A regulatory subunit gene in C. gloeosporioides.

    Teknologi DNA mikroatur merupakan salah satu teknologi penting dalam penyelidikan biologi kini. Teknologi tersebut membolehkan analisis pengekspresan gen pelbagai sistem model pada skala genom dijalankan. Prestasi DNA mikroatur ditentukan oleh parameter seperti kepadatan titik, ciri-ciri titik (morfologi, kepadatan prob dan keamatan isyarat penghibridan), latar belakang, kespesifikan dan kesensitifan. Dengan menggunakan mesin pemegun GeneTac™G3, slaid mikroatur cDNA ikan siakap (Lates calcarifer) yang mengandungi 1920 klon-klon cDNA terpilih daripada perpustakaan cDNA hepar dan limpa yang dipegunkan secara duplikasi telah difabrikasikan. Klon-klon yang dipilih mempunyai fungsi putatif berkaitan dengan keimunan, aruhan tekanan, metabolisme, pengangkutan, transkripsi dan translasi. Kawalan negatif seperti oligonukleotida (A)50 dan beberapa gen daripada Saccharomyces cerevisiae dan Escherichia coli serta kawalan positif seperti beberapa siri pencairan DNA genom dan cDNA L. calcarifer juga dipegunkan ke atas slaid kaca. Diperhatikan bahawa kepekatan prob dalam julat antara 150-250 μg/mL, kesesuaian jenis kawalan positif dan negatif yang digunakan berjaya menghasilkan slaid yang berkualiti. Selain itu, perlakuan pengeringan slaid secara semalaman dan rehidrasi semula slaid menghasilkan morfologi titik DNA yang sekata dan membulat. Kawalan kualiti ke atas slaid yang terhasil membuktikan keberkesanan slaid yang difabrikasi untuk kajian respons transkriptom L. calcarifer terhadap jangkitan Cryptocaryon irritans.

    The ability of a locally isolated clinical strain of C. albicans to adapt and response towards oxidative stress were investigated in this study. Treatment of C. albicans with hydrogen peroxide (H2O2) will exert an oxidative stress to C. albicans cells and affect the growth rate of this opportunistic pathogen. Cells that were grown in Yeast Peptone Dextrose (YPD) medium with the presence of 4.0 mM H2O2 gave a doubling time of 51 min/generation compared to 44 min/generation for those grown in 0.4 mM H2O2 and without H2O2. In order to determine the resistance level of C. albicans towards oxidative stress, cells were exposed to different concentration of H2O2. Results showed that percentage of cells viability decreased with the increased concentration of H2O2. These data indicated that C. albicans could overcome oxidative stress to a certain level before they were killed. To determine whether this strain exhibited a stress induce protection, cells were treated with mild oxidative stress for one hour before exposed to a stronger oxidative stress. Data obtained shows that cells that were pre-treated with mild oxidative stress showed higher percentage of survival compared to non pre-treated cells. This strongly suggested that stress induce protection was present in this C. albicans. Finally, in order to determine whether oxidative stress can induce yeast to hyphal morphogenesis in C. albicans, cells were then exposed to 0.4 mM H2O2 at 37 ºC and the number of hyphal formed were compared to hyphal formation when the cells were grown at 37 ºC only. However the results showed that oxidative stress failed to induce yeast to hyphal morphogenesis in this clinically isolated C. albicans strain.

    The functional data model has recently received increasing attention particularly in their application to mortality forecasting. The advantage of this method over the well-known Lee-Carter model is the ability to treat the underlying process as functional, and provide estimations that are robust to outliers. This research investigates the accuracy of functional data approach in estimating the mortality rates and life expectancy at births in developing countries including Malaysia, Indonesia, Thailand and Singapore. The functional data method was applied to these countries’ mortality data, and the out-sample forecast errors showed that, in terms of overall, the functional data model was more accurate than that of the original Lee-Carter model for males and females. The results provide evidence that the functional model is accurate to forecast the life expectancy at births for developing countries.

    Spray dryer had long been used to dry liquid materials and produce dry crystalline products. However, the drying of the bittern to produce quality salt crystals has not been widely published. Therefore, the purpose of this study was to examine the effect of drying conditions of the bittern using a spray dryer to produce salt with a high natrium chloride (NaCl) content. Drying was carried out in the hot air temperature (105-125 °C), drying air flow rate (25-45 ml/min), feed flow rate (20-30 ml/min), and concentration of maltodextrin (10-30%). The parameters were observed water content, NaCl content, yield, and mean particle diameter size (MPDS). The results showed that the inlet air temperature of 125 °C can significantly reduce the water content faster and produce higher NaCl levels than the inlet air temperature of 105 °C. The salt crystals produced at higher maltodextrin concentrations have lower water content and high NaCl content. The best-operating conditions are at a hot air temperature of 125 °C, a drying airflow rate of 45 m/s, and a maltodextrin concentration of 25% because it produces salt crystals with high NaCl content. Overall, these results indicate that the bittern can be dried using a spray dryer with potential NaCl content as a raw material for the pharmaceutical industry.

    Colorectal cancer (CRC) is the second most common cause of cancer mortality among men and women worldwide; the risk of its occurrence has been shown to be increased by chronic bacterial infections. A case control study was therefore carried out at Hospital Universiti Sains Malaysia (HUSM) to determine the incidence of colorectal cancer associated with S. bovis infection. A total of 166 stool specimens were collected from diseased patients and healthy individuals and S. bovis isolates were identified. Suspected colon tumor and cancer cases were diagnosed and confirmed. It was found that overall prevalence of S. bovis was 41 (24.7%) out of 166 cases studied. Some 41(48.6%) of these S. bovis isolates was found in patients with colonic polyps, adenocarcinomas, inflammatory bowel disease (IBD) and chronic gastrointestinal tract (GIT). It was also found that colorectal cancer incidence was 24.7%, adenocarinomas accounting for 51% with the highest incidence in the sigmoid part of the colon. Among the IBD and chronic GIT cases, ulcerative colitis featured in the majority of cases (41.4%). In conclusion, there is a high incidence of colorectal cancer associated with S. bovis.
