We present in this paper a novel dataset (MMU VAAC) for violent actions against children recognition. This original dataset has been recorded using Microsoft Kinect with the usage of a child mannequin. MMU VAAC dataset contains skeleton joints, depth, and RGB modalities.
Malignant pedunculated (exophytic) hepatocellular carcinoma (P-HCC) is a rare entity of hepatocellular
carcinoma. Due to its unique characteristics, it gives a different spectrum of clinical findings, radiological
presentation and different management approach with reported better outcome to the patients. We
present a case report of a lady diagnosed with malignant pedunculated hepatocellular carcinoma.
Patients who self-discharge against medical advice (DAMA) are susceptible to life-threatening consequences. By understanding the factors associated with DAMA, healthcare centres can build strategies to assist patients to receive optimal medical care and prevent unfavourable outcome. The objective of this study was to determine the factors associated with DAMA from the Emergency Department (ED) of Universiti Kebangsaan Malaysia Medical Centre (UKMMC). This was a prospective unmatched case control study conducted over a 4-month period. For every DAMA episode, two patients who were admitted on the same day were randomly selected as control. Following patient consent, data was collected using a standardized questionnaire. Patients were contacted by the investigator for information regarding hospitalization within two weeks of DAMA. Ninety three patients were recruited; 31 DAMA patients and 62 admitted patients. Payment method was significantly associated with DAMA (OR 3.17 95% CI 1.29-7.98; p=0.01). The likelihood of self-paying patients to take DAMA was three times higher than those who had a guarantor letter from their employer or insurance provider. Other factors which influence DAMA were family obligations (OR 4.08 95% CI 1.09-15.26; p = 0.03) and work problems (OR 3.83 95% CI 1.13-12.94; p=0.03). A total of 19.4% of DAMA patients left following symptomatic pain relief. A total of 80.6% DAMA patients were admitted to hospital within two weeks of the DAMA episode. Payment method significantly influences DAMA. Payment planning, social welfare services, non-governmental organization funds and the introduction of a national health policy scheme may aid hospital payment, alleviate financial limitation of patients and reduce DAMA episodes.
Medication wastage poses a financial burden on the nation’s economy, as well as, environmental implications associated with wastage disposal. This study was conducted in a public hospital in Brunei and aimed to measure the extent of medication wastage and its disposal. This cross-sectional study used a self-designed questionnaire for patients attending outpatient pharmacy at the Suri Seri Begawan (SSB) hospital in which 253 patients participated over a period of two weeks. Exclusion criteria were: i) patients below 18 yrs, ii) patients visiting the Accident and Emergency Department and iii) patients admitted to the hospital. Majority of the participants were reported to have unused medication at home (75.1%, n=189). Nearly half (54.2%) had unused medicines and this was due to improvements in their medical conditions. Most of the participants (70.8%) were not given necessary advice on how to dispose their medicines in a proper manner. Majority disposed their unused medicines in the household garbage (70%), despite 50.2% (n=189) knowing that disposing of medicines in the garbage can cause detrimental effects on the environment. It was found that 47.7% of the participants would return their unused medicines back to the pharmacy, but in practice, only 18.6% did such. There was no significant association between knowing detrimental effects of medicine on
the environment and returning their medicines to the pharmacy (p=0.065). Results showed that medication wastage exists in SSB hospital and patients’ knowledge and
practices on its proper disposal was relatively poor. Health policy makers should be informed to implement wastage reduction strategies.
This work proposes channel impulse response (CIR) prediction for time-varying ultra-wideband (UWB) channels by exploiting the fast movement of channel taps within delay bins. Considering the sparsity of UWB channels, we introduce a window-based CIR (WB-CIR) to approximate the high temporal resolutions of UWB channels. A recursive least square (RLS) algorithm is adopted to predict the time evolution of the WB-CIR. For predicting the future WB-CIR tap of window wk, three RLS filter coefficients are computed from the observed WB-CIRs of the left wk-1, the current wk and the right wk+1 windows. The filter coefficient with the lowest RLS error is used to predict the future WB-CIR tap. To evaluate our proposed prediction method, UWB CIRs are collected through measurement campaigns in outdoor environments considering line-of-sight (LOS) and non-line-of-sight (NLOS) scenarios. Under similar computational complexity, our proposed method provides an improvement in prediction errors of approximately 80% for LOS and 63% for NLOS scenarios compared with a conventional method.
INTRODUCTION: Antiphospholipid antibodies (aPL) are autoantibodies that attack phospholipid through anti-beta 2-glycoprotein 1. The actions of aPL are associated with events leading to thrombosis and morbidity in pregnancy. Antiphospholipid syndrome (APS) is diagnosed when a patient is persistently positive for aPL and also has recognised clinical manifestations such as recurrent pregnancy losses, arterial or venous thrombosis and in a catastrophic case, can result in death. Unfortunately, the pathogenesis of APS is still not well established. Recently, microRNA expressed in many types of diseased tissues were claimed to be involved in the pathological progression of diseases and has become a useful biomarker to indicate diseases, including APS.
OBJECTIVE: This systematic review aims to search for research papers that are focussing on microRNA expression profiles in APS.
METHOD: Three search engines (Ebcohost, ProQuest and Ovid) were used to identify papers related to expression of specific microRNA in antiphospholipid syndrome.
RESULTS AND DISCUSSION: A total of 357 papers were found and screened, out of which only one study fulfilled the requirement. In this particular study blood samples from APS patients were tested. The microRNAs found to be related to APS were miR-19b and miR-20a. No data was found on specific microRNA being expressed in obstetric antiphospholipid syndrome. Analysis on the microRNA target genes revealed that most genes targeted by miR-19b and miR-20a involve in TGF-Beta Signalling and VEGF, hypoxia and angiogenesis pathways.
CONCLUSION: In view of the limited data on the expressions of microRNA in APS we recommend further research into this field. Characterization of microRNA profile in blood as well as in placenta tissue of patients with APS could be useful in identifying microRNAs involved in obstetric APS.
In 2015, a female patient returning to Australia from Sabah, Malaysia, was diagnosed with a suspected sylvatic dengue virus type 2 (DENV-2) infection, becoming the second case of imported highly divergent dengue virus infection recorded in Australia. We describe here the complete genome sequencing of the DENV-2 strain isolated from this patient.
Fluid replacement is the mainstay treatment for diabetic ketoacidosis (DKA). Currently, the best choice of fluids is still debatable. An amount of 0.9% sodium chloride is commonly used. Sterofundin® is an alternative crystalloid that is assumed to expedite resolution of acidosis. Advantages in sterofundin content being smaller significant ion difference (SID) to plasma and lower chloride content. The main objective of the study was to compare rate of acidosis resolution in DKA patients between treatment with 0.9% normal saline and Sterofundin over 12 hrs. Other objectives were to compare significant ion difference (SID), 12-hr blood ketone clearance and electrolyte balance between the two groups. The study was a prospective open labelled randomized control trial. This study was conducted over 6 months. Sample size of 18 was obtained with 9 for each arm. Main difference between two groups was initial median 2-hr pH level improvement (NS = +0.006 vs. Sterofundin = +0.05, P=0.063), however not being significant. Ketone, anion gap reduction, bicarbonate normalisation, sodium, chloride, urea and creatinine levels failed to show any significant differences between both groups. Twelve-hour median chloride levels increments were higher in the NS group (+11) compared to the sterofundin group (+6). There was no difference between mortality and morbidity. Comparing the two fluid groups, there was no significant biochemical differences during treatment of DKA. This was a pilot study that can initiate further clinical trials.

Acute myeloid leukaemia (AML) is the most common subtype of acute leukaemias with a poor outcome. Msi2 protein is a newly discovered prognostic marker and it has been considered as a new target for therapy in AML. The study of Msi2
protein expression in AML cases has not been performed in Malaysia, to date. The main aim of the present study was to observe the expression of Msi2 protein in AML patients by immunohistochemistry (IHC) and to correlate its expression
with the well-established prognostic and clinical parameters in AML as well as the overall survival (OS). Sixty four bone marrow trephine biopsy sections were immunostained for Msi2 protein. The percentage of blasts with positive reaction
and the intensity of the cytoplasmic and nuclear staining were evaluated. The expression of Msi2 protein was found in 95.3% cases with Msi2 pattern varying between the cases. In 71.9% of cases, the blasts showed total cellular positivity and 23.4% cases showed only cytoplasmic positivity. Majority showed high expression of Msi2 for cytoplasmic staining. Interestingly, there was significant correlation between total cellular staining and the intermediate cytogenetic subgroup (P=0.04). In conclusion, the results showed that the majority of the patients had high expression of Msi2 but this did not correlate to OS. However, the Msi2 expression correlated to the cytogenetic findings. The results suggest future extensive research to be conducted in order to ascertain the exact role of Msi2 positive blast cells in AML in our population and their association with prognosis and outcome.

The light train (LRT) is one of the important public transportation modes in the congested city of Kuala Lumpur. Train drivers drive the trains within their shift time on similar routes every day. This job activity may lead to stress. Physical fatigue is one of the health problems commonly experienced by the train drivers. There were three methods applied in this study, namely, Nordic Questionnaire, NASA-TLX and observations. There were a total of 52 respondents, consisting of the train drivers from Rapid Rail Sdn Bhd, a subsidiary company of Prasarana Malaysia Berhad. The questionnaires were analyzed using Statistical Software for the Social Science (SPSS) with a suitable statistical analysis. It was found that, the train drivers experienced pain on their necks, shoulders and lower backs. The main causes of physical fatigue that affect the performance of a train driver are sleep disorder and uncomfortable seating. Moreover, other effects of physical fatigue also prevent the drivers from carrying out activities at home. The industry has improved the drivers’ workstations such as their seats to reduce the rate of physical fatigue. However, more improvements are expected to be done.
An innovative assembly to maintain isoflurane anaesthesia in rats is described. This assembly was used successfully in laboratory rats for dental procedures that may last as long as sixty minutes. Repeated exposures to isoflurane anaesthesia, of up to eight times within a 30-day study period in healthy laboratory rats did not result in any observable adverse-effects.
Neuroblastoma is usually presented with abdominal distension. However, central nervous system manifestations of neuroblastoma are uncommon. In this case report, patient presented with uncommon presentation of neuroblastoma and the diagnostic dilemma.
Thyroid disorders could be broadly categorised into benign and malignant diseases. Approaches for the
management of benign and malignant thyroid disorders have been well documented including the treatment
using radioactive iodine (RAI). RAI has long been used to treat hyperthyroidism and well differentiated
thyroid cancer. Nevertheless, there are various factors that may influence the outcome of RAI treatment
including matters related to patient preparations. Thus, healthcare personnel play an important role in
assisting patients to make the necessary preparations. This article aims to give nurses and clinicians of
various specialities an insight into the overview of RAI treatment for thyroid disorders particularly in
Malaysia and further discusses the issues related to preparations of patients.
Unsafe behaviour is the key factor which contributes to the high number of incidents. Applying Behavior-Based
Safety (BBS) into safety risk controls has been adopted in major industry toward improving safety performance.
This approach to prevent incident has a number of advantages. The objective of this study is to identify the BBS
factors which can contribute to reduce incidents at the workplace. Human factors, behaviour and environment have
been identified as BBS factors. Human factors are influenced by knowledge and understanding of the workers while
behaviour factors are influenced by culture and practices in a daily life. In addition, environmental factor involves
equipment and work area. For an effective BBS approach, level of knowledge, understanding and practices by the
workers become the main concerns in the implementation of BBS. The findings can become the reference model for
future implementation in other organizations and as a guidance for better safety management.
In this paper, we review the history of Dengue, the mechanism of infection, the molecular characteristics and components of Dengue, the mechanism of entry to the target cells, cyclization of the genome and replication process, as well as translation of the proteins for virus assembly. The major emphasis of this work is on natural products and plant extracts, which were used for as palliative or adjuvant treatment of Dengue. This review article also summarizes the latest findings in regards to the marine products as effective drugs to target different symptoms of Dengue. Furthermore, an update on synthetic drugs for treating Dengue is provided in this review. As a novel alternative, we describe monoclonal antibody therapy for Dengue management and treatment.
Multirotor drones have been one of the most important technological advances of the last decade. Their mechanics are simple compared to other types of drones and their possibilities in flight are greater. For example, they can take-off vertically. Their capabilities have therefore brought progress to many professional activities. Moreover, advances in computing and telecommunications have also broadened the range of activities in which drones may be used. Currently, artificial intelligence and information analysis are the main areas of research in the field of computing. The case study presented in this article employed artificial intelligence techniques in the analysis of information captured by drones. More specifically, the camera installed in the drone took images which were later analyzed using Convolutional Neural Networks (CNNs) to identify the objects captured in the images. In this research, a CNN was trained to detect cattle, however the same training process could be followed to develop a CNN for the detection of any other object. This article describes the design of the platform for real-time analysis of information and its performance in the detection of cattle.
Product reviews are the individual's opinions, judgement or belief about a certain product or service provided by certain companies. Such reviews serve as guides for these companies to plan and monitor their business ventures in terms of increasing productivity or enhancing their product/service qualities. Product reviews can also increase business profits by convincing future customers about the products which they have interest in. In the mobile application marketplace such as Google Playstore, reviews and star ratings are used as indicators of the application quality. However, among all these reviews, hereby also known as opinions, spams also exist, to disrupt the online business balance. Previous studies used the time series and neural network approach (which require a lot of computational power) to detect these opinion spams. However, the detection performance can be restricted in terms of accuracy because the approach focusses on basic, discrete and document level features only thereby, projecting little statistical relationships. Aiming to improve the detection of opinion spams in mobile application marketplace, this study proposes using statistical based features that are modelled through the supervised boosting approach such as the Extreme Gradient Boost (XGBoost) and the Generalized Boosted Regression Model (GBM) to evaluate two multilingual datasets (i.e. English and Malay language). From the evaluation done, it was found that the XGBoost is most suitable for detecting opinion spams in the English dataset while the GBM Gaussian is most suitable for the Malay dataset. The comparative analysis also indicates that the implementation of the proposed statistical based features had achieved a detection accuracy rate of 87.43 per cent on the English dataset and 86.13 per cent on the Malay dataset.
Chronic subdural hematoma manifests differently and may mimic the
presentation of psychiatric illnesses. Many a time, physicians are quick to
judge that new onset of psychiatric symptoms is due to the worsening of the
existing psychiatric illness. We reported a case of a lady with learning
disability presenting with neuropsychiatric symptoms who was found to have
an acute-on-chronic subdural hematoma. We discussed regarding the new
onset of neuropsychiatric features seen in patients with chronic subdural
hematoma and its management.
