    The incidence of neurenteric cyst (NC) is rare, accounting for 0.3%-1.3% of all spine tumors. The occurrence of quadriplegia caused by NC is even scarcer. Herein we report on a young girl with a rare NC over the C2-C5 spinal cord, which led to the morbidity of permanent quadriplegia despite early surgical intervention. This case highlights the rare morbidity of cervical cord NC presenting with permanent quadriplegia that failed to respond despite early surgical excision.

    Parkinson's disease (PD) is the second most common neurodegenerative disorder. It is usually seen in those above 50 years old. Current medical treatments only provide symptomatic relief but cannot cure the disease. There are claims that PD can be cured by stem cell transplant. The present study is aimed to assess the clinical potency and safety of stem cell in treating PD. A total of eleven articles were included for analysis, with four randomised control trials (RCTs), five non-RCTs and 2 follow up studies. All the four non-RCTs showed improvement of Unified Parkinson's Disease Rating Scale with no adverse events. However, results from RCTs showed no significant differences in the rating score among the transplant group and the Sham surgery group. The secondary analysis of one study showed a significant improvement of the rating score in those patients aged 60 and younger. Transplant group also associated with an overall higher incidence of adverse events. In conclusion, the RCTs and non-RCTs produced opposite results. When the studies were performed as non-RCTs in small number of patients, they showed promising result in the patients. It could say that currently the use of stem cell/progenitor cells in treating PD need much research despite having the implanted stem cell to be able to survive and integrated. The survival of implanted dopamine neurons in the striatum, however, does not indicate a success in correcting PD symptoms. Further investigations will shed light on the application and mechanism of action of stem cells in treating PD.

    AIM: The 36-month Long-Term Research Grant Scheme project: Towards Usual Aging - Neuroprotective Model for Healthy Longevity among Malaysian Elderly was designed to address multidimensional aspects including psychosocial, biophysical health, nutrition and dietary pattern, and auditory and visual function to highlight the magnitude of these associations in a single study.

    METHODS: A total of 2322 respondents aged ≥60 years were recruited at baseline using the multistage sampling method, followed up at 18 months and 36 months.

    RESULTS: Response rates at baseline, 18 months and 36 months were 87.8%, 77.3% and 67.1%, respectively. At baseline, the prevalence of successful aging, usual aging and mild cognitive impairment was 11%, 73% and 16%, respectively. The prevalence of single and multimorbidity at baseline were 25.9% and 50.3%, respectively. The incidence rates of mild cognitive impairment at 18 months and 36 months were 6.5 and 5.6 per 100 person-years. The incidence rates of multimorbidity at 18 months and 36 months were 23.7 and 21.5 per 100 person-years, respectively.

    CONCLUSIONS: The Long-Term Research Grant Scheme project: Towards Usual Aging study provides an opportunity to investigate the interactions between wide ranges of aspects of the older population in a nationally representative sample of the older population.     Aims: Hip fracture is frequently studied to reflect osteoporosis related fractures. The aim of this study was to look at ethnic differences in the incidence of hip fractures between the three races in Malaysia. Methods: This was a retrospective study performed for the years 1996 and 1997. Data from hospitals treating hip fractures in elderly patients were collected throughout the whole country. Result: The overall incidence of hip fractures was 90 per 100 000 individuals. Sixty-three percent of patients presenting with hip fractures were Chinese. This was followed by Malays at 20% and Indians, 13%. Race-specific incidence data showed that the fracture rates are highest among the Chinese (160 per 100 000) followed by Indians (150 per 100 000) and Malays (30 per 100 000). Females were twice as commonly affected compared to males. Race and sex-specific incidence data showed that the incidence was highest among Chinese females (220 per 100 000), followed by Indian females (200 per 100 000). The age-specific incidence was 500 per 100 000 for patients above 75 years, compared to 10 per 100 000 in those between 50 and 54 years. Conclusion: Hip fractures arecommon in Malaysia.     Labial and buccal gingival recession is a common finding in the adult population, which can occur due to various factors. When present, it can lead to an unesthetic appearance and can cause hypersensitivity. However, very limited literature is available regarding the incidence of palatal gingival recession. Many times it goes unnoticed during routine clinical examinations. Although there are no esthetic concerns in these regions, recession can cause sensitivity and pose a technical challenge in surgical correction because of poor accessibility and visibility. This case reports the treatment of a single palatal recession defect using an epithelial embossed connective tissue graft. The 12-month follow-up of the case showed significant root coverage (93.6%) and decreased sensitivity. The advantage of the harvested graft was that the connective tissue was not exposed, as it was tucked into the underlying flap, and the mucosal contour was maintained.

    INTRODUCTION: Overnight transfusion (OT) is the blood transfusion taking place from 9pm to 8am. During this period, patients are exposed to increased risk of errors. This cross-sectional study aims to determine the incidence and practice of OT in Universiti Kebangsaan Malaysia Medical Centre.

    MATERIALS & METHODS: Data from all OT in June and mid-July 2017 were collected from recipients' cards, transfusion request forms and patient's case files, regarding discipline involved, indications, time intervals from request of blood transfusion to the completion of OT on patients, monitoring of patients and adverse reactions.

    RESULTS: A total of 1285 transfusion cases were identified during the study period. 216 (16.8%) cases were OT while the 1069 (83.2%) cases were non-OT. Surgery discipline has the highest (30.1%) OT. The indications of OT were acute clinical need: 82.9%, less acute clinical need: 13.9% and no clinical need: 3.2%. A huge delay (average: 5 hours 40 minutes) in starting transfusion after grouping and crossmatching (GXM) completion was noted. Besides, 25.9% cases took <4 hours to complete OT; 83.4% cases did not have proper transfusion monitoring and three transfusion reactions were reported.

    DISCUSSION: Although most of the OT cases had appropriate clinical indications, the transfusion can be commenced earlier at day time rather than overnight. Cases without absolute indication should avoid OT. The poor monitoring of patient during OT had posed risks to patients' life if an adverse transfusion reaction happened. The major reason for OTs was a huge delay in starting transfusion after the GXM completion. The contravention of 4-hour infusion rule increased the patients' risk of developing bacterial sepsis. The practice of OT should be discouraged wherever possible except for clinically indicated cases.

    Collar rot of chili (Capsicum annuum L.) is a very destructive disease caused by a soil-borne fungal pathogen Sclerotium rolfsii Sacc. Generally, chemical fungicides are used to combat the menace but this practice is being discouraged because of health and environmental concerns. In the present study, an alternative environment friendly strategy was used to manage this disease by using farmyard manure (FYM) and two commercial biofertilizers namely Biopower and Feng Shou. S. rolfsii inoculated pot soil was amended with 1% and 2% FYM and the two commercial biofertilizers. Inoculation of soil with S. rolfsii only (positive control) resulted in the highest disease incidence (73%) and plant mortality (60%). Biopower and Feng Shou application reduced disease incidence to 20% and 7%, respectively and plant mortality to 0%. Likewise, 1% and 2% FYM amendment reduced disease incidence to 33% and plant mortality to 26% and 7%, respectively. Under biotic stress of S. rolfsii, FYM and biofertilizers applications, either alone or in combination, significantly enhanced root and shoot growth over positive control. S. rolfsii inoculation significantly increased peroxidase and polyphenol oxidase activities in chili plants which were further increased by application of either of the two biofertilizers. The present study concludes that biofertilizers Biopower and Feng Shou alone or in combination with 2% FYM can be effectively utilized to manage southern blight of chili.

    The aim of the study is to assess and compare the impact of antiviral drug alone and in combination with antibiotic for prevention of Influenza-A H1N1 induced acute kidney injury (AKI) in hospitalized patients. Hospitalized admitted patients with confirmed diagnosis of Influenza-A H1N1 infection were divided into two groups: group 1, which received antiviral (oseltamivir) drug alone and group 2, which received antiviral (oseltamivir) in combination with empirically prescribed antibiotic. Patients of both groups were assessed for incidences of AKI by two criteria i.e Acute Kidney Injury Network (AKIN) and RIFLE. A total of 329 patients (176 for group 1 and 153 for group 2) were enrolled. According to RIFLE criteria, 23(13%) of group 1 and 9(6%) patients of groups 2 were suffered from AKI with statistically significant difference (P<0.05). Also as per AKIN criteria, the incidence of AKI is statistically significantly difference (P<0.05) between both groups with 18(10%) patients and 6(4%) patients of group 1 and 2 respectively. Length of hospitalization was statistically less (P<0.05) in group 2 patients. The incidences of AKI in Influenza-A H1N1 treated with antiviral and antibiotic combination was statistically less as compared to patients who were given antiviral alone for treatment of influenza infection.

    BACKGROUND: A surgical audit study among Batu Pahat population was conducted in determining the commonest position of appendix in post appendectomy.

    METHODOLOGY: This is a retrospective study. A total of 204 cases of patients underwent an appendectomy admitted to the surgical ward from January 2017 until January 2018 at Hospital Sultanah Nora Ismail (HSNI) were audited retrospectively.

    RESULTS: This findings showed different figures of ascendancy in gender among patients who underwent an appendectomy with females 58.8% and males 41.2%. The perforation rate was 40.7% and delay in diagnosis was found to be 19.1%. The perforated appendix had a significantly higher incidence in males with a correlation of p-value 0.04. Retrocaecal appendix (RA) remained the commonest position for patients who underwent an appendectomy with 26.9%. RA is associated with an increased incidence of perforation (p-value 0.01).

    CONCLUSION: The position of appendix in our patients who underwent an appendectomy is parallel to the reports available globally in that it is retrocaecal followed by retroileal as the commonest position among residence of Batu Pahat.

    Dengue fever (DF) is a global health problem and considered to be endemic in Malaysia. Conventional mosquito traps currently applied as vector control do not effectively reduce Aedes mosquito population. AedesTech Mosquito Home System (AMHS) is an autocidal ovitraps for Aedes mosquitoes that uses the ‘lure and kill’ concept and is expected to be able to reduce Aedes mosquito population. The effectiveness of AMHS in reducing Aedes mosquito population was investigated in Block A, B and D (control) of the 17th College, Universiti Putra Malaysia (UPM). For the first two weeks (pre-intervention), the conventional ovitraps were used to obtain the initial abundance of mosquito population in Block A, B and D. Subsequently, AMHS was used for the next three months and again followed by the conventional ovitrap for the final two weeks (post-intervention). Ovitrap Index, Hatching Index and percentage of emergence of adult mosquitoes were calculated once every two weeks. Data were analysed using Paired Sample T-test. Values were considered significant at p≤0.05. The Ovitrap Index that indicates the mosquito population at Block A and B was significantly higher (p≤0.05) than of Block D. Hatching Index of AMHS was significantly lower (p≤0.05) then conventional ovitraps. All mosquito eggs collected in AMHS did not develop into adult mosquitoes. There was a significant reduction (p≤0.05) in the mosquito population between the pre- and post-intervention. In conclusion, AMHS was effective in reducing the mosquito population in 17th College, UPM. Therefore, it is believed to be a very promising vector management option to control the incidence of DF.

    Introduction: Aedes albopictus is known for its aggressiveness towards human and recently expanded to more coun- tries outside the native regions. Thus, the demographic parameters of Aedes albopictus are important to determine the characteristics of this species mosquitoes in terms of the reproduction rates and dispersal distance. Materials and Methods: This study, was performed using a Shah Alam strain of Aedes albopictus originally collected in twenty district areas of the central zone of Shah Alam. This research applies field work-study with a cross-sectional design to investigate the demographic parameters of Aedes albopictus. The demographic evaluation of Aedes albopictus was conducted under the control environment in insectarium. Results: Investigation on the demographic parameters of Aedes albopictus clearly showed that there is a significance different observed in the total number of mosquito eggs produced in both high and low incidence rate IR areas (p=0.03). In contrast, other parameters showed insignificant value between high and low IR areas. Conclusion: The key to control the mosquito vectors population is by tracking the vector’s life cycle including its survival. Therefore, the outcome of this study may provide as a baseline to esti- mate the dengue outbreak in the current episystem.

    Introduction: Altered weather patterns and changes in precipitation, temperature and humidity resulting
    from climate change could affect the distribution and incidence of cholera. This study is to quantify climateinduced increase in morbidity rates of cholera. Material and Methods: Monthly cholera cases and monthly
    temperature, precipitation, and relative humidity data from 2004 to 2014 were obtained from the Malaysian
    Ministry of Health and Malaysian Meteorological Department, respectively. Poisson generalized linear models
    were developed to quantify the relationship between meteorological parameters and the number of reported
    cholera cases. Results: The findings revealed that the total number of cholera cases in Malaysia during the 11
    year study period was 3841 cases with 32 deaths. Out of these, 45.1% of the cases were among children below
    12 years old and 75% of the cases were from Sabah. Temperature and precipitation gave significant impact on

    The incidence of rectal dissection is increasing with the rise of rectal cancer all over the world. This
    technique has been used regularly to provide a reasonable quality of life for patients. The concern raised
    was the functions of these neorectum to replace the original rectum. Several configurations have been
    suggested namely the straight end-to-end coloanal anastomosis, side-to-end coloanal anastomosis, colonic Jpouch and the transverse coloplasty to suit the anatomy of the deep pelvis. Apparently, there was no
    difference in terms of functional outcome among all these four types of reconstructions. However, these
    configurations have seen the emergence of anterior resection syndrome or a pragmatic defaecatory
    dysfunction as their complications. Furthermore, the neorectum functions are affected by many other
    factors such as preoperative chemoradiotherapy, septic complications, and others more.

    Colorectal cancer is the third most common cancer in the world with increasing incidence and mortality rates globally. Standard treatments for colorectal cancer have always been surgery, chemotherapy and radiotherapy which may be used in combination to treat patients. However, these treatments have many side effects due to their non-specificity and cytotoxicity toward any cells including normal cells that are growing and dividing. Furthermore, many patients succumb to relapse even after a series of treatments. Thus, it is crucial to have more alternative and effective treatments to treat CRC patients. Immunotherapy is one of the new alternatives in cancer treatment. The strategy is to utilize patients' own immune systems in combating the cancer cells. Cancer immunotherapy overcomes the issue of specificity which is the major problem in chemotherapy and radiotherapy. The normal cells with no cancer antigens are not affected. The outcomes of some cancer immunotherapy have been astonishing in some cases, but some which rely on the status of patients' own immune systems are not. Those patients who responded well to cancer immunotherapy have a better prognostic and better quality of life.

    BACKGROUND: While football injury and illness epidemiology surveillance at professional club level in Europe is available, epidemiological data from other continents are lacking.

    PURPOSE: Investigating injury and illness epidemiology in professional Asian football.

    STUDY DESIGN: Descriptive prospective study.

    METHODS: Professional teams from the Asian Football Confederation (AFC) league were followed prospectively for three consecutive AFC seasons (2017 through 2019, 13 teams per season, 322 team months). Time-loss injuries and illnesses in addition to individual match and training exposure were recorded using standardised digital tools in accordance with international consensus procedures.

    RESULTS: In total, 232 665 hours of exposure (88.6% training and 11.4% matches) and 1159 injuries were recorded; 496 (42.8%) occurred during matches, 610 (52.6%) during training; 32 (2.8%) were reported as 'not applicable' and for 21 injuries (1.8%) information was missing. Injury incidence was significantly greater during match play (19.2±8.6 injuries per 1000 hours) than training (2.8±1.4, p<0.0001), resulting in a low overall incidence of 5.1±2.2.The injury burden for match injuries was greater than from training injuries (456±336 days per 1000 hours vs 54±34 days, p<0.0001). The two specific injuries causing the greatest burden were complete ACL ruptures (0.14 injuries (95% CI 0.9 to 0.19) and 29.8 days lost (29.1 to 30.5) per 1000 hours) and hamstring strains (0.86 injuries (0.74 to 0.99) and 17.5 days (17.0 to 18.1) lost per 1000 hours).Reinjuries constituted 9.9% of all injuries. Index injuries caused 22.6±40.8 days of absence compared with 25.1±39 for reinjuries (p=0.62). The 175 illnesses recorded resulted in 1.4±2.9 days of time loss per team per month.

    CONCLUSION: Professional Asian football is characterised by an overall injury incidence similar to that reported from Europe, but with a high rate of ACL ruptures and hamstring injury, warranting further investigations.

    Tomato (Solanum lycopersicon L.) fruit is rich in various nutrients, vitamins and health-promoting molecules. Fresh tomatoes are an important part of the Mediterranean gastronomy, and their consumption is thought to contribute substantially to the reduced incidence of some chronic diseases in the Mediterranean populations in comparison with those of other world areas. Unfortunately, tomato fruit is highly perishable, resulting in important economic losses and posing a challenge to storage, logistic and supply management. This review summarizes the current knowledge on some important health-promoting and eating quality traits of tomato fruits after harvest and highlights the existence of substantial cultivar-to-cultivar variation in the postharvest evolution of the considered traits according to maturity stage at harvest and in response to postharvest manipulations. It also suggests the need for adapting postharvest procedures to the characteristics of each particular genotype to preserve the optimal quality of the fresh product.

    Peroneal tendon tear is a relatively common cause of lateral ankle pain but often missed due to mixed presentation or low index of suspicion. Left untreated, peroneal injuries can lead to persistent ankle pain, instability and ultimately substantial functional disabilities. An isolated peroneus longus tear is rare with the lowest incidence rate compared to isolated peroneus brevis tear and mixed tear of both peroneal tendon. This is a case report of a 49-year-old lady with a chronic left ankle pain who ultimately underwent surgery for an isolated peroneus longus tear.

    Cardiovascular disease has been the premier cause of hospital-registered deaths in Malaysia for the past 8 years. Among these reported deaths, 31% were caused by coronary heart disease in 1982. A healthy lifestyle to control the coronary risk factors would help to reduce the incidence of coronary heart disease in future.

    Cough associated with angiotensin converting-enzyme (ACE) inhibitor therapy has been reported in Western communities, where its incidence is disputed. We, therefore, reviewed our patients who were treated with captopril primarily for congestive cardiac failure. 19 of 61 patients (31.1%) receiving an ACE inhibitor had volunteered cough as an important adverse effect, compared with only one of 59 patients who recieved other treatment. In 3 patients, the cough was intolerably severe to require discontinuation of treatment. Another 7 patients were withdrawn from ACE inhibitor treatment because of other adverse effects including deteriorating renal function, insomnia, dizzy spells, ageusia and proteinuria, and skin rash. We suggest that although ACE inhibitors are very beneficial in improving the clinical status of congestive heart failure, intolerable adverse effects are not uncommon and might mandate withdrawal in a sizeable minority (16.4%).
