    How people perceive risk influences their behaviour towards these risks. We do not know how workers perceive risk of dying from activities or technology. This study was conducted among 198 workers of a security company in Malaysia. The workers were asked to score on a Likert scale of 1 to 5 the perceived risk of death of Malaysians from activities and technology. The highest perceived risks of death were, in order of ranking, motorcycles, motor vehicles, handguns, alcoholic beverages and smoking. The difference in perception and reality is discussed.

    We do not know how medical doctors perceive the risk of cancer as a cause of death. Medical doctors provide advice and information on health risks to the general public. How they perceive these risks influences the way they behave towards these risks and the way they will communicate these risks. We would like to ascertain how medical students perceive cancer as a health risk. This will hehv us determine misperceptions among medical students that need to be addressed before they practice medicine. A cross-sectional study was conducted among
    all UPM medical students (2000-2001) using sefadministered questionnaires. A total of 339 (88.3%) students responded to the questionnaires. The internal consistency of items was good (Cronbach Alpha >0,7). Cancer was perceived as the 3rd leading risk of death after motor vehicle accidents (I") and heart diseases/cardiovascular disorders (2"d ). The breast was perceived as the organ with the highest risk of developing cancer. Smoking was perceived to be the leading risk factor for developing cancer. There is a dpjference in risk perception of diseases, cancer target organs and cancer risk factors based on gender and ethnicity.     Individual septic tanks are the most common means of on-site sanitation in Malaysia, but they result in a significant volume of septage. A two-staged vertical flow constructed wetlands (VFCWs) system for the treatment of septage was constructed and studied in Sarawak, Malaysia. Raw septage was treated in the first stage wetlands, and the resulting percolate was fed onto the second stage wetlands for further treatment. Here, the effects of a batch loading regime on the contaminant removal efficiency at the second stage wetlands, which included palm kernel shell within their filter substrate, are presented. The batch loading regime with pond:rest (P:R) period of 1:1, 2:2 and 3:3 (day:day) was studied. The improvement of the effluent redox condition was evident with P:R = 3:3, resulting in excellent organic matters (chemical oxygen demand and biochemical oxygen demand) and nitrogen reduction. The bed operated with P:R = 1:1 experienced constant clogging, with a water layer observed on the bed surface. For the P:R = 3:3 regime, the dissolved oxygen profile was not found to decay drastically after 24 hours of ponding, suggesting that the biodegradation mainly occurred during the first day. The study results indicate that a suitable application regime with an adequate rest period is important in VFCWs to ensure efficient operation.

    We present a case of a fatal motor vehicle collision of a 22-year-old car driver, who died at the scene after losing control of his car, and subsequently collided with a tree by the roadside. The subsequent autopsy revealed penetrating injuries on the neck, severing the larynx, carotid artery, jugular vein and fracturing the cervical vertebrae. Internal examination showed the offending material to be a semi-cylindrical metal fragment, which had lodged within the paravertebral muscle. Examination of the car with the authorities found that the metal fragment originated from a defective airbag booster cannister, which shattered upon deployment.

    Analysis of data, recorded on March 8th 2014 at the Comprehensive Nuclear-Test-Ban Treaty Organisation's hydroacoustic stations off Cape Leeuwin Western Australia, and at Diego Garcia, reveal unique pressure signatures that could be associated with objects impacting at the sea surface, such as falling meteorites, or the missing Malaysian Aeroplane MH370. To examine the recorded signatures, we carried out experiments with spheres impacting at the surface of a water tank, where we observed almost identical pressure signature structures. While the pressure structure is unique to impacting objects, the evolution of the radiated acoustic waves carries information on the source. Employing acoustic-gravity wave theory we present an analytical inverse method to retrieve the impact time and location. The solution was validated using field observations of recent earthquakes, where we were able to calculate the eruption time and location to a satisfactory degree of accuracy. Moreover, numerical validations confirm an error below 0.02% for events at relatively large distances of over 1000 km. The method can be developed to calculate other essential properties such as impact duration and geometry. Besides impacting objects and earthquakes, the method could help in identifying the location of underwater explosions and landslides.

    This is a retrospective study to determine the distribution site, associated fracture and causes of mandibular fractures at HUSM, over a 5 year period, from 1st January 2002 - 31st December 2006. Records of patients who had mandibular fracture were reviewed. Data of age, sex, site of fracture, causes and associated fracture were recorded and analyzed using SPSS version 15.0. There were 113 (84.3%) males and 21 (15.7%) females. The mean age for male was 84.3% and female was 15.7%. The fracture occurs mostly at the age of 11-20 years (45.5%), followed by 21-30 years (30.6%). Motor vehicle accidents (MVA) were the commonest causes of mandibular fracture (92.5%), followed by fight and assault (3.7%), industrial accidents (3.0%), fall (0.7%). There were no cases recorded due to sport injury. The commonest site of mandibular fracture occurs at angle and para-symphysis (23%), followed by body (20.1%), symphysis (16.7%), condyle (15.5%) and ramus (1.7%). The most common associated fractures were head injury (23.5%), followed by clavicle fracture (17.2%) and fracture of radius (10.7%). Mandibular fracture was common in males with the mean age 24.63 years and mostly due to MVA. Angle and para-symphysis is the commonest site of mandibular fracture with most of the patient suffered from concomitant head injury.

    Concrete and masonry waste are the main types of waste typically generated at a construction project. There is a lack of studies in the country regarding the cost implication of managing these types of construction waste To address this need in Malaysia, the study is carried out to measure the disposal cost of concrete and masonry waste. The study was carried out by a site visit method using an indirect measurement approach to quantify the quantity of waste generated at the project. Based on the recorded number of trips for waste collection, the total expenditure to dispose the waste were derived in three construction stages. Data was collected four times a week for the period July 2014 to July 2015. The total waste generated at the study site was 762.51 m3 and the cost incurred for the 187 truck trips required to dispose the waste generated from the project site to the nearby landfill was RM22,440.00. The findings will be useful to both researchers and policy makers concerned with construction waste.

    This study had developed a passenger safety perception model specifically for buses taking into consideration the various factors, namely driver characteristics, environmental conditions, and bus characteristics using Bayesian Network. The behaviour of bus driver is observed through the bus motion profile, measured in longitudinal, lateral, and vertical accelerations. The road geometry is recorded using GPS and is computed with the aid of the Google map while the perceived bus safety is rated by the passengers in the bus in real time. A total of 13 variables were derived and used in the model development. The developed Bayesian Network model shows that the type of bus and the experience of the driver on the investigated route could have an influence on passenger's perception of their safety on buses. Road geometry is an indirect influencing factor through the driver's behavior. The findings of this model are useful for the authorities to structure an effective strategy to improve the level of perceived bus safety. A high level of bus safety will definitely boost passenger usage confidence which will subsequently increase ridership.

    Introduction: Fracture of the ulnar coronoid process is uncommon. It is commonly associated with posterior dislocation of the elbow, but it may also present as an isolated fracture. In general, all ulnar coronoid process fractures with elbow joint instability and large fracture fragments are fixed surgically. We report two cases of the rare isolated Regan-Morrey type III ulnar coronoid process fractures and their outcomes.

    Case Report: Case Report 1: Mrs P, a 27-year-old right-hand dominant female, was involved in a motor vehicle accident (MVA) and sustained an isolated Regan-Morrey type III ulnar coronoid process fracture. In view of the large coronoid process fragment causing elbow joint instability, she underwent an open reduction and internal fixation when the elbow swelling had subsided. An anterior approach was used to identify the fracture fragment and it was fixed with two half-threaded cancellous lag screws with washers to achieve an anatomical reduction. Postoperatively, she recovered with excellent outcome based on the Mayo elbow performance score (MEPS).Case Report 2: Mr M, a 23-year-old right-hand dominant gentleman, was involved in a MVA and sustained an isolated Regan-Morrey type III ulnar coronoid process fracture. During examination under general anesthesia, passive range of the movement of his right elbow was noted to be <90°due to the impaction of the fracture fragment. An open reduction through an anterior approach was performed and Kirschner wires were inserted to fix the coronoid process fracture. Kirschner wires were opted for the ease of post-operative removal as the patient was not keen to have a retained implant after recovery. He recovered with good outcome based on the MEPS.

    Conclusion: Isolated fracture of the ulnar coronoid process is rare. Open reduction and internal fixation is mandatory for patients with coronoid process fracture and unstable elbow joint to achieve good functional outcomes.

    A survey was carried out using a medical examination format that was prepared by the Malaysian Medical Association. The findings of the survey show that of the 266 cases surveyed, 64 drivers (24% of cases surveyed) are either totally unfit to drive or temporarily unfit to drive heavy goods and passenger vehicles. This is clear indication that the current format that is being used by the Road Transport Department is inadequate and needs to be reviewed. It must also be stressed that all the above 64 drivers have been certified fit using the existing Road Transport Department format and are currently driving in our highways and roads. Heavy vehicle goods and passenger vehicle drivers if not properly examined and medically certified are not only be endangering their own lives but also that of others. It is therefore recommended that based on the data available from this survey, the Road Transport Department should seriously consider adopting the medical examination format that was formalised by the Malaysian Medical Association and used in this survey.

    The topcoat color of motor vehicles offers vital information while investigating vehicular accidents, especially in instance of hit-and-run, since witnesses seldom perceive and retain the plate details. Differences in color perceptions among individuals with normal vision may lead to confusion in determining the color of the car involved. In this way, witnesses of crash accidents could potentially initiate flawed leads in forensic investigation, and thus affect the administration of justice. In this study, the inter-rater reliability of vehicle color determination by different volunteers was explored. Six individuals observed the topcoat colors of 500 stationary and 500 moving vehicles from five locations, employing a common system of color gradation. The outcome was binary: the vehicle color was either a "match" or "non-match". This was followed by statistical analysis in terms of the colors' frequencies and inter-rater reliability, based on which more suitable color descriptions were determined for subsequent comparisons of stationary and moving vehicles. Higher match frequencies and greater inter-rater reliability were observed when color gradations were disregarded. The frequency of correct matches could have been closely related to their relative on-the-road distribution, regardless of the statuses of observed vehicles. It was also found that black and white were associated with a greater number of matches than were intermediate colors, which should be carefully interpreted during forensic investigation to avoid wrong leads. In conclusion, the present study demonstrated the forensic significance of vehicle topcoat color determination, particularly in cases where witness statements are crucial.

    Structured lipid such as medium-and long-chain triacylglycerol (MLCT) is claimed to be able to suppress body fat accumulation and be used to manage obesity. Response surface methodology (RSM) with four factors and three levels (+1,0,-1) faced centered composite design (FCCD) was employed for optimization of the enzymatic interesterification conditions of palm-based MLCT (P-MLCT) production. The effect of the four variables namely: substrate ratio palm kernel oil: palm oil, PKO:PO (40:60-100:0 w/w), temperature (50-70 °C), reaction time (0.5-7.5 h) and enzyme load (5-15 % w/w) on the P-MLCT yield (%) and by products (%) produced were investigated. The responses were determined via acylglycerol composition obtained from high performance liquid chromatography. Well-fitted models were successfully established for both responses: P-MLCT yield (R (2) = 0.9979) and by-products (R (2) = 0.9892). The P-MLCT yield was significantly (P  0.05). Substrate ratio PKO: PO (100:0 w/w) gave the highest yield of P-MLCT (61 %). Nonetheless, substrate ratio of PKO: PO (90:10w/w) was chosen to improve the fatty acid composition of the P-MLCT. The optimized conditions for substrate ratio PKO: PO (90:10 w/w) was 7.26 h, 50 °C and 5 % (w/w) Lipozyme TLIM lipase, which managed to give 60 % yields of P-MLCT. Up scaled results in stirred tank batch reactor gave similar yields as lab scale. A 20 % increase in P-MLCT yield was obtained via RSM. The effect of enzymatic interesterification on the physicochemical properties of PKO:PO (90:10 w/w) were also studied. Thermoprofile showed that the P-MLCT oil melted below body temperature of 37 °C.

    Background. Syngnathid fishes produce three kinds of sounds, named click, growl and purr. These sounds are generated by different mechanisms to give a consistent signal pattern or signature which is believed to play a role in intraspecific and interspecific communication. Commonly known sounds are produced when the fish feeds (click, purr) or is under duress (growl). While there are more acoustic studies on seahorses, pipefishes have not received much attention. Here we document the differences in feeding click signals between three species of pipefishes and relate them to cranial morphology and kinesis, or the sound-producing mechanism. Methods. The feeding clicks of two species of freshwater pipefishes, Doryichthys martensii and Doryichthys deokhathoides and one species of estuarine pipefish, Syngnathoides biaculeatus, were recorded by a hydrophone in acoustic dampened tanks. The acoustic signals were analysed using time-scale distribution (or scalogram) based on wavelet transform. A detailed time-varying analysis of the spectral contents of the localized acoustic signal was obtained by jointly interpreting the oscillogram, scalogram and power spectrum. The heads of both Doryichthys species were prepared for microtomographical scans which were analysed using a 3D imaging software. Additionally, the cranial bones of all three species were examined using a clearing and double-staining method for histological studies. Results. The sound characteristics of the feeding click of the pipefish is species-specific, appearing to be dependent on three bones: the supraoccipital, 1st postcranial plate and 2nd postcranial plate. The sounds are generated when the head of the Dorichthyes pipefishes flexes backward during the feeding strike, as the supraoccipital slides backwards, striking and pushing the 1st postcranial plate against (and striking) the 2nd postcranial plate. In the Syngnathoides pipefish, in the absence of the 1st postcranial plate, the supraoccipital rubs against the 2nd postcranial plate twice as it is pulled backward and released on the return. Cranial morphology and kinesis produce acoustic signals consistent with the bone strikes that produce sharp energy spikes (discrete or merged), or stridulations between bones that produce repeated or multimodal sinusoidal waveforms. Discussion. The variable structure of the sound-producing mechanism explains the unique acoustic signatures of the three species of pipefish. The differences in cranial bone morphology, cranial kinesis and acoustic signatures among pipefishes (and seahorses) could be attributed to independent evolution within the Syngnathidae, which warrants further investigation.

    Dengue is a viral infection caused by four types of viruses (DENV-1, DENV- 2, DENV-3, DENV-4) and transmitted through the bite of infected Aedes aegypti and Aedes albopictus female mosquitoes that feed both indoors and outdoors during the daytime (from dawn to dusk). These mosquitoes thrive in areas with standing water, including puddles, water tanks, containers and old tyres. There were a total of 35 dengue outbreaks under PKK KK with four active outbreaks in the year 2017. One uncontrolled outbreak occurred at Ibu Pejabat Kontigen (IPK) Kota Kinabalu from 16 November 2017 to 21 December 2017.

    Isolated lower cranial nerve (CN) palsy affecting the CN X resulting from a skull base fracture is very rare. The clinical manifestation and natural history is related closely to the complex anatomy of this region and mechanism of injury. Here, we report a case of a 54 year-old man who presented with a delayed onset of dysphonia and dysphagia with aspiration following a closed head injury sustained from a motor vehicle accident. Injection laryngoplasty was implemented to alleviate symptoms of his CN X palsy, which eventually almost completely resolved. High index of suspicion should be maintained when investigating possible skull base fractures, especially with a suggestive clinical presentation of lower CN palsies affecting one or all the lower CNs. Delayed onset of these CN palsies are likely to have more favourable outcomes.

    History A 58-year-old woman was seen in the rheumatology clinic for bilateral wrist and knee pain that was unresponsive to physiotherapy and intra-articular steroid injections. Remote fracture of the left tibia from a motor vehicle collision was reported and was previously treated with conservative management. Serologic work-up for inflammatory disease was negative. The patient reported no prior surgical or medical history. Social history revealed remote immigration from Malaysia. Radiographs of the hands and knees were obtained ( Figs 1 - 4 ).     A retrospective cross-sectional study of injury patients who attended the Accident and Emergency Unit was conducted at the University of Malaya Medical Centre. Demographic data with regards to age, sex, race, occupation and the relevant data such as type of accident, type of injury sustained and also severity seen during the month of January 2005 were selected retrospectively from medical record. The objectives were to review road traffic accidents and their severity of injuries among the patients. There were two hundred and forty-three (197 males and 46 females) patients involved: 60% Malays, 16% Chinese, 21% Indians and 3% other ethnic groups. Peak accidents occurred among the 21 to 30 years age group. Among the type of accidents, 65.8% involved motorbike riders, 10.3% car drivers, 8.6% pillion riders, 7.8% car passengers, 5.8% pedestrians and 1.6% were bicycle rider injuries. About 57.6% sustained minor injuries and 42.4% major injuries. There was no significant association between outcome of road traffic accidents and sex, age group and race. However, there was a significant association between type of accidents and severity of injuries (p = 0.014). Males sustained more major injuries and motorbike accidents were the highest among road injuries. Therefore, further prevention and control with emphasis on behavioural changes, education and law enforcement may reduce the number of road traffic accidents in the future.

    Fatigue-related crashes have long been the topic of discussion and study worldwide. The relationship between fatigue-related crashes and time of day is well documented. In Malaysia, the possibility of banning express buses from operating during the early-hours of the morning has emerged as an important consideration for passenger safety. This paper highlights the findings of an impact assessment study. The study was conducted to determine all possible impacts prior to the government making any decision on the proposed banning. This study is an example of a simple and inexpensive approach that may influence future policy-making process. The impact assessment comprised two major steps. The first step involved profiling existing operation scenarios, gathering information on crashes involving public express buses and stakeholders' views. The second step involved a qualitative impact assessment analysis using all information gathered during the profiling stage to describe the possible impacts. Based on the assessment, the move to ban early-hour operations could possibly result in further negative impacts on the overall road safety agenda. These negative impacts may occur if the fundamental issues, such as driving and working hours, and the need for rest and sleep facilities for drivers, are not addressed. In addition, a safer and more accessible public transportation system as an alternative for those who choose to travel at night would be required. The proposed banning of early-hour operations is also not a feasible solution for sustainability of express bus operations in Malaysia, especially for those operating long journeys. The paper concludes by highlighting the need to design a more holistic approach for preventing fatigue-related crashes involving express buses in Malaysia.

    Transport of fuel is essential to ensure supplies are delivered as per requested by the industrial sites or other demands. Numerous accidents have been reported and recorded in which loss of containment of hazardous chemicals occurred and led to disastrous outcomes. This paper presents the analysis of Boiling Liquid Expanding Vapour Explosion (BLEVE) due to loss of containment for Liquefied Petroleum Gas (LPG) road tankers. The main objective of this paper is to evaluate the potential consequences resulting from overpressure blast and thermal radiation of tankers carrying LPG to the people and the surrounding. The aim is also to compare the outcomes obtained from PHAST software simulator 8.11 with that of established mathematical model. Malaysia North-south Expressway (NSE) was selected as the location of the incident. The volume, weather parameters and properties of LPG were identified. It was found that the effect of BLEVE on people and structures was catastrophic. The results obtained from the mathematical model were similar with that modelled using PHAST software simulator.

    Recent statistics from the Social Security Organization (SOCSO) of Malaysia shows Commuting Accident (CA)
    increased consistently by 1500 cases per year from 2008 to 2012. This has resulted in the rising of fatality rate,
    extensive claims of compensation payment and not to mention loss of valuable talents. However, little is known
    about the contributing factors to the occurrences of CA in Malaysia because CA is never considered to be work
    related before. This study aims to explore work related factors with occurrence of CA using 5-year statistical data
    from SOCSO, Malaysia. Moreover, we also reviewed studies published between 1990s and 2014 to support the
    statistical findings. Motorbike is found as the most common vehicle used which involved in CA. Individual factors
    concerning workers behaviour is a main risk factor of CA. Family related factors (parenting responsibility), work
    burden, workplace support as well as environmental factors such as bad weather and bad road conditions are also
    significant contributions of CA. It is very important to develop behavioural intervention strategies and provide proper
    training. Hence, more attention should be directed to young individual workers in balancing them with capabilities and
    organization performance demand. This may lead to the elimination of the other causes of CA. In conclusion, solutions
    to this problem involve not only a particular party to ensure the wellbeing of workers in Malaysia, but all authorities
    should play roles in enhancing safety and health matter of workers especially on the occurrence of accident.
