    The stomach contents of Omobranchus sp. (family Blenniidae) larvae were investigated in a seagrass-mangrove based ecosystem in Johor Strait, Malaysia from October 2007 to September 2008. Specimens of larval fish were collected through subsurface towing of a Bongo net from five different stations. The stomach sacs of 267 Omobranchus sp. larvae were separated and observed, which comprised of 24 significant food stuffs belonging to 6 main groups viz. phytoplankton (62.45%), zooplankton (18.24%), algae (5.56%), plant-like particles (5.75%), debris (4.22%) and unidentified particles (2.03%). In situ water parameters were also measured throughout the sampling cruises. There was a strong and significant positive correlation between stomach phytoplankton and salinity (r = 0.658, p < 0.05).? Canonical correlation analysis indicated a weak relationship (29.8%) between stomach contents and physico-chemical parameters. Only salinity appeared to be the controlling factor for the stomach contents of Omobranchus sp. larvae in the investigated area. Based on the stomach content analysis, it could be concluded that Omobranchus sp. were mainly herbivorous during the larval stages. ?

    The tropical conch, Laevistrombus canarium (Linnaeus, 1758) and Canarium urceus (Linneaus, 1758) are ecologically and economically important shellfish species in Malaysia and neighboring region. Their populations, however are currently declining and this histopathological study investigates the aspect of parasitism and diseases that may affect their well-being. Conch samples were randomly collected from their natural habitat and histological sections (4-5 µm) of various organs and tissues were examined under light microscope. This was followed by ultrastructure analysis on infected tissues using transmission electron microscope (TEM). Based on the histological analysis, large numbers of gamonts, sporocysts and trophozoites of Apicomplexa-like parasites were observed in the vacuolated cells and pyramidal crypt cells of the digestive tubules, and in the digestive ducts. Furthermore, coccidian and oocysts-like Pseudoklossia sp. stages were also observed in the cells of the kidney. Apart from that, spores with cyst-like structure were observed in the digestive gland and kidney. Although the parasites were present in most of the organs analyzed, there was no obvious symptom, inflammatory response or mortality incurred on both species, which implies the possibility of a non-virulent relationship like commensalisms or mutualism. However, more investigations, including molecular studies, are needed to confirm the parasite identification and dynamics, and to further evaluate the nature of relationship between Apicomplexa parasites and their host.

    In this study we aimed to analyze the effects of water temperature and diet on the length-weight relationship and condition of juvenile Malabar blood snapper Lutjanus malabaricus over a 30-d experimental period. The experiment was conducted in the laboratory using a flow-through-sea-water system. The fish were subjected to four different temperatures (22, 26, 30, and 34 °C) and two diets (commercial pellet and natural shrimp). Fish were fed twice daily. L. malabaricus exhibited negative allometric growth (b<3) at the beginning of the experiment (Day 0) at all temperatures and both diets except for 22 °C fed with shrimp, which showed isometric growth (b=3). Conversely, at the end of the experiment (Day 30) fish showed isometric growth (b=3) at 30 °C fed with the pellet diet, indicating that the shape of the fish did not change with increasing weight and length, and a positive allometric growth (b>3) at 30 °C fed with shrimp diet, which indicated that fish weight increases faster than their length. The rest of the temperatures represented negative allometric growth (b<3) on both diet, meaning that fish became lighter with increasing size. The condition factors in the initial and final measurements were greater than 1, indicating the state of health of the fish, except for those fed on a pellet diet at 34 °C. However, the best condition was obtained at 30 °C on both diets. Nevertheless, diets did not have a significant effect on growth and condition of juvenile L. malabaricus. The data obtained from this study suggested culturing L. malabaricus at 30 °C and feeding on the pellet or shrimp diet, which will optimize the overall production and condition of this commercially important fish species.

    Vitamin E is composed of various subfamilies that include tocopherols and tocotrienols. These compounds have antioxidant properties but differ in structure, dietary source and potency. In this study we evaluated the efficacy of alpha-tocopherol as an antioxidant and its role in wound closure in normal and streptozotocin-induced diabetic rats. The healing of 6 cm linear incisions created on the back of each male Sprague-Dawley rat (250-300 g) was monitored by measuring the length of the wounds daily. The rats were divided into two categories; normal and streptozotocin-induced diabetic rats. For each category, the animals were further divided into two groups; those untreated and those receiving 200 mg/kg bodyweight alpha-tocopherols daily by oral gavage. All rats were fed standard food and water ad libitum. Blood samples were taken at 0, 5 and 10 days after the wounds were created for the determination of malondialdehyde levels and red cell superoxide dismutase, catalase and glutathione peroxidase activities. The results showed that alpha-tocopherol reduced plasma malondialdehyde levels, increased glutathione peroxidase activity and accelerated the rate of wound closure in treated rats.

    INTRODUCTION: Accurate preoperative imaging of the temporal bone in patients receiving cochlear implants is important. High resolution computed tomography (HRCT) and magnetic resonance (MR) imaging are the 2 preoperative imaging modalities that provide critical information on abnormalities of the otic capsule, pneumatisation of the mastoid, middle ear abnormalities, cochlear ducts patency and presence of cochlear nerve.

    MATERIALS AND METHODS: The HRCT and MR imaging in 46 cochlear implant patients in our department were reviewed.

    RESULTS: Majority of our patients [34 patients (73.9%)] showed normal HRCT of the temporal bone; 5 (10.9%) patients had labyrinthitis ossificans, 2 (4.3%) had Mondini's abnormality and 2 (4.3%) had middle ear effusion. One patient each had high jugular bulb, hypoplasia of the internal auditory canal and single cochlear cavity, respectively.

    CONCLUSION: The above findings contribute significantly to our surgical decisions regarding candidacy for surgery, side selection and surgical technique in cochlear implantation.

    The purpose of this study is to validate Sugar Craving Assessment Tool (MySCAT) among type II diabetes mellitus patients. A total of 168 respondents were recruited to participate in this cross sectional study. It was conducted in out-patient health clinics in Kuala Terengganu and Kuantan. Patients’ sugar craving and dietary behavior were measures using structured questionnaires which were MySCAT, three-day dietary recall and demographic data. The interview sessions were conducted by a dietician. MySCAT internal consistency test had a Cronbach’s alpha value of 0.75 and showed a significant correlation (r=0.56, p < 0.001) with actual sugar intake collected via three days diet recall. ROC analysis reported a cut-off point for MySCAT as 16.5; the sensitivity value of 0.83 and 1-specificity of 0.38. 62% of respondents were categorized as cravers and 38% as non-cravers. Male and female repondents had no significant differences in craving status. The mean intake of sugar was 46 g/day (SD= 2.26), respondents had a relatively high intake of sugar in comparison to their recommendation. This study found that MySCAT provides an easy efficient tool which is sensitive enough to identify those with sugar craving problem. It also provides an overview of patients’ dietary intake and points out their problem with dietary intake compliance. We suggest MySCAT as an important tool that can assist dietitians in their consultation session.

    The Golden Apple Snail (GAS), Pomacea canaliculata is an invasive species that has become a serious pest organism in rice fields in Southeast Asia since 1980s. This study was conducted to assess the hatchability of eggs under a combination effect of temperature and light. The clusters of eggs were exposed to different temperature (28oC, 30oC, 32oC, 34oC and 36oC) and under the presence and the absence of light. The finding showed that the cluster of egg hatched faster with an increase of temperature (2-way ANOVA; P < 0.05). However, the egg hatchability was not impacted by the presence or the absence of light. The combined effect between temperature and light on egg hatchability was also insignificant (2-way ANOVA; P > 0.05). Our findings indicate that temperature alone produces the main impact on the egg hatchability. Therefore, this knowledge provides an initial understanding to predict the population dynamic and geographical distribution of Pomacea from Malaysia particularly in the effort to hamper their ecological invasion.

    DNA Barcoding, primarily focusing on cytochrome coxidase subunit I (COI) gene has been appraised as an effective tool for species identification. Nonetheless, species identification based on molecular approach is essential for discrimination of look-alike species. In this study, we focused on the marine fishes Family Nemipteridae, one of the commercially important group distributed within the surrounding seas of Malaysia. Some of the samples were collected during the National Demersal Trawl Survey in the Exclusive Economic Zone of East Coast Peninsular Malaysia by the Department of Fishery Malaysia. A 652bp region of COI was sequenced for 74 individuals from nine putative species. Additional 34 COIsequences from GenBank were also included in this study making the total number of samples analysed to 108 individuals. The averageKimura 2-parameter (K2P) nucleotide divergence was 0.34% among individuals within species and 6.97% within genera. All putative species formed monophyletic clades in both Neighbour-joining (NJ) and Maximum-likelihood (ML) trees. However, there was a potential misidentification in specimen identified as Nemipterus tambuloides,as the specimen did not group with their own taxa. It was genetically grouped in Nemipterus thosaporni clade. This study supports the effectiveness of COIgene in species discrimination of Family Nemipteridae.

    ABSTRACT
    Metabolic footprinting involves the determination of metabolites excreted or secreted by the cells.
    This study aimed to identify the differential extracellular metabolites in colorectal cancer (CRC)
    cells for the determination of molecular changes that occur as CRC progresses. CRC cells at
    different stages ie; SW 1116 (stage A), HT 29 and SW 480 (stage B), HCT 15 and DLD-1 (stage
    C), and HCT 116 (stage D) were grown in culture. The media in which the cells were grown are
    subjected to metabolomics profiling using Liquid Chromatography Mass SpectrometryQuadrupole Time of Flight (LC/MS Q-TOF). Statistical and metabolic pathway analysis was
    performed using Metaboanalyst software and identification of metabolites was determined by the
    METLIN database. A total of 27 differential extracellular metabolites were identified in CRC cells
    of different stages compared to stage A cells. Data from the Partial least squares-discriminant
    analysis (PLS-DA) score plot shows a clear separation between CRC cells of different stages with
    a few overlaps between stage B and C. Further analysis using variable importance in projection
    (VIP) revealed 14 differential extracellular metabolites that were most significant in differentiating
    CRC cells of the advanced stages from stage A which are 5-hydroxy-L-tryptophan,
    indoleacetaldehyde, 4,5-dimethylthiazole, 8-oxodiacetoxyscirpenol, bisnorbiotin, 5-amino-6-
    (5'phosphoribosylamino) uracil, glyceryl 5-hydroxydecanoate, sphinganine, 8,8-diethoxy-2,6-
    dimethyl-2-octanol, l-cystine, thiamine acetic acid, phytosphingosine, PE
    (20:4(5Z,8Z,11Z,14Z)/22:6(4Z,7Z,10Z,13Z,16Z,19Z)), N-(2R-hydroxypentacosano-yl)-2Samino-1,3S,4R-octadecanetriol. The different expressions of metabolites may indicate altered

    The obligatory fasting of Ramadan is not only an Islamic ritual to enhance the God-consciousness (called Taqwa), it may also lead to improvement of health status of fasting person(s). In other words, it is a complete overhauling instrument of Muslims soul/body in terms of faith and health. This can be termed as “Islamic fasting: a combination of spiritual elevation and prevention of diseases”. During fasting Muslims try to maintain integrity of the human body both physically and spiritually. It not only keeps us healthy physically/spiritually but also makes us healthy in social obligations. Furthermore,with full devotion it brings long-lasting changes in the individual’s life and relate to the level of righteousness and strength of character to resist desires. In health fasting plays an important role, for example the improvement of hyperlipidemia, obesity (fat loss), diabetes, cognition, cell functions, immunological factors, inflammation, stress and lifespan etc. This article is an effort to review and discuss research carried out on the listed conditions during the holy month of Ramadan on various aspects of human health.

    The aims of this study were to fabricate cellulose acetate (CA) film from oil palm empty fruit bunch (OP-EPB), as well as to characterize and evaluate their biocompatibility. Several processes were carried out, and these included prehydrolysis-soda method, chlorine free bleaching method, including oxygen, ozone and peroxide, to produce the cellulose pulp. Then, a liquid phase acetylation method was applied through acetic acid-acetic anhydride-sulphuric acid. Triethyl citrate (TEC) ester was used as additive at different percentages of 10, 20, 30 and 40 wt%. The film produced was characterized by FTIR to identify the functional group of the CA film and their tensile properties were further characterized. Biocompatibility of the film was evaluated using cytotoxicity test. Stem cell derived from human deciduous teeth (SHED) was used with MTS assay. The results showed at 30% of TEC, the tensile strength and elongation of CA (OP-EFB) film was at the optimum and is therefore suitable to be used in dental application. The cytotoxicity evaluated showed that the fabricated CA (OP-EFB) films were non-toxic up to the concentration tested, and are thus compatible with SHED.

    The complex pathophysiology of traumatic brain injury, its cascading effects and a varied outcome suggest that factors such as genetics may permeate and modulate the neurocognitive outcomes in patients with mild traumatic brain injury (mTBI). This study was conducted to determine the relationship between genetic polymorphism of apolipoprotein E, and neurocognitive and functional outcomes in mTBI.     Public health care programme evaluation includes determining the programme effectiveness (outcome assessment), efficiency (economic evaluation), accessibility (reachability of services) and equity (equal provision for equal needs). The purpose of this study was to make comparison on cost·( efficiency and costeffectiveness in managing type 2 diabetes between the Ministry of Health (MOH) health clinics with family medicine specialist (FMS) and health clinics without FMS. A costeffectiveness analysis was conducted alongside across-sectional study at two government health clinics in Machang, Kelantan, one with FMS and the other without FMS. A total of 300 patients, of which 155 from the health clinic without FMS and 145 from the other group were evaluated for sociodemographic and clinical characteristics from August 2005 to May 2006. HbA1c
    analysis was measured for each patient during the study period. Macrocosting and microcosting were used to determine costs. The provider cost for diabetic management ranged from RM270.56 to RM4533.04 per diabetic patient per year, withla mean cost of RM1127.91(t906.08) per diabetic patient per year in health clinic with FMS. In health clinic without FMS, the provider cost ranged from RM225.93 to RM4650.13, with a mean cost of RM802.15 (:626.26). Proportion ofgood HbA1c was 17.2% for health clinic with FMS and 10.3% for the health clinic without FMS. The annual mean provider cost per proportion of good HbA1c control (< 7%) (Costefkctiveness ratio/ CER) was RM6557.65.for health clinic with FMS and RM7787.88 for health clinic without FMS. This provider cost-epfectiveness ratio was not different statistically between the health clinic with FMS and health clinic without FMS (p=0.063). The cost of building, equipments, overheads, staff and consumables were higher for FMS group. Sensitivity analysis was performed for three discount rates (0, 5 and 7%). Relative cost-effectiveness of diabetes management in health clinic with FMS and health clinic without FMS was unchanged in all sensitivity scenarios. Even though, there was no significantly difference in provider CER in type 2 diabetes management at Malaysian MOH health clinics, but the provider CER in health clinic with FMS was lower compared to health clinic without FMS. Therefore, we can conclude that the presence of FMS in the health clinic will effectively improved the management of type 2 diabetes.

    Ludwig angina is a submandibular space cellulitis secondary to oral cavity infection. It is strongly associated with difficult intubation due to limitation in the mouth opening. The presentation of Ludwig angina varies according to the severity of the infection. The extreme presentations include upper airway obstruction and respiratory failure. We present a female teenager with right submandibular abscess as the consequence of Ludwig angina, who was planned for incision and drainage. Successful awake fibre optic intubation was performed as a method of induction due to trismus, deferring the need for tracheostomy.

    Plasmodium knowlesi has been discovered as the fifth species causing malaria in humans. It is a major public health problem in South East Asia especially in Borneo. We report a case of pericardial effusion that rapidly progressing to cardiac tamponade, an atypical presentation of P. knowlesi malaria. Our patient had no underlying known medical illness, presented with high grade fever with chills and rigors, epigastric pain, nausea, vomiting and with poor oral intake. Initial bedside cardiac ultrasound showed minimal pericardial effusion. Within a few hours, she became hypotensive, deteriorated rapidly despite fluid resuscitation requiring mechanical ventilation and inotropic sup- port. Bedside cardiac ultrasound showed cardiac tamponade and pericardiocentesis was done. We highlight the importance of having high level of suspicion for this atypical presentation of cardiac tamponade when a patient is hypotensive in P. knowlesi infection. Prompt diagnosis and management may prevent potentially fatal complication.

    Objectives: Warfarin use in atrial fibrillation has been established for preventing occurrence of stroke in patients with atrial fibrillation. However, safety and clinical monitoring of warfarin use is crucial due to its risk of bleeding complications. This study aims to compare and establish relation of time in therapeutic range (TTR) of warfarin in patients with atrial fibrillation in the first 6 months and 6 month thereafter of anticoagulation therapy.
    Methods: This is a retrospective study carried out at a tertiary-care hospital with anticoagulation clinic in the state of Selangor, Malaysia. Data collected included patients’ demographics, co-morbidities, and international normalized ratio (INR). TTR were determined using Rosendaal method based on records found in database (INR Desk 4.0 system) and patients’ hemorrhage events were also recorded. Samples of the study were patient who started warfarin from January 2009 until March 2013.
    Results: A total of 167 patients with atrial fibrillation were enrolled and only 6% (n=10) achieved TTR of more than 75% for the first 6 months of warfarin use as compared to 16.8% (n=28) of TTR more than 75% 6 months thereafter. As for bleeding incidences, 29% (n=45) of patients in the group of TTR less than 75% in the first 6 months had bleeding complications as compared to 18.7% (n=26) in patients of TTR less than 75% 6 months after.
    Conclusions: A more regular follow up is necessary during the first 6 months of new warfarin users as they tend to be out of the TTR and have a higher bleeding risk.

    BACKGROUND: Cymbopogon citratus (Poaceae) a tropical perennial herb plant that is widely cultivated to be eaten either fresh with food or dried in tea or soft drink has been reported to possess a number of medicinal and aromatic properties. This study aimed at evaluating the protective effects of C. citratus aqueous extract against liver injury induced by hydrogen peroxide (H2O2), in male rats.

    MATERIALS AND METHODS: Twenty-five rats were randomly divided into five different groups of five animals in each group; (1) Control. (2) Received H2O2 (0.5%) with drinking water. (3), and (4) received H2O2 and C. citratus (100 mg·kg(-1) b wt), vitamin C (250 mg·kg(-1) b wt) respectively. (5), was given C. citratus alone. The treatments were administered for 30 days. Blood samples were collected and serum was used for biochemical assay including liver enzymes activities, total protein, total bilirubin and malonaldehyde, glutathione in serum and liver homogenates. Liver was excised and routinely processed for histological examinations.

    RESULTS: C. citratus attenuated liver damage due to H2O2 administration as indicated by the significant reduction (p<0.05), in the elevated levels of ALT, AST, ALP, LDH, TB, and MDA in serum and liver homogenates; increase in TP and GSH levels in serum and liver homogenates; and improvement of liver histo-pathological changes. These effects of the extract were similar to that of vitamin C which used as antioxidant reference.

    CONCLUSION: C. citratus could effectively ameliorate H2O2-induced oxidative stress and prevent liver injury in male rats.

    The methanol extracts of three Macaranga species (M. denticulata, M. pruinosa, and M. gigantea) were screened to evaluate their total phenolic contents and activities as cholinesterase inhibitors, nitric oxide (NO) production inhibitors, tyrosinase inhibitors, and antioxidants. The bark of M. denticulata showed the highest total phenolic content (2682 mg gallic acid equivalent (GAE)/100 g) and free radical scavenging activity (IC50 = 0.063 mg/mL). All of the samples inhibited linoleic acid peroxidation by greater than 80%, with the leaves of M. gigantea exhibiting the highest inhibition of 92.21%. Most of the samples exhibited significant antioxidant potential. The bark of M. denticulata and the leaves of both M. pruinosa and M. gigantea exhibited greater than 50% tyrosinase inhibition, with the bark of M. denticulata having the highest percentage of inhibition (68.7%). The bark and leaves of M. denticulata exhibited greater than 50% inhibition (73.82% and 54.50%, resp.) of the acetylcholinesterase enzyme (AChE), while none of the samples showed any significant inhibition of butyrylcholinesterase (BChE). Only the bark of M. denticulata and M. gigantea displayed greater than 50% inhibition of nitric oxide production in cells (81.79% and 56.51%, resp.). These bioactivities indicate that some Macaranga spp. have therapeutic potential in medicinal research.
