    As physical abilities and health decline with age, older adults tend to lose their driving abilities, which affects their mobility. As mobility is important to older adults' wellbeing, there is a need to explore alternative modes of transportation to increase their ability to actively participate in society. Hence, this paper aims to understand the characteristics of private chauffeuring and companionship services for older adults, and to assess their possible effects on older adults' wellbeing. We gathered the views of transport operators, government agencies, and city councils that offer private chauffeuring and companionship services for older adults. We frame the model of private chauffeuring and companionship services as alternative mobility for older adults and outline a conceptual framework for its possible effects on their wellbeing. The underlying mobility characteristics were availability, accessibility, safety, and affordability-all of which influence wellbeing. The study found that the private chauffeuring and companionship model for older adults includes an additional model of government-to-consumer services in addition to the existing peer-to-peer and business-to-consumer services. While the services are available, the services provided are not standardized, with different operators offering different services and prices, and limiting certain geographical areas. Transport operators perceived that the services they offer promote older adults' physical and mental health, improve their social participation in the community, and empower them in making their travel decisions. The findings of the paper provide insights for policy makers for future planning of alternative transportation for older adults to enhance their mobility.

    Surface ozone or tropospheric ozone has been recognized as one of the major factors that can give adverse impact on crops including rice plants. Effects of ozone on rice plants could be seen in decreased of biochemical activities and physiological performance which contribute to yield reduction. In Malaysia, surface ozone is on the rise due to increment anthropogenic sources i.e. urbanization, transportation and also industrialization process. This condition is alarming due to the facts that rice is the major staple food to the majority of Malaysian population. In this study, exceedence of ozone exposure above an hourly threshold concentration of 40 ppb (AOT40) and ozone trends in four major rice growing areas in Malaysia were assessed using time series analysis of ozone data recorded in each area from January 2000 until December 2010 with a total of 132 readings. The results showed a steady increase in exceedence ozone of yearly AOT40 and statistical significant upward trend for ozone concentrations in each rice growing area in Malaysia. This finding was particularly alarming because ozone is able to inhibit production of rice yields. Preventive actions need to be implemented as soon as possible in order to alleviate ozone threat to our national food security agenda.

    BACKGROUND: The objective of this paper is to quantify the cost of periodontitis management at public sector specialist periodontal clinic settings and analyse the distribution of cost components.

    METHODS: Five specialist periodontal clinics in the Ministry of Health represented the public sector in providing clinical and cost data for this study. Newly-diagnosed periodontitis patients (N = 165) were recruited and followed up for one year of specialist periodontal care. Direct and indirect costs from the societal viewpoint were included in the cost analysis. They were measured in 2012 Ringgit Malaysia (MYR) and estimated from the societal perspective using activity-based and step-down costing methods, and substantiated by clinical pathways. Cost of dental equipment, consumables and labour (average treatment time) for each procedure was measured using activity-based costing method. Meanwhile, unit cost calculations for clinic administration, utilities and maintenance used step-down approach. Patient expenditures and absence from work were recorded via diary entries. The conversion from MYR to Euro was based on the 2012 rate (1€ = MYR4).

    RESULTS: A total of 2900 procedures were provided, with an average cost of MYR 2820 (€705) per patient for the study year, and MYR 376 (€94) per outpatient visit. Out of this, 90% was contributed by provider cost and 10% by patient cost; 94% for direct cost and 4% for lost productivity. Treatment of aggressive periodontitis was significantly higher than for chronic periodontitis (t-test, P = 0.003).     This study involves the adoption of the Geographic Information System (GIS) modeling approach to determine the quickest routes for fresh vegetable delivery. During transport, fresh vegetables mainly deteriorate on account of temperature and delivery time. Nonetheless, little attention has been directed to transportation issues in most areas within Kuala Lumpur. In addition, perishable food normally has a short shelf life, thus timely delivery significantly affects delivery costs. Therefore, selecting efficient routes would consequently reduce the total transportation costs. The regression model is applied in this study to determine the parameters that affect route selection with respect to the fastest delivery of fresh vegetables. For the purpose of this research, ArcGIS software with network analyst extension is adopted to solve the problem of complex networks. The final output of this research is a map of quickest routes with the best delivery times based on all variables. The variables tested from regression analysis are the most effective parameters to make the flow of road networks slower. The objective is to improve the delivery services by achieving the least drive time. The main findings of this research are that Land use such as residential area and population as variables are the effective parameters on drive time.

    Perishable products must be transported quickly from its production area to the markets due to the climatic conditions of Malaysia. Deterioration of fresh produce is affected by temperature and delivery time. The cost to achieve such timely delivery of perishable food can affect the revenue of suppliers and retailers. Choosing an efficient delivery route at right time can reduce the total transportation cost. However, insufficient attention has been given to transportation issues with regards to fresh food delivery of greater Kuala Lumpur. The present study involves adoption of the Geographic Information System (GIS) modelling approach to determine the fastest delivery routes for fresh products to several hypermarkets. For this purpose, ArcGIS software was adopted for solving the problem of a complex road networks. With a goal of realizing the shortest time for delivery route planning, impedance function would be integrated by taking into account the time emphasized in the study. The main findings of this study include determination of efficient routes for delivery of fresh vegetables based on minimal drive time. It has been proposed that the fastest route model for delivery of fresh products is based on comparing two time frames within a day. The final output of this research was a map of quickest routes with best delivery time based on two time frames.

    Bike-sharing service has become a popular sustainable means of transportation due to its direct impact on traffic congestion, energy consumption, the environment, and people's quality of life. Existing literature suggests that sustainable consumption can be promoted by engaging consumers with green products. This study examined drivers and the outcome of consumer engagement with bike-sharing services based on the technology acceptance model (TAM). A survey was conducted to collect the data from the users of the bike-sharing service in Kuala Lumpur. Structural equation modelling was used to analyse the data and find the relationship between variables. The empirical analyses showed that perceived ease of use and perceived usefulness of the bike-sharing service positively impact all facets of consumer engagement with bike-sharing service, which subsequently influences the continuance usage intention of bike-sharing service. The findings of this study offer useful insights that could enhance the consumption of bike-sharing service. This study also offers some guidelines to transportation practitioners, policymakers, and urban planners regarding promoting healthy and sustainable travel behaviour among urban commuters through bike-sharing service.

    INTRODUCTION: Past empirical studies indicated that there is a Kuznets or reverse U-shaped relationship between road deaths and per capita income, such that the number of road death increases at a low level of per capita reverse U-shaped relationship was observed between road injuries and per capita income. While these studies explored the impact of per capita income on road deaths and road injuries, no studies have examined the relationship between per capita income and road death to road injury ratio (DPI).

    METHOD: Using a fixed effects panel regression analysis from 67 countries spanning over a period of five decades (1960-2013), the present study sought to explore the impact of per capita gross domestic product (per capita GDP) on the DPI ratio and the underlying factors responsible for the relationship.

    RESULTS: Our result suggests that per capita GDP followed a reverse U-shaped relationship with DPI. Moreover, the relative improvements in higher mobility roads as compared to improvements in higher accessibility roads, motorcycle ownership to passenger car ownership ratio, percentage of population living in an urban area, infant mortality rate, and the percentage of population below 15 years of age and above 64 years of age contributed to this relationship. Practical Applications: This implies that, at lower level of economic growth where road deaths exceed road injuries, countries should implement low-cost measures to combat road deaths cases. Such measures include mandating wearing of quality helmets and daytime running headlights for motorcycles. On the other hand, at higher level of economic growth where road injuries surpass road deaths, countries are encouraged to devote more resources to improving medical technology and services to treat road injury victims, mandating seatbelt use, as well as enhancing and promoting public transportation service.

    A bank explosion in a neighbouring country over 1000 km away resulted in ten badly burned victims being airlifted to the Burns Centre, Singapore General Hospital (BCSGH) for treatment. The severely injured included patients with 90%, 80%, 74%, 66%, 45%, 33% and 31% burns. Nine had respiratory burns (four severe, one moderate, four mild). One patient died, thus, the mortality rate for the six most severely injured was 16.7%. This differs from predicted mortality rates of 78% according to McCoy or 54% according to Thompson, Herndon et al. The factors contributing to this result were the small size of the disaster, the use of an established Burns Mass Disaster plan and an individual management policy that incorporates carefully monitored fluid resuscitation, recognition of respiratory burns with early treatment by intubation thus pre emptying complications, early surgery and a multidisciplinary approach to complications such as infection and renal failure. The average length of stay was 43 days (range 5-122 days). The cost of the hospitalisation of the ten casualties was $312,317.00.

    Introduction: The purpose of this study was to determine the prevalence of defaulters of immunization, and their associated risk factors among children age 12 to 24 months. Materials and Methods: A cross-sectional study was conducted in all government's maternal child health clinics in District of Kota Kinabalu, Sabah. Data was collected using a standardised questionnaire from July to November 2006. Results: The prevalence rate for defaulting immunization was 16.8% from the 315 respondents. Bivariable analysis showed various significant factors associated with defaulters such as mother’s employment status, family mobility, transportation and cost. Nonetheless, multivariable analysis showed only mother’s age, mother employment status and family size were the significant predictors for defaulting immunization. Immunization that had the highest rate of defaulters was DPT–OPV booster dose (56.6%), followed by MMR immunization (43.4 %) and
    DPT-Hib/OPV and Hep B third dose (37.7%).Conclusion: Employed mothers with bigger family size should be more closely monitored and advised to reduce the chance of defaulting on the immunization. Health promotion activities also should focus to these groups of mothers.

    Peningkatan aktiviti perbandaran dan perindustrian telah mencetuskan masalah pengurusan sisa pepejal. Sebagai usaha penyelesaian, pendekatan bersepadu telah dipilih bagi menguruskan sisa pepejal. Membangunkan dan melaksanakan rancangan pengurusan sisa pepejal bersepadu perlu melibatkan gabungan teknologi dan pilihan yang sesuai dengan keadaan dan undang-undang tempatan. Kajian ini menunjukkan Proses Analisis Hierarki (PAH) berpotensi sebagai kaedah membuat keputusan yang boleh digunakan dalam proses pemilihan teknologi pengurusan sisa pepejal. Tiga aras hierarki dibangunkan dengan matlamat di aras tertinggi, diikuti oleh kriteria dan alternatif. Dengan menggunakan teknik ini, penentuan keutamaan untuk semua teknologi pengurusan sisa pepejal yang dipertimbangkan akan ditentukan dan teknologi dengan nilai keutamaan tertinggi lebih sesuai untuk dibangunkan. Analisis sensitiviti dilakukan bagi menguji sensitiviti keputusan akhir terhadap perubahan kecil penilaian. Aplikasi PAH dalam menentukan keutamaan proses pemilihan teknologi pengurusan sisa pepejal diperjelaskan dalam kajian ini berdasarkan kepada kajian kes di Majlis Perbandaran Port Dickson. Hasil analisis menunjukkan kombinasi teknologi kitar semula dan pengkomposan sesuai diaplikasikan di daerah Port Dickson.

    Working for a minimum of 8 hours, 6 days a week might have exposed the workers of public transportation to
    high noise risks. However, occupational exposures in their workplace have not been adequately characterized and
    identified. Assessment of occupational noise exposure among workers at five public transportation stations was made
    using Sound Level Meter and through questionnaire survey. The data obtained was combined to estimate the work
    shift exposure level and health impacts to the workers by using statistical analysis. The respondents participated in the
    survey to identify the symptoms of noise-induced hearing loss and other health-related problems. Results of the study
    indicated that occupational noise exposure among workers for Mean Continuous Equivalent Level, Leq= 76.17 dB(A)
    presents small risks of developing a hearing disability. Some of the workers show symptoms of noise-induced hearing
    loss and are annoyed by the sources of noise present at the public transportation.

    The continuous depletion of global oil reserves with the propensity for light distillates
    propels the oil and gas industry to explore heavier fractions of crude oils with significant
    amount of paraffin waxes. However, the precipitation and deposition of waxes during the
    transportation of these waxy crude oils in the pipelines contribute to several issues, such as
    the flowability reduction, excessive pumping cost, and wax gel formation, that adversely
    affect the supposedly steady offshore oil production. As a result, substantial resources are
    expended to resolve these flow assurance problems. The wax inhibitors and pour point
    depressants are developed and modified to meet the wax remediation criteria. Essentially,
    the wax crystals are formed through the nucleation, growth, and agglomeration processes,
    while the deposition of these waxes occurs via molecular diffusion and shear dispersion.
    The wax inhibitors are able to control the growth of wax crystals through nucleation, cocrystallization,
    adsorption, and dispersion interactions. This paper particularly assessed
    the following compounds: (1) polymeric wax inhibitors, (2) nano-hybrid pour point
    depressants, (3) organic solvents, and
    (4) surfactants. Given the significance of
    these compounds in the deposition and
    precipitation of waxes, it is imperative to
    comprehensively explore the types and
    nature of these compounds and their recent
    applications as well as to critically assess
    their strengths and drawbacks, which were
    addressed in this paper. Furthermore, the
    challenges of using these compounds and the factors that govern their efficiencies were also discussed. Accordingly, the carbon
    length and the molecular weight of both paraffin waxes and wax inhibitors are among the
    most influential factors.

    Stress has been recognized one of the factors causing disease. About 70-80% of all diseases may be stress related. Thus, stress management can be a part of an early measure of disease prevention. A descriptive cross sectional, randomized study was conducted to determine the stress inducing factors among preclinical students (universal sampling) in a public university in Selangor, Malaysia from 24th April to May 2005. A total of 163 students (52.8% year 1, 36.8% year 2 and 10.4% year 3) were interviewed in the data collection process. The main reasons students entered - medical school was because of their own interest or ambition (65%) and family influence (20.9%). Majority (76.4%) suffered moderate to great stress over hot conditions in lecture hall, tutoriaV small group session rooms and laboratories while 53.4% suffered when using the other facilities like cafeteria, toilet and transportation:. Almost all (95.1%) felt that examination was the most stressful, followed by early clinical exposure sessions (68.1%), problem·based learning sessions (62.5%), hospital visitations (59.7%), tutoriay small group sessions (49.3%), practical class (44.5%) and attending lectures (3 8.5%). Musculoskeletal System was the most stressful module among the first year students, followed by Nervous System and Gastrointestinal System with the percentage of 94.2%, 90.7% and 88.4% respectively while, 95% of the second year students felt that General, Hemopoietic ci? Lymphoid and Nervous System are the most stressful modules. This study revealed that academic sessions and lack of conducive teaching and learning environment as the main stress inducing contributors to preclinical medical students.

    Inventory Routing Problem (IRP) has been continuously developed and improved due to pressure from global warming issue particularly related to greenhouse gases (GHGs) emission. The burning of fossil fuel for transportations such as cars, trucks, ships, trains, and planes primarily emits GHGs. Carbon dioxide (CO2) from burning of fossil fuel to power transportation and industrial process is the largest contributor to global GHGs emission. Therefore, the focus of this study is on solving a multi-period inventory routing problem (MIRP) involving carbon emission consideration based on carbon cap and offset policy. Hybrid genetic algorithm (HGA) based on allocation first and routing second is used to compute a solution for the MIRP in this study. The objective of this study is to solve the proposed MIRP model with HGA then validate the effectiveness of the proposed HGA on data of different sizes. Upon validation, the proposed MIRP model and HGA is applied on real-world data. The HGA is found to be able to solve small size and large size instances effectively by providing near optimal solution in relatively short CPU execution time.

    A vehicular ad hoc network (VANET) is an emerging and promising wireless technology aimed to improve traffic safety and provide comfort to road users. However, the high mobility of vehicles and frequent topology changes pose a considerable challenge to the reliable delivery of safety applications. Clustering is one of the control techniques used in VANET to make the frequent topology changes less dynamic. Nevertheless, research has shown that most of the existing clustering algorithms focus on cluster head (CH) election with very few addressing other critical issues such as cluster formation and maintenance. This has led to unstable clusters which could affect the timely delivery of safety applications. In this study, enhanced weight-based clustering algorithm (EWCA) was developed to address these challenges. We considered any vehicle moving on the same road segment with the same road ID and within the transmission range of its neighbour to be suitable for the cluster formation process. This was attributed to the fact that all safety messages are expected to be shared among the vehicles within the vicinity irrespective of their relative speedto avoid any hazardous situation. To elect a CH, we identified some metrics on the basis of the vehicle mobility information. Each vehicle was associated with a predefined weight value based on its relevance. A vehicle with the highest weight value was elected as the primary cluster head (PCH). We also introduced a secondary cluster head (SeCH) as a backup to the PCH to improve the cluster stability. SeCH took over the leadership whenever the PCH was not suitable for continuing with the leadership. The simulation results of the proposed approach showed a better performance with an increase of approximately40%- 45% in the cluster stability when compared with the existing approaches. Similarly, cluster formation messages were significantly minimized, hence reducing the communication overhead to the system and improving the reliable delivery of the safety applications.

    OBJECTIVE: To examine the effect of emergency immobilization on neurologic outcome of patients who have blunt traumatic spinal injuries.

    METHODS: A 5-year retrospective chart review was carried out at 2 university hospitals. All patients with acute blunt traumatic spinal or spinal cord injuries transported directly from the injury site to the hospital were entered. None of the 120 patients seen at the University of Malaya had spinal immobilization during transport, whereas all 334 patients seen at the University of New Mexico did. The 2 hospitals were comparable in physician training and clinical resources. Neurologic injuries were assigned to 2 categories, disabling or not disabling, by 2 physicians acting independently and blinded to the hospital of origin. Data were analyzed using multivariate logistic regression, with hospital location, patient age, gender, anatomic level of injury, and injury mechanism serving as explanatory variables.

    RESULTS: There was less neurologic disability in the unimmobilized Malaysian patients (OR 2.03; 95% CI 1.03-3.99; p = 0.04). This corresponds to a <2% chance that immobilization has any beneficial effect. Results were similar when the analysis was limited to patients with cervical injuries (OR 1.52; 95% CI 0.64-3.62; p = 0.34).

    CONCLUSION: Out-of-hospital immobilization has little or no effect on neurologic outcome in patients with blunt spinal injuries.

    The physico-mechanical properties data of fruits are important in the design of various handling, packing, and storage and transportation system. The physical-mechanical properties of pineapple fruit from the Josapine variety, namely the weight of the fruit (with and without peel), pulp to peel ratio, diameter of the whole fruit (with and without peel), at three different positions along the longitudinal axis of the fruit, length of the fruit (with and without peel) and the length of crown were studied using the standard method at seven stages of maturity during storage at 25°C and 52% (RH). The effect of fruit maturity on the firmness of each fruit at three different locations was measured using a cylindrical die of 6 mm in diameter with the Instron Universal Testing Machine. The results indicated that the average total weight of a single fruit is 886.86 ± 49.67 g. The average pulp to peel ratio is 1.91. The average diameter (with and without peel) was 86.83 ± 5.24 mm and 80.95 ± 4.15 mm (top section), 100.77 ± 3.84 mm and 90.19 ± 3.73 mm (middle section) and 97.17 ± 3.49 mm and 73.30 ± 5.11 mm (bottom section), respectively. The average length of the fruit (with and without peel) was 126.65 mm and 113.64 mm, respectively. The average length of crown was 89.13 mm. The firmness of the fruits was found to decrease with the stage of maturity. These data are important in determining the optimum stage of maturity for fruit processing.
