    Large doses of ionizing radiation can damage human tissues. Therefore, there is a need to investigate the radiation effects as well as identify effective and non-toxic radioprotectors. This study evaluated the radioprotective effects of Kelulut honey (KH) from stingless bee (Trigona sp.) on zebrafish (Danio rerio) embryos. Viable zebrafish embryos at 24 hpf were dechorionated and divided into four groups, namely untreated and non-irradiated, untreated and irradiated, KH pre-treatment and amifostine pre-treatment. The embryos were first treated with KH (8 mg/mL) or amifostine (4 mM) before irradiation at doses of 11 Gy to 20 Gy using gamma ray source, caesium-137 (137Cs). Lethality and abnormality analysis were performed on all of the embryos in the study. Immunohistochemistry assay was also performed using selected proteins, namely γ-H2AX and caspase-3, to investigate DNA damages and incidences of apoptosis. KH was found to reduce coagulation effects at up to 20 Gy in the lethality analysis. The embryos developed combinations of abnormality, namely microphthalmia (M), body curvature and microphthalmia (BM), body curvature with microphthalmia and microcephaly (BMC), microphthalmia and pericardial oedema (MO), pericardial oedema (O), microphthalmia with microcephaly and pericardial oedema (MCO) and all of the abnormalities (AA). There were more abnormalities developed from 24 to 72 h (h) post-irradiation in all groups. At 96 h post-irradiation, KH was identified to reduce body curvature effect in the irradiated embryos (up to 16 Gy). γ-H2AX and caspase-3 intensities in the embryos pre-treated with KH were also found to be lower than the untreated group at gamma irradiation doses of 11 Gy to 20 Gy and 11 Gy to 19 Gy, respectively. KH was proven to increase the survival rate of zebrafish embryos and exhibited protection against organ-specific abnormality. KH was also found to possess cellular protective mechanism by reducing DNA damage and apoptosis proteins expression.

    BACKGROUND: Traumatic brain injury (TBI) survivors require attention and dependence from their primary caregiver. This is because TBI is a defect that affects both the psychological and physical functions of the victim. Caregivers play an essential role in providing the adequate care victims need to adjust to the new problems they may experience due to their condition, as family members alone may not be able to provide for their needs. In particular, TBI caregivers may face specific challenges when assisting their patients in handling unexpected changes to their daily routines. Therefore, this quantitative study aims to explore the needs of caregivers, their coping mechanisms towards stressful and traumatic circumstances, and how they provide care to their loved ones during the COVID-19 pandemic.

    DESIGN AND METHODS: It was conducted with ten caregivers of individuals suffering from traumatic brain injury, that were selected using a theoretical sampling method. Data were obtained using a semi-structured interview guide, which helped the caregivers provide their responses. Meanwhile, data analysis was performed using the NVIVO analysis software.

    RESULTS: The results showed that there were, three significant themes namely, (a) Support needed, (b) the information need for care, and (c) developing self-resilience. The results also showed that caregivers really need support from the various parties, and the participants lack information on specific care techniques for the severe traumatic brain injury (TBI) survivors.

    CONCLUSION: In conclusion, caregivers require approval and seek more useful information to provide excellent care to their loved ones. Being aware of the caregiver's needs would enable them to offer improved customized care.

    Penyakit lemah jantung merupakan antara lima punca utama kematian penyakit kronik tidak berjangkit berdasarkan rujukan World Health Organization. Sekitar 20% pesakit kegagalan jantung di seluruh dunia memasuki wad semula dalam jangka masa 30 hari selepas discaj. Hal ini demikian biasanya akibat daripada tidak mematuhi penyekatan sukatan air diminum atau pun perkembangan penyakit. Kajian ini bertujuan untuk mengenalpasti kadar masuk wad dalam 30 hari selepas discaj daripada kegagalan jantung dan faktor berkaitan di Pusat Perubatan Universiti Kebangsaan Malaysia (PPUKM) pada tahun 2016-2017 dengan menggunakan data casemix. Berdasarkan data kod, perbelanjaan digunakan untuk masuk wad semula dalam 30 hari diambilkira. Kadar masuk wad untuk kegagalan jantung dalam 30 hari selepas discaj bagi punca yang sama adalah 53.5% dan 43.5% bagi tahun 2016 dan 2017, sama kadar dengan hospital mengajar yang lain tetapi lebih tinggi daripada kadar di negara maju. Selepas diselaraskan, hanya faktor umur pesakit dan tahap keterukan penyakit menunjukkan kaitan yang bermakna dengan kemasukan wad dalam 30 hari selepas discaj bagi kegagalan jantung. Jangka masa berada dalam wad dan perbelanjaan untuk kemasukkan wad adalah lebih rendah daripada yang sepatutnya dicapai. Sebanyak RM80,329.73 telah dijimatkan pada tahun 2017 atas rawatan yang pantas dan rancangan penjagaan yang komprehensif diberi kepada 19.3% pesakit gagal jantung tahap keterukan tiga. Penurunan kadar kemasukan wad semula dalam 30 hari selepas discaj juga menjimatkan kos pesakit dan menunjukkan penjagaan klinikal yang mantap dan strategi rawatan yang rapi. Kajian casemix ini boleh digunakan sebagai rujukan untuk penentuan bajet bagi unit kardiologi di PPUKM.

    Hypermature cataracts are commonly seen in developing countries. Spontaneous rupture of the anterior capsule, resulting in dislocation of the lens nucleus into the anterior chamber, presents rarely in hypermature cataracts. We describe a middle-aged woman who presented with spontaneous anterior dislocation of the nucleus in both eyes. The presence of calcification spots in the posterior capsule at the pupillary edge strongly suggested that our patient had hypermature cataracts. It is important to highlight this uncommon cause of nucleus dislocation in a patient with no previous history of ocular trauma.

    The current standard of managing facial bone fractures is the use of rigid internal fixation. This method provides good stabilization and repair for mid-face fractures such as the zygomatico-orbitalfracture. Nowadays, for the young and old, patients want not only rapid bone healing, but also good facial aesthetics after surgical treatment following maxillofacial trauma. Therefore, osteosynthesis of fractures and inconspicuous post-operative scars are considered essential. The lower eyelid approach or modified blepharoplasty provides rapid access to the infraorbital rim as well as the orbital floor, while the lateral eyebrow incision gives direct access for fixation at the fronto-zygomatic suture. Combining the Gillies' approach and a single form of rigid internal fixation, good cosmetic results and function can be achieved. Cases using the micro-plate-system for thin infraorbital bones and the miniplate- system for thicker facial bones involving zygomatico-orbital fractures are described.

    Health care workers (HCW) are constantly exposed to blood-borne illnesses through needle stick injuries (NSI). Despite the increasing trend of NSI, evidence regarding the actual practice of universal precautions among these HCWs is lacking. This study assessed the practice of universal precautions towards prevention of NSI among HCWs in a teaching hospital setting.
    Methods: This cross-sectional survey involved a newly-designed self-completed questionnaire assessing demographic data, exposure to NSI and practice of universal precautions. Questionnaires were distributed to every ward and completed questionnaires were collected after a period of 7 days.
    Results: A total of 215 HCWs responded to the survey. 35.8% were exposed to bodily fluid, with 22.3% had NSI in the last 12 months. Blood taking was the commonest procedure associated with NSI. Of practices of universal precautions, recapping needle and removing needle from syringe were still wrongly practiced by the HCWs assessed.
    Conclusion: NSI among HCW are still common despite the introduction of universal precautions in our hospital. Incorrect practices in handling sharps should be looked into in order to reduce the incidence of blood-borne illnesses through NSI in the hospital.

    Introduction: Interest in mindfulness and its enhancements have quietly exploded in recent years, bringing with it the need for validated instruments to assess mindfulness in the Malaysian population. The study aims to assess the reliability, factor structure and validity of the Malay version of The Mindful Attention Awareness Scale (MMAAS) in a group of nursing students. Methods: The MMAAS was ‘forward-backward’ translated from English to Malay. Two hundred and sixty six nursing students answered the MMAAS. At the same time, they responded to the General Health Questionnaire (GHQ) and Rosenberg Self-Esteem Scale (RSES). We performed Exploratory Factor Analysis (EFA) with varimaxrotation to examine the factor structure of the MMAAS. Associations of retained factors were estimated by Spearman correlation coefficients. Results: Internal consistency reliability of MMAAS was good (Cronbach’s α = 0.851) and showed temporal stability in a 3-week period. Exploratory Factor Analysis (EFA) suggested three factors labelled as “Attention related to generaldomain”, “Attention related to the physicaldomain” and “Attention related to psychological domain”.     Background: The occupationally acquired accident and injuries in Malaysian medical laboratories are still largely unexplored prior to this survey. Some of these questions are attempted in this survey and act as source of reference for the number and accident injuries in medical laboratories in the area of Klang Valley and also in Malaysia.
    Methods : This survey was carried out based on recordable cases throughout the calendar year of 2001 to 2005 from 3 main medical laboratories of Hospital Kuala Lumpur (HKL), Hospital Universiti Kebangsaan Malaysia (HUKM) and Pusat Perubatan Universiti Malaya (PPUM).
    Results : The average annual incident rate for this three medical laboratories is 2.05/100 full time equivalent (FTE) employees. The annual incident rate in individual medical laboratory is 2.04/100 FTE (HKL), 2.07/100 FTE (HUKM) and 2.04/100 FTE (PPUM) employees, respectively. The most common injury that is 25.3% of the total cases reported was due to cuts by sharp objects and the second most common injury was exposure to biohazard and chemical substances which constitutes 19.9% of the total cases. . Needle prick injury (16.8%), fire (8.4%), fall/slip (6.3%) and gases leak and locked in cold room were reported as one case each.
    Conclusion : The average incident rate from this study is remarkably similar compared with the incident injury rate reported by BLS (2006) which is 2.1/100 FTE in the average size of medical laboratory and diagnostic. Besides this incident rate of injury and illness as a comparison, it also can be used as a benchmark to evaluate the safety performance among medical laboratories in Malaysia.

    Int roduction: Treatment of calcaneal fracture is still controversial and indication for surgery is not well
    established. We are reporting the mid term outcome of calcanel fractures treated conservatively. Material and Methods: Patients admitted with calcaneal fractures from 1st November 2002 till 31st December 2004 and were treated conservatively were included in this study. The fractures were grouped according to Essex-Lopresti classification and their outcomes were assessed with the Maryland foot score. We also looked at time to weight bearing and returning to occupational activity. Results: Forty-four patients were included for evaluation. Patients with extraarticular calcaneal fractures had significantly higher rating scores compared to those with intraarticular fractures (98.2 and 88.8 respectively, with a p value = 0.0001). Generally, both group of patients had a good clinical outcome. 18 of the 44 patients (41%) started partial weight bearing before or at 6 weeks and 31 patients (72%) were able to start full weight bearing by 12 weeks. 31 patients (72%) were back to work 12 weeks after the injury. Conclusion: Conservative management for calcaneal fractures is an acceptable mode of treatment with satisfactory.

    Intracranial nail gun injury is a rare subset of penetrating head injury. Here we report a case of intracranial nail gun injury in a Vietnamese patient who attempted suicide with no neurological deficit. Three nails were launched. Because the nail head acted as a brake, the launched nail could make a hole into the skull but could not entirely pass it. A rational management strategy should permit these patients to be discharged with no additional injury. Some medical and surgical management in penetrating head injury are discussed. The use of antibiotics and antiepileptic drugs and the retraction of the nail aided by the performance of a craniotomy surrounding the entry point are recommended.

    We are all aware that there has been a dramatic increase in the number of anterior cruciate ligament (ACL) reconstructions that are carried out here in Malaysia as well as around the world. The numbers of ACL injuries have undoubtedly increased over the years with greater participation of young adults in sporting activities. However it is not certain whether the increase in the numbers of reconstructions can be accounted for by the increasing numbers of ACL injuries. Without doubt commercial interests as well the influence of the biomedical companies have a role to play. In the past the rationale for surgical treatment of an ACL tear was that the ACL is vital for knee function and that in the long term ACL deficiency will lead to more injuries of the meniscus and more degeneration of the joint. This belief was prevalent because the natural history of an ACL deficient knee and the ultimate outcome of reconstruction of the ACL were both not known. However in recent years a substantial amount of research has been published, which has elucidated the natural history of ACL deficient knees as well as the long term outcome of reconstruction of the ACL.

    Needle Stick Injury is injuries caused by suture needle and hollow bore needle and does not include injuries via scalpel blades, lancet and glass pieces or by other means for the purpose of this training program. This problem needs to be prevented or minimized by embarking on an effective training program. The applicability of this program will be determined by the response of the participants to the contents and teaching methods which will in future effect the long term out come and the success of the Needle Stick Injury prevention and Universal Precautions training program. This Health Education program was designed to be used for the induction training of new workers as well as those already in service and is applicable to all levels of Health Care Workers especially those directly dealing with or handling syringes and needles. This Needle Stick Injury and Universal Precautions Training Program was carried out in one of the health facility under the Ministry of Health in Sabah and the outcome evaluation and analysis using paired t-test has shown a significant difference in their effectiveness in increasing the knowledge of the Health Care Workers after being exposed to the program (p < 0.01). The process evaluation was able to show that this approach was instrumental in creating awareness among the Health Care Workers who had attended this program. From the study, 98% (49) had not attended any formal training since joining the service, followed by 90% who had given a good overall rating for the program. For the lecture and its relevance, 90% and 74% for the hands–on training. About 77% of the participant was satisfied with the discussion. The program can be developed further and with it a comprehensive training module can be produced. The long-term benefits of such a training program will need to be evaluated in a future study.

    Introduction: Splenic pseudo-aneurysm is an uncommon complication following splenic injury caused by blunt abdominal trauma. Pseudo-aneurysm is usually treated with splenic arterial embolisation. Spontaneous thrombosis of a splenic pseudo-aneurysm can occur but it is rare. Method: We report a case of spontaneous thrombosis in a post-traumatic splenic pseudo-aneurysm. The patient was managed conservatively as he was haemodynamically stable with a repeat CT scan on the 7th day showing a tubular enhancing structure in the upper pole of the spleen demonstrating the same attenuation value .
    Conclusion: This case illustrates a spontaneous resolution of splenic pseudo-aneurysm, which is a rare outcome of the condition.

    The aim of this research is to study how the physical changes in the maxillofacial trauma patients affect them psychologically in patients of different ages, sexes, races, socioeconomic backgrounds, types and severity of injury. The study was conducted by doing a questionnaire survey, which was divided into physical and psychological components. In the physical component, the questions were mainly related to the physical injuries to the patient, which could be visualized clinically and functionally. The psychological component consisted of analyzing the impact of the maxillofacial trauma to the psychological profile. Twenty patients of both sexes and ages, ranged from 15 to 62 years old were interviewed. The main cause of trauma was from motor vehicle accidents. This study appears to show that maxillofacial trauma may have a psychological impact on patients.

    The incidence of pseudoaneurysm after total hip arthroplasty is extremely rare. The most common mechanism of vascular injury is due to direct trauma during the operative procedure and most reported cases are acute in a presentation. We report an unusual case of pseudoaneurysm of the external iliac artery, presenting 6 years after initial surgery, and occurring following septic loosening of total hip arthroplasty. This case highlights the importance of prompt recognition of infection of total hip arthroplasty, and also indicates the need for prompt and appropriate treatment for these infections.

    Thyroid enlargement is one of the common surgical presentations in the Department of Surgery, Hospital Universiti Sains Malaysia (HUSM). Among them, benign non-toxic multinodular goiter constitutes one third (30%) of patients who underwent thyroidectomy. Common complications of thyroidectomy include recurrent laryngeal nerve (RLN) injury, hypocalcaemia, and recurrence of the thyroid lesion. Objective & Methods: This is a retrospective study of patients diagnosed with multinodular goiter treated in HUSM between January 1996 and December 2005. A total of 111 patients were studied and 52 of them underwent subtotal thyroidectomy while 59 underwent total thyroidectomy. The outcome in terms of RLN injury, hypocalcaemia and mass recurrence were analyzed. Results: Post operative complications were studied in both groups. Permanent recurrent laryngeal nerve injury occurs in 2.4% (1 case) in subtotal thyroidectomy group compared to total thyroidectomy group (3.6%, 2 cases). Five cases from total thyroidectomy group suffered from permanent hypocalcaemia but none in the other group. 70.7% (29 cases) from subtotal thyroidectomy group have functional remnant of thyroid tissue. Recurrence rate post subtotal thyroidectomy after 5 years is only 4.9% (2 cases). Conclusion: The post operative outcome in patients who underwent subtotal thyroidectomy in HUSM from January 1996 to
    December 2005 was better than total thyroidectomy with significant functional thyroid remnant.

    Traumatic brain injury (TBI) is responsible for causing global deaths exceeding 1.27 million per year. Various aspects of traumatic brain injuries have been studied worldwide in order to reduce the mortality and morbidity statistics. One such strategy has been to manage these patients at the scene of the accident itself. This pre hospital strategy has been shown to reduce the mortality in severely head injured patients. The pre hospital “team” consists of paramedics, nursing personnel and occasionally clinicians who are trained in resuscitation as well as managing initially the traumatic brain injury. Though definitive treatment for TBI is started in the intensive care at the trauma unit, primary management of these patients at the accident site itself has its advantages. This article reviews the current practices, pros and cons and the future directions in pre hospital care for TBI.

    Pseudohyperkalemia is a spuriously high serum potassium measurement in a patient with no clinical evidence of hyperkalemia. It has been reported to occur in patients with leukocytosis and thrombocytosis. Only a few cases of pseudohyperkalemia have been reported in patients after splenectomy. Two cases of pseudohyperkalemia occurring after emergency splenectomy for abdominal trauma are presented to highlight their clinical presentation and sequalae. Consecutive patients who underwent emergency splenectomy for trauma and subsequently developed pseudohyperkalemia were monitored in Universiti Kebangsaan Malaysia Medical Centre for their clinical sequalae. Both the patients developed leukocytosis, thrombocytosis and high serum potassium level within 2-weeks of their splenectomies. They did not demonstrate any symptoms and signs of hyperkalemia. Their conditions resolved spontaneously without any specific treatment. Pseudohyperkalemia should be suspected in a similar clinical scenario to avoid unnecessary treatment that could lead to severe hypokalemia.

    Penetrating injuries to bladder occur in 20 % of cases. Synchronous bladder and rectal perforation occur in 30-64 % of cases. The management of rectal and bladder injuries depend on whether it is an extra-peritoneal or intra-peritoneal injury. We hereby, report a case of penetrating trauma in a 13 year old boy who fell off a tropical fruit (Rambutan - Nephelium lappaceum) tree. He sustained an extra-peritoneal rectal injury with intra-peritoneal bladder injury. The rectal injury was repaired primarily via per anal route while the bladder injury needed an open repair following laparotomy. Upon removal of bladder clots, a leaf of the ‘Rambutan’ tree was found intra-vesically. It was removed and bladder repaired as per standard method. We review the literature on rare intra-vesicle foreign bodies and discuss the treatment of synchronous rectal and bladder injuries.

    There is a high incidence of 36% to 83% of respiratory dysfunction in patients with acute cervical spinal cord
    injury. Complications arising from respiratory dysfunction remain one of the most common causes of morbidity
    and mortality in the spinal cord injured population. Specialized pulmonary care and therapy can help individuals
    with tetraplegia to maintain a stable respiratory status allowing active participation in active rehabilitation.
    This would allow them to achieve rehabilitation goals of independent function and community reintegration.
    Mechanical insufflation-exsufflation (MIE) therapy is an option for secretion management in individuals with
    acute spinal cord injury. In this paper, we present our experience using MIE as an adjunct to management of
    secretions in the spinal cord injured population at the University of Malaya Medical Centre.
