    The increase in the number of fatalities caused by road accidents involving heavy vehicles every year has raised the level of concern and awareness on road safety in developing countries like Malaysia. Changes in the vehicle dynamic characteristics such as gross vehicle weight, travel speed, and vehicle classification will affect a heavy vehicle's braking performance and its ability to stop safely in emergency situations. As such, the aim of this study is to establish a more realistic new distance-based safety indicator called the minimum safe distance gap (MSDG), which incorporates vehicle classification (VC), speed, and gross vehicle weight (GVW).

    Many major accidents due to toxic release in the past have caused many fatalities such as the tragedy of MIC release in Bhopal, India (1984). One of the approaches is to use inherently safer design technique that utilizes inherent safety principle to eliminate or minimize accidents rather than to control the hazard. This technique is best implemented in preliminary design stage where the consequence of toxic release can be evaluated and necessary design improvements can be implemented to eliminate or minimize the accidents to as low as reasonably practicable (ALARP) without resorting to costly protective system. However, currently there is no commercial tool available that has such capability. This paper reports on the preliminary findings on the development of a prototype tool for consequence analysis and design improvement via inherent safety principle by utilizing an integrated process design simulator with toxic release consequence analysis model. The consequence analysis based on the worst-case scenarios during process flowsheeting stage were conducted as case studies. The preliminary finding shows that toxic release consequences analysis tool (TORCAT) has capability to eliminate or minimize the potential toxic release accidents by adopting the inherent safety principle early in preliminary design stage.

    Work shift has been shown to correlate with accident rates. Understanding of such correlation is pertinent especially among emergency response personnel since the decisions that they make determines not only the outcome of their responses but also the risks of accidents to themselves. A questionnaire data derived study used together with a semi quantitative risk analysis method was adopted to estimate the levels of accident risks between firefighters working on two work shifts. Two hundred and forty eight Malaysia’s Fire and Rescue Department firemen from 24 fire stations working on shifts were selected as respondents. The accident rate among firefighters in year 2006 was 52.8 %. Results showed that the Accident Risk Index (ARI) among firefighters working the 24 hours shift was higher (ARI = 3.14) compared with those in the 12 hours shift (ARI = 2.98). However, there were no significant difference in overall severity of the accidents between the two shifts (p>0.05). The difference in risk levels was attributed to the difference in the likelihood of accident occurrence.

    OBJECTIVE: The objective of this prospective cohort study was to analyse the characteristics of severe Traumatic Brain Injury (TBI) in a regional trauma centre Hospital Kuala Lumpur (HKL) along with its impact of various prognostic factors post Decompressive Craniectomy (DC).
    MATERIALS AND METHODS: Duration of the study was of 13 months in HKL. 110 consecutive patients undergoing DC and remained in our centre were recruited. They were then analysed categorically with standard analytical software.
    RESULTS: Age group have highest range between 12-30 category with male preponderance. Common mechanism of injury was motor vehicle accident involving motorcyclist. Univariate analysis showed statistically significant in referral area (P = 0.006). In clinical evaluation statistically significant was the motor score (P = 0.040), pupillary state (P = 0.010), blood pressure stability (P = 0.013) and evidence of Diabetes Insipidus (P < 0.001). In biochemical status the significant statistics included evidence of coagulopathy (P < 0.001), evidence of acidosis (P = 0.003) and evidence of hypoxia (P = 0.030). In Radiological sector, significant univariate analysis proved in location of the subdural clot (P < 0.010), location of the contusion (P = 0.045), site of existence of both type of clots (P = 0.031) and the evidence of edema (P = 0.041). The timing of injury was noted to be significant as well (P = 0.061). In the post operative care was, there were significance in the overall stability in intensive care (P < 0.001), the stability of blood pressure, cerebral perfusion pressure, pulse rates and oxygen saturation (all P < 0.001)seen individually, post operative ICP monitoring in the immediate (P = 0.002), within 24 hours (P < 0.001) and within 24-48 hours (P < 0.001) period, along with post operative pupillary size (P < 0.001) and motor score (P < 0.001). Post operatively, radiologically significant statistics included evidence of midline shift post operatively in the CT scan (P < 0.001). Multivariate logistic regression with stepwise likelihood ratio (LR) method concluded that hypoxia post operatively (P = 0.152), the unmaintained Cerebral Perfusion Pressure (CPP) (P = 0.007) and unstable blood pressure (BP) (P = <0.001). Poor outcome noted 10.2 times higher in post operative hypoxia [OR10.184; 95% CI: 0.424, 244.495]. Odds of having poor outcome if CPP unmaintained was 13.8 times higher [OR: 13.754; CI: 2.050, 92.301]. Highest predictor of poor outcome was the unstable BP, 32 times higher [OR 31.600; CI: 4.530, 220440].
    CONCLUSION: Our series represent both urban and rural population, noted to be the largest series in severe TBI in this region. Severe head injury accounts for significant proportion of neurosurgical admissions, resources with its impact on socio-economic concerns to a growing population like Malaysia. This study concludes that the predictors of outcome in severe TBI post DC were postoperative hypoxia, unmaintained cerebral perfusion pressure and unstable blood pressure as independent predictors of poor outcome.     Since the world’s first nuclear reactor major breakthrough in December 02, 1942, the nuclear power industry has undergone tremendous development and evolution for more than half a century. After surpassing moratorium of nuclear power plant construction caused by catastrophic accidents at Three-Mile Island (1979) and Chernobyl (1986), today, nuclear energy is back on the policy agendas of many states, both developed and developing nations, signaling nuclear revival or nuclear renaissance. Selection of suitable nuclear power technology has thus been subjected to primary attention. This short paper attempts to draw preliminary technology assessment for the first nuclear power reactor technology for Malaysia. Methodology employed is qualitative analysis collating recent finding of TNB-KEPCO Preliminary Feasibility Study for Nuclear Power Program in Peninsular Malaysia and other published presentations and/or papers by multiple experts.
    The results suggested that the pressurized water reactor (PWR) is the prevailing technology in terms of numbers and plant performances, and while the commercialization of Gen IV reactors is remote (e.g. not until 2030), Generation III/III+ NPP models are commercially available on the market today. Five (5) major steps involved in reactor technology selection were introduced with a focus on introducing important aspects of selection criteria. Three (3) categories for the of reactor technology selection were used for the cursory evaluation. The outcome of these analyses shall constitute deeper and full review analyses of the recommended reactor technologies for the intended full feasibility study in the near future. Recommendations for reactor technology option were also provided for both strategic and technical recommendations. The paper shall also postulate or rather implore what could be the best way for Malaysian and also other aspiring new entrant nations to select systematically their first civilian nuclear power reactor.

    Trauma is thought to complicate 1 in 12 pregnancies. The management of trauma during pregnancy requires special consideration because pregnancy alters maternal physiology and the foetus is a potential collateral victim. The approach of these cases in the setting of the Emergency Department should not only be diagnostic for any foetal injuries but also prognostic for any future undue outcome. Antenatal traumatic brain injury is a rare but real complication of maternal blunt force trauma. Our case involves a 22-year-old primigravida who suffered a motor vehicle accident and on initial assessment revealed normal foetal assessment but subsequently after premature labour revealed a new born with traumatic brain injury. Early ultrasonographic evaluation and observational period with continuous electronic foetal monitoring may improve the detection and emergent treatment in these cases.

    Blood alcohol levels from 155 UKM forensic postmortem cases of various causes of death from August 1988 to mid-September 1989 were studied. 59 cases (38%) were related to fatal road traffic accidents. 13 of these 59 cases (22%) showed blood alcohol levels of more than 50 mg/dl. 11 of these 13 cases (84.6%) revealed blood alcohol levels of more than 100 mg/dl after correction. Further analysis showed 53.8% were Indian, 76.9% aged between 20 and 40 years, 84.6% of the accidents occurred between 8.00 p.m. and 4.00 a.m. and all the alcohol related fatal road traffic accident victims studied in this series were males. This study provides helpful information.

    Individual factors have been considered to be a key issue to organizations’ performance and survival. Specifically, previous studies provide evidence that individual factors are closely associated with accidents in the workplace. Therefore, this study seeks to investigate the relationship between individual factors and workplace safety. 50 respondents have been randomly chosen from a manufacturing company in Johor Bahru, Malaysia. Data are collected using a survey and are analyzed using SPSS version 16.0. A descriptive statistic, cross tabulation, independent sample t-test and one-way between groups ANOVA are employed to interpret findings of the study. Findings show that employees’ perception on safety performance is at high level (Mean= 115, Standard Deviation= 17.73). Furthermore, the results of cross tabulation demonstrate that individual factors play a key role to determine the organization safety performance level. Another significant finding reveals that there is a significant difference of safety performance mean to two of individual factors namely gender and tenure of service. Individual factors namely gender, age, tenure of service and academic qualification give a significant impact to employees’ perception on safety performance.

    Recent development in industrialization and global economy has contributed to the increased number of workplace
    injuries and accidents. Safety culture has been seen as a central medium to curb the worrying trend of workplace
    accidents. The term of safety culture has emerged from the Chernobyl nuclear disaster in 1986 and the concept has
    been widely used until today. However, the conceptualization of safety culture suffers from several major drawbacks
    such as no definite understanding of how safety culture is being defined and measured. This paper hypothesized that
    higher levels of safety culture in the workplace will display lower numbers of accidents. Due to this concern, this
    paper seeks to provide a clear conceptualization of safety culture, dimensions used to measure it and common research
    methodology used in the previous safety research.

    BACKGROUND: Sleep apnoea is a diagnosed condition in which appurtenances interrupt breathing whilst sleeping. The consequence of obstructive sleep apnoea (OSA) includes road traffic accidents due to drowsiness, systemic hypertension, heart disease, diabetes mellitus and neurocognitive disorders. This condition extends the duration of recovery phase after traumatic brain injury.

    OBJECTIVE: This study was intended to assess the knowledge and attitude towards OSA and compare it among dental and medical undergraduate students of University Sains Malaysia (USM).

    METHODS: In this study, a total of 216 clinical undergraduate students (36 from each year; 108 from medical and 108 from dental school) of University Sains Malaysia (USM) Health campus were recruited for the study by non-probability stratified random sampling method. Total study period was October 2017 to October 2018. A self-administered questionnaire was used to assess the sociodemographic status and OSAKA questionnaire was used to assess knowledge and attitude regarding sleep apnoea of the respondents. Descriptive analysis was carried out to assess the knowledge and attitude of OSA amongst dental and medical undergraduate students of USM. The Mann-Whitney U test was carried out to compare the knowledge and attitude of OSA amongst dental and medical undergraduate students of USM.

    RESULTS: Our study findings revealed that 0.9 %and 6.5 %of the dental undergraduate students and medical undergraduate students could answer all the questions correctly regarding knowledge of OSA.     Retroperitoneal duodenal rupture due to blunt abdominal trauma is relatively uncommon. Diagnosis is difficult because the physical signs are subtle or they are difficult to interpret. A case of retroperitoneal duodenal rupture due to blunt abdominal trauma in whom the diagnosis was delayed is reported. Radiographic features are discussed. The role of early roentgen diagnosis is stressed in order to reduce the very high mortality which is associated with delay in diagnosis and treatment.

    Intracavernous carotid artery aneurysm following head injury is a rare occurrence. Two such cases presenting with delayed but massive and repeated epistaxis are reported and the literature reviewed. The first case required a trapping procedure while the second case had only cervical carotid ligation to control the bleeding. Both resulted in no neurological sequelae.

    Most highly motorized countries in the world have implemented different speed limits for light weight and heavy weight vehicles. The heavy vehicle speed limit is usually chosen to be lower than that of passenger cars due to the difficulty for the drivers to safely maneuver the heavy vehicle at high speed and greater impact during a crash. However, in many cases, the speed limit for heavy vehicle is set by only considering the vehicle size or category, mostly due to simplicity in enforcement. In this study, traffic and vehicular data for all vehicle types were collected using a weigh-in-motion system installed at Federal Route 54 in Malaysia. The first finding from the data showed that the weight variation for each vehicle category is considerable. Therefore, the effect of gross vehicle weight (GVW) and category of heavy vehicle on free flow speed and their interaction were analyzed using statistical techniques. Empirical analysis results showed that statistically for each type of heavy vehicle, there was a significant relationship between free flow speed of a heavy vehicle and GVW. Specifically, the results suggest that the mean and variance of free flow speed decrease with an increase GVW by the amount unrelated to size and shape for all GVW range. Then, based on the 85th percentile principle, the study proposed a new concept for setting the speed limit for heavy vehicle by incorporating GVW where a different speed limit is imposed to the heavy vehicle, not only based on vehicle classification, but also according to its GVW.

    In recent years, driver drowsiness has been one of the major causes of road accidents and can lead to severe physical injuries, deaths and significant economic losses. Statistics indicate the need of a reliable driver drowsiness detection system which could alert the driver before a mishap happens. Researchers have attempted to determine driver drowsiness using the following measures: (1) vehicle-based measures; (2) behavioral measures and (3) physiological measures. A detailed review on these measures will provide insight on the present systems, issues associated with them and the enhancements that need to be done to make a robust system. In this paper, we review these three measures as to the sensors used and discuss the advantages and limitations of each. The various ways through which drowsiness has been experimentally manipulated is also discussed. We conclude that by designing a hybrid drowsiness detection system that combines non-intrusive physiological measures with other measures one would accurately determine the drowsiness level of a driver. A number of road accidents might then be avoided if an alert is sent to a driver that is deemed drowsy.

    Occupational Health and Safety (OHS)-related injuries are vexing problems for construction projects in developing countries, mostly due to poor managerial-, governmental-, and technical safety-related issues. Though some studies have been conducted on OHS-associated issues in developing countries, research on this topic remains scarce. A review of the literature shows that presenting a predictive assessment framework through machine learning techniques can add much to the field. As for Malaysia, despite the ongoing growth of the construction sector, there has not been any study focused on OHS assessment of workers involved in construction activities. To fill these gaps, an Ensemble Predictive Safety Risk Assessment Model (EPSRAM) is developed in this paper as an effective tool to assess the OHS risks related to workers on construction sites. The developed EPSRAM is based on the integration of neural networks with fuzzy inference systems. To show the effectiveness of the EPSRAM developed, it is applied to several Malaysian construction case projects. This paper contributes to the field in several ways, through: (1) identifying major potential safety risks, (2) determining crucial factors that affect the safety assessment for construction workers, (3) predicting the magnitude of identified safety risks accurately, and (4) predicting the evaluation strategies applicable to the identified risks. It is demonstrated how EPSRAM can provide safety professionals and inspectors concerned with well-being of workers with valuable information, leading to improving the working environment of construction crew members.

    Given the safety issues associated with flammability characteristics of alternative environmentally-friendly refrigerants, it is vital to establish measurement systems to accurately analyse the flammability of these mildly flammable refrigerants. In this study, we used a customised Hartmann bomb analogue to measure the minimum ignition energy (MIE) and laminar burning velocity (BV) for refrigerant/air mixtures of pure ammonia (R717), R32, R1234yf and mixtures of R32 and R1234yf with non-flammable refrigerants of R134a, R125 and carbon dioxide (R744). The MIEs of R717, R32, and R1234yf were measured at an ambient temperature of 24 °C to be (18.0 ± 1.4), (8.0 ± 1.5) and (510 ± 130) mJ at equivalence ratios of 0.9, 1.27 and 1.33, respectively. Adding the non-flammable refrigerants R134a, R125 and R744 along with R32 at volumetric concentrations of 5% each to R1234yf reduced the latter compound's flammability and increased its MIE by one order of magnitude. The laminar burning velocities of pure R717 and R32 were measured at an equivalence ratio of 1.1 using the flat flame method and found to be 8.4 and 7.4 cm/s, respectively. Adding 5% R1234yf to R32 decreased the laminar burning velocity by 11%, while a further 5% addition of R1234yf resulted in a decrease of over 30% in the laminar burning velocity.

    Traumatic chest injury with complete tracheo- bronchial disruption is uncommon and occurs in approximately 1% of motor vehicle accidents (MVA) (1,2). Such injuries carry a high mortality and patients rarely survive transfer to hospital. A high index of suspicion facilitates early diagnosis. Early operative intervention is vital for survival. We describe a rare case of traumatic complete disruption of the right mainstem bronchus (RMB) due to blunt chest trauma. The transected airway was reanastomosed emergently avoiding a lung resection.
