    We have previously isolated a novel avian Orthobunyavirus, Kedah Fatal Kidney Syndrome (KFKS) virus from a broiler farm in Kedah, Malaysia in 2020 with a severe kidney lesion in chickens. The virus was designated as KFKS2_CS virus. Sequence analysis of partial nucleocapsid (N) and nonstructural (NSs) sequence of this virus showed the highest sequence identity with previous KFKS1 from Malaysia (100%) and 97% with a zoonotic Umbre (UMB) virus, which was reported to cause encephalitis in immunocompromised humans in India. Phylogenetic analysis revealed that this virus was clustered together with previous KFKS1 virus from Malaysia, UMB and Cristoli viruses. This study aimed to assess the zoonotic potential of this KFKS2_CS virus in vitro by determining its ability to inhibit the production of interferon (IFN) in human glioblastoma multiforme (GBM) brain cells using reverse-transcriptase polymerase reaction (RT-PCR). This virus blocked the production of interferon-a in this human brain cells. In conclusion, this KFKS2_CS virus may have a zoonotic potential and become a public health concern in the future.

    PURPOSE: Guillain-Barré Syndrome (GBS) associated with SARS-CoV-2 vaccine administration is very rare. Early recognition of GBS at early stage could prevent extensive nerve damage with potential respiratory and autonomic failure.

    CASE REPORT: We report a case of paraparetic spectrum of GBS in a 53-year-old lady who presented with rapidly progressive acute flaccid paralysis involving both lower extremities with areflexia eight days after the first dose of Sinovac vaccine for SARS-CoV-2 in Malaysia. Cerebrospinal fluid (CSF) albuminocytological dissociation was seen and nerve conduction study (NCS) revealed sensory neuropathy. The diagnosis of GBS was made based on the Brighton criteria. Patient responded well to intravenous immunoglobulin (IVIG).

    CONCLUSION: Though there is currently no convincing evidence of any causation between GBS and SARS- CoV-2 vaccination, clinicians should remain vigilant and consider GBS in the differential diagnosis for patient who presents with weakness with reduced or absent deep tendon reflex after vaccination against SARS-CoV-2.

    Contract appointment policy for newly graduated medical officers was implemented by the Ministry of Health Malaysia in 2016 to overcome the lack of permanent posts. Contract officers faced disadvantages in terms of salary, leave provision, and career prospects. A nationwide strike, Hartal Doktor Kontrak (HDK) was organised on 26th July 2021. Besides generating widespread public attention, HDK was also closely scrutinised by the medical fraternity and stakeholders. This content analysis aimed to explore how the medical fraternity and stakeholders viewed the strike as their perception would offer vital insights into the fundamental causes and viable solutions to the contract appointment policy. A qualitative content analysis of Facebook (FB) posts on the HDK strike was conducted from 1st June 2021 until 28th February 2022. A total of 182 FB posts were retrieved from stakeholders, medical fraternity groups, and medical key opinion personnel. Inductive coding was used in the thematic analysis to identify pertinent themes. Three main themes emerged: triggering factors, reactions to the strike, and outcomes of the strike. Factors that led to the strike included unequal treatment faced by contract officers, frustration with the government's lack of long-term solutions, and aggravation by the COVID-19 pandemic. In terms of reactions, there was a mixture of supportive and opposing voices. No substantial negative impact on the healthcare service resulted from the strike. Instead, it generated widespread attention that propelled the government into implementing solutions to prevent adverse short and long-term consequences. Various suggestions were proposed, including the reform of human resource planning and undergraduate medical education. The results highlight the importance of proactive systemic measures by the government to prevent further strikes that may jeopardise healthcare provision. In summary, social media was found to influence the progress and outcome of HDK, thus demonstrating the impact of media influence on similar issues.

    The present study aims to identify the Acanthamoeba genotypes and their pathogenic potential in three recreational lakes in Malaysia. Thirty water samples were collected by purposive sampling between June and July 2022. Physical parameters of water quality were measured in situ while chemical and microbiological analyses were performed in the laboratory. The samples were vacuum filtered through nitrate filter, cultured onto non-nutrient agar and observed microscopically for amoebic growth. DNAs from positive samples were extracted and made to react with polymerase chain reaction using specific primers. Physiological tolerance tests were performed for all Acanthamoeba-positive samples. The presence of Acanthamoeba was found in 26 of 30 water samples by PCR. The highest rate in lake waters contaminated with amoeba was in Biru Lake (100%), followed by Titiwangsa Lake (80%) and Shah Alam Lake (80%). ORP, water temperature, pH and DO were found to be significantly correlated with the presence of Acanthamoeba. The most common genotype was T4. Temperature- and osmo-tolerance tests showed that 8 (30.8%) of the genotypes T4, T9 and T11 were highly pathogenic. The presence of genotype T4 in habitats related to human activities supports the relevance of this amoeba as a potential public health concern.

    The food choices in childhood have high a probability of being carried through into their adulthood life, which then contributes to the risk of many non-communicable diseases. Therefore, there is a need to gather some information about children's views on foods which may influence their food choices for planning a related dietary intervention or programme. This paper aimed to explore the views of children on foods and the types of foods which are usually consumed by children under four food groups (snacks, fast foods, cereals and cereal products; and milk and dairy products) by using focus group discussions. A total of 33 school children aged 7-9 years old from Selangor and Kuala Lumpur participated in the focus groups. Focus groups were audio-taped, transcribed and analyzed according to the listed themes. The outcomes show that the children usually consumed snacks such as white bread with spread or as a sandwich, local cakes, fruits such as papaya, mango and watermelon, biscuits or cookies, tea, chocolate drink and instant noodles. Their choices of fast foods included pizza, burgers, French fries and fried chicken. For cereal products, they usually consumed rice, bread and ready-to-eat cereals. Finally, their choices of dairy products included milk, cheese and yogurt. The reasons for the food liking were taste, nutritional value and the characteristics of food. The outcome of this study may provide additional information on the food choices among Malaysian children, especially in urban areas with regard to the food groups which have shown to have a relationship with the risk of childhood obesity.

    Implementing asthma guideline recommendations is challenging in low- and middle-income countries. We aimed to explore healthcare provider (HCP) perspectives on the provision of recommended care. Twenty-six HCPs from six public primary care clinics in a semi-urban district of Malaysia were purposively sampled based on roles and experience. Focus group discussions were guided by a semi-structured interview guide and analysed thematically. HCPs had access to guidelines and training but highlighted multiple infrastructure-related challenges to implementing recommended care. Diagnosis and review of asthma control were hampered by limited access to spirometry and limited asthma control test (ACT) use, respectively. Treatment decisions were limited by poor availability of inhaled combination therapy (ICS/LABA) and free spacer devices. Imposed Ministry of Health programmes involving other non-communicable diseases were prioritised over asthma. Ministerial policies need practical resources and organisational support if quality improvement programmes are to facilitate better management of asthma in public primary care clinics.

    Translation equivalents are widely used in bilingual research concerning word processing (e.g., Eddington & Tokowicz, 2013; Jouravlev & Jared, 2020) and second-language vocabulary learning (e.g., Bracken et al., 2017; Degani et al., 2014). Although translation norms exist in several languages, to date there are no Malay-English translation norms. This study presents the first Malay-English translation norms collected with highly proficient Malay-English bilinguals. Furthermore, the study investigates the impact of lexical characteristics on translation ambiguity. The forward translation (FT) task (N = 30) collected English translations for 1004 Malay words selected from the Malay Lexicon Project (Yap et al., 2010), and subsequently the backward translation (BT) task (N = 30) gathered Malay translations for 845 English words obtained from the FT phase. The data revealed a high prevalence of translation ambiguity in both translation directions. Specifically, verbs, adjectives, and class-ambiguous words were more translation-ambiguous than nouns. Furthermore, within-language semantic variability and word length were positively correlated with translation ambiguity, whereas word frequency only correlated with translation ambiguity in FT. Word length and word frequency of the source words and their translations were positively correlated. Intriguingly, only in FT were bilinguals with higher Malay proficiency more likely to provide accurate and dominant translations for the Malay words. These findings contrast with those reported in translation norming studies involving other language pairs. The translation norms provide a useful resource for bilingual language studies involving Malay-English bilinguals.

    OBJECTIVE: To examine the relationship between affiliate stigma, stress, and perceived quality of life among parents of children with cerebral palsy (CP).

    METHOD: Surveys (Study 1) and semi-structured interviews (Study 2) were used to collect data. Thirty-eight respondents completed the Affiliate Stigma Scale (ASS), Caregiver Burden Inventory (CBI), and Care-related Quality of Life (CarerQol) scales (GMFCS). Eleven respondents took part in semi-structured interviews, which were then thematically evaluated.

    RESULTS: Parents did not feel stigmatized because they had a child with CP. However, parents needed some form of short-term relief from caregiving. After controlling for demographics, high stress respondents experienced high affiliate stigma whereas low stress respondents had better quality of life. Although the respondents with less stigma had a better quality of life, this effect was moderate. Three major themes emerged from the interviews.

    CONCLUSION: Counseling, support groups, and helper services should be offered to parents. Also, healthcare professionals participating in CP rehabilitation, community-based rehabilitation, and health institutions should be educated on how to better help parents of children with CP.

    BACKGROUND: The natural hydrodynamic process of Kuala Nerus, Terengganu, has changed since the extension of Sultan Mahmud Airport runway in 2008. Consequently, severe coastal erosion has occurred in the area, particularly during the northeast monsoon season (NEM). Numerous types of coastal defense structures (CDS) have been constructed to protect the coastline. Despite the loss of esthetic values, the effect of CDS construction on marine organisms in the area remains unknown. Hence, this study aims to assess the ecological aspects of macrobenthic compositions at the CDS area of Kuala Nerus, Terengganu, based on the differences between the southwest (SWM) and northeast (NEM) monsoon seasons.

    METHODS: Macrobenthos were collected from the sediment in July (SWM) and December 2021 (NEM) using the Ponar grab at 12 substations from five sampling stations.

    RESULTS: The density of macrobenthos was higher in SWM (48,190.82 ind./m2) than in NEM (24,504.83 ind./m2), with phylum Mollusca recording the highest species composition (60-99.3%). The macrobenthos species had a low to moderate level of diversity (H' = 1.4-3.1) with the species were almost evenly distributed (J' = 0.2-0.8). Windward substations exhibited coarser grain sizes (38.56%-86.84%), whereas landward substations exhibited very fine grain sizes (44.26%-86.70%). The SWM season recorded a higher organic matter content (1.6%-6.33%) than the NEM season (0.4%-3.1%). However, metal concentrations in the surface sediment were within the safe range and permissible limits for both seasons, inferring that the macrobenthos composition was unaffected.

    DISCUSSION: This study demonstrated that the CDS associated with the monsoon system has controlled the hydrodynamics and nearshore sedimentary processes in the Kuala Nerus coastal zone, thereby affecting the macrobenthos population, in terms of richness and density. The ecological and energetic effects of the coastal structures in different seasons have resulted in a more significant result, with the SWM exhibiting a higher macrobenthos composition than the NEM.

    The paediatric orthopaedic expert system analyses and predicts the healing time of limb fractures in children using machine learning. As far we know, no published research on the paediatric orthopaedic expert system that predicts paediatric fracture healing time using machine learning has been published. The University Malaya Medical Centre (UMMC) offers paediatric orthopaedic data, comprises children under the age of 12 radiographs limb fractures with ages recorded from the date and time of initial trauma. SVR algorithms are used to predict and discover variables associated with fracture healing time. This study developed an expert system capable of predicting healing time, which can assist general practitioners and healthcare practitioners during treatment and follow-up.     Derris trifoliata is mainly found in mangrove area in tropical regions and the plant extract is traditionally used for fishing by poisoning. This is the first case report of rotenone poisoning in a child from ingesting Derris trifoliata seed. The child developed altered consciousness, vomiting, hypotension, metabolic acidosis, and acute kidney injury. Species identification of this case requires the collaborative efforts of various agencies. She survived from the poisoning with no neurological sequelae.

    Edible bird's nest (EBN) is a high-value health food with various nutrients and bioactive components. With increasing demand for EBN, they are often adulterated with cheaper ingredients or falsely labeled by the origin information, thus harming consumer interests. In this study, high- and low-field nuclear magnetic resonance (HF/LF-NMR) technology combined with multivariate statistical analysis was used to identify the geographical marker of EBN from different origins and authenticate the adulterated EBN with various adulterants at different adulteration rates. Authentic EBN samples from Malaysia were used to simulate adulteration using gelatin (GL), agar (AG) and starch (ST) at 10 %, 20 %, 40 %, 60 %, 80 %, and 100 % w/w, respectively. The results showed significant differences in composition among EBN from different origins, with isocaproate and citric acid serving as geographical markers for Malaysia and Vietnam, respectively. Leucine, glutamic acid, and N-acetylglycoprotein serving as geographical markers for Indonesia. In addition, PLS model further verified the accuracy of origin identification of EBN. The LF-NMR results of adulteration EBN showed a linear correlation between the transverse relaxation (T2, S2) and the adulterated ratio. The OPLS-DA based on T2 spectra could accurately identify authentic EBN from adulterated with GL, AG and ST at 40 %, 20 %, and 20 %, respectively. Fisher discrimination model was able to differentiate at 20 %, 20 %, and 40 %, respectively. These results show that the 1H NMR combined with multivariate statistical analysis method could be a potential tool for the detection of origin and adulteration of EBN.

    Species composition of seaweed and distribution were investigated in the coastal waters of Bintulu, Sarawak. The seaweed samples were collected during low tide between May 2011 and May 2012 from the six different stations. In total 54 species of seaweeds were identified from study areas of Bintulu coastal waters. Among them, 23 species were from Rhodophyta with 11 families, 15 species were from Phaeophyta with 2 families and 16 species were from Chlorophyta with 10 families: Seventeen species of seaweeds were recorded from the Tanjung Batu, while 23 species from Pantai Telekom, 14 species from Golden Beach, 26 species from Kuala Similajau, 12 species from Kuala Nyalau and 21 species from Batu Mandi. Seaweeds abundance was high in rocky substrate and Rhodophyta (11 families and 23 species) was the common and highest group of seaweeds in this coastal areas. Present study recorded high diversified seaweed species at the rocky shore area compare to reef area.

    Microplastics (MPs) pose a threat to ecosystems due to their capacity to bind with toxic chemicals. While the occurrence of MPs in aquatic environmental matrices like water, sediments, and biota is well studied, their presence in the atmosphere remains less understood. This study aimed to determine the presence of airborne MPs and their characteristics through ground-based sampling in the coastal city of Kuala Nerus, Terengganu, Malaysia. Airborne MP samples were collected using passive sampling technique in December 2019. MPs were manually counted and identified using a stereomicroscope based on their colour and shape. The average deposition rate of airborne MPs during the sampling period was 5476 ± 3796 particles/m2/day, ranging from 576 to 15,562 particles/m2/day. Various colours such as transparent (38%), blue (25%), black (20%), red (13%), and others (4%) were observed. The predominant shape of airborne MPs was fibres (> 99%). The morphology structure of MPs observed using a scanning electron microscope (SEM) showed a cracked surface on MPs, suggesting weathering and irregular fragmentation. Further elemental analysis using energy dispersive X-ray spectroscopy (EDS) showed the presence of heavy metals such as aluminium (Al) and cadmium (Cd) on the surface of MPs, attributed to the adsorption capacities of MPs. Polymer types of airborne MPs were analysed using attenuated total reflectance-Fourier transform infrared spectroscopy (ATR-FTIR), which revealed particles composed of polyester (PES), polyethylene (PE), and polypropylene (PP). The preliminary findings could provide additional information for further investigations of MPs, especially in the atmosphere, to better understand their sources and potential human exposure.

    Presently, microplastic pollution has emerged as a growing environmental risk around the world. Nevertheless, knowledge of the occurrence and characteristics of microplastics in tropical agricultural soil is limited. This study investigated the pollution of surface soil microplastics in two agricultural farms located at Klang Valley, Malaysia. An extraction method based on density separation by using saturated extraction solution (sodium sulfate, ρ = 2 g cm-3 and sucrose, ρ = 1.59 g cm-3 with a ratio 1:1, v/v) was carried out. The study revealed the mean particle size of soil microplastics with 3260.76 ± 880.38 μm in farm A and 2822.31 ± 408.48 μm in farm B. The dominant types of soil microplastics were fragments and films with major colors of white (59%) and transparent (28%) in farm A, while black (52%) and white (37.6%) in farm B. Representatives of soil microplastics detected polymers of polyvinyl chloride (PVC), high density polyethylene (HDPE), polycarbonate (PC), and polystyrene (PS). The sources of plastic products were black and white plastic pipes, black plastic films for vegetation, fertilizer bottles, plastic water containers and polystyrene storage boxes, and the breakdown processes, contributed to the microplastic pollution in these farms. The outcomes of this study will establish a better understanding of microplastic pollution in tropical agricultural soil in the Southeast Asian region. The findings would be beneficial as supportive reference for the endeavor to reduce microplastic pollution in agricultural soil.

    Considering the fact that publications serve as an important criterion to evaluate the scientific accomplishments of an individual within respective fields in academia, there has been an increasing trend to publish scientific articles whereby multiple authors are defined as primary, co-, or corresponding authors according to the roles performed. This article analyzes the authorship pattern in 4,561 papers (including 60 single-authored papers) from 1990 till 2020 of 94 academics who hold a position as professors and are affiliated with the Faculty of Medicine at three different research universities in Malaysia. Only 708 papers (15.5% of 4,561 papers) were authored by less than three authors. In 3,080 papers (67.5% of 4,561 papers), those academics appeared as coauthors. Using different years as cutoff periods, it was observed that the appearance as coauthor in the papers had steeply risen around the years: 2006, 2007, 2008 and onwards. The increased number of authors in the multi-author papers and the appearance of the selected academics as coauthors reflect the extent of boosting of collaborative research in that period which corresponds to the adoption of the "publish or perish policy" by the Ministry of Higher Education in Malaysia.

    This paper reviewed the impacts of climate change on the management of the water sector in Malaysia discussing the current status of water resources, water service, and water-related disasters. The implementation of engineering practices was discussed to provide the detailed assessment of climate change impacts, risks, and adaptation for sustainable development. The narrative methods of reviewing the literatures were used to get an understanding on the engineering practices of water infrastructures, implication of the government policies, and several models as the main motivation behind the concept of integrated water resource management to contribute as part of the sustainable development goals to achieve a better and more sustainable future for all. The findings of this review highlighted the impacts of climate change on the rivers, sea, lakes, dams, and groundwater affecting the availability of water for domestic and industrial water supplies, irrigation, hydropower, and fisheries. The impacts of climate change on the water-related disasters have been indicated affecting drought-flood abrupt alternation and water pollution. Challenges of water management practices facing climate change should be aware of the updated intensity-duration-frequency curves, alternative sources of water, effective water demand management, efficiency of irrigation water, inter-basin water transfer, and nonrevenue water. The transferability of this review findings contribute to an engagement with the society and policy makers to mobilize for climate change adaptation in the water sector.
