    The present work sought to investigate the nutritional composition and phytochemical properties of red pitaya (Hylocereus polyrhizus) juices from Malaysia and Australia and to determine the optimum ethanol concentration (in the range of 0 – 100% ethanol) for the extraction of phenolic, flavonoid and betacyanin contents. The predominant macronutrient in red pitaya juice was carbohydrate while potassium and vitamin A were the major mineral and vitamin content. Red pitaya juice from Malaysia achieved optimal total phenolic content at 20% of ethanol (20 mL ethanol in 100 mL water, v/v); total flavonoid content at 60% (v/v); and betacyanin content at 0% (v/v). Red pitaya juice from Australia achieved the maximum total phenolic content at 60% (v/v); total flavonoid content at 20% (v/v); and betacyanin content at 80% (v/v). Nutritional composition and the phytochemical properties of red pitaya in Malaysia and Australia were significantly different suggested the role of environmental factors like soil and climate on the phytochemical properties of red pitaya.

    Pengkomposan tandan buah kosong (EFB) dan sisa efluen kilang (POME) merupakan pendekatan lestari bagi menukarkan EFB dan POME kepada baja kompos. Pengkomposan adalah satu proses biologi yang memerlukan pengawalan yang optimum agar nutrien yang terhasil adalah tinggi dan stabil. Cabaran utama yang dihadapi oleh industri kompos ialah untuk menghasilkan kompos yang bernutrien tinggi dan stabil. Objektif kajian ini ialah untuk menganalisis prestasi jangka panjang bagi loji kompos terpilih melalui kandungan nutrien kompos yang dihasilkan. Penyelidikan ini mengkaji prestasi tiga tahun bagi empat loji kompos di Malaysia iaitu loji kompos Merotai, loji kompos Binuang, loji kompos Tanah Merah dan loji kompos Elphil. Prestasi kandungan nutrien yang diambil kira termasuklah nitrogen, kalium, nisbah karbon kepada nitrogen, kandungan kelembapan dan agregat kompos. Sebanyak 1,465 set data nutrien dianalisis secara deskriptif. Secara keseluruhannya, prestasi kandungan nutrien kesemua loji kompos yang dikaji berjaya memenuhi keperluan minimum industri sawit. Namun terdapat kes kompos yang dihasilkan mengandungi nutrien kalium yang tidak mencapai keperluan minimum industri dan kandungan kelembapan kompos yang tinggi. Kesimpulannya, masih terdapat ruang untuk penambahbaikan dalam proses pengkomposan agar kompos yang dihasilkan mengandungi nutrien yang stabil dan memenuhi keperluan minimum industri

    The elemental profiles of six honey samples from Malaysia had been constructed using the data obtained from both ICP-AES and ICP-MS. Potassium and sodium were the most abundant minerals covering from 69.3-78.6% and 14.1-28.7%, respectively. The ratio of potassium to sodium was more than one. Even though the minerals and trace elements composition varied dependent on the type of honey samples, there was no statistically significant difference between the analysed honey samples, namely tualang, gelam, acacia and a few forest honeys based on two-factor ANOVA and cluster analysis. The total element content of honey samples were strongly correlated with the electrical conductivity, but only have moderate correlation with the ash content and honey colour based on the regression analysis. PCA result on the available elemental data from worldwide honeys, including honey samples from Malaysia revealed that potassium and sodium were the mineral markers to distinguish honey origin. Both tualang and gelam honey samples from Malaysia have close mineral profile with sesame honeys from Egypt and multifloral honeys from India, whereas forest honeys Malaysia were near to avocado honeys from Spain and multifloral honeys from India.

    Mutations in KCNJ5, ATP1A1, ATP2B3, CACNA1D, and CTNNB1 are thought to cause the excessive autonomous aldosterone secretion of aldosterone-producing adenomas (APAs). The histopathology of KCNJ5 mutant APAs, the most common and largest, has been thoroughly investigated and shown to have a zona fasciculata-like composition. This study aims to characterize the histopathologic spectrum of the other genotypes and document the proliferation rate of the different sized APAs. Adrenals from 39 primary aldosteronism patients were immunohistochemically stained for CYP11B2 to confirm diagnosis of an APA. Twenty-eight adenomas had sufficient material for further analysis and were target sequenced at hot spots in the 5 causal genes. Ten adenomas had a KCNJ5 mutation (35.7%), 7 adenomas had an ATP1A1 mutation (25%), and 4 adenomas had a CACNA1D mutation (14.3%). One novel mutation in exon 28 of CACNA1D (V1153G) was identified. The mutation caused a hyperpolarizing shift of the voltage-dependent activation and inactivation and slowed the channel's inactivation kinetics. Immunohistochemical stainings of CYP17A1 as a zona fasciculata cell marker and Ki67 as a proliferation marker were used. KCNJ5 mutant adenomas showed a strong expression of CYP17A1, whereas ATP1A1/CACNA1D mutant adenomas had a predominantly negative expression (P value =1.20×10-4). ATP1A1/CACNA1D mutant adenomas had twice the nuclei with intense staining of Ki67 than KCNJ5 mutant adenomas (0.7% [0.5%-1.9%] versus 0.4% [0.3%-0.7%]; P value =0.04). Further, 3 adenomas with either an ATP1A1 mutation or a CACNA1D mutation had >30% nuclei with moderate Ki67 staining. In summary, similar to KCNJ5 mutant APAs, ATP1A1 and CACNA1D mutant adenomas have a seemingly specific histopathologic phenotype.

    BACKGROUND: Excessive and inappropriate antimicrobial use in the community is one risk factor that can result in the spread of antimicrobial resistance. Upper respiratory tract infections are most frequently reported among children and mainly of viral origin and do not require antibiotics. We have conducted Knowledge, Attitude and Perception (KAP) survey of parents to explore the parent's knowledge, attitude & perception of Saudi parents.

    METHODS: A knowledge attitude perception questioner was adopted from a previous study conducted in Greece by Panagakou et al. Raosoft online sample size calculator calculated the sample size by adding the total estimated Makkah population of 5,979,719 with a response rate of 30%, 5% margin of error and 99% confidence interval. Based on the described criteria five hundred & fifty-eight was the required sample size of the study. Incomplete questioners were excluded from the statistical analysis. SPSS version 21 was used to analyse data and to produce descriptive statistics.

    RESULTS: Most of the mothers (95%) responded among parents. 67% had no health insurance to cover medications costs. Most of them (74%) were related to medium income level. Seventy per cent of the parents believed physicians as a source of information for judicious antibiotics use. Interestingly, only 8% were agreed that most of the upper respiratory tract infections are caused by viral reasons. Majority of Saudi parents (53%) expect pediatricians to prescribe antimicrobials for their children for symptoms like a cough, nose discharge, sore throat and fever. Moreover, most the parents had the poor knowledge to differentiate commonly used OTC medications for URTI and antibiotics like Augmentin (Co-amoxiclav), Ceclor (cefaclor) and Erythrocin (Erythromycin). While comparing males and female's knowledge level, few males have identified Amoxil (Amoxicillin). Similarly, parents of age 20-30 years have good knowledge about the antibiotics.

    CONCLUSIONS: Majority of Saudi parents believe in pediatricians and use antibiotics on physician's advice. Most of them expect antibiotics from their physicians as a primary treatment for upper respiratory tract infections. There is need for more educational activities to parents by the pharmacists to prevent antibiotics overuse among children.

    BACKGROUND: Poor availability and unaffordability of key access antibiotics may increase antimicrobial resistance in the community by promoting inappropriate antibiotic selection and abridged therapy compliance.

    OBJECTIVE: To check the prices, availability, and affordability of the World Health Organization (WHO) key access antibiotics in private sector pharmacies of Lahore, Pakistan.

    METHODOLOGY: A survey of WHO key access antibiotics from WHO essential medicine list 2017 was conducted in private sector pharmacies of 4 different regions of Lahore employing adapted WHO/HAI methodology. The comparison of prices and availability between originator brands (OB) and lowest price generics (LPG) were conducted followed by the effect of medicine price differences on patient's affordability. The data were analyzed using a preprogrammed WHO Microsoft excel workbook.

    RESULTS: The mean availability of OB products was 45.20% and the availability of LPGs was 40.40%. The OBs of co-amoxiclav, clarithromycin and metronidazole and LPGs of azithromycin and ciprofloxacin were easily available (100%) in all private sector pharmacies. Whereas, antibiotics like chloramphenicol, cloxacillin, nitrofurantoin, spectinomycin, and cefazolin were totally unavailable in all the surveyed pharmacies. The OBs and LPGs with high MPRs were ceftriaxone (OB; 15.31, LPG; 6.38) and ciprofloxacin (OB; 12.42, LPG; 5.77). The median of brand premium obtained was 38.7%, which varied between the lowest brand premium of 3.97% for metronidazole and highest for ceftriaxone i.e. 140%. The cost of standard treatment was 0.5 day's wage (median) if using OB and 0.4 day's wage (median) for LPG, for a lowest paid unskilled government worker. Treatment with OB and LPG was unaffordable for ciprofloxacin (OB; 2.4, LPG; 1.1) & cefotaxime (OB; 12.7, LPG; 8.1).

    CONCLUSION: There is dire need to properly implement price control policies to better regulate fragile antibiotic supply system so that the availability of both OB and LPG of key access antibiotics should be increased. The prices could be reduced by improving purchasing efficiency, excluding taxes and regulating mark-ups. This could increase the affordability of patients to complete their antibiotic therapy with subsequent reduction in antimicrobial resistance.

    Evidence show that gamma-aminobutyric acid (GABA) receptors are involved in depression, so the aim of this study was to investigate the effect of nitrazepam as agonist of GABAA receptors on depression and curiosity in male mice and the role of potassium channel in antidepressant-like response. For this purpose, we studied the antidepressant-like properties of fluoxetine, nitrazepam, glibenclamide, and cromakalim by both forced swimming test (FST) and tail suspension test (TST). Animals were injected by various doses of nitrazepam (0.05, 0.1, and 0.5mg/kg). Nitrazepam at dose of 0.5mg/kg significantly decreased the immobility time compared to control group in both FST and TST. Fluoxetine also showed such a response. Co-administration of nitrazepam (0.05mg/kg) with glibenclamide in TST (1mg/kg) and in FST (0.3, 1mg/kg) also showed antidepressant-like response. Beside, cromakalim (0.1mg/kg) could reverse the antidepressant-like effect of nitrazepam (0.5mg/kg) in both FST and TST, while cromakalim and glibenclamide alone could not change the immobility time compared to control group (P>0.05). The hole-board test revealed that nitrazepam at doses of 0.5 and 0.1mg/kg could increase the activity of the animal's head-dipping and boost the curiosity and exploration behavior of mice. The results of this study revealed that nitrazepam may possess antidepressant-like properties and this effect is dependent to potassium channels in both FST and TST.

    BACKGROUND: Sickle cell disease is a group of disorders characterized by deformation of erythrocytes. Renal damage is a frequent complication in sickle cell disease as a result of long-standing anemia and disturbed circulation through the renal medullary capillaries. Due to the improvement in life expectancy of people with sickle cell disease, there has been a corresponding significant increase in the incidence of renal complications. Microalbuminuria and proteinuria are noted to be a strong predictor of subsequent renal failure. There is extensive experience and evidence with angiotensin-converting enzyme (ACE) inhibitors over many years in a variety of clinical situations for patients who do not have sickle cell disease, but their effect in people with this disease is unknown. It is common practice to administer ACE inhibitors for sickle nephropathy due to their renoprotective properties; however, little is known about their effectiveness and safety in this setting. This is an update of a Cochrane Review first published in 2013.

    OBJECTIVES: To determine the effectiveness of ACE inhibitor administration in people with sickle cell disease for decreasing intraglomerular pressure, microalbuminuria and proteinuria and to to assess the safety of ACE inhibitors as pertains to their adverse effects.

    SEARCH METHODS: The authors searched the Cochrane Cystic Fibrosis and Genetic Disorders Group's Hameoglobinopathies Trials Register comprising references identified from comprehensive electronic database searches and handsearches of relevant journals and abstract books of conference proceedings.Date of the most recent search: 03 June 2015.

    SELECTION CRITERIA: Randomized or quasi-randomized controlled trials of ACE inhibitors designed to reduce microalbuminuria and proteinuria in people with sickle cell disease compared to either placebo or standard treatment regimen.

    DATA COLLECTION AND ANALYSIS: Three authors independently applied the inclusion criteria in order to select studies for inclusion in the review. Two authors assessed the risk of bias of studies and extracted data and the third author verified these assessments.

    MAIN RESULTS: Five studies were identified through the searches, only one met our inclusion criteria. The included study randomized 22 participants (seven males and 15 females) having proteinuria or microalbuminuria with sickle cell disease and treated the participants for six months (median length of follow up of three months) with captopril or placebo. The overall quality of the outcomes reported was high, since most aspects that may contribute to bias were regarded to be of low risk, although allocation concealment was not reported. At six months, the study reported no significant difference in urinary albumin excretion between the captopril group and the placebo group, although the mean urinary albumin excretion in the captopril group was lower by a mean difference of -49.00 (95% confidence interval -124.10 to 26.10) compared to that of placebo. However, our analysis on the absolute change score showed significant changes between the two groups by a mean difference of -63.00 (95% confidence interval -93.78 to -32.22). At six months albumin excretion in the captopril group was noted to decrease from baseline by a mean of 45 ± 23 mg/day and the placebo group was noted to increase by 18 ± 45 mg/day. Serum creatinine and potassium levels were reported constant throughout the study. The potential for inducing hypotension should be highlighted; the study reported a decrease of 8 mmHg in systolic pressure and 5 mmHg in diastolic and mean blood pressure.

    AUTHORS' CONCLUSIONS: There is not enough evidence to show that the administration of ACE inhibitors is associated with a reduction of microalbuminuria and proteinuria in people with sickle cell disease, although a potential for this was seen. More long-term studies involving multiple centers and larger cohorts using a randomized-controlled design are warranted, especially among the pediatric age group. Detailed reporting of each outcome measure is necessary to allow a clear cut interpretation in a systematic review. One of the difficulties encountered in this review was the lack of detailed data reported in the included study.

    BACKGROUND: Apium leptophyllum (Pers.) is an annual herb with traditional appreciation for various pharmacological properties; however, the scientific information on this herb is insufficient. The aim of the present investigation was undertaken to evaluate flavonoidal fraction of A. leptophyllum fruit (FFALF) against diarrhoea on albino rats.

    METHODS: The antidiarrhoeal study was conducted by castor oil induce diarrhoea, prostaglandin E2 (PGE2) induced enteropooling and intestinal transit by charcoal meal test. The rats were divided into five groups (six/group). Group I served as control and received orally 2% acacia suspension; Group II served as standard and received orally loperamide (3 mg/kg) or atropine sulphate (5 mg/kg); Group III, IV and V served as test groups and received the FFALF at doses of 5, 10 and 20 mg/kg orally, respectively.

    Long QT syndrome (LQTS) is predominantly a genetic cardiac arrhythmia disorder. We report here our study on long QT syndrome from two children from Kelantan, Malaysia. Clinical and genetic findings of these two unrelated Malay children with LQTS is discussed. We found a Long QT, type 1 causal mutation, p.Ile567Thr in the KCNQ1 gene in the first child. A pathogenic mutation could not be detected in the second child, explaining the heterogeneity of this disease.

    Pseudohyperkalemia is a spuriously high serum potassium measurement in a patient with no clinical evidence of hyperkalemia. It has been reported to occur in patients with leukocytosis and thrombocytosis. Only a few cases of pseudohyperkalemia have been reported in patients after splenectomy. Two cases of pseudohyperkalemia occurring after emergency splenectomy for abdominal trauma are presented to highlight their clinical presentation and sequalae. Consecutive patients who underwent emergency splenectomy for trauma and subsequently developed pseudohyperkalemia were monitored in Universiti Kebangsaan Malaysia Medical Centre for their clinical sequalae. Both the patients developed leukocytosis, thrombocytosis and high serum potassium level within 2-weeks of their splenectomies. They did not demonstrate any symptoms and signs of hyperkalemia. Their conditions resolved spontaneously without any specific treatment. Pseudohyperkalemia should be suspected in a similar clinical scenario to avoid unnecessary treatment that could lead to severe hypokalemia.

    The objectives of this systematic review were to investigate the efficacy of amoxicillin/amoxicillin-clavulanic acid for reducing the risk of postoperative infection after third molar surgery and to evaluate the adverse outcomes in these patients, as well as in healthy volunteers. A systematic search of four databases was performed on May 26, 2017. Eleven studies qualified for the qualitative analysis and eight were found suitable for meta-analysis. The results suggest that both amoxicillin-clavulanic acid and amoxicillin significantly reduce the risk of infection after third molar extraction (overall relative risk (RR) 0.25, P<0.001). However, with the exclusion of randomized controlled trials with a split-mouth design (due to an inadequate crossover period after antibiotic treatment), only amoxicillin-clavulanic acid was found to be effective (RR 0.21, P<0.001). The risk of adverse effects was significantly higher in the amoxicillin-clavulanic acid group (RR=4.12, P=0.023) than in the amoxicillin group (RR 1.57, P=0.405). In conclusion, amoxicillin-clavulanic acid and amoxicillin may significantly reduce the risk of infection after third molar extraction. However, their use in third molar surgery should be viewed with caution, as recent clinical trials on healthy volunteers have shown evidence of the negative impact of amoxicillin use on bacterial diversity and antibiotic resistance.

    Understanding the influence of co-dopants in the luminescence enhancement of carbonate glasses is the key issue in dosimetry. A series of borate glasses modified by lithium and potassium carbonate were synthesized by the melt-quenching method. The glass mixture activated with various concentrations of TiO2 and MgO was subjected to various doses of gamma-rays ((60)Co). The amorphous nature of the samples was confirmed by x-ray diffraction (XRD) spectra. The simple glowing curve of the glass doped with TiO2 features a peak at 230°C, whose intensity is maximal at 0.5 mol% of the dopant. The intensity of the glowing curve increases with the concentration of MgO added as a co-dopant up to 0.25 mol%, where it is two times higher than for the material without MgO thermoluminescence properties, including dose response, reproducibility, and fading were studied. The effective atomic number of the material was also determined. Kinetic parameters, such as kinetics order, activation energy, and frequency factor are estimated. The photoluminescence spectra of the titanium-doped glass consist of a prominent peaks at 480 nm when laser excitation at 650 nm is used. A three-fold photoluminescence enhancement and a blue shift of the peak were observed when 0.1% MgO was introduced. In addition, various physical parameters, such as ion concentration, polaron radius and internuclear distances were calculated. The mechanism for the thermoluminescence and photoluminescence enhancements are discussed.

    The presence of natural radioactivity and (137)Cs has been investigated in fresh media obtained from South China Sea locations off the coast of peninsular Malaysia. The media include seafood, sea water and sediment. The samples were collected some weeks prior to the devastating 2011 Tōhoku earthquake and associated tsunami, the occurrence of which precipitated the Fukushima incident. All samples showed the presence of naturally occurring (226)Ra, (228)Ra and primordial (40)K, all at typically prevailing levels. The concentrations of natural radioactivity in molluscs were found to be greater than that of other marine life studied herein, the total activity ranging from 337 to 393 Bq kg(-1) dry weight. The total activity in sea water ranged from 15 to 88 Bq l(-1). Sediment samples obtained at deep sea locations more than 20 km offshore further revealed the presence of (137)Cs. The activity of (137)Cs varied from ND to 0.5 Bq kg(-1) dry weight, the activity increasing with offshore distance and depth. The activity concentrations presented herein should be considered useful in assessing the impact of any future radiological contamination to the marine environment.

    Batu Dam is of considerable importance to the metropolis of Kuala Lumpur, its existence and the quality assessment of its waters being essential in helping to maintain the lives of a large sector of the Malaysian population. Concerning the level of naturally occurring radioactivity contained within its waters, a well characterised HPGe γ-ray technique has been used in making measurements of the concentrations of primordial radionuclides in samples of surface water from the Dam. Based on the mean individual daily consumption of dam water, estimation has been made of the concomitant radiation dose. Activity concentrations, in units of Bq l-1, have been found to be in the range 2.4-3.2 for 226Ra, 1.1-1.3 for 232Th and 22.7-40.7 for 40K, in line with literature data for surface waters. The total annual ingestion dose for infants (<1 y) and adolescents (12-17 y) are found to be significant and greater than the World Health Organization recommended maximum dose of 0.1 mSv y-1 from the imbibing of drinking water. However, the Dam water does not pose a threat to public health, the Dam water not being used as the sole source of drinking water. Noting that this is the only known study of water from Batu Dam, the reported levels allow for evaluation of future changes in the natural radioactivity profile.

    Activity concentrations of primordial radionuclides in sand samples collected from the coastal beaches surrounding Penang Island have been measured using conventional γ-ray spectrometry, while in-situ γ-ray doses have been measured through use of a portable radiation survey meter. The mean activity concentrations for 226Ra, 232Th and 40K at different locations were found to be less than the world average values, while the Miami Bay values for 226Ra and 232Th were found to be greater, at 1023±47 and 2086±96Bqkg̶ 1 respectively. The main contributor to radionuclide enrichment in Miami Bay is the presence of monazite-rich black sands. The measured data were compared against literature values and also recommended limits set by the relevant international bodies. With the exception of Miami Bay, considered an elevated background radiation area that would benefit from regular monitoring, Penang island beach sands typically pose no significant radiological risk to the local populace and tourists visiting the leisure beaches.
