    Adult laryngeal haemangioma is normally seen in the supraglottic or glottic region. Transglottic haemangioma is unusual, and treatment with primary endolaryngeal surgical excision may lead to undesirable bleeding and poor voice outcomes. A 25-year-old female presented with hoarseness and progressive upper airway obstruction symptoms. Videoendoscopy showed haemangioma involving all unilateral subunits of the larynx obstructing half of the subglottis. The transglottic haemangioma was treated with endolaryngeal ethanol injection with prior tracheostomy under local anaesthesia. Endolaryngeal laser surgery was performed later on to remove small residual haemangioma and granuloma. The haemangioma resolved; however, the ethanol injection to the paraglottic space results in vocal fold immobility but with favourable position and good muscle tone and bulk. The patient was successfully decannulated. Post-intervention subjective and objective voice assessments showed normal parameters except slight impairment of voice handicap index-10 with a total score of 12. Adult transglottic haemangioma can cause upper airway obstruction and requires intervention. Excision of the lesion endoscopically without sacrificing voice is achievable.

    BACKGROUND: Nasopharyngeal carcinoma (NPC) is the fourth common cancer in Malaysia. A few studies have looked into the incidence of NPC in Malaysia in general, but there has been no published data on its incidence in Sabah specifically, where NPC is very common among the population. The aim of this study is to present the first report on the incidence of NPC in the state of Sabah, Borneo.

    METHODS: Data of all patients diagnosed with NPC over a 5-year period from January 2015 to December 2019 inclusive were collected from the NPC registry of 3 main hospitals in Sabah. Age-standardized rates (ASRs) for different genders, ethnicities, and districts of origin were calculated.

    RESULTS: 215 NPC patients were identified with a mean age at diagnosis of 49 (range 9-82). The ASR of NPC was 7.9/100,000 where the average age-adjusted male-to-female ratio was 2.4. The highest ASR was found in Dusun ethnicity in both male (3.19/100,000) and female (1.69/100,000) individuals, followed by Chinese (both genders), and Kadazan (for male individuals) and Bajau (for female individuals). The highest ASR was found in patients originating from Sandakan, Kota Kinabalu, Keningau, and Tawau.

    CONCLUSION: This is the first report on the incidence of NPC in Sabah, Borneo. The data suggest high ASRs among the population, especially in male Dusun and Chinese ethnic groups. Further research looking into NPC in this state, especially on risk factors and ways to improve diagnosis and prevention among the population, is recommended.

    To understand the effects of fish predator’s kairomones on Aedes mosquitoes’
    oviposition, we established an experiment using gravid Aedes females. Kairomones
    concentrations were established using Hampala macrolepidota. One individual fish was
    placed inside containers with varying water levels (1 L, 5 L, and 10 L of water). The fish
    were kept in the containers for 24 hours and were removed immediately at the start of
    each trial in order to have the kairomones remnants. Twenty gravid adult females of
    Aedes aegypti and Aedes albopictus were allowed to lay eggs on oviposition site with
    various treatments: (1) control without any kairomones; (2) kairomone remnant in 1 L of
    water; (3) kairomone remnant in 5 L of water; and (4) kairomone remnant in 10 L of water.

    BACKGROUND: Coronavirus disease-2019 (COVID-19) causes severe illness and multi-organ dysfunction. An abnormal electrocardiogram is associated with poor outcome, and QT prolongation during the illness has been linked to pharmacological effects. This study sought to investigate the effects of the COVID-19 illness on the corrected QT interval (QTc).

    METHOD: For 293 consecutive patients admitted to our hospital via the emergency department for COVID-19 between 01/03/20 -18/05/20, demographic data, laboratory findings, admission electrocardiograph and clinical observations were compared in those who survived and those who died within 6 weeks. Hospital records were reviewed for prior electrocardiograms for comparison with those recorded on presentation with COVID-19.

    RESULTS: Patients who died were older than survivors (82 vs 69.8 years, p 455 ms (males) and >465 ms (females) (p = 0.028, HR 1.49 [1.04-2.13]), as predictors of mortality. QTc prolongation beyond these dichotomy limits was associated with increased mortality risk (p = 0.0027, HR 1.78 [1.2-2.6]).

    CONCLUSION: QTc prolongation occurs in COVID-19 illness and is associated with poor outcome.

    During a forensic entomological study conducted at an oil palm plantation in Tanjung Sepat, Kuala Langat, Selangor, a Bengalia emarginata Malloch, 1927 (Diptera: Calliphoridae: Calliphorinae: Bengalini) was collected for the first time. Two adults were collected nearby the pig carcass by the first author and identified by the second. Prior to this finding, nine species of Bengalia were recorded from peninsular Malaysia or Borneo. Male of B. emarginata are different from Bengalia varicolor Fabricious by the following characters: Sternite 5 projection rounded with small identation and mid tibia double-fringed in ventral surface.

    Corona virus (COVID-19) infection has been growing as a biggest threat to human society. World Health Organization (WHO) has already declared it as a pandemic for the whole world, with nearly six million positive cases. Highly contagious nature of the virus has challenged the medical facilities of all the developed and developing country health system. Early identification of the infection is very important to provide medical facilities and cease the chain of infection to new persons. The symptoms such as fever, dry cough, breathing issues generally show in patients not before 5-7 days. However, the patients feel the loss of smell or taste (anosmia) as early as second day onwards due to the presence of virus in nose and throat. Low-cost techniques such as SniffIn-sticks ® Smell Test and UPSIT etc. can be used to test anosmia along with medically approved olfactory test leading to identification of COVID-19 infections. With leading researchers findings anosmia test will be effective in breaking the chain infection of COVID-19 virus. In the exit ports, anosmia test kits may be added to thermal testing to identify the infected patients with low symptoms. Additionally, home test kits may be developed at low cost and supplied for large scale testing of the infection.

    Inner myometrial laceration is a rarely encountered intrapartum uterine trauma, which usually present as post-partum hemorrhage (PPH). The diagnosis can only be confirmed by direct examination of the uterine cavity but it should be suspected in PPH with good uterine contraction. Discussed here are the characteristics and management of the reported cases including two women managed at our center.

    Peripheral, predominantly motor polyneuropathy is associated with a plethora of possible aetiologies and the investigative procedures to rule them out are extensive. A 31 year old lady presented with progressive lower limb weakness over a period of 2 years. Examination revealed symmetrical weakness (3+/5) of all distal muscle groups in the lower limbs with absent ankle jerk. There was no sensory loss. The nerve conduction study revealed a predominantly axonal motor neuropathy. Cerebrospinal fluid examination and other investigations to rule out potential causes of predominantly motor peripheral neuropathy were normal. The only positive findings were multiple qualitative assays for porphobilinogen in her urine.

    Due to unavailability of a computer keyboard layout for the Jawi script, users of this script make use of the Arabic keyboard layout for typing texts in this particular script. Obviously, the layout of the Arabic script keyboard is not designed for the ancient writing of the Jawi script. Keeping this in view, a research was conducted to design a new keyboard layout suitable for the users of the Jawi script from an ergonomics point of view, and the outcome of the research is presented in this study. In order to design the Jawi script keyboard layout, the relative finger strengths of both male and female subjects were determined  experimentally. The relative frequency of the characters and two special characters (full-stop and comma) that appeared in the script were determined by counting their presence in a large number of texts in the script that represent the workload of the fingers. The keys were rearranged in such a way that the workload of each finger was approximately matched with its relative strength. The newly proposed arrangement of the keys was not much different compared to the Arabic script keyboard layout, and hence it is convenient for users to switch between the layouts.

    Hairy polyps are rare developmental malformations. They are benign lesions presented as a pedunculated mass that may arise from the naso-oropharyngeal region. Larger mass can cause upper respiratory obstruction causing respiratory distress or feeding difficulty, while smaller mass will present as intermittent respiratory distress due to a ball-valve type of obstruction. They are commonly seen in female, with ratio of 6:1 and majority of the cases occur in the infantile period. We are reporting a case of hairy polyp in a female infant that causing intermittent respiratory distress.

    The main objective of this study was to investigate the effectiveness of portable devices (Mp4) in delivering imagery based on video modelling (expert model) among 30 female netball players, examining their personal experience. In the pre-test-intervention-post-test study design, 15 participants were randomly assigned into each of the Mp4 (initial and repeated instructions). Participants practised the imagery for seven days and were interviewed at post-test. In interviews most of the participants in the Mp4 conditions reported that their confidence for shooting increased. They considered the Mp4 to be a useful device to practice imagery at any time and place, but imagery training routines varied among participants and were influenced by their personal life and free time activities. The results of this study signified that the Mp4 has the potential to be a reliable tool to deliver imagery training based on video modelling among netballers.

    Catamenial epilepsy refers to changes in the frequency of seizures over the course of the menstrual cycle. A thorough history and detailed review of the patient's seizure diary are imperative to classify the seizures accurately and select the most appropriate antiepileptic treatment. As catamenial epilepsy rarely responds to antiepileptic medications, the physician should regularly revise the treatment plan of the women with epilepsy that is refractory to the current treatment. We describe the case of a 34-year-old single woman who presented with refractory seizures.

    The purpose of this study was to assess the foundation depth of track and field events and objectively identify if there had been a significant advancement of performance in athletics at a foundation level. Data was taken from all 45 athletics events (23 male, 22 female) over the last seven biannual National Games from 2002 to 2014. The performances for the top five finalists across each event were analyzed. A correlation co-efficient was calculated to assess the strength of linear performance relationship over time. T-tests were performed to assess mean differences across high prospect events versus major international benchmarks. Using r = 0.70 as a high correlation, only five events (men’s 400 m, 400 m hurdles, high jump, and both men’s and women’s hammer throw) out of 41 individual events demonstrated strong positive linear relationships over the assessment period. Only men’s high jump and women’s hammer throw had a non-significant difference (p = > 0.05) when compared with the means performance measure at the two Southeast Asian Games, indicating the events’ degree of capacity to compete at an international level. In comparison to higher level competitions, men’s high jump and women’s hammer throw also demonstrated fragility. The performance gap between the National Games and comparable international event was very large, ranging from 5.3 to 71.0%. Overall, there appears to be a trivial or stagnant trend for many athletics events, which has been unable to create a foundation needed for developing consistent elite performance. Taking into account this data, consideration of the development of new intervention action plans within the overall strategy should be determined and implemented.

    Spinal tuberculosis is not common in the paediatric age group. Initial clinical features are often vague and non specific until the disease progresses to later stages. We highlight the diagnostic difficulties and management challenges of a complicated extradural tuberculoma with neurological deficits in a very young girl.
