    BACKGROUND: Epilepsy has a high incidence among infants during their first year of life, yet the prognosis can vary significantly. Although considerable research has been conducted on infantile spasms, studies examining infantile-onset epilepsy, excluding infantile spasms, remain limited, particularly concerning the factors influencing outcomes. Therefore, our study aims to elucidate seizure control, developmental outcomes, and prognostic factors in infants with epilepsy during their first year of life, within a single-center study in Malaysia.

    METHODS: We retrieved data from patients who experienced seizures before age 12 months and were followed for over two years, using electronic patient records at Hospital Raja Perempuan Zainab II in Kelantan, a state in Malaysia's east coast. We retrospectively reviewed these records and assessed clinical outcomes based on the last follow-up.

    RESULTS: Of 75 patients, 61 (81.3%) achieved good seizure control or remission. At the last follow-up, 24 (32%) exhibited developmental delay, whereas 19 (25.3%) displayed abnormal neuroimaging. Patients with abnormal background electroencephalographic (EEG) activity, as well as abnormal radiological findings, were more likely to experience poor seizure control and unfavorable developmental outcomes (P

    PURPOSE: Kelulut honey contains trehalulose and has high antioxidant content, such as phenolic and flavonoid substances, which can promote wound healing. This study evaluated the effectiveness of Kelulut honey in diabetic wound healing compared to a commercially available conventional gel dressing (Intrasite gel).

    METHODS: A prospective, randomized, single-blinded control trial was performed on eligible diabetic patients with full-thickness cavity wounds. Patients' demographics, size and site of wounds, and baseline routine blood investigations were recorded. The wounds were dressed every other day with Kelulut honey for the intervention group or gel for the control group. The wound size reduction and granulation tissue formation percentage were calculated every 6 days for 1 month.

    RESULTS: Seventy-one patients were randomized. After 30 days of follow-up, 62 participants were available for analysis: 30 from the control group and 32 from the treatment group. The control group had increased granulation tissue at baseline and more wounds on the lower limb and posterior trunk.     Microstrip couplers play a crucial role in signal processing and transmission in various applications, including RF and wireless communication, radar systems, and satellites. In this work, a novel microstrip 180° coupler is designed, fabricated and measured. The layout configuration of this coupler is completely new and different from the previously reported Rat-race, branch-line and directional couplers. To obtain the proposed coupler, the meandrous coupled lines are used and analyzed mathematically. To improve the performance of our coupler, an optimization method is used. The designed coupler is very compact with an overall size of 0.014λg2. The obtained values of S21 and S31 are -3.45 dB and -3.75 dB, respectively at the operating frequency, while the fractional bandwidth (FBW) is 56.2%. It operates at fo = 1.61 GHz (suitable for 5G applications) and can suppress harmonics up to 2.17fo. Another advantage of this coupler is its low phase imbalance, while the phase difference between S21 and S31 is 180°± 0.023°. Therefore, our device is a balanced coupler with ±0.3 dB magnitude unbalance at its operating frequency. It is important to note that it is very difficult to find a coupler that has all these advantages at the same time. The proposed 180° coupler is fabricated and measured. The comparison shows that the measurement and simulation results are in good agreement. Therefore, the proposed coupler can be easily used in designing high-performance 5G communication systems.

    The halophilic genus Joostella is one of the least-studied genera in the family of Flavobacteriaceae. So far, only two species were taxonomically identified with limited genomic analysis in the aspect of application has been reported. Joostella atrarenae M1-2T was previously isolated from a seashore sample and it is the second discovered species of the genus Joostella. In this project, the genome of J. atrarenae M1-2T was sequenced using NovaSeq 6000. The final assembled genome is comprised of 71 contigs, a total of 3,983,942 bp, a GC ratio of 33.2%, and encoded for 3,416 genes. The 16S rRNA gene sequence of J. atrarenae M1-2T shows 97.3% similarity against J. marina DSM 19592T. Genome-genome comparison between the two strains by ANI, dDDH, AAI, and POCP shows values of 80.8%, 23.3%, 83.4%, and 74.1% respectively. Pan-genome analysis shows that strain M1-2T and J. marina DSM 19592T shared a total of 248 core genes. Taken together, strain M-2T and J. marina DSM 19592T belong to the same genus but are two different species. CAZymes analysis revealed that strain M1-2T harbors 109 GHs, 40 GTs, 5 PLs, 9 CEs, and 6 AAs. Among these CAZymes, while 5 genes are related to cellulose degradation, 12 and 24 genes are found to encode for xylanolytic enzymes and other hemicellulases that involve majorly in the side chain removal of the lignocellulose structure, respectively. Furthermore, both the intracellular and extracellular crude extracts of strain M1-2T exhibited enzymatic activities against CMC, xylan, pNPG, and pNPX substrates, which corresponding to endoglucanase, xylanase, β-glucosidase, and β-xylosidase, respectively. Collectively, description of genome coupled with the enzyme assay results demonstrated that J. atrarenae M1-2T has a role in lignocellulosic biomass degradation, and the strain could be useful for lignocellulosic biorefining.

    PURPOSE: Alzheimer's disease (AD) is a common and devastating neurological ailment that affects millions of the elderly worldwide. Therapeutic toys and games have emerged as potential non-pharmacological interventions for AD. However, despite a growing number of documents on the subject, research on the future direction of therapeutic toys and games for AD remains scarce. To address this gap, this study aims to (1) map the future trends of therapeutic toys and games for AD and (2) identify the categories and design characteristics.

    MATERIALS AND METHODS: Using a thematic review framework, a systematic literature search was conducted in two electronic databases (Scopus and WoS) using established criteria. Thematic analysis was done using ATLAS.ti 23 to identify prominent themes, patterns and trends.

    RESULTS: A total of 180 documents were found. Twenty-five articles met the inclusion criteria. A thematic review of these 25 articles identified 13 initial codes, which were been clustered into four themes: detection and evaluation; intervention; toy/game category; and design characteristics. The word "Cognitive" appears most frequently in documents according to word cloud.

    CONCLUSIONS: Therapeutic toys and games are used to detect and as an intervention for AD. Most of the current studies focused on specific cognitive functions. More research is needed about play therapy for neuropsychiatric symptoms. This thematic review also proposed a conceptual framework for designing toys and games tailored to the needs of the elderly with AD, offering valuable insights to future researchers focusing on this domain.

    The HT-29 cell line, derived from human colon cancer, is valuable for biological and cancer research applications. Early detection is crucial for improving the chances of survival, and researchers are introducing new techniques for accurate cancer diagnosis. This study introduces an efficient deep learning-based method for detecting and counting colorectal cancer cells (HT-29). The colorectal cancer cell line was procured from a company. Further, the cancer cells were cultured, and a transwell experiment was conducted in the lab to collect the dataset of colorectal cancer cell images via fluorescence microscopy. Of the 566 images, 80 % were allocated to the training set, and the remaining 20 % were assigned to the testing set. The HT-29 cell detection and counting in medical images is performed by integrating YOLOv2, ResNet-50, and ResNet-18 architectures. The accuracy achieved by ResNet-18 is 98.70 % and ResNet-50 is 96.66 %. The study achieves its primary objective by focusing on detecting and quantifying congested and overlapping colorectal cancer cells within the images. This innovative work constitutes a significant development in overlapping cancer cell detection and counting, paving the way for novel advancements and opening new avenues for research and clinical applications. Researchers can extend the study by exploring variations in ResNet and YOLO architectures to optimize object detection performance. Further investigation into real-time deployment strategies will enhance the practical applicability of these models.

    Moina micrura represents a promising model species for ecological and ecotoxicological investigations in tropical freshwater ecosystems. Illumina NovaSeq™ 6000 sequencing was employed in this study to analyze M. micrura across three distinct developmental stages: juvenile, adult, and male. Current study successfully annotated 51,547 unigenes (73.11%) derived from seven (7) different databases. A total of 554 genes were found to be significantly upregulated, while 452 genes showed significant downregulation between juvenile and male. Moreover, 1001 genes were upregulated, whereas 830 genes exhibited downregulation between the adult and male. Analysis of differentially expressed genes revealed upregulation of chitin, cuticle, myosin (MYO), mitogen-activated protein kinases (MAPK), fibrillin (FBN), cytochrome (CYP), glutathione s-transferase (GST), vitellogenin (VTG), acetylcholinesterase (AChE), and transforming growth factor beta (TGFB) under unfavorable environmental conditions (male), as compared to favorable environmental conditions (juveniles and adults). These alterations in gene expression significantly impact the phenological and life-history traits of M. micrura. Furthermore, the upregulation of hemoglobin (HMB), doublesex (DSX), juvenile hormone analogs (JHA), heat shock protein (HSP), and methyltransferase (METT) genes in males initiates the sex-switching effects observed in M. micrura. These findings hold substantial value for researchers interested in determining M. micrura sequences for future investigations of gene expression and comparative reproductive genome analysis within the Moina genus and cladoceran families.

    PURPOSE: The aim of this study was to evaluate the mechanical stimuli transfer at the bone-implant interface via stress and strain energy density transfer parameters. This study also aimed to investigate the effect of different implant stiffness and parafunctional loading values on the defined mechanical stimuli transfer from the implant to the surrounding bone.

    METHODS: A three-dimensional finite element model of two-piece threaded dental implant with internal hexagonal connection and mandibular bone block was constructed. Response surface method through face-centred central composite design was applied to examine the influence of two independent factors variables using three levels. The analysis model was fitted to a second-order polynomial equation to determine the response values.

    RESULTS: The results showed that the implant stiffness was more effective than the horizontal load value in increasing the stress and strain energy density transfers. The interaction between both factors was significant in decreasing the likelihood of bone resorption. Decreasing the implant stiffness and horizontal load value led to the increased stress transfer and unexpected decrease in the strain energy density, except at the minimum level of the horizontal load. The increase in the implant stiffness and horizontal load value (up to medium level) have increased the strain energy transfer to the bone.

    CONCLUSIONS: The stress and strain energy density were transferred distinctively at the bone-implant interface. The role of both implant stiffness and parafunctional loading is important and should be highlighted in the preoperative treatment planning and design of dental implant.

    This study was a co-operative investigation of myxosporean infections of Notopterus notopterus, the bronze featherback, which is a popular food fish in the South Asian region. We examined fish from Lake Kenyir, Malaysia and the River Ganga, Hastinapur, Uttar Pradesh, India, and observed infections with two myxosporeans: Myxidium cf. notopterum (Myxidiidae) and Henneguya ganapatiae (Myxobolidae), respectively. These species were identified by myxospore morphology, morphometry and host tissue affinity, and the original descriptions supplemented with small-subunit ribosomal DNA sequences and phylogenetic analysis. Free myxospores of M. cf. notopterum were found in the gallbladder, and measured 14.7 ± 0.6 μm long and 6.3 ± 0.6 μm wide; host, tissue and myxospore dimensions overlapped with the type, but differed in morphological details (spore shape, valve cell ridges) and locality (Malaysia versus India). Plasmodia and spores of H. ganapatiae were observed in gills, and myxospores had a spore body 9.7 ± 0.4 μm long, 4.5 ± 0.5 μm wide; sample locality, host, tissue, spore morphology and morphometry matched the original description. Small-subunit ribosomal DNA sequences were deposited in GenBank (M. cf. notopterum MT365527, H. ganapatiae MT365528) and both differed by >7% from congeneric species. Although the pathogenicity and clinical manifestation of myxozoan in humans are poorly understood, consumption of raw fish meat with myxozoan infection was reported to be associated with diarrhea. Identification of current parasite fauna from N. notopterus is an essential first step in assessing pathogen risks to stocks of this important food fish.

    This study utilizes bibliometric analysis to examine historical and present research patterns in the area of energy transition and green finance and to forecast potential future domains. Using the bibliometric method, 328 scholarly articles from the Web of Science database were evaluated. This paper identifies influential publications, maps the research landscape, and forecasts emerging tendencies through co-citation and co-word analyses. Co-citation analysis found three main clusters, while co-word analysis revealed four main clusters. Despite the growing significance of research on energy transition and green finance research, further in-depth investigation is necessary to offer a thorough depiction of the research domain. This research represents a pioneering endeavour in the utilization of bibliometric analysis to investigate the interrelationship between two items. It offers valuable insights into the rapidly expanding field of energy transition and green finance, effectively highlighting its contours and indicating potential future developments.

    Even though Pacific - Indian Ocean exchange [Indonesian Throughflow (ITF)] has been measured for the last three decades, the measurements of microplastic in the region is very limited. This study was the initial investigation of the vertical distribution of microplastic in the deep-sea areas across the ITF Pathway. Niskin water samples were utilized to obtain the samples from a water column in a range of 5 to 2450 m. A total of 924 microplastic particles with an average abundance of 1.062 ± 0.646. n/L were found in the water column. Our findings indicate that water temperature and water density are the most significant factors correlated to the microplastic concentration. This study will be the first report discussing the distribution of microplastics in the deep-sea water column that could be highly significant in determining the fate and transport of microplastic within Indonesian waters that exits into the Indian Ocean.

    Medical students in Malaysia and globally were considered among the most exposure groups in the community to stress due to several sources. Stress is a very important issue that leads to a worsening of health problems. Stress develops in the body and leads to oxidative stress which in turn leads to a disorder in the whole body. Oxidative stress may lead to abnormal haematological indices elevated white blood cells (WBCs) count. Oxidative stress can lead to massive destruction of red blood cells (RBCs). The brain and the gastrointestinal system (GI) are intimately connected as one system. The brain has a direct impact on the GI tract. A stressful brain can send signals to the gut, just as a troubled intestine can send signals to the brain. Therefore, stress can be the cause of block the breakdown and assimilation of food for energy and nutriment. This malabsorption can then lead to a reciprocal negative effect to the stress and can be another cause of anaemia through malabsorption minerals and vitamins that are to erythropoiesis. So, stress can be one of the leading causes of anaemia among medical students. Stress is a chronic epidemic in the most medical students and can directly affect how well body works. This review article discovers the effect of stress in medical students that can be effect on their studies and further create researcher's interest to generate database that help to reduce stress response and bring about the empowerment of balanced life among Malaysian medical students besides the increasing level of health and academic performance.

    Alzheimer's disease (AD) is characterized by aggregation of amyloid beta (Aβ) plaque. RhoA may serve as a potential target for prevention against AD given its role in the amyloidogenic pathway. The recent emergence of the gut-brain axis has linked lactic acid bacteria (LAB) to neuroprotection against AD. This study assessed the importance of RhoA inhibition in mediating the neuroprotective potential of LAB. To this end, de Man, Rogosa and Sharpe (MRS) broth fermented by lactobacilli or pediococci were tested against SK-N-SH (a human neuroblastoma cell line) in the presence of RhoA activator II for 24 h after which the RhoA activity was measured using the G-LISA Kit. Fluorescence staining of f-actin stress fibres was performed to validate RhoA inhibition. SK-N-SH was transfected with plasmid expressing amyloid precursor protein (APP) gene. The Aβ concentration in transfected cells exposed to LAB-derived cell free supernatant (CFS) in the presence of RhoA activator II was measured using the ELISA kit. Furthermore, this study measured organic acids in LAB-derived CFS using the gas chromatography. It was found that LAB-derived CFS yielded strain-dependent inhibition of RhoA, with LAB6- and LAB12-derived CFS being the most potent Pediococcal- and Lactiplantibacillus-based RhoA inhibitor, respectively. Lesser stress fibres were formed under treatment with LAB-derived CFS. The LAB-derived CFS also significantly inhibited Aβ in SK-N-SH transfected with APP gene in the presence of RhoA activator II. The LAB-derived CFS was presented with increased lactic acid, acetic acid, butyric acid and propionic acid. The present findings warrant in-depth study using animal models.

    BACKGROUND: Iron deficiency (ID) is prevalent in Malaysian children. The incidence of ID in infants under 6 months of age is unknown. Our aim was to determine the prevalence of iron deficiency (ID) and iron deficiency anemia (IDA) in healthy, term infants aged below 6 months in our hospital population.

    METHODS: A prospective longitudinal pilot study of mother-infant pairs was conducted on infants receiving routine immunizations in a mother and child clinic at a university hospital, in Kuala Lumpur, Malaysia. Mothers completed standardized questionnaires at 3- and 5-month postnatal visits. Maternal and infant full blood count, ferritin, and C-reactive protein (CRP) levels were measured at 3 months and for the infants repeated at 5 months. Infant anthropometric measurements were obtained at both visits. We conducted a univariate analysis to identify factors associated with ID and IDA.

    RESULTS: Altogether, 91 mother-infant pairs were enrolled, with 88 completing the study. No infant had ID or IDA at 3 months; the lowest ferritin level was 16.6 µg/L. At 5 months, 5.9% (5/85) of infants had ID, and 2.4% (2/85) had IDA. Median (interquartile range) infant ferritin levels significantly declined from 113.4 (65.0-183.6) µg/L at 3 months to 50.9 (29.2-70.4) µg/L at 5 months, p

    Microalgal lipids are promising and sustainable sources for the production of third-generation biofuels, foods, and medicines. A high lipid yield during the extraction process in microalgae could be influenced by the suitable pretreatment and lipid extraction methods. The extraction method itself could be attributed to the economic and environmental impacts on the industry. This review summarizes the pretreatment methods including mechanical and non-mechanical techniques for cell lysis strategy before lipid extraction in microalgae biomass. The multiple strategies to achieve high lipid yields via cell disruption techniques are discussed. These strategies include mechanical (shear forces, pulse electric forces, waves, and temperature shock) and non-mechanical (chemicals, osmotic pressure, and biological) methods. At present, two techniques of the pretreatment method can be combined to increase lipid extraction from microalgae. Therefore, the extraction strategy for a large-scale application could be further strengthened to optimize lipid recovery by microalgae.

    OBJECTIVES: Unexplained subfertility (UEI) describes a couple whose standard subfertility workout consider acceptable but unable to conceived.

    METHODS: This retrospective study was conducted in the Advanced Reproductive Centre, UKM Hospital, Kuala Lumpur, from January 2016 to December 2019. The data of 268 UEI couples were obtained from the clinical database. Women aged 21-45 years old was included and further divided into four groups according to the female partner's age and subfertility duration: group A (age <35 years and subfertility <2 years), group B (age <35 years and subfertility >2 years), group C (age >35 years and subfertility <2 years), and group D (age >35 years and subfertility <2 years). All statistical analyses were performed using SPSS 22.0 for Windows.

    RESULTS: A total of 255 cases were included in this study. The mean age of the women was 32.9 ± 4.04 years, and the mean subfertility duration was 5.04 ± 2.9 years. A total of 51 (20 %) cases underwent timed sexual intercourse, 147 (57.6 %) cases had intrauterine insemination (IUI), whereas 57 (22.4 %) cases opted for in vitro fertilization (IVF). A total of 204 cases underwent active management (IUI/IVF), which showed a significant difference (p<0.05). Out of eight clinical pregnancies, half of them were from group B.

    CONCLUSIONS: Active management in younger women with a shorter subfertility duration revealed a better pregnancy outcome. Otherwise, individualized treatment should be considered in selecting a suitable treatment plan.

    In recent research, Durandal, a signature scheme based on rank metrics following Schnorr's approach, was introduced to conceal secret key information by selectively manipulating the vector subspace of signatures. Later, an enhancement, namely the SHMW signature scheme, with smaller keys and signatures while maintaining EUF-CMA security, was proposed. Both Durandal and SHMW require adversaries to solve hard problems (i.e., Rank Support Learning, Rank Syndrome Decoding, and Affine Rank Syndrome Decoding) for secret key retrieval, in which the parameters are designed to withstand at least 128-bit computational complexity. The authors claimed that the security of the SHMW scheme is deemed superior to that of the original Durandal scheme. In this paper, we introduce a novel approach to identifying weak keys within the Durandal framework to prove the superiority of the SHMW scheme. This approach exploits the extra information in the signature to compute an intersection space that contains the secret key. Consequently, a cryptanalysis of the SHMW signature scheme was carried out to demonstrate the insecurity of the selected keys within the SHWM scheme. In particular, we proposed an algorithm to recover an extended support that contains the secret key used in the signature schemes. Applying our approach to the SHMW scheme, we can recover its secret key with only 97-bit complexity, although it was claimed that the proposed parameters achieve a 128-bit security level. The results of our proposed approaches show that the security level of the SHMW signature scheme is inferior compared to that of the original Durandal scheme.

    OBJECTIVE: In this article, we review the current studies on the role of podoplanin in oral cancer and the potential application of podoplanin inhibitors as a therapeutic agent for oral cancer.

    DESIGN: The narrative review approach was conducted, providing a comprehensive perspective of related literature. Publications addressing podoplanin and its inhibitors in the context of oral cancer were retrieved from PubMed and Scopus databases.

    RESULTS: Podoplanin has emerged as a biomarker and therapeutic agent for oral cancer. Numerous studies have reported high podoplanin expression in oral cancer and pre-cancerous lesions compared to normal cells. A specific inhibitor targeting podoplanin may have the potential to prevent oral carcinogenesis via interfering with the pathway of cancerous cells involved in cell proliferation and metastasis. Antibodies, chimeric antigen receptor (CAR)-T cells, cancer-specific mAb (CasMab), synthetic molecules, and lectins are among the materials used as anticancer agents targeting podoplanin. Plant-derived lectins appear to demonstrate a unique advantage against alternative candidates.

    CONCLUSIONS: The use of podoplanin inhibitors in place of existing therapeutic approaches could be a promising and novel approach to the prevention and treatment of oral cancer. Nevertheless, further research is required to investigate the practical application of such inhibitors.

    The conventional hydrodenitrogenation method is expensive and involves the use of catalysts and harsh procedures. In the last few years, ionic liquids (ILs) have gained attention as a promising alternative solvent for fuel oil extractive denitrogenation. In this work, the Conductor-like Screening Model for Real Solvents (COSMO-RS) was used to screen 173 potential ILs as solvents for fuel oil. Two ILs (1-ethyl-3-methylimidazolium dicyanamide ([EMIM][N(CN)2]) and 1-ethyl-3-methylimidazolium methanesulfonate ([EMIM][MeSO3])) were selected for experimental investigation. The experimental liquid-liquid extraction of pyrrole (taken as the model nitrogen compound) from n-hexadecane (the model fuel) was conducted at 298 K and 1 atm with feed concentrations of pyrrole ranging from 10 to 50 wt%, using either the two pure ILs or their mixtures with dimethylformamide or ethylene glycol. Moreover, the NRTL model was effectively used to correlate the experimental tie lines. This work shows that the use of a binary mixture of ILs with a conventional solvent results in good selectivity, but has a low capacity for extracting pyrrole compounds. On the other hand, using an IL-IL mixture exhibits good results for both capacity and selectivity. All the ternary systems tested showed positive slopes, indicating that the nitrogen compounds had a higher affinity for the IL and binary mixture extract phase. In fact, the extraction efficiency for all the systems shows promising results. This characteristic is advantageous, as it requires less solvent to remove nitrogen compounds.

    Ocular ischemic syndrome is a rare, blinding condition that usually presents rather late. This occurs mainly due to stenosis or occlusion of the carotid artery which supplies the ocular structures. Bilateral involvement may occur in one in five cases but may be asymmetrical. We report a case of a 72-year-old gentleman with bilateral ocular ischemic syndrome secondary to left common carotid artery total occlusion and severe right proximal internal carotid artery stenosis in a patient who is an active smoker with chronic tophaceous gout. His vision remained stable after a year of follow-up, with the main emphasis on optimizing his medical condition and smoking cessation.
