    OBJECTIVE: To study the impact of various clinicopathological factors on short-term survival in a cohort of breast cancer patients treated at the University of Malaya Medical Centre (UMMC).
    METHODS: All cases of breast cancer treated at UMMC from January 1999 to June 2001, except for stage IV disease, were included in the study. Survival analysis was carried out using Kaplan-Meier for univariate analysis and Cox regression for multivariate analysis. The log-rank test was used to test the significance of differences between the different survival curves.
    RESULTS: A total of 385 patients were included. The mean patient age at presentation was 50.3 years (SD, 11.4); 198 (51.4%) patients had lymph node-positive disease, and 187 (48.6%) had node-negative disease. The mean follow-up period was 18.7 months (SD, 8.8). The Malay ethnic group, tumours of larger size, node-positive disease, more than five positive lymph nodes, oestrogen receptor (ER) negativity and the presence of lymphovascular invasion were significant prognostic factors for shorter recurrence-free survival (RFS) in the univariate analysis. In the multivariate analysis, ER negativity was the only independent adverse prognostic factor for RFS. For overall survival (OS), tumours of larger size, node-positive disease, more than five positive lymph nodes, ER negativity and high grade tumours were associated with significantly shorter OS. However, more than five positive lymph nodes was the only independent prognostic factor for shorter OS in the multivariate analysis. Further multivariate analysis of the patients with node-positive disease showed that the Malay ethnic group, ER negativity and more than five positive lymph nodes were independent prognostic factors for shorter RFS. On the other hand, ER negativity and more than five positive lymph nodes were independent negative prognostic factors for OS in this subgroup of patients.
    CONCLUSION: The evaluation of various prognostic factors would provide useful information on disease progression in local patients, especially for the planning of adjuvant therapies and follow-up protocols. Differences in the pattern of breast cancer among the different ethnic groups in Malaysia warrant further studies.

    Objectives: To evaluate the effect of light-cure devices and curing times on the shear bond strength (SBS) of orthodontic brackets.

    Material and Methods: 60-extracted human premolars were divided into 6-groups of 10-teeth each and bonded with stainless-steel brackets by using 3M Unitek Transbond XT composite. Specimens were cured with halogen, LED and plasma arc lights with two different times for each. The specimens were subjected to shear force till debond with a crosshead speed of 1mm/min and tested after 5min. The stress was calculated and data were subjected to statistical analysis.

    Results: one-way ANOVA and Dunnett T3 post hoc comparison test were used. There were no significant differences between the 6 groups (p < 0.05).

    Conclusions: all curing light methods with loading force after 5 min achieved SBS more than the normal range; therefore, arch wire can be inserted at the same visit using any of tested curing light device or curing time.

    Introduction: Owing to the variety of materials and methods employed, comparison of the results and findings from bonding studies is difficult. Until recently, several types of teeth have been used in published research papers as a substrate in orthodontic bonding research including bovine incisors, fresh and rebonded human premolars. Objectives: The purpose of this study was to compare the shear bond strength of an adhesive bonded to different tooth surfaces (human premolar, bovine incisor and rebonded human premolar). Methods: Two groups of thirty premolar teeth and one group of bovine incisors had brackets attached in a standardized manner using Transbond XT (3M Unitek). The adhesive was cured using conventional halogen light and a specially designed tool to standardize the distance between the light curing tip and the adhesive.     We performed a comparative study to select the efficient mother wavelet (MWT) basis functions that optimally represent the signal characteristics of the electrical activity of the human brain during a working memory (WM) task recorded through electro-encephalography (EEG). Nineteen EEG electrodes were placed on the scalp following the 10-20 system. These electrodes were then grouped into five recording regions corresponding to the scalp area of the cerebral cortex. Sixty-second WM task data were recorded from ten control subjects. Forty-five MWT basis functions from orthogonal families were investigated. These functions included Daubechies (db1-db20), Symlets (sym1-sym20), and Coiflets (coif1-coif5). Using ANOVA, we determined the MWT basis functions with the most significant differences in the ability of the five scalp regions to maximize their cross-correlation with the EEG signals. The best results were obtained using "sym9" across the five scalp regions. Therefore, the most compatible MWT with the EEG signals should be selected to achieve wavelet denoising, decomposition, reconstruction, and sub-band feature extraction. This study provides a reference of the selection of efficient MWT basis functions.

    PURPOSE: To investigate the monthly and seasonal variation in adult osteoporotic fragility fractures and the association with weather.

    METHODS: 12-year observational study of a UK Fracture Liaison Service (outpatient secondary care setting). Database analyses of the records of adult outpatients aged 50 years and older with fragility fractures. Weather data were obtained from the UK's national Meteorological Office. In the seasonality analyses, we tested for the association between months and seasons (determinants), respectively, and outpatient attendances, by analysis of variance (ANOVA) and Tukey's test. In the meteorological analyses, the determinants were mean temperature, mean daily maximum and minimum temperature, number of days of rain, total rainfall and number of days of frost, per month, respectively. We explored the association of each meteorological variable with outpatient attendances, by regression models.

    RESULTS: The Fracture Liaison Service recorded 25,454 fragility fractures. We found significant monthly and seasonal variation in attendances for fractures of the: radius or ulna; humerus; ankle, foot, tibia or fibula (ANOVA, all p-values <0.05). Fractures of the radius or ulna and humerus peaked in December and winter. Fractures of the ankle, foot, tibia or fibula peaked in July, August and summer. U-shaped associations were showed between each temperature parameter and fractures. Days of frost were directly associated with fractures of the radius or ulna (p-value <0.001) and humerus (p-value 0.002).

    CONCLUSION: Different types of fragility fractures present different seasonal patterns. Weather may modulate their seasonality and consequent healthcare utilisation.

    Extraction of collagen from muscles of cultured catfish (Clarias gariepinus × C. macrocephalus) with the aid of pepsin digestion was investigated using a statistical tool. Fractional factorial design (FFD) was applied to evaluate the effects of eight process parameters: acetic acid concentration, acid extraction time, acid extraction temperature, acetic acid to muscles ratio, NaOH concentration, NaOH to muscles ratio, NaOH treatment time, and stirring speed. Contribution of every parameter in influencing the extraction efficiency was evaluated and factors that significantly affected the extraction were elucidated by employing experimental design and analysis of variance in FFD. The result of first order factorial design showed that acetic acid concentration, acid extraction time, acid to muscles ratio, and stirring speed had significant effect (P < 0.05) to the yield of pepsin soluble collagen (PSC) obtained at the end of the experiment. Effects of these process factors on the efficiency of collagen extraction were investigated, and are discussed in detail. Optimum conditions were found at 0.5 M acetic acid, 16 hr extraction period, solvent to muscles ratio at 25 ml/g, and stirring speed of 400 rpm, resulting in yield of PSC as high as 211.49 ± 15.51 mg/g.

    The study of lead exposure among workers in Selangor and the Federal Territory was carried out based on the delta-aminolevulinic acid (ALA) level in urine. Occupations which are expected to have higher lead exposure were chosen in this research. The ALA level in the workers' urine was linked to a few variables which may contribute to the lead level in the body. The result of this study showed that the ALA level of the urine of university students (0.352 +/- 0.038 mg/100 ml) < clerical staff (0.560 +/- 0.043 mg/100 ml) < traffic police (0.612 +/- 0.064 mg/100 ml) < vehicle workshop workers (0.673 +/- 0.099 mg/100 ml) < petrol kiosk workers (0.717 +/- 0.069 mg/100 ml) < bus drivers/conductors (0.850 +/- 0.055 mg/100 ml) which was similar to workers in the printing industry (0.852 +/- 0.110 mg/100 ml). The ALA levels in the urine of the exposed workers were significantly different from the control group (university students). However, results obtained from clerical staff revealed that they were also in the exposed group category. Analysis of variance showed that the exposed groups are in a population which is different from the control population. Correlation tests suggest that there is no significant connection between the ALA level in the urine and the variables tested. Furthermore, Duncan's Multiple Range Test showed no significant differences between the smoking/non smoking group, alcoholic/non-alcoholic group, race and sex (p > 0.05).

    This study aimed to identify the significant factors that give large effects on the efficiency of Cu(II) extraction from aqueous solutions by soybean oil-based organic solvents using fractional factorial design. Six factors (mixing time (t), di-2-ethylhexylphosphoric acid concentration ([D2EHPA]), organic to aqueous phase ratio (O:A), sodium sulfate concentration ([Na(2)SO(4)]), equilibrium pH (pH(eq)) and tributylphosphate concentration ([TBP])) affecting the percentage extraction (%E) of Cu(II) were investigated. A 2(6-1) fractional factorial design was applied and the results were analyzed statistically. The results show that only [D2EHPA], pH(eq) and their second-order interaction ([D2EHPA] × pH(eq)) influenced the %E significantly. Regression models for %E were developed and the adequacy of the reduced model was examined. The results of this study indicate that fractional factorial design is a useful tool for screening a large number of variables and reducing the number of experiments.

    BACKGROUND: Iron deficiency and its consequent anemia constitute the commonest micronutrient deficiency in the world.
    OBJECTIVE: We investigated whether long-term, weekly iron-folate supplements administered at school would improve hemoglobin and ferritin concentrations in adolescent girls, including those with mild-to-moderate anemia and hemoglobin concentrations indicating borderline anemia.
    DESIGN: Subjects were 266 girls with hemoglobin concentrations of 80-119.9 g/L (group A) and 358 girls with hemoglobin concentrations of 120-130 g/L (group B) who were otherwise healthy. Two hundred sixty-six girls in group A and 268 girls in group B were randomly assigned to receive either 60 or 120 mg Fe plus 3.5 mg folic acid weekly for 22 wk. Ninety of the girls in group B were randomly assigned to receive only 5 mg folic acid weekly. Capillary hemoglobin and plasma ferritin were measured at baseline and after 12 and 22 wk of supplementation.
    RESULTS: By the end of the study, 2% of the girls had dropped out and > 96% had taken > or = 20 of the 22 tablets; side effects were minimal. Mean plasma ferritin increased significantly in all iron-supplemented groups, independently of initial hemoglobin values and iron doses. Ferritin concentrations decreased in the girls supplemented with folic acid only. As expected, hemoglobin responses to iron were higher in group A than in group B and increases were positively correlated with initial plasma ferritin. Hemoglobin failed to respond to folate supplementation if initial plasma ferritin concentrations were low. Mean hemoglobin increased significantly and consistently in relation to the length of treatment.
    CONCLUSION: Long-term, weekly iron-folate supplementation was found to be a practical, safe, effective, and inexpensive method for improving iron nutrition in adolescent schoolgirls.

    The optimization data presented here are part of the study planned to remove the caffeine from aqueous solution through the large surface area optimized H3PO4-activated Acacia mangium wood activated carbon (OAMW-AC). The maximum adsorption capacity of the OAMW-AC for caffeine adsorption was achieved (30.3 mg/g) through optimized independent variables such as, OAMW-AC dosage (3.0 g/L), initial caffeine concentration (100 mg/L), contact time (60 min), and solution pH (7.7). The adsorption capacity of OAMW-AC was optimized with the help of rotatable central composite design of response surface methodology. Under the stated optimized conditions for maximum adsorption capacity, the removal efficiency was calculated to be 93%. The statistical significance of the data set was tested through the analysis of variance (ANOVA) study. Data confirmed the statistical model for caffeine adsorption was significant. The regression coefficient (R2) of curve fitting through the quadratic model was found to be 0.9832, and the adjusted regression coefficient was observed to be 0.9675.

    INTRODUCTION: Previous studies have shown that Piper betle L. leaves extract inhibits the adherence of Streptococcus mutans to glass surface, suggesting its potential role in controlling dental plaque development.

    OBJECTIVES: In this study, the effect of the Piper betle L. extract towards S. mutans (with/without sucrose) using scanning electron microscopy (SEM) and on partially purified cell-associated glucosyltransferase activity were determined.

    MATERIAL AND METHODS: S. mutans were allowed to adhere to glass beads suspended in 6 different Brain Heart Infusion broths [without sucrose; with sucrose; without sucrose containing the extract (2 mg mL(-1) and 4 mg mL(-1)); with sucrose containing the extract (2 mg mL(-1) and 4 mg mL(-1))]. Positive control was 0.12% chlorhexidine. The glass beads were later processed for SEM viewing. Cell surface area and appearance and, cell population of S. mutans adhering to the glass beads were determined upon viewing using the SEM. The glucosyltransferase activity (with/without extract) was also determined. One- and two-way ANOVA were used accordingly.

    RESULTS: It was found that sucrose increased adherence and cell surface area of S. mutans (p<0.001). S. mutans adhering to 100 µm² glass surfaces (with/without sucrose) exhibited reduced cell surface area, fluffy extracellular appearance and cell population in the presence of the Piper betle L. leaves extract. It was also found that the extract inhibited glucosyltransferase activity and its inhibition at 2.5 mg mL(-1) corresponded to that of 0.12% chlorhexidine. At 4 mg mL(-1) of the extract, the glucosyltransferase activity was undetectable and despite that, bacterial cells still demonstrated adherence capacity.

    CONCLUSION: The SEM analysis confirmed the inhibitory effects of the Piper betle L. leaves extract towards cell adherence, cell growth and extracellular polysaccharide formation of S. mutans visually. In bacterial cell adherence, other factors besides glucosyltransferase are involved.

    Scales are calcium carbonate and collagen-contained structures embedded within the fish epidermis and useful for
    species identification. This study aimed to describe morphological characteristics of scales and use the differences to
    prepare keys to species. Fishes were sampled from selected rivers of Tembat Forest Reserve, Hulu Terengganu. Specimens
    caught were from 3 families (Cyprinidae, Channidae, Nandidae) and 17 species. Each species was represented by ten
    individuals (size ranges 2.5 - 50 cm TL). The scales were removed, soaked in H2
    O2
    (0.5%), NH3 (0.3%), DH2
    O and
    mounted between a pair of glass slides for digital photographing. The morphological descriptions were based on types
    of scales, distinctiveness of radii arrangement at the anterior field, radii cover, radii distribution, overall shape, focus
    position and focus pattern. Keys to species were constructed based on these scale morphological characters described.
    Measurements of scale total length (L), total width (W), rostral field length (L1) and caudal field length (L2) of the scales
    were taken using Image J software. The inter-specific variation among scales was indicated by L1/L, L2/L, L1/L2 and
    W/L indices through multiple comparison tests (ANOVA). It was found that all 17 species showed significant differences
    with at least one other species in all four indices. Pristolepis grootii (Bleeker 1852) was the only one that significantly
    different (p<0.05) from other 16 species in the first three indices. Species that showed the least significant differences
    among species was Probarbus jullieni (Sauvage 1880). The differences among the scales were primarily due to the
    different types of scale that was either ctenoid or cycloid.

    Animal research plays an important role in the pre-clinical phase of clinical trials. In animal studies, the power analysis approach to sample size calculation is recommended. Whenever it is not possible to assume the standard deviation and the effect size, an alternative to the power analysis approach is the 'resource equation' approach, which sets the acceptable range of the error degrees of freedom (DF) in an analysis of variance (ANOVA). The aim of this article is to guide researchers in calculating the minimum and maximum numbers of animals required in animal research by reformulating the error DF formulas.

    Akumulasi logam berat dan bahan pencemar ke dalam ekosistem akuatik memberi impak negatif kepada alam sekitar dan organisma akuatik. Salvinia molesta dan Pistia stratiotes merupakan spesies yang mempunyai kadar pertumbuhan yang cepat dan berkemampuan dalam mengakumulasi logam berat dan menyerap nutrien menjadikan ia sesuai digunakan untuk merawat air sisa melalui kaedah bioteknologi iaitu fitoremediasi. Kajian ini dijalankan untuk menentukan kualiti air sisa kumbahan sebelum dan selepas rawatan menggunakan kaedah fitoremediasi. Selain itu, objektif kajian ini juga adalah untuk menilai keberkesanan Salvinia molesta dan Pistia stratiotes sebagai agen fitoremediasi bagi rawatan air sisa. Sampel tumbuhan akuatik berbeza berat iaitu 10, 20 dan 30 g diuji untuk rawatan tersebut. Ujian ANOVA satu hala menunjukkan perbezaan kadar pengurangan jumlah pepejal terampai dan ammoniakal nitrogen yang bererti (p<0.05) bagi 10, 20 dan 30 g Pistia stratiotes dan Salvinia molesta sepanjang kajian dijalankan iaitu daripada hari 0 sehingga hari ke-7. Keputusan kajian juga menunjukkan 30 dan 20 g Pistia stratiotes dan Salvinia molesta dapat menyingkirkan jumlah pepejal terampai dan ammoniakal nitrogen dengan lebih cepat berbanding berat tumbuhan 10 g. Ujian ANOVA satu hala juga tidak menunjukkan perbezaan yang bererti bagi kadar pengurangan jumlah pepejal terampai dan ammoniakal nitrogen antara Pistia stratiotes dan Salvinia molesta.

    Cell- based targeted delivery is recently gain attention as a promising platform for delivery of anticancer drug in selective and efficient manner. As a new biotechnology platform, bacterial ghosts (BGs) have novel biomedical application as targeted drug delivery system (TDDS). In the current work, Salmonellas' BGs was utilized for the first time as hepatocellular cancer (HCC) in-vitro targeted delivery system. Successful BGs loading and accurate analysis of doxorubicin (DOX) were necessary steps for testing the applicability of DOX loaded BGs in targeting the liver cancer cells. Loading capacity was maximized to reach 27.5 µg/mg (27.5% encapsulation efficiency), by incubation of 10 mg BGs with 1 mg DOX at pH 9 in constant temperature (25 °C) for 10 min. In-vitro release study of DOX loaded BGs showed a sustained release (182 h) obeying Higuchi sustained kinetic release model. The death rate (tested by MTT assay) of HepG2 reached to 64.5% by using of 4 μg/ml, while it was about 51% using the same concentration of the free DOX (P value

    To investigate whether the craniofacial sagittal jaw relationship in patients with non-syndromic cleft differed from non-cleft (NC) individuals by artificial intelligence (A.I.)-driven lateral cephalometric (Late. Ceph.) analysis. The study group comprised 123 subjects with different types of clefts including 29 = BCLP (bilateral cleft lip and palate), 41 = UCLP (unilateral cleft lip and palate), 9 = UCLA (unilateral cleft lip and alveolus), 13 = UCL (unilateral cleft lip) and NC = 31. The mean age was 14.77 years. SNA, SNB, ANB angle and Wits appraisal was measured in lateral cephalogram using a new innovative A.I driven Webceph software. Two-way ANOVA and multiple-comparison statistics tests were applied to see the differences between gender and among different types of clefts vs. NC individuals. A significant decrease (p < 0.005) in SNA, ANB, Wits appraisal was observed in different types of clefts vs. NC individuals. SNB (p > 0.005) showed insignificant variables in relation to type of clefts. No significant difference was also found in terms of gender in relation to any type of clefts and NC group. The present study advocates a decrease in sagittal development (SNA, ANB and Wits appraisal) in different types of cleft compared to NC individuals.

    The aim of the present study was to determine if paraventricular-spinal vasopressin neurones participate in the sympatho-inhibitory effects of systemically administered atrial natriuretic peptide (ANP) on renal sympathetic nerve activity (RSNA). Experiments were carried out on male Sprague-Dawley rats anesthetized with 1.3 g/kg urethane. Changes in mean arterial pressure (mm Hg), heart rate (beats per minute) and RSNA (%) were measured following intravenous bolus administration of ANP (250 ng, 500 ng and 5 microg). Intrathecal application of selective V 1a receptor antagonist was performed to test for the involvement of supraspinal vasopressin pathways in mediating the effect on sympathetic outflow evoked by intravenous ANP administration. The results obtained demonstrated that both low and high doses of ANP caused renal sympathoinhibition (250 ng; - 7.5 +/- 1%, 500 ng; - 14.2 +/- 1%, 5 microg; - 16.4 +/- 2%), concomitant with vasodilation and bradycardia. After spinal vasopressin receptor blockade, the inhibitory effects of ANP were prevented and there was a small renal sympatho-excitation (250 ng; + 1.7 +/- 0.2%, 500 ng; + 6.1 +/- 0.03%, 5 microg; + 8.0 +/- 0.03%, P < 0.05). Therefore, the renal sympathetic nerve inhibition elicited by circulating ANP is dependent on the efficacy of a well established supraspinal vasopressin pathway. Since supraspinal vasopressin neurones without exception excite renal sympathetic neurones, it is suggested that ANP elicits this effect by activating cardiac vagal afferents that inhibit the spinally projecting vasopressin neurones at their origin in the paraventricular nucleus of the hypothalamus.
