    The aim of this study is to identify the optimum thermal conversion of Chlorella vulgaris with neuro-evolutionary approach. A Progressive Depth Swarm-Evolution (PDSE) neuro-evolutionary approach is proposed to model the Thermogravimetric analysis (TGA) data of catalytic thermal degradation of Chlorella vulgaris. Results showed that the proposed method can generate predictions which are more accurate compared to other conventional approaches (>90% lower in Root Mean Square Error (RMSE) and Mean Bias Error (MBE)). In addition, Simulated Annealing is proposed to determine the optimal operating conditions for microalgae conversion from multiple trained ANN. The predicted optimum conditions were reaction temperature of 900.0 °C, heating rate of 5.0 °C/min with the presence of HZSM-5 zeolite catalyst to obtain 88.3% of Chlorella vulgaris conversion.

    This paper elucidates the capability of isolated indigenous bacteria to remove aluminium from wastewater and soil. Two indigenous species of Brochothrix thermosphacta and Vibrio alginolyticus were isolated from an aluminium-contaminated site. These two species were used to treat aluminium-containing wastewater and contaminated soil using the bioaugmentation method. B. thermosphacta showed the highest aluminium removal of 57.87 ± 0.45% while V. alginolyticus can remove aluminium up to 59.72 ± 0.33% from wastewater. For aluminium-contaminated soil, B. thermosphacta and V. alginolyticus, showed a highest removal of only 4.58 ± 0.44% and 5.48 ± 0.58%, respectively. The bioaugmentation method is more suitable to be used to treat aluminium in wastewater compared to contaminated soil. The produced biomass separation after wastewater treatment was so much easier and applicable, compared to the produced biomass handling from contaminated soil treatment. A 48.55 ± 2.45% and 40.12 ± 4.55% of aluminium can be recovered from B. thermosphacta and V. alginolyticus biomass, respectively, with 100 mg/L initial aluminium concentration in wastewater.

    The existing mold concept of fabricating magnetorheological elastomer (MRE) tends to encounter several flux issues due to magnetic flux losses inside the chamber. Therefore, this paper presents a new approach for enhancing particle alignment through MRE fabrication as a means to provide better rheological properties. A closed-loop mold, which is essentially a fully guided magnetic field inside the chamber, was designed in order to strengthen the magnetic flux during the curing process with the help of silicone oil (SO) plasticizers. The oil serves the purpose of softening the matrix. Scanning electron microscopy (SEM) was used to observe the surface morphology of the fabricated MRE samples. The field-dependent dynamic properties of the MREs were measured several ways using a rheometer, namely, strain sweep, frequency sweep, and magnetic field sweep. The analysis implied that the effectiveness of the MRE was associated with the use of the SO, and the closed-loop mold helped enhance the absolute modulus up to 0.8 MPa. The relative magnetorheological (MR) effects exhibited high values up to 646%. The high modulus properties offered by the MRE with SO are believed to be potentially useful in industry applications, particularly as vibration absorbers, which require a high range of stiffness.

    Background: Hand infection in diabetics is an often ignored but challenging condition. If not addressed effectively, it may result in long term disability, contracture, amputation and even death. Methods: From August 2014 to December 2015, a study was done in our centre, where 49 diabetic hand infection cases were analyzed in two groups, superficial and deep hand infection. Results: Mean age of the patients was 51.63 years. There were 21 superficial infections and 28 deep infections. Cause of infection was unknown or spontaneous in 16 cases, traumatic laceration or crush in 14 patients, following minor prick in 10 cases. Most of the cases were the results of neglected minor wound. Forty-one patients were insulin dependent. Five cases were diagnosed as diabetic at the time of treatment. Four patients were treated conservatively and 45 (92%) cases required operation in the form of incision, drainage and debridement. In 16 (35%) cases, wound was left open and was healed by secondary intention following regular dressing. In five patients, wound was closed secondarily. Partial thickness skin graft was applied in 15 cases. Seven patients were treated with flap coverage. Partial digital or ray amputation were done in 16 cases. All fingers except thumb were amputated in one case and amputation from wrist was done in another patient. Wound swabs were taken, and antibiotics were changed or continued accordingly. But reports of 26 cases were available. No growth was found in four cases, monomicrobial infection was found in 15 patients and polymicrobial in seven cases. Infection resolved with healing in 47 cases. Two patients died during treatment from sepsis, both were insulin dependent, had associated renal failure and from deep infection group. One patient developed severe mental disorder. Conclusions: For diabetic hand infection, early diagnosis and prompt treatment with appropriate antibiotics and emergency surgery with extensile incision is crucial. Primary amputation of the part could be life and limb saving.

    BACKGROUND: Limited information is available from longitudinal studies regarding the predictors and incidence of MCI in older Asian adults. Thus, a community-based longitudinal study was conducted to determine the incidence of MCI among multi-ethnic older adults in Malaysia. The role of health and lifestyle as predictors of MCI was also examined.

    METHODS: Analysis of data obtained from the Towards Useful Aging (TUA) study (2014-2016), wave 1 (baseline) and wave 2 (1½ years of follow-up) was conducted. For the baseline, comprehensive, interview-based questionnaires were administered to 1227 subjects who were 60 years old and above. MCI is a unique transitional state between normal ageing and dementia. MCI characteristics include a decline and disturbance of cognition, minimal impairment of complex activities, ability to perform regular daily functions, and absence of dementia. The incidence of MCI was assessed using comprehensive neuropsychological batteries. The study then performed a logistic regression analysis to examine the effect of each possible predictor of MCI. This analysis began with univariate analyses and a separate review of the effect of every variable. Binary logistic analyses followed hereafter.

    RESULTS: During the follow-up after 1½ years, 179 (14.6%) of the participants who did not exhibit MCI at baseline were observed to have developed MCI. Among the participants who did not exhibit MCI at baseline, the incidence rate was 10.5 per 100 person-years. Male sex and lack of engagement in mental activities were predictors of MCI among participants without MCI at baseline.

    CONCLUSION: After the 1½-year follow-up, the incidence rate for MCI was considerably high among the respondents. Being male and being less engaged in mental activities were predictors of the occurrence of MCI. Mental activities need to be promoted for the prevention of MCI incidence among older adults.

    PURPOSE: To assess the utility of the cobas EGFR Mutation Test, with tissue and plasma, for first-line osimertinib therapy for patients with EGFR-mutated (EGFRm; Ex19del and/or L858R) advanced or metastatic non-small cell lung cancer (NSCLC) from the FLAURA study (NCT02296125).

    EXPERIMENTAL DESIGN: Tumor tissue EGFRm status was determined at screening using the central cobas tissue test or a local tissue test. Baseline circulating tumor (ct)DNA EGFRm status was retrospectively determined with the central cobas plasma test.

    RESULTS: Of 994 patients screened, 556 were randomized (289 and 267 with central and local EGFR test results, respectively) and 438 failed screening. Of those randomized from local EGFR test results, 217 patients had available central test results; 211/217 (97%) were retrospectively confirmed EGFRm positive by central cobas tissue test. Using reference central cobas tissue test results, positive percent agreements with cobas plasma test results for Ex19del and L858R detection were 79% [95% confidence interval (CI), 74-84] and 68% (95% CI, 61-75), respectively. Progression-free survival (PFS) superiority with osimertinib over comparator EGFR-TKI remained consistent irrespective of randomization route (central/local EGFRm-positive tissue test). In both treatment arms, PFS was prolonged in plasma ctDNA EGFRm-negative (23.5 and 15.0 months) versus -positive patients (15.2 and 9.7 months).

    CONCLUSIONS: Our results support utility of cobas tissue and plasma testing to aid selection of patients with EGFRm advanced NSCLC for first-line osimertinib treatment. Lack of EGFRm detection in plasma was associated with prolonged PFS versus patients plasma EGFRm positive, potentially due to patients having lower tumor burden.

    Blomia tropicalis has been recognized as a cause of allergic diseases in the tropical and subtropical regions. Here we report the immuno-characterization of its group 2 allergen, Blo t 2. Allergen Blo t 2 was amplified from the cDNA of B. tropicalis using degenerate primers, expressed in Escherichia coli as a recombinant protein and purified to homogeneity. The mature protein of Blo t 2 was 126 amino acids long with 52% sequence identity to Der p 2 and apparent molecular mass of 15 kDa. Circular dichroism spectroscopy showed that Blo t 2 is mainly a beta-sheeted protein. We confirmed the presence of three disulfide bonds in recombinant (r) Blo t 2 protein using electrospray mass spectrometry. Thirty-four percent of dust-mite allergic individuals from the Singapore showed specific IgE binding to rBlo t 2 as tested using immuno dot-blots. IgE-cross reactivity assays showed that Blo t 2 had between 20-50% of unique IgE-epitopes compared to Der p 2.     Phyllanthus species (family; Euphorbiaceae) have been intensively studied for their immunomodulating effects due to their wide-ranging uses to treat immune-related diseases in indigenous medicine, which are primarily lack of scientific basis. The focuses of this review are on the significance of Phyllanthus species and their bioactive metabolites particularly corilagin (1), geraniin (2), gallic acid (3), phyllanthin (4), hypophyllanthin (5), ellagic acid (6), phyltetralin (7), niranthin (8), catechin (9), quercetin (10), astragalin (11), and chebulagic acid (12) in the modulation of both innate and adaptive immune systems through various mechanisms and their possible therapeutic benefits for treatment of immune-related diseases. We have compiled all significant findings published in the literature, and the data were analyzed critically to provide perspectives and directions for future research for the plants as a prospective source of novel immunomodulating agents. Various Phyllanthus species particularly Phyllanthus amarus, Phyllanthus emblica, Phyllanthus niruri, and Phyllanthus urinaria have been documented to possess significant immunomodulatory effects. However, the possible challenges encountered by the application of extracts of various Phyllanthus species and their bioactive constituents as immunomodulators need to be addressed. Most reports on the biological and pharmacological studies of the plants were based on crude extracts. The extracts were not chemically characterized, and the contributions of their chemical constituents to the bioactivities were not identified. The underlying mechanisms involved in the immunomodulatory effects of the Phyllanthus species were not indepthly studied due to limitations in terms of design, conduct, and interpretation. Extensive experimental and preclinical studies on the immunomodulating potential of Phyllanthus species should be carried out to provide sufficient data to prove that their traditional uses are inherently effective and safe and will allow clinical trials to be pursued for their further development as therapeutic agents to treat immune-related disorders.

    This study intended to validate customer inspiration (CI)in Malaysian/developing country context. Data were collected from two different respondents for two studies - from Millennial customers of the auto industry and Generation Z customers of the smartphone industry. The survey conducted through a standardized and structured questionnaire. The variables of the both studies were customer-defined market orientation (MO) (customer orientation, competitor orientation, and interfunctional coordination), CI (inspired-by and inspired-to), and customer loyalty (CL). This research strategy, in terms of quantity, is descriptive and correlational. Statistical analysis of the data was carried out, using ADANCO 2.0. The finding of the study suggests all results of data 1 and data 2 were significant, and CI mediates the sub-constructs of MO with CL.

    Collisions arising from lane departures have contributed to traffic accidents causing millions of injuries and tens of thousands of casualties per year worldwide. Many related studies had shown that single vehicle lane departure crashes accounted largely in road traffic deaths that results from drifting out of the roadway. Hence, automotive safety has becoming a concern for the road users as most of the road casualties occurred due to driver's fallacious judgement of vehicle path. This paper proposes a vision-based lane departure warning framework for lane departure detection under daytime and night-time driving environments. The traffic flow and conditions of the road surface for both urban roads and highways in the city of Malacca are analysed in terms of lane detection rate and false positive rate. The proposed vision-based lane departure warning framework includes lane detection followed by a computation of a lateral offset ratio. The lane detection is composed of two stages: pre-processing and detection. In the pre-processing, a colour space conversion, region of interest extraction, and lane marking segmentation are carried out. In the subsequent detection stage, Hough transform is used to detect lanes. Lastly, the lateral offset ratio is computed to yield a lane departure warning based on the detected X-coordinates of the bottom end-points of each lane boundary in the image plane. For lane detection and lane departure detection performance evaluation, real-life datasets for both urban roads and highways in daytime and night-time driving environments, traffic flows, and road surface conditions are considered. The experimental results show that the proposed framework yields satisfactory results. On average, detection rates of 94.71% for lane detection rate and 81.18% for lane departure detection rate were achieved using the proposed frameworks. In addition, benchmark lane marking segmentation methods and Caltech lanes dataset were also considered for comparison evaluation in lane detection. Challenges to lane detection and lane departure detection such as worn lane markings, low illumination, arrow signs, and occluded lane markings are highlighted as the contributors to the false positive rates.

    This study analyzes the association between trait driving anger and driving styles in a sample of Colombian professional drivers. Additionally, the internal and external validity of the Deffenbacher's Driving Anger Scale (DAS-14) was examined in the study population. The DAS-14 and the Spanish Version of the Multidimensional Driving Style Inventory (S-MDSI) were administered to 492 urban bus and taxi operators. Average trait driving anger scores in the study population were similar to those reported in previous validation studies from Spain, Argentina, China, and Malaysia. After deleting three cross-loaded items, confirmatory factor analyses revealed a three-dimensional latent structure for the DAS-14, similar but not equal to the previous Spanish speaking validations. This factorial structure fits the data reasonably well. Finally, linear regression analyses revealed that the three factors of the DAS-14 (impeded progress by others, illegal driving, and direct hostility) significantly predict adaptive and maladaptive driving styles. Overall, the results of this study suggest that the DAS-14 is a reasonably reliable measure of driving anger traits among professional drivers, and it also provides relevant insights for the prevention of risky driving styles in this occupational group.

    Southeast Asia has undergone rapid developments in terms of urbanization, economic and population growth. The progress in sewerage treatment infrastructure has not kept pace with such developments. The inadequacy and inefficiency of sewerage systems has prompted the release of untreated sewage into the aquatic environment of Southeast Asia causing many waterborne illnesses since surface water is utilized for recreational, agricultural and aquaculture purposes and, above all, as a source of water intake in Southeast Asia. This paper will review the current data on molecular markers of sewage pollution including sterols and linear alkylbenzenes (LABs) in Southeast Asian aquatic environment to clarify the state of sewage pollution and the competence of sewage treatment plants (STPs) in this area. Despite the importance of sewage pollution research in the region, the number of studies using molecular markers to trace the sources of sewage pollution is limited. So far, indicators of sewage pollution have been investigated in aquatic environments of Indonesia, Vietnam, Malaysia, the Philippines, Thailand, Cambodia and Brunei among Southeast Asian countries. The concentrations and diagnostic ratios of faecal sterols and LABs show the release of untreated and primary treated urban waste into water bodies of these countries. Further studies are required to fill the data gaps in Southeast Asia and come to a better understanding of the trends of sewage pollution in this part of the world. Graphical abstract.

    PURPOSE: The internal drainage provided by a ureteral stent helps with the relief and prevention of ureteral obstruction. By definition, correct stent placement is one with a complete loop in both the renal pelvis and bladder. This prevents stent migration proximally or distally despite urinary flow, patient movement, and ureteral peristalsis.

    METHODS: We performed a comparative prospective cross-sectional study assessing the impact of intravesical stent position on the quality of life in 46 patients with a ureteral stent. This is done using the Ureteral Stent Symptom Questionnaire (USSQ).

    RESULTS: 52.5% of patients had an ipsilateral positioned intravesical stent, while the remaining had their stent positioned contralaterally. Intravesical stent position significantly influenced the quality of life. The USSQ score was worse for the contralateral group. Subscore analysis found that urinary symptoms and body pain index contribute significantly to the morbidity. Majority of patients in the ipsilateral group reported no discomfort as compared to the contralateral group.

    CONCLUSIONS: To the best of our knowledge, this is the first study assessing the impact of intravesical stent position on the quality of life in the Asian population. Intravesical stent position has a significant influence on patient's morbidity and quality of life in particular towards their urinary irritative symptoms and body pain. It is imperative to ensure correct distal placement of ureteric stent that does not cross the midline to the contralateral site. We believe that the USSQ should be used in daily clinical practice in assessing the symptoms related to indwelling ureteric stents.

    There have been an increasing number of studies conducted on community preparedness, particularly on changing individual health behaviors in ways that minimizes individual risk to cope with the stress of a natural disaster. A variety of behavioral change theories and models used by disaster academics scrutinize the manner in which individual behavior is sought and transformed into disaster preparedness. This reflects the lack of knowledge about how these models identify certain behaviors regarding natural disaster preparation. This article seeks to address this lack of knowledge. It presents a set of health behavioral change models that can be used by scholars to comprehend variation in the nature and extent of individual disaster preparedness. The purpose of this study is to provide a review of the existing models on the subject, and also to present a comparative analysis of the models that may contribute to ways of understanding the investigation on natural disaster preparedness behaviors.

    CHRM3 codes for the M3 muscarinic acetylcholine receptor that is located on the surface of smooth muscle cells of the detrusor, the muscle that effects urinary voiding. Previously, we reported brothers in a family affected by a congenital prune belly-like syndrome with mydriasis due to homozygous CHRM3 frameshift variants. In this study, we describe two sisters with bladders that failed to empty completely and pupils that failed to constrict fully in response to light, who are homozygous for the missense CHRM3 variant c.352G > A; p.(Gly118Arg). Samples were not available for genotyping from their brother, who had a history of multiple urinary tract infections and underwent surgical bladder draining in the first year of life. He died at the age of 6 years. This is the first independent report of biallelic variants in CHRM3 in a family with a rare serious bladder disorder associated with mydriasis and provides important evidence of this association.

    BACKGROUND: There is limited evidence detailing the cost-effectiveness of psoriasis treatments in the Asian region. Therefore, this study is aimed to evaluate the cost-effectiveness of 3 psoriasis treatments tailored for moderate to severe psoriasis, namely topical and phototherapy (TP), topical and systemic (TS), and topical and biologic (TB) regimens, respectively.

    METHODS: This has been achieved by the participation of a prospective cohort involving a total of 90 moderate to severe psoriasis patients, which has been conducted at 5 public hospitals in Malaysia. The main outcome measures have been evaluated via cost and effectiveness psoriasis area severity index (PASI)-75 and/or body surface area (BSA) <5 and/or dermatology life quality index (DLQI) ≤5), estimated from the societal perspective over a 6-months duration. All costs are based on 2015's recorded Malaysian Ringgit (RM) currency.

    RESULTS: Consequently, TS has been found to be the most cost-effective treatment with the lowest cost/PASI-75/and/or BSA <5 and/or DLQI ≤5, valued at RM9034.56 (US$2582.55). This is followed by TP, which is valued at RM28 080.71 (US$8026.93) and TB, valued at RM54 287.02 (US$15 518.06). Furthermore, one-way sensitivity analysis has highlighted the cost of medication as the most sensitive parameter.

    CONCLUSION: Thus, the input from this study is helpful for policy-makers in determining the first line treatment for moderate to severe psoriasis with consideration of the costs and its effectiveness in Malaysia. This will consequently allow hospitals to justify and provide the essential resources for further research and development, as well as the adoption of better treatment options.

    AIMS: Refugees are confronted with the task of adapting to the long-term erosion of psychosocial systems and institutions that in stable societies support psychological well-being and mental health. We provide an overview of the theoretical principles and practical steps taken to develop a novel psychotherapeutic approach, Integrative Adapt Therapy (IAT), which aims to assist refugees to adapt to these changes. This paper offers the background informing ongoing trials of IAT amongst refugees from Myanmar.

    METHODS: A systematic process was followed in formulating the therapy and devising a treatment manual consistent with the principles of the Adaptation and Development After Persecution and Trauma (ADAPT) model. The process of development and refinement was based on qualitative research amongst 70 refugees (ten from West Papua and 60 Rohingya from Myanmar). The therapeutic process was then piloted by trained interventionists amongst a purposively selected sample of 20 Rohingya refugees in Malaysia.

    RESULTS: The final formulation of IAT represented an integration of the principles of the ADAPT model and evidence-based techniques of modern therapies in the field, including a transdiagnostic approach and the selective use of cognitive behavioural treatment elements such as problem-solving and emotional regulation techniques. The steps outlined in refining the manual are outlined in relation to work amongst West Papuan refugees, and the process of cultural and contextual modifications described during early piloting with Rohingya refugees in Malaysia.

    CONCLUSIONS: IAT integrates universal principles of the ADAPT model with the particularities of the culture, history of conflict and living context of each refugee community; this synthesis of knowledge forms the basis for participants gaining insights into their personal patterns of psychosocial adaptation to the refugee experience. Participants then apply evidence-based techniques to improve their capacity to adapt to the serial psychosocial changes they have encountered in their lives as refugees. The overarching goal of IAT is to provide refugees with a coherent framework that assists in making sense of their experiences and their emotional and interpersonal reactions to the challenges they confront within the family and community context. As such, the principles of a general model (ADAPT) are used as a springboard for making concrete, manageable and meaningful life changes at the individual level, a potentially novel approach for psychosocial interventions in the field.

    Stretchable self-healing urethane-based biomaterials have always been crucial for biomedical applications; however, the strength is the main constraint of utilization of these healable materials. Here, a series of novel, healable, elastomeric, supramolecular polyester urethane nanocomposites of poly(1,8-octanediol citrate) and hexamethylene diisocyanate reinforced with cellulose nanocrystals (CNCs) are introduced. Nanocomposites with various amounts of CNCs from 10 to 50 wt% are prepared using solvent casting technique followed by the evaluation of their microstructural features, mechanical properties, healability, and biocompatibility. The synthesized nanocomposites indicate significantly higher tensile modulus (approximately 36-500-fold) in comparison to the supramolecular polymer alone. Upon exposure to heat, the materials can reheal, but nevertheless when the amount of CNC is greater than 10 wt%, the self-healing ability of nanocomposites is deteriorated. These materials are capable of rebonding ruptured parts and fully restoring their mechanical properties. In vitro cytotoxicity test of the nanocomposites using human dermal fibroblasts confirms their good cytocompatibility. The optimized structure, self-healing attributes, and noncytotoxicity make these nanocomposites highly promising for tissue engineering and other biomedical applications.
