    Plastic packaging is widely used by hawkers to pack food items within the foodservice industry. Despite of the various benefits brought about by plastics within the foodservice industry, concerns associated to food safety and solid waste disposal have been raised. Therefore, a study that involved 131 food hawkers at night markets in Kuala Selangor, Selangor, had been carried out to evaluate their perceptions pertaining to the use of plastic packaging to pack hot food items. As such, this study investigated the correlations between perceptions of food hawkers towards awareness, health hazards, environmental hazards, regulations, and the practice of using plastic packaging to pack hot edible food items. In addition, this study determined the most relevant predictor that influences the practice of using plastic packaging. A cross-sectional survey that integrated a self-administered structured questionnaire was employed for the purpose of this study. The retrieved data were analysed by using SPSS version 21. The study outcomes revealed that the food hawkers appeared to have good perceptions towards environmental hazard (3.52 ± 0.48) and regulation (3.51 ± 0.38), but poor perceptions on awareness (2.73 ± 0.66) and health hazard (2.55 ± 0.64). Nevertheless, significantly positive and moderate correlations were observed between health hazard (r = 0.45, p < 0.05), environmental hazard (r = 0.43, p < 0.05), regulations (r = 0.42, p < 0.05) and plastic packaging practices. The study reveals that health hazard emerges to be the main predictor of the plastic packaging practices. Hence, future studies may incorporate larger sample size and various locations in Malaysia as this study only serves as a platform to comprehend the practice of packing hot food items using plastic packaging from the stance of food hawkers.

    Sibutramine is a prohibited weight-reducing agent used in many weight-loss
    products, which has been banned in many countries since 2010 including
    Malaysia due to it toxicities and mortality effect. The widespread use of these
    sibutramine-containing products has raised concerns regarding their side
    effects, which include psychological symptoms. In this case report, we
    highlight a case of psychosis induced by the use of this product. We present a
    case of 36-year-old Burmese lady who has three admissions to psychiatry
    ward with psychotic presentation after on-going consumption of weight-loss
    product containing sibutramine.

    Pre-pregnancy Clinic (PPC) services is one the plausible efforts towards achieving the Millennium Development Goals. However, various issues still need to be addressed for improvement of the services. Considering this view, an attempt was made to explore the barriers, strength and weakness of current practice of pre-pregnancy clinic services in Sarawak, since the programme has been implemented in this state from the year 2011. This cross-sectional study was conducted at nine selected health care facilities throughout Sarawak. A multistage sampling procedure was adapted to select the health care facilities. An unstructured open-ended questionnaire was administered to get the in-depth perceived views and current practice of pre-pregnancy clinic services. A total of 322 health care providers from nine selected health care facilities gave their feedback. In the present paper, a qualitative analysis was done for the openended questions to get in-depth views of barriers, strength and weakness of pre-pregnancy clinic services. The results of the study were narrated in textual form and a thematic analysis was done manually. The identified themes for perceived barriers to the provision of pre-pregnancy care were perception, attitude and acceptance of PPC services, socio-economic issues, services and client factors. The perceived weaknesses of the services are listed under two main themes: working environment and service factors, while, the strength of services produced three thematic areas which are preparation for pregnancy, prevention of mortality and morbidity and comprehensive services. Though prepregnancy services are beneficial for society wellbeing, various issues still need to be considered for the improvement of the quality of services. Lack of awareness, no ministerial guidelines or Standard Operating Procedures (SOP) and knowledge pertaining to the services were few of the main areas which need to be pondered upon. Promotional activities and campaigns should be geared up ensuring availability the services to the general population.

    A majority of the clinical use of positron emission tomography (PET)-computed tomography (CT) is related to cancer management. Its application in evaluating inflammatory diseases and pyrexia of unknown origin is becoming popular. We reviewed the fluorine-18-fluorodeoxyglucose PET-CT findings of an 80-year-old woman with nonspecific clinical presentation consisting of generalised malaise, moderately high fever and weight loss. Prior CT and magnetic resonance imaging were not helpful in providing a clinical diagnosis. The diagnosis was Horton's arteritis, and the patient responded well to high-dose steroids.

    BACKGROUND: Neonatal nasal obstruction may result in respiratory distress, feeding difficulties, sleep apnoea and failure to thrive; hence, it requires thorough evaluation and prompt intervention. Congenital inferior turbinate hypertrophy is relatively uncommon, and its presentation can mimic other congenital nasal anomalies.

    RELEVANCE: This paper reports two cases of congenital inferior turbinate hypertrophy in neonates that resulted in significant respiratory distress, feeding difficulties and sleep disturbance. Both patients were successfully treated surgically by endoscopic nasal dilatation and stenting. A literature search was performed to identify articles on congenital inferior turbinate hypertrophy in neonates and its management.

    CONCLUSION: Albeit rare, congenital inferior turbinate hypertrophy should be considered a differential diagnosis in newborns presenting with respiratory distress at birth.

    BACKGROUND: Injection laryngoplasty (IL) is a common procedure performed by the laryngologist to treat glottal insufficiency. It can be performed under general anaesthesia (GA) or as an office-based procedure. One of the common problems faced during IL is the disconnection between the injection needle and injection material syringe due to high-pressure effect. This may lead to leakage of the injectates especially when injecting high-viscosity materials such as calcium hydroxylapatite (CaHa) or injecting through hard fibrotic scar of the vocal fold.

    PURPOSE: Due to this recurring issue, we propose anti-reflux valve as a connector to link up these two devices. The function of anti-reflux valve is to ensure secure connection between these two devices and overcome the problem.

    METHODS: The anti-reflux valve that can be used is NeutraClearTM needle-free connector EL-NC1000 or MicroClaveTM clear connector. In our practice, these anti-reflux valves are used together with Integra MicroFrance straight malleable injection needle (0.5 mm diameter, 250 mm length) for IL under GA. However, any other injection needle suitable for IL may also be used with these anti-reflux valves.

    RESULTS: Our 3 years' experience has shown good outcome with no reported incidence of device detachment or leakage of injectates during IL procedure.

    CONCLUSIONS: Anti-reflux valve is a readily available device in the operating theatre or clinic and only involve simple preparation prior to IL. It is beneficial as an added device in IL procedure.

    The Crimean-Congo haemorrhagic fever virus (CCHFV), which is transmitted by the ticks of Hyalomma spp. in general and H. marginatumin particular, can cause severe disease in humans, with mortality rates of 3-30%. Other than from the bites of infected ticks, CCHFV can also be transmitted through contact with patients with the acute phase of infection or contact with blood or tissues from viraemic livestock.  Outbreaks of human cases of haemorrhagic manifestations have been documented since 1945 and described in parts of Africa, Asia, Eastern Europe and the Middle East and most recently India in 2011. In addition, serological evidence of the disease has been reported in some countries where no human cases were reported. As regional neighbours China and India have been affected by this virus, this study was conducted to determine the seroprevalence of CCHFV among Orang Asli population of Malaysia as the most at risk people who residing in the deep forests.

    Energy is an indispensable factor for the economic growth and development of a country. Energy consumption is rapidly increasing worldwide. To fulfill this energy demand, alternative energy sources and efficient utilization are being explored. Various sources of renewable energy and their efficient utilization are comprehensively reviewed and presented in this paper. Also the trend in research and development for the technological advancement of energy utilization and smart grid system for future energy security is presented. Results show that renewable energy resources are becoming more prevalent as more electricity generation becomes necessary and could provide half of the total energy demands by 2050. To satisfy the future energy demand, the smart grid system can be used as an efficient system for energy security. The smart grid also delivers significant environmental benefits by conservation and renewable generation integration.

    In this study, we determined the molecular mechanism of γ-tocotrienol (GTT) in preventing cellular aging by focusing on its anti-apoptotic effect in stress-induced premature senescence (SIPS) model of human diploid fibroblasts (HDFs). Results obtained showed that SIPS exhibited senescent-phenotypic characteristic, increased expression of senescence-associated β-galactosidase (SA β-gal) and promoted G(0)/G(1) cell cycle arrest accompanied by shortening of telomere length with decreased telomerase activity. Both SIPS and senescent HDFs shared similar apoptotic changes such as increased Annexin V-FITC positive cells, increased cytochrome c release and increased activation of caspase-9 and caspase-3 (P < 0.05). GTT treatment resulted in a significant reduction of Annexin V-FITC positive cells, inhibited cytochrome c release and decreased activation of caspase-9 and caspase-3 (P < 0.05). Gene expression analysis showed that GTT treatment down regulated BAX mRNA, up-regulated BCL2A1 mRNA and decreased the ratio of Bax/Bcl-2 protein expression (P < 0.05) in SIPS. These findings suggested that GTT inhibits apoptosis by modulating the upstream apoptosis cascade, causing the inhibition of cytochrome c release from the mitochondria with concomitant suppression of caspase-9 and caspase-3 activation. In conclusion, GTT delays cellular senescence of human diploid fibroblasts through the inhibition of intrinsic mitochondria-mediated pathway which involved the regulation of pro- and anti-apoptotic genes and proteins.

    Several genes have been used as housekeeping genes and choosing an appropriate reference gene is important for accurate quantitative RNA expression in real time RT-PCR technique. The expression levels of reference genes should remain constant between the cells of different tissues and under different experimental conditions. The purpose of this study was to determine the effect of different experimental treatments on the expression of glyceraldehyde 3-phosphate dehydrogenase (GAPDH) mRNA so that the reliability of GAPDH as reference gene for quantitative real time RT-PCR in human diploid fibroblasts (HDFs) can be validated. HDFs in 4 different treatment groups viz; young (passage 4), senescent (passage 30), H2O2-induced oxidative stress and gamma-tocotrienol (GTT)-treated groups were harvested for total RNA extraction. Total RNA concentration and purity were determined prior to GAPDH mRNA quantification. Standard curve of GAPDH expression in serial diluted total RNA, melting curve analysis and agarose gel electrophoresis were used to determine the reliability of GAPDH as reference gene.

    Over the past few years, high step-up dc-dc converters have been drawn substantial attention because of their wide-ranging application not only in the renewable energy sector but also in many other applications. To acquire a high voltage gain in photovoltaic (PV) and other renewable energy applications, a high step-up dc-dc converter is proposed in this paper. The proposed converter structure consists of a full-bridge (FB) module along with an input boost inductor and a voltage multiplier based on the Cockcroft-Walton (CW) principle with a parallel inductor. The key features of the proposed converter are: 1) high voltage gain with lower voltage stress on the switches, diodes and other passive elements without affecting the number of cascaded stages, 2) a minimum size of boost inductance and cascaded stage capacitance that ensures its compactness and low cost, and 3) a minimal number of major components. Circuit operation, steady-state analysis and various design parameters of the proposed converter are explained in details. In order to prove the performance of the theoretical analysis, a laboratory prototype is also implemented. The peak voltage gain and the maximum efficiency obtained are 11.9 and 94.6% respectively with very low input current ripple and output voltage ripple generated.

    INTRODUCTION: Chronic histiocytic intervillositis (CHI) is a rare placental lesion strongly associated with recurrent miscarriages and fetal losses. It requires histopathological diagnosis and can only be made after delivery of the products of conception (POC). We describe a case of CHI in a 41-yearold lady with a 16-year history of thirteen recurrent consecutive first trimester miscarriages.

    CASE REPORT: The patient is a 41-year-old lady who suffered first trimester miscarriages in all her thirteen pregnancies. The relevant clinical investigations revealed neither significant nor helpful findings in determining the cause of recurrent miscarriages. Histological findings in each except one of the submitted conceptual tissue showed similar features of histiocytic aggregates primarily within the intervillous spaces, a characteristic description of CHI. One of the samples showed degenerative changes.

    DISCUSSION: Practicing pathologists are not familiar with the histological features of CHI and this may be a potential pitfall in routine examination of POCs. Recognising this entity allows for accurate diagnosis and hence better management. The aetiology remains unclear, although an immunopathological basis are being explored.

    In 2017, the World Health Organization (WHO) named A. baumannii as one of the three antibiotic-resistant bacterial species on its list of global priority pathogens in dire need of novel and effective treatment. With only polymyxin and tigecycline antibiotics left as last-resort treatments, the need for novel alternative approaches to the control of this bacterium becomes imperative. Vaccines against numerous bacteria have had impressive records in reducing the burden of the respective diseases and addressing antimicrobial resistance; as in the case of Haemophilus influenzae type b . A similar approach could be appropriate for A. baumannii. Toward this end, several potentially protective antigens against A. baumannii were identified and evaluated as vaccine antigen candidates. A licensed vaccine for the bacteria, however, is still not in sight. Here we explore and discuss challenges in vaccine development against A. baumannii and the promising approaches for improving the vaccine development process.

    Total laryngectomy (TL) is the treatment of choice for advanced glottic cancer. Post-operative complications can be debilitating for patients, family members and healthcare workers. Complications following TL have been reported in many studies, with pharyngocutaneous fistula and wound infection being the most common. Identifying the risk factors that may give rise to these complications is vital to minimise post-operative morbidity. We present the case of a 62-year-old male who underwent salvage TL following radiation therapy for recurrent glottic carcinoma. The patient developed diffuse submental swelling upon the commencement of oral feeding. A flexible nasopharyngolaryngoscopy revealed a sloughy area at the neopharynx, with the finding of a sealed anastomotic leak on a repeat barium swallow study. We report persistent Pseudomonas aeruginosa infection following salvage TL, after a sealed anastomotic leak.

    Polymyxins are last-line antibiotics against multidrug-resistant Klebsiella pneumoniae but using polymyxins alone may not be effective due to emerging resistance. A previous study found that combining polymyxin B with chloramphenicol effectively kills MDR K. pneumoniae, although the bone marrow toxicity of chloramphenicol is concerning. The aim of this study is to assess the antibacterial efficacy and cytotoxicity of polymyxin B when combined with chloramphenicol and its derivatives, namely thiamphenicol and florfenicol (reported to have lesser toxicity compared to chloramphenicol). The antibacterial activity was evaluated with antimicrobial susceptibility testing using broth microdilution and time-kill assays, while the cytotoxic effect on normal bone marrow cell line, HS-5 was evaluated using the MTT assay. All bacterial isolates tested were found to be susceptible to polymyxin B, but resistant to chloramphenicol, thiamphenicol, and florfenicol when used alone. The use of polymyxin B alone showed bacterial regrowth for all isolates at 24 h. The combination of polymyxin B and florfenicol demonstrated additive and synergistic effects against all isolates (≥ 2 log10 cfu ml-1 reduction) at 4 and 24 h, respectively, while the combination of polymyxin B and thiamphenicol resulted in synergistic killing at 24 h against ATCC BAA-2146. Furthermore, the combination of polymyxin B with florfenicol had the lowest cytotoxic effect on the HS-5 cells compared to polymyxin B combination with chloramphenicol and thiamphenicol. Overall, the combination of polymyxin B with florfenicol enhanced bacterial killing against MDR K. pneumoniae and exerted minimal cytotoxic effect on HS-5 cell line.

    BACKGROUND: Statins are the most widely used lipid-lowering agents for patients with hyperlipidemia. However, interindividual variations in efficacy and risk of adverse drug reactions to statin treatment have been widely reported. Ethnicity is well known to be one of the contributing factors to this variation, particularly among Asians.

    OBJECTIVES: To identify genetic variants associated with statin treatment responses among Asian populations with a focus on four commonly prescribed statins: atorvastatin, rosuvastatin, simvastatin, and pravastatin.

    METHODS: A literature search was conducted in Medline and Embase databases. Studies published from 2008 to 2021 were included. The title and abstract of each article were screened by two reviewers and verified by another two reviewers. Data charted include information on authors, year of study, study population, statin studied, gene studied, study findings, and data of significant statistical value.

    RESULTS: A total of 35 articles were included from the 1,939 original studies related to treatment efficacy and 5 articles out of the 284 original studies related to adverse effects. Genetic variants in transmembrane transporters, cytochrome P450 isoenzymes, and apolipoproteins are the most extensively studied among Asian populations, with a main focus on ethnic Chinese. However, Asia consists of genetically different populations, and the results of this review indicated that there is a paucity of studies on other ethnic groups within Asia.

    CONCLUSIONS: Considering the ethnicity of patients could provide a potential value to personalized medicine in statin therapy.

    High conversion ratio dc-dc converters have received significant attention in renewable energy systems, primarily due to their necessary high-gain characteristics. This research proposes a high step-up ratio full-bridge resonant cascaded (FBRC) dc-dc converter designed for use in photovoltaics (PV), fuel cells (FC), electric vehicles (EV), and other low-voltage output energy sectors to achieve high voltage gain. This converter contains a full-bridge cell with a boost input inductor, a diode-capacitor cascaded stage that replaces the transformer as a voltage multiplier and an inductor-capacitor (LC) parallel-series resonant network across the FB terminal. One of the strategic features of the converter is its high voltage step-up characteristic combined with lower duty cycle operation that limits the maximum current through the active devices, making it particularly suitable for systems that generate low output voltage. In addition, zero-voltage switching (ZVS) is achieved during the turn-off and turn-on operation of the FB switches from 25% to full load, thereby lessening the switching losses. Moreover, the diminished necessity for passive components and the decreased voltage stress on both active and passive devices lead to the use of smaller and more cost-effective components. The theoretical analysis of the proposed converter is validated using a 500 W laboratory-scale prototype wherein high-performance SiC-based MOSFETs have been utilized as switching devices. It offers reduced ripples, with input current ripple at 5% and output voltage ripple at 0.76%. When the load is 400 W and 60 V as the input voltage, the maximum efficiency is found 95.8% at 400 V output voltage. The proposed dc-dc converter, with its high voltage gain and reduced component stress, shows significant promise for application in renewable energy systems.

    BACKGROUND: Induction of lactation occurs when breast milk is produced in a human (woman), without going through the process of pregnancy and delivery. Efforts to produce milk by women who had never been pregnant and give birth are not easy. According to the many who have attempted it, it is far more arduous a task than initiating postpartum lactation, but it is possible and worth the effort.

    RESEARCH AIM: This study aimed to explore and understand the challenges for women during induced lactation in Malaysia.

    METHODS: This qualitative study was done in 2017 and utilized in-depth interview among women who induced lactation in five states based on five regions in Malaysia [Central Region (Selangor), Northern Region (Penang), Southern Region (Johor), East Coast Region (Kelantan), Malaysia Borneo (Sabah)]. All interviews were consented, audio-recorded then transcribed verbatim, followed by identification of main themes.

    RESULTS: Data saturation was achieved after interviewing a total of 23 induced lactation women. Data synthesized using thematic analysis revealed six themes describing the main challenges during the induction process were (1) health condition, (2) work commitment, (3) overexertion, (4) not eligible to take leave, (5) inability to follow the treatment plan, and (6) difficulty attaching the adopted baby to initiate breastfeeding during induced lactation process.

    CONCLUSIONS: The challenges faced by women during the process of induced lactation were complex and the induced lactation process requires considerable dedication and determination. The findings of this research will help other women and their spouses/partners comprehend the challenges, obstacles, and support that are needed along the induced lactation process. The Government and other stakeholders have roles in more efforts and programs to help these mothers breastfeed their adoptive child and make them feel more accomplished as women and mothers.

    Validation of housekeeping gene is important for accurate quantitation of RNA in real time RT-PCR technique. The purpose of this study was to determine the validity of glyceraldehyde 3-phosphate dehydrogenase (GAPDH) as a housekeeping gene for quantitative real time RT-PCR assessment in human skin fibroblast senescent model. The cells were divided into different treatment groups; young (passage 4), senescent (passage 30), treatment with H2O2 and treatment with A-tocotrienol prior to H2O2 treatment. Our results showed that the expression level of GAPDH was constant with different treatment groups. Therefore, we concluded that GAPDH was suitable to be used as housekeeping gene in human skin fibroblast senescent model.
