    This study aimed to highlight the COVID-19 response by the Ministry of Health (MOH) and the Government of Malaysia in order to share Malaysia's lessons and to improve future pandemic preparedness. The team conducted a rapid review using publicly available information from MOH, PubMed, and World Health Organisation (WHO) Global Research on Coronavirus Disease Database to compile Malaysia's responses during the COVID-19 pandemic. Measures taken between 31 December 2019 and 3 June 2020 were classified into domains as well as the pillars described in the WHO COVID-19 Strategic Preparedness and Response Plan (WHO SPRP). Malaysia's response incorporated all pillars in the WHO SPRP and consisted of five domains, (i) whole-of-government, (ii) cordon sanitaire/lockdown, (iii) equity of access to services and supports, (iv) quarantine and isolation systems, and (v) legislation and enforcement. Some crucial measures taken were activation of a centralised multi-ministerial coordination council where MOH acted as an advisor, with collaboration from non-government organisations and private sectors which enabled an effective targeted screening approach, provision of subsidised COVID-19 treatment and screening, isolation or quarantine of all confirmed cases, close contacts and persons under investigation, with all strategies applied irrespective of citizenship. This was provided for by way of the Prevention and Control of Infectious Diseases Act 1988. A combination of these measures enabled the nation to contain the COVID-19 outbreak by the end of June 2020.

    Few studies to date have evaluated dementia care in Malaysia, and the focus of studies has primarily been on epidemiological and laboratory research. In this study, we aimed to identify potential challenges for the delivery of dementia care in Malaysia and priorities for research and enhancing existing dementia care. This study used thematic analysis to evaluate the open and focus group workshop discussions guided by semi-structured questions. Triangulation of the collected data (sticky notes, collated field notes, and transcripts of discussions) was achieved through stakeholder consensus agreement during a workshop held in 2017. Five main themes as priorities for dementia care were identified: (1) availability of a valued multi-disciplinary care service, (2) accessibility of training to provide awareness, (3) the functionality of the governance in establishing regulation and policy to empower care services, (4) perceived availability and accessibility of research data, and (5) influence of cultural uniqueness. The findings of this study seek to enhance existing dementia care in Malaysia but have potential application for other low and middle-income countries with a similar social and health care set up. The constructed relationship between themes also tries to tackle the challenges in a more efficient and effective manner, as none of these aforementioned issues are standalone challenges. In addition, we demonstrated how a carefully constructed workshop with defined aims and objectives can provide a useful analysis tool to evaluate health and social care challenges in a multidisciplinary forum.

    BACKGROUND: Climate change poses a real challenge and has contributed to causing the emergence and re-emergence of many communicable diseases of public health importance. Here, we reviewed scientific studies on the relationship between meteorological factors and the occurrence of dengue, malaria, cholera, and leptospirosis, and synthesized the key findings on communicable disease projection in the event of global warming.

    METHOD: This systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 flow checklist. Four databases (Web of Science, Ovid MEDLINE, Scopus, EBSCOhost) were searched for articles published from 2005 to 2020. The eligible articles were evaluated using a modified scale of a checklist designed for assessing the quality of ecological studies.

    RESULTS: A total of 38 studies were included in the review. Precipitation and temperature were most frequently associated with the selected climate-sensitive communicable diseases. A climate change scenario simulation projected that dengue, malaria, and cholera incidence would increase based on regional climate responses.

    CONCLUSION: Precipitation and temperature are important meteorological factors that influence the incidence of climate-sensitive communicable diseases. Future studies need to consider more determinants affecting precipitation and temperature fluctuations for better simulation and prediction of the incidence of climate-sensitive communicable diseases.

    Caries is one of the non-communicable diseases that has a high prevalence trend. The current methods used to detect caries require sophisticated laboratory equipment, professional inspection, and expensive equipment such as X-ray imaging device. A non-invasive and economical method is required to substitute the conventional methods for the detection of caries. UV absorption spectroscopy coupled with chemometrics analysis has emerged as a good potential candidate for such an application. Data preprocessing methods such as mean centre, autoscale and Savitzky-Golay smoothing were implemented to enhance the signal-to-noise ratio of spectra data. Various classification algorithms namely K-nearest neighbours (KNN), logistic regression (LR) and linear discriminant analysis (LDA) were implemented to classify the severity of dental caries into International Caries Detection and Assessment System (ICDAS) scores. The performance of the prediction model was measured and comparatively analysed based on the accuracy, precision, sensitivity, and specificity. The LDA algorithm combined with the Savitzky-Golay preprocessing method had shown the best result with respect to the validation data accuracy, precision, sensitivity and specificity, where each had values of 0.90, 1.00, 0.86 and 1.00 respectively. The area under the curve of the ROC plot computed for the LDA algorithm was 0.95, which indicated that the prediction algorithm was capable of differentiating normal and caries teeth excellently.

    Bilimbi fruit is widely eaten in Malaysia. Rarely reported is its potential to cause acute kidney injury (AKI) in patients with prior normal renal function. The possible dangers of its consumption are still relatively unknown among many. This case highlights the importance of taking a thorough dietary history in patients with AKI. We also hope to increase awareness among healthcare professionals on the nephrotoxic and neurotoxic effects of bilimbi fruits.

    The Asia and Oceania Society for Comparative Endocrinology (AOSCE) was founded in 1987, when the first congress was held in Nagoya, Japan. The purpose of the AOSCE is to progress scientific activities in the field of comparative endocrinology in Asia and Oceania and to establish a deep relationship among the members. For this purpose, the AOSCE holds a congress or an intercongress symposium every 2 years, which organizes an attractive scientific program covering the latest progress in the broad aspect of comparative endocrinology. 2012 was the 25th anniversary of AOSCE. Our scientific activities have increased dramatically during the past 25 years. The 7th AOSCE congress was held in Kuala Lumpur, Malaysia in 2012. The theme of this congress was "Overcoming challenges in the 21st century". To overcome challenges in the 21st century, we further need to create new research directions in comparative endocrinology from Asia and Oceania. This paper describes a brief history of the AOSCE and also highlights the discovery of gonadotropin-inhibitory hormone (GnIH) and the progress of GnIH research as one of new research directions in comparative endocrinology. In 2000, GnIH was discovered in Japan and now more than 50 laboratories are working on GnIH in the world. The discovery of GnIH has changed our understanding about regulation of the reproductive axis drastically in the past decade.

    Landfill leachate can threaten the environment and human life. Therefore, this study aims to investigate the efficiency of ozone (O3 ), O3 with zirconium tetrachloride (O3 /ZrCl4 ), and O3 with tin tetrachloride (O3 /SnCl4 ) in remediating the stabilized anaerobic landfill leachate (SAL) from Alor Pongsu, Perak. Hydroxyl radical (OH•) is an important oxidizing agent in the ozonation process. Its presence was tested using tert-butyl alcohol. Results showed that using ZrCl4 and SnCl4 in ozonation boosted the generation of hydroxyl radical, thereby enhancing the oxidation process and pollutant removal inside the sample. The O3 /ZrCl4 mix at chemical oxygen demand (COD) to ZrCl4 ratio of 1:1.5, pH 8-9, and 90-min reaction time resulted in the highest reduction rates of COD and color at 91.9% and 99.6%, respectively. All results demonstrated that the optimum performance occurred at alkaline conditions (pH > 8), proving that OH radicals primarily oxidized the pollutants through an indirect reaction pathway. The biodegradability (biochemical oxygen demand/COD) ratio was also considerably improved from 0.02 (raw) to 0.37 using O3 /ZrCl4 , compared with using O3 alone and using O3 /SnCl4 , which only recorded 0.23 and 0.28, respectively, after the treatment. The study demonstrated that O3 /ZrCl4 was the most efficient combination. PRACTITIONER POINTS: The O3 /ZrCl4 recorded the highest COD and color removals. The O3 /ZrCl4 combination also recorded higher OH• concentrations. The biodegradability of leachate (BOD5 /COD ratio) improved from 0.02 to 0.37.

    Coronary artery disease (CAD) is an important cause of mortality across the globe. Early risk prediction of CAD would be able to reduce the death rate by allowing early and targeted treatments. In healthcare, some studies applied data mining techniques and machine learning algorithms on the risk prediction of CAD using patient data collected by hospitals and medical centers. However, most of these studies used all the attributes in the datasets which might reduce the performance of prediction models due to data redundancy. The objective of this research is to identify significant features to build models for predicting the risk level of patients with CAD. In this research, significant features were selected using three methods (i.e., Chi-squared test, recursive feature elimination, and Embedded Decision Tree). Synthetic Minority Over-sampling Technique (SMOTE) oversampling technique was implemented to address the imbalanced dataset issue. The prediction models were built based on the identified significant features and eight machine learning algorithms, utilizing Acute Coronary Syndrome (ACS) datasets provided by National Cardiovascular Disease Database (NCVD) Malaysia. The prediction models were evaluated and compared using six performance evaluation metrics, and the top-performing models have achieved AUC more than 90%. Graphical abstract.

    BACKGROUND: The uptake of risk-reducing salpingo-oophorectomy (RRSO) in Asian countries is variable despite being the most effective option for ovarian cancer risk reduction in BRCA mutation carriers. Exploration of factors which may impact the RRSO decision-making of BRCA mutation carriers from Malaysia, a developing country in Southeast Asia, was undertaken.

    METHODS: In-depth interviews with 28 Malaysian BRCA mutation carriers with a history of breast cancer were conducted in addition to observing their RRSO decision-making consultations in the clinic.

    RESULTS: The decision-making considerations among the carriers were centered around the overarching theme of "Negotiating cancer risk and womanhood priorities," with the following themes: (1) risk perception, (2) self-preservation, (3) motherhood obligation, and (4) the preciousness of marriage. Cognitive knowledge of BRCA risk was often conceptualized based on personal and family history of cancer, personal beliefs, and faith. Many women reported fears that RRSO would affect them physically and emotionally, worrying about the post-surgical impact on their motherhood responsibilities. Nevertheless, some reported feeling obliged to choose RRSO for the sake of their children. For some, their husband's support and approval were critical, with emotional well-being and sexuality reportedly perceived as important to sustaining married life. Despite reporting hesitancy toward RRSO, women's decisions about choosing this option evolved as their priorities changed at different stages of life.

    CONCLUSIONS: Recognizing during clinic encounters with Malaysian women that RRSO decision-making involves negotiating the likelihood of developing cancer with the societal priorities of being a woman, mother, and wife may serve to support their decision-making.

    The continuous growth of population and the steady improvement of people's living standards have accelerated the generation of massive food waste. Untreated food waste has great potential to harm the environment and human health due to bad odor release, bacterial leaching, and virus transmission. However, the application of traditional disposal techniques like composting, landfilling, animal feeding, and anaerobic digestion are difficult to ease the environmental burdens because of problems such as large land occupation, virus transmission, hazardous gas emissions, and poor efficiency. Pyrolysis is a practical and promising route to reduce the environmental burden by converting food waste into bioenergy. This paper aims to analyze the characteristics of food waste, introduce the production of biofuels from conventional and advanced pyrolysis of food waste, and provide a basis for scientific disposal and sustainable management of food waste. The review shows that co-pyrolysis and catalytic pyrolysis significantly impact the pyrolysis process and product characteristics. The addition of tire waste promotes the synthesis of hydrocarbons and inhibits the formation of oxygenated compounds efficiently. The application of calcium oxide (CaO) exhibits good performance in the increment of bio-oil yield and hydrocarbon content. Based on this literature review, pyrolysis can be considered as the optimal technique for dealing with food waste and producing valuable products.

    The ever-growing human population has resulted in the expansion of agricultural activity; evident by the deforestation of rainfoamrests as a means of acquiring fertile land for crops. The crops and fruits produced by such means should be utilized completely; however, there are still losses and under-exploitation of these produces which has resulted in wastes being mounted in landfills. These underutilized agricultural wastes including vegetables and fruits can serve as a potential source for biofuels and green diesel. This paper discusses the main routes (e.g., biological and thermochemical) for producing biofuels such as bioethanol, biodiesel, biogas, bio-oil and green diesel from underutilized crops by emphasizing recent technological innovations for improving biofuels and green diesel yields. The future prospects of a successful production of biofuels and green diesel by this source are also explained. Underutilized lignocelluloses including fruits and vegetables serve as a prospective biofuel and green diesel generation source for the future prosperity of the biofuel industry.

    OBJECTIVES: This systematic review aimed to investigate the effects if probiotics can inhibit oral carcinogenesis.

    DESIGN: PubMed, Web of Science, Scopus, and PLOS databases were searched up to February 2020 to identify randomised controlled trials that fulfilled the eligibility criteria. Joanna Briggs Institute (JBI) Critical Appraisal Tool was used for quality assessment of articles. This review was performed according to the Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA-P) 2015 protocol guidelines.

    RESULT: The initial search retrieved 774 articles. Of these, only five articles were included in the qualitative synthesis. Two out of the five papers were further analysed for quantitative synthesis in meta-analysis. The majority of the included studies were found to be of "moderate quality". The qualitative synthesis found four probiotics that exhibited potential therapeutic effects in oral carcinogenesis, includingAcetobacter syzygii, AJ2, Lactobacillus plantarum, and Lactobacillus salivarius REN.     This study has aimed to investigate the impact of sustainable project management on sustainable project planning and success in manufacturing firms. Data was collected from project management professionals in a manufacturing firm in Malaysia. A total of 231 responses were analyzed using the partial least square (PLS) method. The findings revealed that sustainable project management has a significant impact on sustainable project success and sustainable project planning. Sustainable project planning is positively correlated with sustainable project success. The results also indicated that sustainable project planning mediates the effect of sustainable project management on sustainable project success. The findings have significant insight into the body of knowledge of the project life cycle and indicated that sustainable project planning is a crucial tool attributed to project management towards the project success of the manufacturing firm. The results can be used as a guideline for organizations, providing direction in project management to achieve sustainable development for business.

    Recently, Bayesian brain-based models emerged as a possible composite of existing theories, providing an universal explanation of tinnitus phenomena. Yet, the involvement of multiple synergistic mechanisms complicates the identification of behavioral and physiological evidence. To overcome this, an empirically tested computational model could support the evaluation of theoretical hypotheses by intrinsically encompassing different mechanisms. The aim of this work was to develop a generative computational tinnitus perception model based on the Bayesian brain concept. The behavioral responses of 46 tinnitus subjects who underwent ten consecutive residual inhibition assessments were used for model fitting. Our model was able to replicate the behavioral responses during residual inhibition in our cohort (median linear correlation coefficient of 0.79). Using the same model, we simulated two additional tinnitus phenomena: residual excitation and occurrence of tinnitus in non-tinnitus subjects after sensory deprivation. In the simulations, the trajectories of the model were consistent with previously obtained behavioral and physiological observations. Our work introduces generative computational modeling to the research field of tinnitus. It has the potential to quantitatively link experimental observations to theoretical hypotheses and to support the search for neural signatures of tinnitus by finding correlates between the latent variables of the model and measured physiological data.

    Curcuma longa (Turmeric) is a tropical herbaceous perennial plant of the family Zingiberaceae and contains curcuminoids, sesquiterpenoids and monoterpenoids as its major components. Given the broad range of activities that Curcuma longa possesses and also its use as a traditional epilepsy remedy, this review attempts to systematically review the experimentally proven activities of Curcuma longa and its bioactive components, which are related to the management of epileptic seizures. Using the PRISMA model, five databases (Google Scholar, PubMed, ScienceDirect, SCOPUS and SpringerLink) were searched using the keywords ["Curcuma longa" AND "Epilepsy"] and ["Curcuma longa" AND "Seizures"], leaving 34 articles that met the inclusion criteria. The present systematic review elaborated on the experimentally proven potential of Curcuma longa components, such as an aqueous extract of Curcuma longa itself, Curcuma longa oil and active constituents like curcuminoids and bisabolene sesquiterpenoids found in Curcuma longa with anti-seizure potential. Using human equivalent dose calculations, human treatment parameters were suggested for each component by analysing the various studies in this review. This review also determined that the principal components possibly exert their anti-seizure effect via the reduction of corticosterone, modulation of neurotransmitters signalling, modulation of sodium ion channels, reduction of oxidative DNA damage, reduction of lipid peroxidation, upgregulation of brain-derived neurotrophic factor (BDNF) and γ-aminobutyric acid (GABA) mediated inhibition. It is anticipated that this review will help pave the way for future research into the development of Curcuma longa and its neuroactive constituents as potential drug candidates for the management of epilepsy.

    BACKGROUND: Increased intra-abdominal pressure (IAP) is seen in patients after congenital diaphragmatic hernia (CDH) repair owing to reduction of thoracic contents into the relatively smaller abdominal cavity. In infants, IAP ≥11 mmHg is considered intra-abdominal hypertension (IAH). We aim to determine the incidence of IAH and its relationship with duration of ventilatory support, and gastrointestinal function post CDH repair.

    METHODS: We prospectively recruited all neonates who had CDH repair in four hospitals in Malaysia from June 2018 to October 2020. Intra vesical pressure was used as a proxy for IAP and was measured for 5 consecutive days post surgery. The daily median value was used for analysis. We categorized IAP as <11 mmHg (no IAH), 11-15 mmHg (IAH), and >15 mmHg (severe IAH). Incidence of IAH, its effects on the duration of ventilatory support, and gastrointestinal function were studied.

    RESULTS: There were 24 neonates included in this study. They were operated between day 1 and 6 of life (median: 4 days old). IAH was detected within the first 3 days post surgery, with 83% occurring on day one. Those requiring ventilatory support for more than 3 days contributed the largest proportion of IAH (n = 17, 71%).     OBJECTIVE: To evaluate the effects of cryopreservation in post-thaw umbilical cord blood units for the survivability of Gram-positive bacteria strains.

    BACKGROUND: Microbial screening is required for all cord blood units (CBUs). Four gram-positive contaminants were documented to survive cryopreservation poorly and isolation of other contaminants were reported.

    METHODS: Forty-eight contaminated CBUs detected with either Staphylococcus epidermidis, Corynebacterium species, Peptostreptococcus or Streptococcus species before cryopreservation were used in this study. CBUs were processed, DMSO-infused and microbial screened before cryopreservation. Post-thaw microbial screening was achieved using 1 and 10 ml inoculants in BACTEC culture bottles. Positive bottles were subjected for microbial identification and results were compared with those from pre-freeze.

    RESULTS: A higher rate of microbial contamination was found using the 10 ml inoculant. Screening of 11 CBUs did not detect any contaminants while 30 CBUs screened detected more than one unknown contaminants and majority of contaminants were identified to be gram-negative species.

    CONCLUSION: A higher inoculation volume used at post-thaw for microbial screening improves contamination detection but leads to the loss of precious cord blood. Some contaminants did not survive cryopreservation or were not identified due to their low microbial levels. Contrasting contaminants found at post-thaw suggest the improvements made in detection and identification of contaminants over the years.
