    The first representative of the genus Cerapus in Malaysian waters, Cerapus bumbumiensis sp. nov. is described from specimens sampled from Pulau Bum Bum, Sabah (east Malaysia). The main identifying characteristics of this new amphipod species are: pereonites 1, 2 with constriction; male gnathopod 2 carpochelate, carpus large with long defining posterior tooth and well-developed anterodistal tooth; pereopod 6 coxa with fine hair/fringe setae ventrally; and telson with deep cleft. An updated identification key for the 23 known species in the genus is also presented.

    Acanthamoeba spp. merupakan ameba hidup bebas yang tersebar luas di persekitaran. Ameba ini menyebabkan penyakit
    keratitis terutamanya kepada individu yang memakai kanta sentuh terkontaminasi atau mempunyai sejarah trauma
    pada mata. Tujuh puluh empat sampel kikisan kornea pesakit keratitis daripada 4 buah hospital di sekitar Lembah
    Kelang telah dikultur untuk menemukan Acanthamoeba spp. Keputusan kajian mendapati Acanthamoeba spp. berjaya
    dipencilkan daripada 14.87% sampel kikisan kornea pesakit keratitis. Berdasarkan kajian ini, kesemua pesakit keratitis
    Acanthamoeba adalah merupakan pemakai kanta sentuh yang majoritinya (90.9%) terdiri daripada kaum wanita. Kanta
    sentuh lembut terlibat dalam 90.9% kes manakala jenis kanta separa keras cuma satu kes 9.1%. Kesemua 11 pencilan
    Acanthamoeba yang dipencilkan adalah daripada kumpulan polyphagids yang biasanya patogenik kepada manusia dan
    menyebabkan jangkitan ini. Kegagalan pengguna kanta sentuh mengamalkan tahap kebersihan kanta sentuh yang baik
    merupakan faktor risiko yang penting dalam kejadian keratitis Acanthamoeba di Malaysia.

    INTRODUCTION: This single-group, open-label, prospective, noncomparative, multicenter, Phase IV study explored the efficacy and tolerability of paliperidone palmitate (PP) in hospitalized patients with acute exacerbation of schizophrenia.

    METHODS: Asian patients of either sex, between 18 and 65 years of age, diagnosed with schizophrenia (Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition) with acute exacerbations within the previous 4 weeks, were enrolled. Intramuscular PP was initiated at doses of 150 milligram equivalent (mg eq) (day 1) and 100 mg eq (day 8), followed by a monthly maintenance dose between 75 mg eq and 150 mg eq (days 36 and 64). Primary efficacy endpoint was the change from baseline in the Positive and Negative Syndrome Scale (PANSS) total score (last-observation-carried-forward) at week 13.

    RESULTS: Of the 212 enrolled patients, 152 (71.7%) completed the 13-week treatment; withdrawal of consent (24 [11.3%] patients) was the most common reason for study discontinuation. Mean (standard deviation) PANSS total score from baseline (90.0 [17.41]) improved significantly at day 4 (-6.1 [9.27]; 95% confidence interval: -7.38, -4.85; P<0.001) and week 13 endpoint (-23.9 [23.24]; 95% confidence interval: -27.10, -20.78; P<0.001). Similarly, the secondary endpoints (Clinical Global Impression-Severity, Physical and Social Performance, each PANSS subscale, and Marder factor scores) improved significantly from baseline to week 13 endpoint (P<0.001 for all). At week 13, 112/210 (53.3%) patients had a 40% improvement in the PANSS total score (responder rate), and 133/212 (62.7%) patients were ready for hospital discharge. Overall, 139 (65.6%) patients experienced at least one treatment-emergent adverse event (TEAE). Most common (>5%) TEAEs were hyperprolactinemia, constipation, nasopharyngitis, insomnia, increased weight, and tremor. Worsening of schizophrenia (3.3%) and sinus bradycardia (2.0%) were serious TEAEs; no deaths were reported.

    CONCLUSION: PP was generally tolerable and efficacious in a hospital setting for the treatment of acute exacerbated schizophrenia with significant improvements in psychotic symptoms, social functioning, and severity of illness.

    Psychosocial risks are considered as a burning issue in the Asia-Pacific region. The aim of this study was to investigate the impact of psychosocial work environment factors on health of petrochemical industry workers of Malaysia. In lieu to job demands-resources theory, significant positive associations were found between quantitative demands, work-family conflict, and job insecurity with stress, while a significant negative association of role clarity as a resource factor with stress was detected. We also found that quantitative demands were significantly associated with the mean arterial pressure (MAP). Multistage sampling procedure was used to collect study sample. Structural Equation Modeling was used to identify relationship between the endogenous and exogenous variables. Finally, the empirically tested psychosocial work environment model will further help in providing a better risk assessment in different industries and enterprises.

    To determine the number of cases that are at risk of poor stability in terms of arch width changes following fixed appliances treatment at the Orthodontic Unit, Klinik Pergigian Cahaya Suria, Kuala Lumpur. In a retrospective audit, 101 pre- and post-treatment lower study casts were selected from cases completed in the year 2015 at the Orthodontic Unit, Klinik Pergigian Cahaya Suria, Kuala Lumpur. Samples were measured using a universal caliper by a single calibrated operator. Samples was categorised as extraction or non-extraction types. Arch width changes was determined using paired T-test. The recommended limit was 0mm for inter-canine width, 2 mm for inter-first premolar width and 3mm for inter-second premolar and inter-molar width. Differences were considered “within limits”, if the changes were within the recommended limit ±0.25mm (for possible marginal measurement error) and “expanded”, if above the range for within limits. 42.6% were non-extraction while 57.4% were extraction cases. In the non-extraction group, 52.2% cases had expanded inter-canine widths, followed by inter-first and second premolars (27.9%) and interfirst molar (20.9%) widths. Arch width changes for the inter-first and second premolars and inter-molars widths were statistically significantly different (p<0.05) but bot clinically significant. In the extraction group, 67.2% had expanded inter-canine widths, followed by inter-first premolar (64.3%), inter-second premolar (9.1%) and inter-first molar (5.2%) widths. The inter-canine (M=1.43; SD=2.71, p<0.05) and inter-first premolar (M=2.87; SD=2.61, p<0.05) widths statistically and clinically significant expansion but the inter-second premolar and molar were significantly contracted (p<0.05). The number of cases with expanded arch widths was high regardless of the extraction type.

    The effects of children’s exposure on high concentration of airborne pollutants at schools often associated with increased rate of absenteeism, low productivities and learning performances, and development of respiratory problems. Recent studies have found that the presence of occupants in the classroom seems to give major effect towards the elevation of concentration of airborne pollutants in indoors. In order to evaluate and further understand on the significance of occupancy factor on IAQ, this study has been designed to determine and compare the level of selected physical (particulate matter (PM)) and chemical (carbon dioxide (CO2) and temperature) IAQ parameters and biological contaminants via colony forming unit (CFUm-3 ) for bacteria and fungi inside the selected classrooms during occupied and non-occupied period (first objective). The second objective is to describe the possible sources of airborne pollutants inside the classrooms at the selected primary schools around Kuantan, Pahang. Assessments of physical and chemical IAQ were done by using instruments known as DustMate Environmental Dust Detector and VelociCalc® MultiFunction Ventilation Meter 9565.The data were recorded every 30 minutes for 8 hours during schooldays and weekend at the selected sampling point in the classrooms. For microbial sampling, Surface Air System Indoor Air Quality (SAS IAQ) was used to capture the bacteria and fungi. The data obtained were compared with the established standard reference known as the Industrial Code of Practice on Indoor Air Quality (2010) constructed by the Department of Occupational Safety and Health (DOSH), Malaysia. This study has found that some of the IAQ parameters in the selected classrooms were exceeding the established standards during occupied period in schooldays compared to non-occupied period during weekend. Findings of this study provide the insights for future research including the site selection of school, arrangement of the classrooms and numbers of students per class.

    PURPOSE: To develop an evidence-based Clinical Practice Guideline (CPG) on caries management for the Malaysian population using the ADAPTE trans-contextual adaptation framework.

    MATERIALS AND METHODS: A systematic search was conducted to identify all CPGs related to caries management on guideline repository websites and other platforms. The search findings were screened and the quality of the identified guidelines was evaluated using the AGREE II tool. The currency and the content of the recommendations were assessed by multidisciplinary experts for local adaptation.

    RESULTS: Following an extensive assessment, six high-quality CPGs were selected for adaptation. Subsequent to the content assessment, the multidisciplinary experts agreed to adopt 24 recommendations, adapt 55, and exclude two recommendations. The adaptation process generated 21 recommendations for caries management in Malaysia. The formulation of the final evidence-based recommendations for caries management in Malaysia was based on the feedback given by the external reviewers.

    CONCLUSION: The use of the trans-contextual adaptation process is feasible for the development of local guidelines when there are scarce resources and insufficient local evidence. The involvement of the multidisciplinary experts ensures the comprehensiveness of the CPG in terms of its quality and validity and subsequently promotes adherence and ownership of the CPG at the local settings.

    Kerajaan Malaysia telah memberikan penumpuan terhadap aspek keselamatan dan pembangunan
    pertahanan sejak dari kemerdekaan negara lagi. Pada ketika ini, ancaman yang dihadapi oleh negara
    adalah melibatkan dua bentuk ancaman iaitu ancaman dari dalam negara dan ancaman dari luar negara.
    Kebanyakkan negara khususnya di rantau Asia Tenggara telah mencapai kemerdekaan di antara tahun
    1950-an dan 1960-an. Sebagai sebuah negara yang juga boleh dikategorikan juga baru mencapai
    kemerdekaan, negara-negara di rantau Asia Tenggara ini tidak lari dari menghadapi pelbagai ancaman
    yakni ancaman dari sudut dalaman negara seperti ancaman pemberontakan komunis semasa insurgensi,
    konflik etnik, ketidakstabilan politik, rusuhan kaum, masalah perpaduan dan sebagainya telah menjadi
    faktor yang juga telah mempengaruhi perbentukan Dasar Pertahanan Negara (DPN). Manakala
    ancaman dari sudut luar seperti persempadan, persaingan kuasa besar antara Amerika Syarikat dan
    Kesatuan Soviet semasa perang dingin yang juga menyebabkan berlakunya ketidakstabilan politik,
    konflik dan peperangan di rantau Asia Tenggara, persaingan ideologi, ketegangan hubungan dan
    konflik hubungan antara negara, ancaman terrorisme dan pemberontakan serta sebagainya telah
    mempengaruhi dan memaksa negara Malaysia untuk membangun serta memodenkan sistem pertahanan
    negara.

    “Fenomena anarki” telah mempengaruhi kebanyakan negara untuk memberikan tumpuan dan
    perhatian terhadap pembangunan Angkatan ketenteraan bagi menjamin keselamatan untuk tujuan
    kelangsungan hidup. Manakala merujukkan kepada teori neo-realisme, teori ini mengatakan dilema
    keselamatan sebagai situasi yang sentiasa ada ancaman yang cuba untuk menggugat keselamatan
    negara mahupun ianya dari bentuk unsur ancaman tradisional mahupun dari unsur dari ancaman bukan
    dari tradisional. Akibat dari senario keselamatan dan sifat anarki dalam sistem antarabangsa, keadaan
    ini akan menyebabkan negara perlu bergantung pada diri sendiri untuk kelangsungan hidup. Ini
    merujuk kepada konsep self help atau self reliance di mana setiap negara mempunyai kepentingan
    negara mereka tersendiri dalam meneruskan kelangsungan hidup dalam sistem antarabangsa. Melihat
    dari sudut Angkatan Tentera Malaysia (ATM,) perspektif pembangunan dan pemodenan ATM semasa
    era Perang Dingin dan Pasca Perang Dingin ternyata mempunyai perubahan yang signifikan di mana
    pembangunan ATM sebelum tahun 1990 lebih bersifat konvensional dan pasca Perang Dingin
    memperlihatkan pembangunan ATM bersifat lebih strategik.
