    Ustekinumab adalah agen biologi yang berkesan untuk rawatan kulit psoriasis dengan menyasarkan molekul interleukin 12 dan 23. Namun begitu ada beberapa halangan ketika penggunaannya seperti kegagalan sekunder dan juga peristiwa paradoks. Penyakit juga didapati kembali menyerang selepas terapi dihentikan. Kajian kohort retrospektif telah dilakukan ke atas pesakit kulit kronik psoriasis yang telah menerima ustekinumab di antara tahun 2013 ke 2018 di Pusat Perubatan Universiti Kebangsaan Malaysia (PPUKM). Demografi, ciri-ciri klinikal, keberkesanan rawatan, komplikasi, kadar dan corak penyakit berulang dikaji melalui rekod-rekod pesakit. Enam (75%) pesakit adalah lelaki. Enam (75%) pesakit ini tidak pernah menerima agen biologi sebelum ini. Usia median pesakit adalah 41.5 tahun (IQR 26.8-48.3), tempoh median penyakit ialah 16.5 tahun (IQR 6.5- 23.0). Tempoh median untuk mencapai kadar 75% pengurangan penyakit (PAS I75) adalah 16 minggu. PASI75 pada minggu ke 12 dicapai oleh 37.5% pesakit manakala tempoh median untuk mencapai sekurang-kurangnya PASI75 adalah 16 minggu. Tempoh median keseluruhan rawatan adalah selama 102 minggu. Semua pesakit berjaya dirawat. Dua (25%) pesakit mengalami kegagalan sekunder. Seorang (12.5%) pesakit mendapat peristiwa paradoks dengan tumbuhnya fenomena nanah dan plak di kulit semasa rawatan. Tempoh median untuk penyakit berulang adalah 40 minggu. Semua pesakit kecuali seorang memerlukan rawatan biologi untuk penyakit berulang. Ustekinumab berkesan untuk semua pesakit. Kejayaan rawatan diperolehi 4 minggu selepas piawai yang dijangkakan. Kohort ini telah merekodkan kegagalan sekunder dan fenomena paradoks yang jarang berlaku. Penyakit didapati akan berulang dan corak psoriasis yang berbeza ditemui selepas rawatan dihentikan.

    Tuberkulosis (TB) paru-paru dan penyakit “Chronic Obstructive Pulmonary Disease” (COPD) adalah punca utama kematian dan morbiditi di Malaysia. Di Malaysia, jumlah kes TB semakin meningkat dan terdapat 500,000 rakyat Malaysia yang didiagnosa dengan COPD pada tahun 2016. Sejumlah besar pesakit TB mendapat “COPD” pada jangka panjang. Jangkitan TB yang lepas telah terbukti menyumbang kepada etiologi COPD. Kami menjalankan kajian keratan rentas selama satu tahun yang melibatkan pesakit dewasa di klinik pesakit luar di Pusat Perubatan Universiti Kebangsaan Malaysia (UKMMC) yang mempunyai sejarah TB paru-paru 3 tahun sebelum kajian.     Ovitrap autosidal berperanan penting dalam pemantauan dan kawalan vektor denggi, Aedes aegypti and Aedes albopictus. Beberapa jenis ovitrap ini telah digunakan di Malaysia termasuklah Mosquito Larvae Trapping Device (MLTD) yang diperkenalkan oleh Dewan Bandaraya Kuala Lumpur. Kajian ini bertujuan untuk menilai kepadatan larva Aedes dan pembolehubah meteorologi setempat di sebuah penempatan pelajar di Cheras, Kuala Lumpur. MLTD (n=30) dengan umpan larutan baja NPK (5:5:5) organik telah digunakan. Pengumpulan larva dan penyelenggaraan ovitrap telah dilakukan setiap minggu selama lapan minggu. Pemeliharaan larva dan penentuan spesies telah dijalankan di makmal. Daripada sejumlah 2,152 larva Aedes, 85% merupakan Ae. albopictus manakala selebihnya Ae. aegypti. Ae. albopictus lebih banyak dikumpulkan di luar berbanding di dalam bangunan (purata larva per ovitrap, 9.28 lawan 6.08). Sebaliknya, Ae. aegypti lebih banyak dikumpulkan di dalam berbanding di luar bangunan (1.72 lawan 0.86). Ovitrap indeks yang paling tinggi adalah semasa minggu kelima (90%) dan keenam (93%). Minggu pertama menunjukkan ovitrap indeks yang paling rendah (30%). Analisis mendapati korelasi positif antara kepadatan larva Aedes dan suhu maksimum (r=0.830, p=0.011), manakala kelembapan relatif minimum menunjukkan korelasi negatif (r=-0.778, p=0.023). Pemantauan rutin bagi peringkat pramatang vektor denggi boleh memberikan maklumat mengenai kepadatan dan naik turun populasi nyamuk setempat. Apabila digandingkan dengan faktor meteorologi sejajar, maklumat ini dapat memberi panduan bagi perlaksanaan kawalan vektor sebagai sebahagian strategi pencegahan denggi.

    Dalam situasi wabak COVID-19 ketika ini, maklumat yang tidak disahkan telah membanjiri platform media sosial, lalu meningkatkan tahap kegelisahan dan gejala kemurungan di kalangan orang ramai. Penyelidikan ini bertujuan untuk menilai hubungan antara pengetahuan, sikap, dan amalan (KAP) yang berkaitan dengan COVID-19 dan kesan psikologinya, seperti kegelisahan dan gejala kemurungan pada rakyat Malaysia. Tinjauan dalam talian telah dilaksanakan dari 8-22 April 2020, yang melibatkan 227 responden. Set soal selidik tersebut merangkumi ciri-ciri sosiodemografi, soal selidik KAP - 11 soalan mengenai pengetahuan (K1-K11), 3 soalan mengenai sikap (A1-A3), 4 soalan mengenai amalan (P1-P4), bersama dengan Soal Selidik ‘Patient Health Questionnaire (PHQ-9)’ dan ‘Generalized Anxiety Disorder (GAD-7) Scale’. Untuk mentafsir hasil penyelidikan, analisis deskriptif, ujian chi-square dan analisis teori respons barang telah dilaksanakan. Peratusan responden yang menjawab soalan penyataan benar dari komponen Pengetahuan (K1-K3) dengan betul adalah hampir 100%. Walau bagaimanapun, item yang menguji maklumat palsu COVID-19 (K4-K11) berkisar dari 38.8% hingga 95.2%. Satu-satunya item dari komponen Sikap yang kurang dari 95% dijawab dengan betul adalah A1 (78.9%), mengenai penggunaan topeng muka. Dari komponen Latihan, lebih daripada 80% responden mengenal pasti bahawa mereka menggunakan media sosial untuk mendapatkan kemas kini mengenai COVID-19 sepanjang masa. Ini merupakan sesuatu yang membimbangkan kerana kebanyakan maklumat yang beredar di media sosial belum disahkan oleh pihak berwajib, yang mungkin menyebabkan, dan terus-menerus memperkuat maklumat palsu yang menimbulkan kegelisahan. Kajian ini menunjukkan bahawa skor KAP keseluruhan yang lebih rendah, terutamanya untuk item komponen Pengetahuan, berkaitan langsung dengan tahap kegelisahan dan kemurungan yang lebih tinggi.

    Pengetahuan yang baik tentang teknik perahan, penyimpanan dan penggunaan susu perahan ibu adalah sangat penting kepada ibu-ibu yang ingin meneruskan penyusuan ibu selepas kembali bekerja. Objektif kajian ini adalah untuk menilai pengetahuan dan sikap ibu bekerja terhadap perahan, penyimpanan dan penggunaan susu ibu. Kajian hirisan lintang ini disertai oleh 300 ibu bekerja sepenuh masa yang melahirkan bayi di sebuah hospital tertiari di Kuala Lumpur, Malaysia. Kajian ini menggunakan soal selidik yang mengandungi 28 soalan menguji pengetahuan dan 9 soalan menilai sikap yang telah disahkan kandungannya. Skor tertinggi adalah 28 untuk pengetahuan dan 45 untuk sikap. Skor purata untuk pengetahuan adalah 20.47 (SD 4.06). Ibu yang mendapat skor ≥21 (≥75% skor maksimum) dikategorikan sebagai mempunyai “pengetahuan yang baik” manakala mereka yang mendapat skor

    Soal-selidik Kecelaruan Keresahan Menyeluruh-7 (GAD-7) versi Bahasa Melayu telah menunjukkan kesahan bersamaan, iaitu kepekaan dan kekhususan yang tinggi sebagai alat saringan untuk keresahan. Namun, ciri-ciri psikometriknya dari segi kesahan faktor belum lagi dikaji dengan lebih lanjut. Kajian ini memeriksa struktur faktor GAD-7 versi Bahasa Melayu di antara 300 orang pesakit luar kencing manis (purata umur: 60.4 tahun, sisihan piawai: 13.4 tahun; 52.7% lelaki) di sebuah hospital universiti di Kuala Lumpur, Malaysia.     Objective. Our innovation was to propose a multilevel model to explain how an organizational factor, psychosocial safety climate (PSC) - the climate for worker psychological health - related to work investment (work engagement and workaholism) and, in turn, psychological distress. Methods. Longitudinal data were collected in Peninsular Malaysia across 26 police departments from 392 police personnel, matched across 4 months, and were tested using hierarchical linear modeling. Results. The analysis revealed between-group effects linking PSC to job resources, to work engagement and to workaholism. When PSC operated by improving job resources, aside from increased work engagement, it could unwittingly boost workaholism. However, this only existed under low PSC conditions. The secondary function of PSC buffered the impact of job resources on workaholism and psychological distress. When PSC was high, job resources reduced both workaholism and psychological distress, suggesting that PSC enabled resources to do their job of mitigating unfavorable conditions. Conclusions. Results support a multilevel PSC-extended job demands-resources motivational path with cross-links, and PSC's moderation function, as an explanation of worker psychological health. Confirming PSC as a leading indicator and the importance of a motivational path, this article presents new evidence in support of targeting PSC to improve worker psychological health.

    The objective of this study is to identify the relationship between task characteristics (variety of skills, task identity, task significance, autonomy, feedback) and employee engagement. The study was conducted in a healthcare service in Sabah, Malaysia. A self administered questionnaire was used to collect data by using convenience sampling. A total of 78 respondents participated in the study. Data was analyzed using Spearman Correlation. The results revealed a significant and positive relationship between task characteristics (variety of skills, task identity, task significance, autonomy, feedback) and employee engagement in the studied organization. This has important implications to employers. It is recommended that managers, human resource development (HRD) practitioners and supervisors provide employees with more challenging tasks, autonomy and feedback from time to time. Moreover, work designs should include how the tasks may improve employee identity and significance.

    Sports-related ocular traumas may be rare, but can have devastating and disabling consequences. The causes of eye-related injuries depend on the type of sports popular in a particular area or country. Badminton is a popular sport played by all age groups and socioeconomic segments and is popular in many parts of the world. It is most popular in South-East Asia, accounting for two-thirds of all ocular sports injury in Malaysia. In India, badminton has become quite popular in recent years. Shuttlecocks in badminton have been shown to be responsible for a high number of outpatient ocular sports-related, mostly blunt injuries. We report an unusual case of penetrating ocular injury due to a feather shuttlecock and its surgical management.

    BACKGROUND: Resistance to colistin, the last line therapy for infections caused by multidrug-resistant Gram-negative bacteria, represents a major public health threat. Citrobacter freundii B9-C2 which was isolated from the stool of preterm neonate on the first week of life, displayed resistance to almost all major antibiotics, including colistin. Through whole genome sequencing (WGS), we characterised the genome features that underline the antibiotic-resistance phenotype of this isolate.

    METHODS: Genome of C. freundii B9-C2 was sequenced on an Illumina MiSeq platform. The assembled genome was annotated and deposited into GenBank under the accession number CP027849.

    RESULTS: Multiple antimicrobial resistance genes including blaCMY-66 were identified. Further, the presence of 15 antibiotic efflux pump-encoding resistance genes, including crp, baeR, hns, patA, emrB, msbA, acrA, acrB, emrR, mdtC, mdtB, mdtG, kdpE, mdfA and msrB, were detected and likely to account for the observed cephalosporins, carbapenems, aminoglycosides and monobactams resistance in C. freundii B9-C2. The isolate also presented unique virulence genes related to biofilm formation, motility and iron uptake. The genome was compared to publicly available genomes and it was closely related to strains with environmental origins.

    CONCLUSION: To the best of our knowledge, this is the first report of intestinal carriage of colistin-resistant C. freundii from the stool of a neonate in Malaysia. Using genomic analysis, we have contributed to the understanding of the potential mechanism of resistance and the phylogenetic relationship of the isolates with draft genomes available in the public domain.

    Rivers in Malaysia are classified based on water quality index (WQI) that comprises of six parameters, namely, ammoniacal nitrogen (AN), biochemical oxygen demand (BOD), chemical oxygen demand (COD), dissolved oxygen (DO), pH, and suspended solids (SS). Due to its tropical climate, the impact of seasonal monsoons on river quality is significant, with the increased occurrence of extreme precipitation events; however, there has been little discussion on the application of artificial intelligence models for monsoonal river classification. In light of these, this study had applied artificial neural network (ANN) and support vector machine (SVM) models for monsoonal (dry and wet seasons) river classification using three of the water quality parameters to minimise the cost of river monitoring and associated errors in WQI computation. A structured trial-and-error approach was applied on input parameter selection and hyperparameter optimisation for both models. Accuracy, sensitivity, and precision were selected as the performance criteria. For dry season, BOD-DO-pH was selected as the optimum input combination by both ANN and SVM models, with testing accuracy of 88.7% and 82.1%, respectively. As for wet season, the optimum input combinations of ANN and SVM models were BOD-pH-SS and BOD-DO-pH with testing accuracy of 89.5% and 88.0%, respectively. As a result, both optimised ANN and SVM models have proven their prediction capacities for river classification, which may be deployed as effective and reliable tools in tropical regions. Notably, better learning and higher capacity of the ANN model for dataset characteristics extraction generated better predictability and generalisability than SVM model under imbalanced dataset.

    This article proposes a novel data selection technique called the mixed peak-over-threshold-block-maxima (POT-BM) approach for modeling unhealthy air pollution events. The POT technique is employed to obtain a group of blocks containing data points satisfying extreme-event criteria that are greater than a particular threshold u. The selected groups are defined as POT blocks. In parallel with that, a declustering technique is used to overcome the problem of dependency behaviors that occurs among adjacent POT blocks. Finally, the BM concept is integrated to determine the maximum data points for each POT block. Results show that the extreme data points determined by the mixed POT-BM approach satisfy the independent properties of extreme events, with satisfactory fitted model precision results. Overall, this study concludes that the mixed POT-BM approach provides a balanced tradeoff between bias and variance in the statistical modeling of extreme-value events. A case study was conducted by modeling an extreme event based on unhealthy air pollution events with a threshold u > 100 in Klang, Malaysia.

    Leaf litter and its breakdown products represent an important input of organic matter and nutrients to mangrove sediments and adjacent coastal ecosystems. It is commonly assumed that old-grown stands with mature trees contribute more to the permanent sediment organic matter pool than younger stands. However, neither are interspecific differences in leaf decay rates taken into account in this assumption nor is our understanding of the underlying mechanisms or drivers of differences in leaf chemistry sufficient. This study examines the influence of different plant species and ontogenetic stage on the microbial decay of mangrove leaf litter. A litterbag experiment was conducted in the Matang Mangrove Forest Reserve, Malaysia, to monitor leaf litter mass loss, and changes in leaf litter chemistry and microbial enzyme activity. Four mangrove species of different morphologies were selected, namely the trees Rhizophora apiculata and Bruguiera parviflora, the fern Acrostichum aureum and the shrub Acanthus ilicifolius. Decay rates of mangrove leaf litter decreased from A. ilicifolius to R. apiculata to B. parviflora to A. aureum. Leaf litter mass, total phenolic content, protein precipitation capacity and phenol oxidase activity were found to decline rapidly during the early stage of decay. Leaf litter from immature plants differed from that of mature plants in total phenolic content, phenolic signature, protein precipitating capacity and protease activity. For R. apiculata, but not of the other species, leaf litter from immature plants decayed faster than the litter of mature plants. The findings of this study advance our understanding of the organic matter dynamics in mangrove stands of different compositions and ages and will, thus, prove useful in mangrove forest management.

    INTRODUCTION: Biosimilars are a cost-effective alternative to original biologic medicines that allow patients access to biologic therapies for various chronic diseases. Our paper aims to provide an overview of biosimilars in Malaysia with emphasis on the comparison of Malaysian guidelines with guidelines from well-established regulatory agencies, a review of biosimilars' market approval and their reported adverse effects (AEs) as well as clinical trials conducted in Malaysia.

    METHODS: We searched the official websites of the National Pharmaceutical Regulatory Agency (NPRA) Malaysia and three other well-established agencies, online databases of Medline® and EMBASE for guidelines on legislation and regulations of biosimilars. Meanwhile, we extracted the reports of AEs involving biosimilars in Malaysia from the NPRA database and for global AEs from the World Health Organisation VigyLize database. The ClinicalTrials.gov Website by the U.S. National Library of Medicines was the source for data on clinical trials.

    RESULTS: Malaysia followed the principles of the European Medicines Agency biosimilar regulations and issued their guideline in 2008. Since then, NPRA has approved 24 biosimilar products and recorded 499 AE reports, of which 43 (8.6%) were serious. NPRA has also approved ten Phase III clinical trials in Malaysia with four trials still ongoing.

    CONCLUSION: Malaysia follows a stringent regulatory pathway for the approval of biosimilars enacted by well-established regulatory agencies to maintain the quality, efficacy and safety of biosimilars. Introducing biosimilars to the Malaysian market would improve patients' accessibility to biologic therapies.

    The determinant factors for macroinvertebrate assemblages in river ecosystems are varied and are unique and specific to the type of macroinvertebrate family. This study aims to assess the determinant factors for macroinvertebrate assemblages in a recreational river. The study was conducted on the Ulu Bendul River, Negeri Sembilan, Malaysia. A total of ten sampling stations were selected. The research methodology included (1) water quality measurement, (2) habitat characterization, and (3) macroinvertebrate identification and distribution analysis. The statistical analysis used in this study was canonical correspondence analysis (CCA) to represent the relationship between the environmental factors and macroinvertebrate assemblages in the recreational river. This study found that most of the families of macroinvertebrates were very dependent on the temperature, DO, NH3-N, type of riverbed, etc. All of these factors are important for the survival of the particular type of macroinvertebrate, plus they are also important for selecting egg-laying areas and providing suitable conditions for the larvae to grow. This study advises that improved landscape design for watershed management be implemented in order to enhance water quality and physical habitats, and hence the protection and recovery of the macroinvertebrate biodiversity.

    Meal and snack patterns are associated with energy and nutrient intakes and consequently health and nutritional status. The aim of this paper is to describe the percentage of daily energy intake from meals and afternoon snack among Malaysian adults. The study included a representative sample of adults aged 18-59 years (n=7349) from a nationwide Food Consumption Survey conducted by the Ministry of Health. Information on dietary intake was obtained using a one day 24-hour diet recall (24-HDR). Dietary data on 6886 adults were analysed using Nutritionist ProTM and statistical analysis was carried out using the SPSS 13.0. The median percentage of daily energy intake is reported only for adults consuming meals and afternoon tea and by socio-demographic characteristics as well as body mass index (BMI) status. More than 80% of Malaysian adults consumed morning meals, lunch and dinner and 54% reported having afternoon tea. The median percentage of energy intake from morning meals, lunch, dinner and afternoon tea was 29.9%, 30.5%, 32.4% and 17%, respectively. There were variations in the median percentage of energy from meals and snacks according to the socio-demographic variables and BMI status. It is important to understand the eating patterns of Malaysians as the information can assist in efforts to address obesity and diet-related chronic diseases among adults.

    This study reports the food consumption patterns of adults aged 18 to 59 years in the Malaysian Adults Nutrition Survey (MANS) carried out between October 2002 and December 2003. A total of 6,742 subjects comprising 3,274 men and 3,468 women representing the northern, central , southern and east coast of Peninsular Malaysia as well as Sabah and Sarawak were interviewed. A semiquantitative food frequency questionnaire (FFQ) which consisted of 126 food items was used to evaluate the food consumption pattern (habitual food intake) of the respondents during the previous one- year period. The results demonstrate that nasi putih (cooked rice) was consumed by 97% of the population twice daily (average 2½ plates per day). Other food items consumed daily were marine fish, (one medium fish per day), green leafy vegetables (one cup per day) and sweetened condensed milk (three teaspoons per day. The mean frequencies for daily intake of rice, leafy vegetables, marine fish, local kuih, anchovy (ikan bilis) and biscuits were significantly higher among the rural compared to the urban adults. In contrast, more urban dwellers consumed chicken and eggs more frequently than their rural counterparts. More men than women consumed chicken and eggs more frequently. Malaysian adults showed a satisfactory habit of drinking plain water, with 99% drinking at least six glasses of plain water daily. Other beverages such as tea (47%), coffee (28%), chocolate-based drinks (23%) and cordial syrup (11%) were also consumed on daily basis, however, in a smaller proportion of the population. There were differences in the prevalence of daily consumption of foods when comparing urban and rural population, and also between men and women. The prevalence of daily consumption of marine fish among rural and urban adults was 51% and 34% respectively. For sweetened condensed milk, men and women consumed 43% and 28% respectively; however, more women drank full cream milk than men. Between the age groups, 21% of adults below 20 years old consumed chicken at least once a day, while this pattern of intake was not shown in the older age groups. Our findings show that adults, aged 50 to 59 years old, had the highest prevalence of daily consumption of full cream milk with 24% while those aged 18 to 19 years old had the lowest prevalence of daily consumption at 15%. The food consumption pattern of Malaysian adults appears to be satisfactory. However, some changes in food habits are recommended especially in substituting the less wholesome sweetened condensed milk with the more nutritious full cream or skimmed milk.
