    Swietenia macrophylla (SM) is a medicinally important plant found in tropical and subtropical regions of the world. The ethyl acetate fraction of the seeds of S. macrophylla (SMEAF) is reported to exhibit potent anticancer, antitumor, anti-inflammatory and antifeedant activities. Till date, there have been no studies reported on the acute oral toxicity profile of the ethyl acetate fraction of the seeds of SM. The objective of the present study was to determine the acute toxicity of SMEAF and evaluate the in-vitro neuroprotective activity of SMEAF using primary neuronal cell cultures. In acute oral toxicity study, the SMEAF did not produce any lethal signs of morbidity and mortality. Histo-pathological findings, support the safety of SMEAF, as there were no significant changes observed in any of the parameters studied. Based on the results obtained in MTT assay, we infer that SMEAF has a significant neuroprotective effect, as it increased the cell viability and exhibited protection to the neuronal cells against TBHP induced oxidative stress. Thus, SMEAF can be suggested for use in the development of herbal drug formulations with neuroprotective potential.

    A middle-aged gentleman with history of left penetrating keratoplatsy presented with left eye perforated corneal graft secondary to infective keratitis. The affected eye was blind from absolute steroid-induced glaucoma. In view of expected poor graft survival in a blind eye, globe removal was offered. However, the patient refused the treatment and request for another corneal graft. This case highlights both the possibility of good outcome of cornea graft in such a case, and also illustrates that patient’s autonomy to refuse treatment option outweighs beneficence.

    Oral cancer is the sixth most common malignancy in the world. Despite recent advances in cancer diagnoses and therapies, the five-year survival rate of oral cancer patients has remained at a dismal 50% in the last few decades. Oral cancer is of major concern in Southeast Asia primarily because of the prevalent oral habits namely betel quid chewing, smoking and alcohol consumption. This paper provides a brief overview on the various aetiological agents and risk factors implicated in the development of oral cancer.

    Objective: Previous studies have shown milk to contain cancer inhibitors. In this context, this study was conducted to screen the potential cytotoxic properties of four different types of milk, namely cow's milk, goat's milk, mare's milk and human milk.
    Methods: In evaluating the cytotoxic properties of milk, two different human leukemia cell lines namely, Raji and CEM-SS were used. The treated and untreated cells of milk were cultured at 37°C in 5% CO2 for 5 days according to standard guidelines. The CellTiter 96® Aqueous (MTS) assay was carried out on the first, third and fifth days to measure cell viability.     This article presents a brief overview on the importance of adopting healthy behavior during and after the completion of primary cancer treatment. Increasing evidences are advocating physical activity engagement in cancer survivors due to its convincing beneficial outcomes. Today, outcomes from numerous trials confirmed the need to examine beyond physical activity engagement, into physical inactivity as an independent factor for cancer recurrences. Reducing cancer-risk related behaviors via increase physical activity and reduce inactivity is now receiving much attention in the field of cancer survivorship. Both realms of activity and inactivity are now acknowledged as influential independent factors contributing to better care in the field of cancer survivorship.

    Ki-67 expression is strongly correlated with tumour cell proliferation and growth. It is widely used as a proliferation marker in the routine pathological investigation. The nuclear protein Ki- 67 (pKi67) is recognised prognostic and predictive indicator for the biopsies assessment for cancer patients. Clinically, pKi67 has been revealed to associate with metastasis and the clinical stage of tumours. Furthermore, it has been presented that the expression of Ki-67 is significantly higher in malignant tissues with poorly differentiated tumour cells, as compared with normal tissue. The Ki-67 labelling index plays a vital role as an independent prognostic factor for survival rate, which includes all stages and grade categories. There is an association between the ratios of Ki-67 positive malignant cells and patient survival. This review provides an overview of recent advances in detecting Ki-67 in ovarian carcinoma.

    Gold nanoparticles (AuNPs) have been extensively investigated as dose enhancement agent to increase the lethal dose to the tumours while minimizing dose to the normal tissue. Their intriguing properties and characteristics such as small size and shape provide favorable option in increasing radiotherapy therapeutic efficiency. In this study, the effects of AuNPs size on the dose enhancement effects irradiated under megavoltage photon beams were investigated. The study was conducted in-vitro on HeLa cells using AuNPs of 5 nm and 15 nm sizes. The cells samples were incubated with AuNPs and irradiated with photon beam of energy 6 MV and 10 MV at 100 cm SSD and 10 cm x 10 cm field size. Clonogenic assay were performed to observe the dose enhancement effects on cell survival. Dose enhancement factor (DEF) were extrapolated and evaluated from the cell survival curves. The results show that both sizes of AuNPs produce dose enhancement with the larger size AuNPs of 15 nm produce more dose enhancement compare to 5 nm AuNPs for 6 MV photon beam. Dose enhancements were observed for 10 MV photon beams but DEF for both sizes AuNPs shows no differences. In conclusion, larger size AuNPs produce higher dose enhancement compare to small size of AuNPs which conclude that nanoparticles size is important factor that need to be taken into account for AuNPs to be applied in radiotherapy.

    The purpose of the study was to determine the effect of out-of-field photon beams radiotherapy to the cancer cell survival. In this study, HeLa and T24 cancer cells were irradiated with 6 MV and 10 MV photon beams in two different conditions, one with intercellular communication with the in-field cell and one without the communication. Cells survival was determined by clonogenic assay. In the presence of intercellular communication, the cell death was increased which indicate the presence of radiation induced bystander effects (RIBE). The effects were also dependent on the cell types and photon energy where the HeLa cells exhibit less survival compares to T24 cells and the effects were prominent at higher photon energy. This study demonstrates that the out-of-field cells in conjunction with RIBE plays important roles in the cells response towards megavoltage photon beam radiation therapy.

    Anammox bacteria can easily undergo starvation due to fluctuations in feed flowrate and concentration in wastewater treatment plants. In this study, we analyzed the effects of different types of storage conditions (presence of ammonium (Ra), nitrite (Rn), hydrazine (Rh), and no substrate (Rc)) in aiding the viability of anammox bacteria during starvation and recovery. After starvation, the bacteria were subjected to a 15-week recovery period. Anammox bacteria showed better results during starvation and recovery in Rh as compared to other conditions. Decay rate values obtained after starvation in Ra, Rn, Rh, and Rc were 0.032/day, 0.042/day, 0.019/day, and 0.037/day, respectively. Meanwhile, µmax values obtained in Rh, Ra, Rn, and Rc on the 15th week of recovery were 0.092, 0.075, 0.011, and 0.067 d-1, respectively. This indicated that the availability of hydrazine helps to reduce the mortality rate of anammox bacteria during starvation and enhances the recovery of anammox process.

    AIM: Endoscopic endonasal nasopharyngectomy (EEN) for recurrent nasopharyngeal carcinoma (rNPC) is being increasingly used due to the added high magnification, reduced morbidities associated with open procedures and good survival outcomes. Most studies looked at usage of EEN in patients with lower recurrent staging (rT1 and rT2) although more and more surgeons are studying the outcome of EEN in advanced rNPC (rT3 and rT4). The aims of this study were to report the long-term 5-year survival outcome of EEN performed in patients with advanced rNPC, and to determine any prognostic factors for patients' survival.

    METHODS: All patients who underwent EEN for advanced rNPC between January 2003 and December 2015 inclusive were analyzed. All surgeries were performed in University Malaya Medical Centre in Kuala Lumpur and Queen Elizabeth Hospital in Sabah. We reported the 5-year overall survival (OS), disease-free survival (DFS) and disease-specific survival (DSS) and any related complications and significant prognostic factors.

    RESULTS: Twelve patients with rNPC (2 rT3 and 10 rT4) were followed-up over a mean duration of 44.8 months (range, 40-440 weeks). The 5-year OS was 50.0% (mean 44.75 months), DFS was 25.0% (mean 35.25 months) and the DSS was 58.3% (mean 43.33 months). No severe operative complications were encountered and no independent prognostic factors for survival outcome were identified.

    CONCLUSION: This is the first report in English that exclusively described the long-term 5-year survival data in patients with both rT3 and rT4 recurrent NPC after EEN. The data suggest that EEN is a feasible treatment to improve survival with minimal morbidities in patients with rT3 and rT4 recurrent NPC. However, more studies with larger patient size is recommended.

    This paper discussed on the effectiveness of BAP and NAA growth hormones on establishment of plant regeneration for selected ornamentals; Agapanthus praecox, Justicia betonica and Celosia cristata. Various explants (leaf, stem, shoot tip and bulb) derived from one-month-old aseptic seedlings of A. praecox and C. cristata, as well as explants from intact plants of J. betonica were utilized to achieve complete plant regeneration of these species. MS medium supplemented with various hormones, with an emphasis on BAP and NAA was tested to obtain direct and indirect regeneration. Both A. praecox (bulbs) and C. cristata (shoots) formed complete plantlets on MS added with 0.5-2.0 mg/L BAP and NAA, while direct regeneration was achieved for J. betonica on MS media containing BAP. Several methods were attempted to acclimatize the regenerants, with A. praecox gave the highest percentage of survival rates (96.67%), followed by J. betonica (80.00%) and C. cristata (75.00%).

    AIMS: Agro-based wastes were evaluated as a medium for mass micropropagule production and optimal efficacy of Trichoderma asperellum B1092 in controlling Fusarium oxysporum f. sp. lycopersici and promoting tomato growth. This study focused on biological control because pathogen persistence in the soil makes the disease difficult to control.

    METHODS AND RESULTS: Rice bran, biochar, empty fruit bunches, coconut fibres, compost, top soil and mixed soil were evaluated as media for mass multiplication of T. asperellum, which is effective in controlling plant pathogens. Yielding the most colony forming units (CFU) among the media, coconut fibre was deemed most suitable for promoting sporulation. After 120 days on the medium, T. asperellum B1902 produced 9·053 × 105  CFU per gram coconut fibre; oil palm empty fruit bunches was second highest (7·406 × 105  CFU per gram). In field tests of T. asperellum B1092 against F. oxysporum f. sp lycopersici (causing Fusarium wilt of cherry tomato), B1092 significantly promoted plant growth compared to the control. The efficacy of this formulation resulted in increased growth of roots and shoots tomato plants and total lycopene, sugar, K, N, Ca, P and Mg content after 120 days.

    CONCLUSIONS: Trichoderma asperellum B1092 showed great field potential for improving productivity and quality of tomatoes and in controlling Fusarium wilt of cherry tomato.

    SIGNIFICANCE AND IMPACT OF THE STUDY: This innovative approach using a cheap agro-waste to control the persistent soil-borne Fusarium pathogen of cherry tomato should increase soil survival rate of Trichoderma and has potential for upscaling in the field for other crops.

    Activation of the synlpathetic nervous system (SNS) and renin-angiotensin system (RAS) plays a pivotal role in the pathophysiology and progression of the disease in chronic heart failure (CHF). Blocking the activation of the RAS with angiotensin converting enzyme inhibitors not only improves sylnptonls but also prolongs life in symptomatic CHF. Does a similar analogy hold true for the use of ß-blockers in CHF? Evidence from a number of small trials and several recent large prospective trials show that b-blockers may improve ventricular function and symptoms in CHF. In a combination of trials investigating the use of carvedilol (an α, and ß-blocker) in congestive heart failure a mortality benefit appears to be evident. There are still a number of key questions that relnaill unanswered regarding the tolerability, patient type and stage of CHF in which ß-blockers should be advocated. Several large scale trials are in progress to answer some of these questions and also to add further information regarding its efficacy and impact on survival.     A retrospective cohort study of 206 consecutive patients with colorectal cancer presenting to three general surgeons in the University Hospital, Kuala Lumpur over a 3-year period is reported. In all, 184 patients had an operation and are grouped according to whether their operation was within 24 hours of admission (n=16), more than 24 hours after admission (n=38), or elective (n=130). Operative mortalities for these groups were 15.9%,15.2°/o and 6.5%, respectively, significantly higher in both the emergency groups. Delayed surgery to allow complete resuscitation did not improve the operative mortality when compared with those patients having urgent surgery. Both groups of emergency patients, delayed (27%) and urgent (19%), showed poorer 5-year survival than the electively treated patients (36%), many dying of non-cancer causes. Patients who undergo emergency surgery for colorectal carcinoma are more l i k e l y to be in poorer physical condition than the patients undergoing elective surgery for the same condition. It appears that the physical status is the principal determinant of outcome after emergency colorectal surgery rather than any other factor. KEYWORDS: colorectal cancer, emergency, outcome

    Zingiber montanum rhizomes are traditionally used for the treatment of numerous human ailments. The present study was carried out to investigate the inhibitory activity of the crude extract, chromatographic fractions, and purified compounds from Z. montanum rhizomes on the migration of MDA-MB-231 cells. The effect of the extract on cell migration was investigated by a scratch assay, which showed significant inhibition in a concentration-dependent manner. Vacuum liquid chromatography on silica gel afforded four fractions (Frs. 1 - 4), which were tested on cell migration in the scratch assay. Frs. 1 and 2 showed the most significant inhibition of MDA-MB-231 cell migration. The effect of the most potent fraction (Fr. 2) was further confirmed in a transwell migration assay. The study of Frs. 1 and 2 by gelatin zymography showed significant inhibition of MMP-9 enzyme activity. Chromatographic separation of Frs. 1 and 2 afforded buddledone A (1: ), zerumbone (2: ), (2E,9E)-6-methoxy-2,9-humuradien-8-one (3: ), zerumbone epoxide (4: ), stigmasterol (5: ), and daucosterol (6: ). In a cell viability assay, compounds 1:  - 4: inhibited the viability of MDA-MB-231 cells in a concentration-dependent manner. The study of buddledone A (1: ) and zerumbone epoxide (4: ) on cell migration revealed that 4: significantly inhibited the migration of MDA-MB-231 cells in both scratch and transwell migration assays. The results of the present study may lead to further molecular studies behind the inhibitory activity of zerumbone epoxide (4: ) on cell migration and support the traditional use of Z. montanum rhizomes for the treatment of cancer.

    Prediction of survival probabilities based on models developed by other countries has shown inconsistent findings among Malaysian patients. This study aimed to develop predictive models for survival among women with breast cancer in Malaysia. A retrospective cohort study was conducted involving patients who were diagnosed between 2012 and 2016 in seven breast cancer centres, where their survival status was followed until 31 December 2021. A total of 13 predictors were selected to model five-year survival probabilities by applying Cox proportional hazards (PH), artificial neural networks (ANN), and decision tree (DT) classification analysis. The random-split dataset strategy was used to develop and measure the models' performance. Among 1006 patients, the majority were Malay, with ductal carcinoma, hormone-sensitive, HER2-negative, at T2-, N1-stage, without metastasis, received surgery and chemotherapy. The estimated five-year survival rate was 60.5% (95% CI: 57.6, 63.6). For Cox PH, the c-index was 0.82 for model derivation and 0.81 for validation. The model was well-calibrated. The Cox PH model outperformed the DT and ANN models in most performance indices, with the Cox PH model having the highest accuracy of 0.841. The accuracies of the DT and ANN models were 0.811 and 0.821, respectively. The Cox PH model is more useful for survival prediction in this study's setting.

    BACKGROUND: Mutations in the TP53 gene are the most common among genetic alterations in human cancers, resulting in the formation of mutant p53 protein (mutp53). Mutp53 promotes proliferation, migration, invasion, and metastasis in cancer cells. Not only does the initiation of oncogenesis ensue due to mutp53, but resistance towards chemotherapy and radiotherapy in cancer cells also occurs. This review aims to summarise and discuss the oncogenesis of mutant p53 in cancer cells and introduce the various mutant p53 inhibitors currently being evaluated at the pre-clinical and clinical stages. Compounds that induce the wild-type conformation on the targeted p53 missense mutation, restore or enhance the DNA binding of mutant p53, and inhibit cancer cells' growth are highlighted. In addition, the progression and development of the mutant p53 inhibitors in clinical trials are updated.

    CONCLUSION: The progress of developing a cancer treatment that may successfully and efficiently target mutant p53 is on the verge of development. Mutant p53 proteins not only initiate oncogenesis but also cause resistance in cancer cells to certain chemo or radiotherapies, further endorse cancer cell survival and promote migration as well as metastasis of cancerous cells. With this regard, many mutant p53 inhibitors have been developed, some of which are currently being evaluated at the pre-clinical level and have been identified and discussed. To date, APR-246 is the most prominent one that has progressed to the Phase III clinical trial.

    Mark-release-recapture experiments were undertaken in January 1989, in Pos Betau, Pahang, Malaysia, with the malaria vector Anopheles maculatus. On two consecutive nights, 121 and 175 blood-fed mosquitos were released. A mean recapture rate of 11.5% and survival rates of 0.699-0.705 with an estimated oviposition cycle period of 2.35 days were obtained from the releases. About 68% of all recaptures were taken within a distance of 0.5 km from their release points and the longest detected flight was 1.6 km. No heterogeneity was found between indoor and outdoor biters of An. maculatus.

    BACKGROUND: Liver transplantation (LT) offers patients with decompensated cirrhosis the best chance at long-term survival. With the rising prevalence of diabetes, further clarity is needed on the impact of receiving a liver allograft from a donor with diabetes on post-LT outcomes. This study aims to evaluate the impact of donor diabetes on clinical outcomes after LT.

    METHODS: This is a retrospective analysis of the United Network for Organ Sharing registry data of LT recipients from January 1, 2000, to December 31, 2021. Outcomes analysis was performed using Cox proportional model for all-cause mortality and graft failure. Confounding was reduced by coarsened exact matching causal inference analysis.

    RESULTS: Of 66 960 donors identified, 7178 (10.7%) had diabetes. Trend analysis revealed a longitudinal increase in the prevalence of donor diabetes ( P
