    Painful neuropathy, a common complication of diabetes mellitus is characterized by allodynia and hyperalgesia. Recent studies emphasized on the role of non-neuronal cells, particularly microglia in the development of neuronal hypersensitivity. The purpose of the present study is to evaluate the effect of minocyline, a selective inhibitor of microglial activation to define the role of neuroimmune activation in experimental diabetic neuropathy. Cold allodynia and thermal and chemical hyperalgesia were assessed and the markers of inflammation and oxidative and nitrosative stress were estimated in streptozotocin-induced diabetic rats. Chronic administration of minocycline (40 and 80 mg/kg, i.p.) for 2 weeks started 2 weeks after diabetes induction attenuated the development of diabetic neuropathy as compared to diabetic control animals. In addition, minocyline treatment reduced the levels of interleukin-1β and tumor necrosis factor-α, lipid peroxidation, nitrite and also improved antioxidant defense in spinal cords of diabetic rats as compared to diabetic control animals. In contrast, minocycline (80 mg/kg, per se) had no effect on any of these behavioral and biochemical parameters assessed in age-matched control animals. The results of the present study strongly suggest that activated microglia are involved in the development of experimental diabetic neuropathy and minocycline exerted its effect probably by inhibition of neuroimmune activation of microglia. In addition, the beneficial effects of minocycline are partly mediated by its anti-inflammatory effect by reducing the levels of proinflammatory cytokines and in part by modulating oxidative and nitrosative stress in the spinal cord that might be involved in attenuating the development of behavioral hypersensitivity in diabetic rats.

    BACKGROUND: Early detection and risk factor control prevent chronic kidney disease (CKD) progression. Evaluation of peripheral autonomic dysfunction may detect incident cardiovascular-renal events in type 2 diabetes (T2D).

    METHODS: SUDOSCAN, a non-invasive tool, provides an age-adjusted electrochemical skin conductance (ESC) composite score incorporating hands/feet ESC measurements, with a score ≤53 indicating sudomotor dysfunction. A consecutive cohort of 2833 Chinese adults underwent structured diabetes assessment in 2012-13; 2028 participants without preexisting cardiovascular disease (CVD) and CKD were monitored for incident cardiovascular-renal events until 2015.

    RESULTS: In this prospective cohort {mean age 57.0 [standard deviation (SD) 10.0] years; median T2D duration 7.0 [interquartile range (IQR) 3.0-13.0] years; 56.1% men; 72.5% never-smokers; baseline ESC composite score 60.7 (SD 14.5)}, 163 (8.0%) and 25 (1.2%) participants developed incident CKD and CVD, respectively, after 2.3 years of follow-up. The adjusted hazard ratios (aHRs) per 1-unit decrease in the ESC composite score for incident CKD, CVD and all-cause death were 1.02 [95% confidence interval (CI) 1.01-1.04], 1.04 (1.00-1.07) and 1.04 (1.00-1.08), respectively. Compared with participants with an ESC composite score >53, those with a score ≤53 had an aHR of 1.56 (95% CI 1.09-2.23) for CKD and 3.11 (95% CI 1.27-7.62) for CVD, independent of common risk markers. When added to clinical variables (sex and duration of diabetes), the ESC composite score improved discrimination of all outcomes with appropriate reclassification of CKD risk.

    CONCLUSIONS: A low ESC composite score independently predicts incident cardiovascular-renal events and death in T2D, which may improve the screening strategy for early intervention.

    BACKGROUND: Diabetic peripheral neuropathy (DPN) is one of the most common complications of diabetes mellitus. The main clinical manifestations of DPN include pain, numbness, paraesthesia, and weakness of the lower limbs which often leads to diabetic foot ulceration, eventually resulting in amputation. Based on Traditional Chinese Medicine theory, moxibustion has a great effect on treating and preventing DPN. However, randomized clinical trials done to evaluate the efficacy of this treatment are still lacking. Hence, this study is carried out to evaluate the effectiveness and safety of moxibustion therapy on diabetic peripheral neuropathy.

    METHODS: This study will be a pilot, interventional, randomized, 2-armed, parallel, singled-masked, controlled trial. A total of 40 diabetes mellitus patients with peripheral neuropathy will be recruited and assigned randomly into 2 groups (moxibustion group and waiting group) at a 1:1 ratio. This trial consists of an 8-week intervention period and a 4-week follow-up period. During the intervention period, the moxibustion group will take 3 moxibustion sessions per week, whereas no intervention will be done on the waiting group to act as the control group. The outcome will be assessed by an outcome assessor who is unaware of the group assignment. The primary outcome will be pain assessment measured with algometry, Leeds Assessment of Neuropathic Symptoms and Signs pain scale, visual analogue scale, and neuropathy pain scale. The secondary outcome will be an evaluation of functional performance capacity with 6 minutes walking test, evaluation of the Foot and Ankle Ability Measure, and serum HbA1c and albumin levels.

    DISCUSSION: We hope that this trial will provide valuable insights on the efficacy of moxibustion in the management of diabetic peripheral neuropathy.

    BACKGROUND: The role of endothelial injury and circulating adhesion molecule in the development and progression of diabetic peripheral neuropathy in the long-term has been established previously.
    AIMS: To study the effects of short-term glycemic control using insulin and oral hypoglycemic agent therapy (OHA) on the peroneal nerve function and vascular cell adhesion molecule-1 (VCAM-1) and advanced glycation endproducts (AGE) levels in type 2 diabetic patients.
    SETTINGS AND DESIGN: A randomized controlled study involving poorly controlled (HbA1c, 7.5%-11%) type 2 diabetic patients attending the endocrinology outpatient center in a tertiary hospital in Kuala Lumpur.
    MATERIALS AND METHODS: Twenty-nine patients were randomized to receive insulin (n=15) or OHA (n=14) for 8 weeks. The glycemic variables (HbA1c, fasting plasma glucose [FPG], fructosamine), VCAM-1, serum AGE and the peroneal motor conduction velocity (PMCV) were measured at baseline and at 4-week intervals.
    STATISTICAL ANALYSIS USED: Paired 't' test or Kruskal Wallis test; and the unpaired 't' test or Mann-Whitney U test were used for within-group and between-group analyses, respectively. Correlation was analyzed using Spearman's correlation coefficient.
    RESULTS: Within-group analysis showed significant progressive improvement in HbA1c at weeks 4 and 8 in the insulin group. The PMCV improved significantly in both groups by week 8, and by week 4 (P = 0.01) in the insulin group. PMCV correlated negatively with VCAM-1 (P = 0.031) and AGE (P = 0.009) at week 8.
    CONCLUSION: Aggressive glycemic control with insulin improves the peroneal nerve function within 4 weeks. Improvement in the serum VCAM-1 and AGE levels correlated significantly with improvement in peroneal nerve conduction velocity only in the insulin group.

    Tajuk Elektrokimia didapati sukar dipelajari kerana konsep abstrak yang melibatkan tahap perwakilan makroskopik, mikroskopik dan simbolik. Kajian lepas menunjukkan bahawa animasi dan simulasi menggunakan teknologi maklumat dan komunikasi (TMK) boleh membantu pelajar membayangkan dan seterusnya memperkuatkan pemahaman mereka dalam mempelajari tajuk kimia yang abstrak. Ekoran daripada itu, modul multimedia interaktif dengan agen pedagogi (MMIAP) yang dinamakan Makmal EK (Elektrokimia) dibangunkan dalam usaha membantu pelajar dalam mempelajari Elektrokimia. Reka bentuk penyiasatan kuasi eksperimen dengan ujian pra/pos kumpulan kawalan tidak setara dijalankan dalam usaha untuk menilai keberkesanan MMIAP Makmal EK terhadap pemahaman konsep pelajar dalam mempelajari elektrokimia. Seramai 127 orang pelajar tingkatan empat dari dua buah sekolah menengah terlibat dalam kajian ini. Setiap sekolah mempunyai sebuah kumpulan rawatan dan sebuah kumpulan kawalan yang diajar oleh guru kimia yang sama. Ujian pencapaian pra dan pos diberikan untuk menilai pemahaman konsep Elektrokimia pelajar sebelum dan selepas mempelajari kimia. Hasil kajian menunjukkan bahawa intervensi seperti yang direkabentuk dalam kajian menghasilkan perbezaan yang bererti antara kumpulan kawalan dan kumpulan rawatan dalam memahami konsep Elekotrokimia.

    Peningkatan kes penderaan kanak-kanak sehingga kematian kanak-kanak amat membimbangkan keluarga dan masyarakat. Artikel ini akan membahaskan tentang cabaran pekerja sosial dalam melindungi penderaan kanak-kanak di taska. Kajian lepas amat kurang mengkaji isu penderaan kanak-kanak di taska. Kajian ini dijalankan dengan menggunakan pendekatan kualitatif berpandukan kajian fenomenologi. Teknik pengumpulan data kajian melalui temubual berstruktur secara bertulis. 6 orang informan terdiri daripada pekerja sosial telah dipilih bagi kajian ini dengan menggunakan teknik persampelan bertujuan (purposive sampling). Lokasi kajian yang dipilih ialah taska sekitar Bandar Baru Bangi. Data temubual berstruktur dianalisis secara tematik dan hasil kajian mendapati terdapat tiga cabaran utama iaitu pertama praktis kerja sosial, konflik profesion dan kemahiran dalam mengendalikan pengurusan kes penderaan kanak-kanak. Implikasi kajian ini mencadangkan program kaunseling individu dan kelompok kepada pekerja sosial dan penting meningkatkan latihan dalam kemahiran mengendalikan pengurusan kes penderaan kanak-kanak yang semakin kompleks dan kritikal di masa depan.

    Tinjauan sepanjang lebuhraya Puchong-Kajang telah dijalankan khususnya di kawasan Puchong, Selangor. Terdapat 12 stesen telah dilawati dan 6 daripadanya telah dilakukan pensampelan tanah bagi analisis makmal. Cerun-cerun yang terbabit dengan pensampelan ini melibatkan cerun jenis tanih dengan julat darjah luluhawa di antara gred IV dan VI. Sampel-sampel tanih yang diambil mewakili cerun yang mengalami kegagalan dan cerun yang stabil. Ciri-ciri fiziko-kimia tanih yang dikaji melibatkan kandungan air segar dan dalam keadaan tepu, kandungan bahan organik, taburan saiz zarah, ketumpatan pukal dan sebenar, keporosan tanah, serakan liat, pH dan kandungan oksida ferum. Kandungan air tanah segar menunjukkan peratusan yang lebih tinggi pada cerun gagal berbanding cerun yang stabil. Semua sampel menunjukkan kandungan air tanah lapangan di bawah tahap kandungan air tepu. Kandungan bahan organik adalah sangat rendah (<2%) dan tanah boleh dikelaskan sebagai berpotensi tidak stabil. Terdapat hubungan positif yang jelas antara kandungan air dan bahan organik khususnya pada cerun gagal. Porositi tanah menunjukkan perkaitan positif terhadap lempung dan kandungan bahan organik. Kebanyakan tanah dikelaskan sebagai bertekstur lempung berpasir berbanding tekstur lain. Nilai-nilai koefisien serakan liat menunjukkan struktur tanah berada dalam kategori stabil hingga sangat stabil berbanding satu stesen yang dikelaskan sebagai tidak stabil. Secara umumnya, tanah terdiri daripada jenis asidik (pH4.87 - pH4.34) dengan kehadiran oksida ferum kurang daripada 6%.

    AIM: The present study was aimed to highlight the current prescribing pattern of oral hypoglycemia in type 2 diabetes mellitus and to evaluate the therapeutic effectiveness of these therapeutic categories in achieving target glycemic control.

    METHODS: This is a prospective, cross-sectional, observational study of 6 months' duration conducted in a tertiary care hospital of Lahore, Pakistan.

    RESULTS: The current research recruited 145 patients presented in diabetes management center of a tertiary care hospital in Lahore, Pakistan. Mean age of the participants was 50.2 (± 8.5) years. Out of the 145 patients, 63% were females and 37% were males. Most patients were diagnosed to have diabetes within the past 5 years. Diabetes-induced neuropathy was the most common complication (71.7%) among the patients. A large proportion of these patients (70.3%) were also suffering from other comorbidities among which the most common one is hypertension. The average number of prescribed medications was 1.31. Metformin was prescribed to a majority of patients (64%) as monotherapy while 28.96% received combination therapy. Mean glycated hemoglobin (HBA1c) before and after 3 months of treatment was 8.5 (± 2.3) and 8.04 (± 2.1), respectively. Inferential statistics show a strong association between HBA1c and life style modifications and adherence to medication therapy (P = 0.05). However, the correlation between HBA1c and Morisky score and duration of disease was inverse and weak (P = 0.6, 0.4). The t-test values show a small difference between HBA1c values before and after 3 months (t = 0.440 and 0.466, respectively).

    CONCLUSION: Optimization of medication regimen and continuous patient education regarding life style modification and adherence to medication therapy are necessitated to bring HBA1c values near to target.

    Kebimbangan dan gangguan emosi amat biasa di dalam Penyakit Kecelaruan
    Defisit Perhatian. Laporan kes ini mengetengahkan kes lelaki Melayu berusia
    23 tahun yang menunjukkan masalah memberi perhatian dan kemerosotan
    pencapaian akademiknya secara ketara. Kemerosotan pengajiannya disedari
    oleh ahli keluarganya 2-3 tahun sebelum beliau mendapatkan rawatan psikiatri.
    Beliau juga mengalami masalah kemurungan tetapi tidak mengalami gejala
    biologi kemurungan. Beliau telah dirawat dengan ubat-ubatan Buproprion 150 mg
    setiap hari dan juga ubat Ritalin 10 mg. Kebiasaannya ubat peransang digunakan
    untuk rawatan penyakit kecelaruan deficit perhatian tetapi dalam kes ini ubat
    anti kemurungan telah digunakan. Beliau telah menunjukkan penambahbaikan
    selepas memakan ubat dan seterusnya dapat memberi fokus kepada pembelajaran
    akademiknya.

Beberapa kaedah boleh digunakan untuk menentukan kekasaran permukaan ketakselanjaran. Kertas ini memperkenalkan satu pendekatan yang mudah dalam bentuk tiga (3) persamaan polinominal untuk mengkorelasikan sudut geseran puncak ketakselanjaran (fp) batuan granit segar, terluluhawa sedikit dan terluluhawa sederhana dengan pekali kekasaran kekar, (JRC). Persamaan polinominal ketakselanjaran batuan granit segar ialah fp1 = -0.071JRC2 + 3.56JRC + 35.6. Persamaan polinominal ketakselanjaran batuan granit terluluhawa sedikit dan terluluhawa sederhana masing-masing ialah fp2 = -0.087JRC2 + 4.32JRC + 34.1 dan fp3 = -0.133JRC2 + 5.08JRC + 29. Hasil kajian ini memberikan satu kaedah alternatif penganggaran nilai fp satah ketakselanjaran daripada penentuan nilai PKK pada permukaan ketakselanjaran batuan granit di lapangan.

    A 68-year-old diabetic and hypertensive woman presented with chronic autonomic diarrhoea, syncope and palpitations which were associated with QT prolongation and recurrent episodes of torsade de pointes. She was on glibenclamide, indapamide and probucol (for type V hyperlipidaemia). Despite intravenous infusions of potassium, lignocaine and amiodarone, the unstable rhythm persisted. However, intravenous magnesium sulphate with small doses of intravenous propranolol terminated the torsade de pointes. She was stabilised but following discharge she relapsed, and upon re-admission, succumbed to intractable ventricular fibrillation. Early recognition and aggressive treatment of this condition is emphasised. Multiple aggravating factors ie autonomic diarrhoea resulting in severe potassium and magnesium depletion, kaliuretic effect of indapamide, probable QT prolongation associated with diabetic autonomic neuropathy and probucol; probable underlying coronary artery disease and heightened emotional and sympathetic discharge could have contributed to this very unstable ventricular arrhythmia and sudden death.

    Poor glycaemic control and the duration of diabetes mellitus are known to accelerate development and progression of neuropathy. Diabetic co-morbidities: hypertension and hyperlipidaemia, have been postulated to associate with development of neuropathy. A diabetic foot with low temperature and frequent exposure to low temperature environment has recently been hypothesized to be at higher risk to develop early neuropathy. This cross-sectional study is undertaken to identify risk factors for diabetic neuropathy and the association between foot temperature and development of diabetic neuropathy by using simple clinical examination in the outpatient setting. From April 18, to April 30, 2005, universal sampling method was used to select 134 diabetic patients (type 1 or type 2 for >1 year) with peripheral neuropathy. Excluded are those with chronic alcoholism, drug-induced neuropathy, dietary history of vitamin B deficiency and family history of porphyria and hereditary sensorimotor neuropathy. The patient's duration of diabetes, glycaemic control status and the presence of co-morbids: hypertension and hyperlipidemia, were recorded. The temperature of the foot was measured by using thermo buddy. Of 134 patients representing Malaysian ethnic distribution with an equal number of males and females, 20.1% were in the age group of 61 to 65 years and, 85.1% and 67.9% belonged to lower socioeconomic and educational groups respectively. Associations between diabetic neuropathy and glycaemic control (p = 0.018) and duration of diabetes (p < 0.05) were significant. However, hypertension, hyperlipidaemia and low foot temperature were not significantly associated with development of diabetic neuropathy. Poor glycaemic control is significantly associated with diabetic neuropathy. Foot temperature alteration is merely an effect of autonomic neuropathy with a cold foot is attributed to co-existing peripheral arterial disease.

    With the pathophysiology not clearly understood and fewer than 130 cases having been reported in the literature, diabetic papillopathy presents a special challenge to the ophthalmologist. We report a case of a young patient with more than 12 years of type 1 diabetes mellitus (T1DM) on insulin with poor compliance to treatment who presented with sudden bilateral loss of vision. Ocular examination, fluorescence angiography (FA) and systemic signs were conclusive of diabetic papillopathy. His fasting blood sugar level was high and serum glycosylated haemoglobin (HbA1c) indicated a long term fluctuating blood glucose control. His vision initially improved with treatment, but later deteriorated with tight glycemic control.

    Despite the availability of various clinical trials that used different diagnostic methods to identify diabetic sensorimotor polyneuropathy (DSPN), no reliable studies that prove the associations among diagnostic parameters from two different methods are available. Statistically significant diagnostic parameters from various methods can help determine if two different methods can be incorporated together for diagnosing DSPN. In this study, a systematic review, meta-analysis, and trial sequential analysis (TSA) were performed to determine the associations among the different parameters from the most commonly used electrophysiological screening methods in clinical research for DSPN, namely, nerve conduction study (NCS), corneal confocal microscopy (CCM), and electromyography (EMG), for different experimental groups. Electronic databases (e.g., Web of Science, PubMed, and Google Scholar) were searched systematically for articles reporting different screening tools for diabetic peripheral neuropathy. A total of 22 studies involving 2394 participants (801 patients with DSPN, 702 controls, and 891 non-DSPN patients) were reviewed systematically.     This study was undertaken to clinically estimate the prevalence of diabetic peripheral neuropathy amongst patients attending an outpatient clinic and to evaluate their risk factors for developing peripheral neuropathy. It was a cross-sectional study of 134 diabetes mellitus patients who attended the Primary Care Clinic, University Hospital, Kuala Lumpur. The patients were interviewed for their demographic data, past and present medical/surgical history, social history, personal habits and symptoms of peripheral neuropathy. Foot examination and clinical neurological tests were conducted and the presence of peripheral neuropathy was assessed. The main outcome measures were the Neuropathy Symptom Score and the Neuropathy Disability Score. The prevalence of diabetic peripheral neuropathy was found to be 50.7%. Peripheral neuropathy was related to the age of the patient and the duration of diabetes but did not seem to be significantly related to diabetic control. To conclude, there was a high prevalence of peripheral neuropathy amongst the diabetics in this study. These patients developed peripheral neuropathy at a younger age and shorter duration of diabetes compared to a similar study that was done in the UK.

    Diabetic amyotrophy also known as Bruns-Garland syndrome is diabetic neuropathy subtype affecting the lumbosacral nerve roots and peripheral nerves. There is an ongoing debate on the pathophysiology behind this condition whether is it ischaemic, metabolic (hyperglycaemia) or inflammatory. A 36-year-old man with uncontrolled diabetes mellitus complained of unprovoked weight lost and right proximal thigh discomfort with weakness for one week duration. In neurological examination, his right hip flexion was at medical research council (MRC) grade 3, right hip extension MRC grade 4, his knee and ankle flexion and extension are normal (MRC grade 5). The muscle tones and reflexes were normal. Sensation and proprioception were intact bilaterally. Nerve conduction study (NCS) showed markedly reduced amplitude of the compound muscle action potentials and sensory nerve action potentials, while conduction velocities show only mild slowing. He was started on a course of oral prednisolone 10 mg daily and improved gradually. At three months follow-up, his right lower limb power has recovered fully and he can walk without any assistance. Diabetic amyotrophy was confirmed by suggestive clinical features supported by electrophysiological findings of the affected nerves. This condition is due to metabolic derangement and vasculopathy or immune mediated nerve injury. So, the healthcare providers should be aware about this rare complication of diabetes.

    Satu model matematik dibina bagi menilai pengurusan aktiviti pelancongan rekreasi di Wetland Putrajaya. Kajian ini menggunakan kaedah pengaturcaraan gol (PaG) dan perisian LINDO 6.1.untuk menyelesaikan masalah pelbagai objektif bagi memaksimumkan anggaran keuntungan aktiviti dan bilangan peserta yang terlibat. Tujuh aktiviti di Wetland Putrajaya yang terletak di bawah pengurusan Perbadanan Putrajaya telah dipilih sebagai kes kajian. Data dan maklumat rekod tahun 2008 dijadikan sebagai anggaran untuk kos dan bilangan peserta. Hasil kajian mendapati pihak Wetland Putrajaya boleh mencapai keuntungan lebih 40% daripada jumlah kos dan matlamat untuk memaksimumkan bilangan peserta bagi aktiviti yang terlibat juga tercapai.

    Transistor berasaskan karbon tiub nano (CNTFET) telah dihasilkan melalui kaedah pertumbuhan langsung menggunakan alkohol pemangkin pemendapan wap kimia. Sifat struktur dan ciri elektrik peranti telah dikaji. FET ini menunjukkan operasi ambikutub dan ia selaras dengan jenis-Schottky FET. Ia telah didapati bahawa ketinggian halangan Schottky yang wujud antara sentuhan elektrod dan CNT, boleh termodulat oleh aplikasi berkesan daripada voltan pincang (VDS) dan voltan get (VGS). Aplikasi voltan pincang sahaja tidak mudah untuk merendahkan ketinggian halangan Schottky dan ketebalan halangan Shottky, tetapi ia berkait rapat dengan aplikasi voltan get. Bagi konfigurasi peranti dalam kajian ini, keberkesanan medan boleh dikaitkan dengan VGS:VDS = 10:-1. Peningkatan arus didapati disebabkan oleh pengurangan tenaga pengaktifan. Kaitan yang jelas antara kesan voltan pincang, voltan get dan tenaga pengaktifan telah diperhati dalam kajian ini.
