    Swedish Massage Therapy (SMT) is known for its therapeutic relaxation effects. Hypertension is associated with stress and elevated endothelial inflammatory markers. This randomized control trial measured the effects of whole body SMT (massage group) or resting (control group) an hour weekly for four weeks on hypertensive women. Blood pressure (BP) and heart rate (HR) were measured before and after each intervention and endothelial inflammatory markers: vascular endothelial adhesion molecules 1 (VCAM-1) and intracellular adhesion molecules 1 (ICAM-1) were measured at baseline and after the last intervention. Massage group (n=8) showed significant systolic BP (SBP) reduction of 12 mmHg (P=0.01) and diastolic BP (DBP) reduction of 5 mmHg (P=0.01) after four sessions with no significant difference between groups. Reductions in HR were also seen in massage group after sessions 1, 3, and 4 with significant difference between groups. VCAM-1 showed significant reduction after four sessions: the massage group showed reduction of 998.05 ng/mL (P=0.03) and the control group of 375.70 ng/mL (P=0.01) with no significant differences between groups. There were no changes in ICAM-1. In conclusion, SMT or resting an hour weekly has effects on reducing BP, HR, and VCAM-1 in hypertensive women.

    Hepatocellular carcinoma (HCC) is a malignant tumour of the hepatocyte. It is a common malignancy worldwide and causes almost half a million deaths annually. Asia is a high risk area. Although surgery (hepatectomy or liver transplantation) is the main form of curative treatment, the majority of patients are not eligible for surgery due to extent of tumour and dysfunction of liver. Radiopharmaceuticals used for transarterial treatment of HCC were Yttrium-90 microspheres, Iodine-131 lipiodol, Rhenium-188 lipiodol, and Holmium-166 Chitosan complex. Yittrium-90 microspheres are glass or resin microspheres of mean sphere diameter of 20 to 30 micrometre. The activity administered was about 4 GBq. Reported response rate was about 20%, and median survival was 54 weeks. On inoperable tumours, reported objective response of I-131 lipiodol was 40 to 70%, and median survival was six to nine months. It showed efficacy similar to TACE. In adjuvant treatment following curative resection of HCC, reported three year survival was 86% compared with 46% for the control group. The administered activity in both adjuvant and inoperable HCC was about 2 GBq (55 mCi). Rhenium-188 lipiodol is a new radioconjugate, and using it we treated 70 patients with inoperable HCC. This treatment was a part of a multi-centre trial sponsored by the International Atomic Energy Agency. Partial response was obtained in 17% of cases, while 49% had stable disease at three months, and 34% showed disease progression. In terms of survival, 19% survived one year, 60% for six months, and 90% for three months. The mean activity was about 4.6 GBq (124 mCi). This method was safe and free from adverse effects.

    Objectives: We sought to determine the effectiveness of a voice care program among primary school teachers in a northeastern district in Malaysia.
    Methods: We conducted a randomized community trial in eight primary schools in a northeastern district in Malaysia. The self-administered and validated Malay-Voice Handicap Index-10 (M-VHI-10) questionnaire was used to assess overall voice handicap scores pre-intervention and eight weeks post-intervention. Teachers with a score of five or more (n = 86) were randomized into intervention (n = 41) and control groups (n = 45). The intervention group received portable voice amplifiers and vocal hygiene instruction, which was delivered by lectures and a booklet. The control group was not prescribed any intervention.
    Results: The sociodemographic, lifestyle, and occupational characteristics of the teachers (except maximum number of students per class) were similar between both groups. The baseline M-VHI-10 scores between both groups were also comparable. After the intervention phase, there was a significant effect observed in the total M-VHI-10 scores (p = 0.021, F-stat (df): 5.33 (1,79)) between both groups after controlling for the maximum number of students per class.
    Conclusions: Our results support the use of voice amplification in adjunct with vocal hygiene instruction as a prevention and treatment modality to reduce voice handicap among teachers. Our study demonstrated encouraging evidence on the low-cost voice care program as well as the success of group and workplace-based approaches in the school setting.

    L-arginine is an amino acid, which serves as the sole substrate for nitric oxide (NO) synthesis with the concomitant formation of L-citrulline in biologic system. NO has been demonstrated to be involved in smooth muscle relaxation and vasodilation, immune regulation and neurotransmission. It also has an important function as both intercellular and intracellular signals in many physiological systems, including the reproductive system where NO mediates penis erection. This study was undertaken to determine the effects of L-arginine on sperm motility, sperm count, and the nitric oxide level in the seminal plasma. Twelve sexually matured male rabbits (Oryctolagus cuniculus) were randomly divided into four groups with three rabbits each, which were control, low, medium, and high concentration groups. The treatment groups were force-fed with 100mg/kg, 200mg/kg, and 300mg/kg body weight of L-arginine for four weeks, whereas the control group was force-fed with water. Semen samples were collected every three days alternatively for a week before starting treatment and then after four weeks of treatment. Pre-treatment and post-treatment results were compared. Semen samples were collected using artificial vaginas from each group for sperm analysis such as sperm motility, sperm count and NO level in seminal plasma.     The purpose of this study was to evaluate the effectiveness of short-term stability ball (SB) training on males and females by comparing the strength changes produced in the core muscles. Forty-two previously untrained subjects, mean age = 23.62 ± 2.89 years were matched by their maximum strength (back strength: male = 190-200 kg, female = 45-50 kg and abdominal strength: male = 110-120 kg, female = 35-40 kg 1RM) and randomly placed in either one of these 3 groups; unstable SB group (n = 14), stable floor group (n = 14) and control group (n = 14) who did no exercise. SB training showed greatest improvement (p < 0.001) in back and abdominal strength (25.79 % and 29.51 % respectively), compared with the gain in floor training (FT) back and abdominal strength (10.28 % and 8.47 % respectively). Untrained female subjects achieved a higher percentage of improvement in strength compared to males in both back and abdominal muscles, and this is most evident in the SB training group. It is apparent that performing core training exercises on unstable surfaces stressed the musculature, possibly activating the neuro-adaptive mechanisms that led to the early phase gains in strength.

    Introduction: Dyslipidaemia is one of the risk factors contributing to the pathogenesis of cardiovascular
    diseases (CVDs). This study was conducted to investigate the effect of wet cupping on lipid profile.

    Methods: This randomized controlled trial was conducted in 2012 at the School of Medical Sciences,
    Universiti Sains Malaysia, Malaysia. Sixty-two healthy volunteers ranging from 30 to 60 years old were
    randomized into control and intervention groups. Subjects in the intervention group were assigned to two
    sessions of wet cupping at the beginning of the study and at the third month; individuals in the control group
    did not undergo any cupping procedure. Venous blood sample was collected for serum lipid profile: Total
    Cholesterol (TC), High Density Lipoprotein Cholesterol (HDL-C), Low Density Lipoprotein Cholesterol (LDL-C),
    and triglycerides; measured at baseline, first, third and fourth month.

    After publication of the article [1], it has been brought to our attention that the methodology outlined in the original article was not able to be fully carried out. The article planned a two armed randomized control trial. However, due to a lower response than expected and one housing complex dropping out from the study, the method was changed to pre- and post-intervention with no control group. All other methods were conducted as outlined in the original article.

    The beneficial impact of the differential biofeedback intervention technique on modification in autonomic competence, as well as the ultimate impact on bilateral shooting ability, were studied on young and competitive male soccer players. A total of 45 young high-performing soccer players in the age range of 18 to 24 years were recruited. At first, the players were subjected to baseline evaluation of psychomotor (i.e., reaction time - RT), physiological (viz. resting heart-rate; VO2Max, electrical muscle potentiality, etc.), and psychobiological indices (such as skin conductance components - latency, amplitude and recovery time). Thereafter, they were equally categorized into: Control Group, Experimental Group I [who received skin conductance (Sc) biofeedback training], and Experimental Group II [who received electromyography (EMG) biofeedback intervention training]. Participants of the experimental groups were subjected to their respective interventions (Sc & EMG biofeedback intervention training), which were imparted for 24 sessions (15 mins/day; 2 days/week for 12 weeks). After six weeks of intervention, mid-term evaluation of all of the aforementioned parameters (assessed during baseline evaluation) was performed. Thereafter, intervention sessions continued following the identical protocol for six weeks. At the end of the 12th week, a post-intervention assessment on all of the psychological, psychomotor and psychobiological variables was carried out on all of the participants to verify the effect of training on dependent measures. Post-intervention analyses revealed improvements in physiological, psychobiological and performance parameters, which could be rationally attributed to the differential biofeedback therapeutic interventions introduced to the experimental groups. The outcome suggested that both interventions facilitated improvement in bilateral shooting ability, while indepth analyses clarified that a higher extent of autonomic recovery emerged as the most significant factor facilitating shooting ability in the experimental groups.

    OBJECTIVE: The aim of the present study was to assess the expression of E-cad in oral precancerous lesions and conditions and oral carcinomas in comparison with normal mucosa.

    MATERIALS AND METHODS: Total of 50 samples were selected for the study and were categorized into five groups and 10 samples in each group as Group I-oral leukoplakia (OL), Group II-oral lichen planus (OLP), Group III-oral submucous fibrosis (OSMF), Group IV-oral squamous cell carcinoma (OSCC) and Group V-normal oral mucosa (NOM) as control group. All the samples were assessed for the expression of E-cad by immunohistochemical study.

    RESULTS: Upon assessing the expression of E-cad in OL, OSMF, OLP and OSCC, as majority of the samples with OSCC (90%), OL (80%), OLP (70%) and OSMF (60%) showed mild to moderate expression of E-cad staining, which was suggestive of reduction in dysplastic cells on comparison to NOM cells. This difference in expression and variation of E-cad upon comparison with normal mucosa was statistically significant (P < 0.001).

    CONCLUSION: There is significant (P < 0.001) variation of expression of E-cad with the histopathological dysplasia of the oral precancerous lesions and conditions, and the tumor differentiation of the oral cancers. However, there was no correlation of the degree of loss of expression of E-cad with the degree of dysplasia or the tumor differentiation of oral cancers. We conclude with our study that, there is a variation in the expression of E-cad but its value as a prognostic marker is questionable.

    Background: Penaeus monodon is the second most widely cultured marine shrimp species in the global shrimp aquaculture industry. However, the growth of P. monodon production has been constantly impaired by disease outbreaks. Recently, there is a lethal bacterial infection, known as acute hepatopancreatic necrosis disease (AHPND) caused by Vibrio parahaemolyticus AHPND strain (VpAHPND), which led to mass mortalities in P. monodon. Unfortunately, there is still insufficient knowledge about the underlying immune response of P. monodon upon AHPND infection. The present study aims to provide an insight into the antibacterial immune response elicited by P. monodon hepatopancreas towards AHPND infection.

    Methods: We have employed high-throughput RNA-Seq technology to uncover the transcriptome changes of P. monodon hepatopancreas when challenged with VpAHPND. The shrimps were challenged with VpAHPND through immersion method with dissected hepatopancreas samples for the control group (APm-CTL) and treatment group at 3 (APm-T3), 6 (APm-T6), and 24 (APm-T24) hours post-AHPND infection sent for RNA-Seq. The transcriptome de novo assembly and Unigene expression determination were conducted using Trinity, Tgicl, Bowtie2, and RSEM software. The differentially expressed transcripts were functionally annotated mainly through COG, GO, and KEGG databases.

    Results: The sequencing reads generated were filtered to obtain 312.77 Mb clean reads and assembled into 48662 Unigenes. Based on the DEGs pattern identified, it is inferred that the PAMPs carried by VpAHPND or associated toxins are capable of activating PRRs, which leads to subsequent pathway activation, transcriptional modification, and antibacterial responses (Phagocytosis, AMPs, proPO system). DAMPs are released in response to cell stress or damage to further activate the sequential immune responses. The comprehensive interactions between VpAHPND, chitin, GbpA, mucin, chitinase, and chitin deacetylase were postulated to be involved in bacterial colonization or antibacterial response.

    Conclusions: The outcomes of this research correlate the different stages of P. monodon immune response to different time points of AHPND infection. This finding supports the development of biomarkers for the detection of early stages of VpAHPND colonization in P. monodon through host immune expression changes. The potential genes to be utilized as biomarkers include but not limited to C-type lectin, HMGB1, IMD, ALF, serine proteinase, and DSCAM.

    This retrospective study was conducted at the audiology clinic of UKM in the year 2000. It was intended to detect the effect of walkman on the hearing system of its users and to investigate whether distortion product otoacoustic emission test can provide an early and reliable sign of cochlear damage or not. Distortion product otoacoustic emission test helps to determine the function of outer hair cell. Outer hair cells get damaged with exposure to loud sound, drugs and aging. Thirty subjects (20 in the study group and 10 in the control group), between the ages of 19-25 years, who fulfilled all the criteria of selection, were investigated. There were two sets of criteria (general and specific). Anyone, in the control or study group, failing in any of these criteria was excluded from the study. According to the general criteria, it was essential for all the subjects to have clean ear canals, normal hearing at all the frequencies (250Hz- 8000Hz), normal middle ear function (Type A tympanogram), no middle ear problem, not used/using any ototoxic medicine, no family history of hearing loss, no history of ear surgery, not exposed to any form of loud noise like disco, concert, F1 racing or gun shooting. According to the special criteria it was essential for the subjects in the control group to have never used a walkman and those in the study group must have been using a walkman for at least 6 months. Comparison of distortion product emission levels between control and study groups revealed that emissions were significantly lower in the study group, across all the frequencies, suggesting outer hair cell damage in the walkman users. These differences reached level of statistical significance (p<0.05) at 2, 4, 6 and 8kHz. Using walkman at an intensity unsafe for hearing and a duration longer than recommended is suspected for the outer hair cell damage in our study group. Those using walkman for longer duration and higher intensity manifested much lower emission levels. Our study supports the literature that distortion product emission test is a much sensitive test than pure-tone audiometry, as it can detect cochlear damage long before it appears in an audiogram. It is recommended that people who are exposed to loud noise regularly should be monitored with distortion product emission test. It is suggested that the use of walkman.

    Hydroxyapatite (HA) powder was synthesized via wet method using calcium nitrate hydrate (Ca(NO3)2.H2O) and diammonium hydrogen phosphate ((NH4)2HPO4) as raw materials. Powder obtained was milled using various milling speed ranging from 250 to 400 r.p.m. and sintered at 1300°C for 2hrs. Due to the nature of HA powder that decomposed at high temperature, XRD technique have been used in this work to determine the phase composition of the HA powder and also the crystallite size. The unmilled sample was used as the control group. Results show that sufficient heat supply generated from the milling process, initiates the decomposition of HA phase into ȕ-tricalcium phosphate (ȕ-TCP). Decomposition of HA starts to occur at the milling speed of 300 rpm, i.e the formation of ȕ-TCP was occurred at lower sintering temperature. It was believed that the decomposition of HA was associated with the formation of an intermediate phase, oxyapatite. Moreover, the crystallinity and particle size of the produced powder is very much affected by the milling speed and the stability of the HA. All milled powders possess spherical shape particle.

    Purpose: The present study aims to investigate the effects of nicotine on the endometrial decidual growth and levels of estrogen and progesterone in pseudopregnant rats.

    Methods: Pseudopregnancy (pc) was induced in cyclic Sprague-Dawley rats by sterile mating. Subcutaneous injection of nicotine tartrate (7.5 mg/kg/day) was scheduled from day 1 through day 5, day 5 through day 9 or day 1 through day 9 of pc. In another group of pseudopregnant rats, concomitant treatment of nicotine tartrate concurrently with progesterone (2 mg/day) was scheduled from day 1 through day 9 pc. Control groups received subcutaneous injections of vehicle only. Endometrial decidualization was induced on day 5 pc. On day 10 pc, animals were sacrificed.The degree of decidual growth and circulating levels of estrogen and progesterone were measured.

    Results: The decidual growth in all the first three nicotine-treated groups of animals was significantly reduced, particularly in the animals treated with nicotine from day 1 through day 9 pc. Plasma estrogen levels were significantly elevated in animals treated with nicotine from day 1 through day 9 pc. Conversely, levels of plasma progesterone were found to be significantly attenuated in the same group of nicotine-treated animals compared to controls. Exogenous replacement of progesterone, however, caused a higher degree of endometrial decidualization compared to the nicotine-treated group but it was slightly less than when compared to control.

    Conclusions: In conclusion, nicotine-induced progesterone deficiency with a corresponding elevation of estrogen may possibly attenuate the degree of endometrial decidualization in pseudopregnant rats.

    Prolonged consumption of repeatedly heated vegetable oil increases blood pressure. This study aimed to determine the effects of Citrus leaf extract, (CLE) on blood pressure, blood pressure-regulating enzymes and mediators, as well as aortic histomorphometry in heated palm oil induced-hypertension. Male Sprague Dawley rats (n=56) were divided into seven groups; control group was given normal diet and the other groups were fed with palm oil-enriched diet (15% w/w) either fresh (FPO), five-time-heated (5HPO) or ten-time-heated (10HPO) with or without CLE (0.15%, w/w) supplementation. CLE supplementation reduced the heated oil-raising effect of blood pressure, plasma TBARS, thromboxane and angiotensin-1 converting enzyme in 5HPO but not in 10HPO group. CLE increased serum heme oxygenase-1 in both 5HPO and 10HPO groups. CLE supplementation reduced the increase in aortic intima-media thickness, intima-media area and circumferential wall tension in 5HPO group but not in 10HPO group. These findings suggested that CLE supplementation reduces the blood pressure-raising effects of 5HPO and vascular damage, possibly through its antioxidant effect by modulating vasoactive mediators and blood pressure-regulating enzymes.

    The fruit of Canarium odontophyllum Miq. is a traditional delicacy in Borneo for its anti-aging benefit. This study evaluated the protective effect of C. odontophyllum leaf aqueous extract on damaged liver in streptozotocin-induced diabetic rats. A total of 30 male Spraque-Dawley rats (150-250g) were randomly divided into three groups: control group, diabetic without treatment and diabetic treated with 300 mg/kg aqueous extract of C. odontophyllum for 28 consecutive days. The diabetic condition was induced by intraperitoneal injection of streptozotocin at 65 mg/kg body weight.     In this study, the effects of palm vitamin E (PV) supplementation on glycemic control and lipid profile in diabetic-induce Sprague-Dawley rats have been evaluated. Diabetes in the rats was induced by a single intravenous streptozotocin (50 mg/kg body weight). The diabetic rats were divided into two groups; supplemented with 200 mg/kg body weight/day of PV and non-supplemented with PV (No PV group).     Fenitrothion (FNT) usage has received much attention for its potential to promote free radicals generation and interfere with antioxidant defense system. The aim of the present study was to investigate the effect of palm oil tocotrienol rich fraction (TRF) supplementation on oxidative stress and histological changes in rat brain induced by FNT. A total of 32 male Sprague Dawley rats divided into four groups: control group which received corn oil; TRF group was received palm oil TRF (200 mg/kg bw); FNT group administered with FNT (20 mg/kg bw) and TRF+FNT group pretreated with palm oil TRF (200 mg/kg bw) 30 min prior to administration of FNT (20 mg/kg bw). FNT and TRF were dissolved in corn oil and all supplementations were given by oral gavage once daily for 28 days. After four weeks of supplementation, TRF+FNT rats had significantly lower malondialdehyde (MDA) content and superoxide dismutase (SOD) activity but higher reduced glutathione (GSH) level and total protein level compared to FNT rats (p<0.05). However, protein carbonyl (PC) level was insignificantly lower for TRF+FNT group compared to FNT group. In conclusion, this study suggested that palm oil TRF was effective in preventing brain damage in rats.

    Introduction: Self-awareness and reflective-skill are important components of personal and professional development of medical students.

    Objective: The objective of this study was to determine whether self awareness and reflective skill could be improved by direct teaching intervention.

    Method: This was a cross sectional intervention study conducted among 75 third year medical students of UKM Medical Centre Malaysia in 2010. Malaysian Emotional Quotient Inventory (MEQI) was used to measure self-awareness and reflective writing scripts were used to measure the reflective skills of the respondents before and after teaching intervention. Experimental group comprised of 30 students while the control group constituted the rest 45 students selected randomly. The factors contributing to self awareness with gender and learning styles were determined.

    Result: There was no significant improvement observed in self-awareness of the respondents but reflective skills was significantly improved (p=0.007) following intervention. A non-significant relationship between self-awareness and gender (p=0.588) as well as self-awareness and learning styles (p=0.435) was also noticed.

    Conclusion: It is concluded that intervention programs improved students' reflective skills but not their self-awareness. Effectiveness of the intervention programme is an important factor. Continuous effort should be focused to train students in an effective way in order to be more self-reflective, self-regulative as well as self-corrective, to ensure a high level of personal and professional development in their daily practices.

    Cell mediated immunity is currently thought to be involved in the pathogenesis of oral mucosal lichen planus (OMLP). However, literature reveals there is no large scale data of immunohistochemistry (IHC) study on these immune cell populations. The aim of this study was to assess and compare immune cell surface identification markers CD3, CD4, CD8, CD19 and CD83 between the OMLP (n=40) and non-specific inflammatory lesions (as control group) (n=10) qualitatively and quantitatively. Kruskal-Wallis and Mann Whitney U tests have been used to make comparison between the test and control group, p values of less than 0.05 was considered to be statistically significant. T cell surface markers (CD3+, CD4+ and CD8+), B cells (CD19+) and mature dendritic cells (CD83+) showed intense immunostaining in OMLP tissues with a significantly higher expression of positive cells than in the control group (p<0.05). CD3, CD4 and CD8+ve T cells were the predominant inflammatory cell type in OMLP rather than CD19+ B cells, supporting the role of Th1 cells in the pathogenesis of OMLP. CD83+ mature dendritic cells were present in the least number and were mostly localized to areas where there were aggregates of lymphocyte. There was a positive correlation and direct relationship between T and B lymphocyte subsets whereby as one subset increased, the other follows.

    Background: Periodontitis is a public health concern since it is a major factor in tooth loss worldwide and has association with many systemic diseases. Sleep is a complex and essentially biological process and a critical factor for maintaining mental and physical health. Since inflammation is characteristic of both chronic periodontitis and sleep deprivation, few studies in recent years present the contradictory results regarding this potential association. The objective of the present study was to investigate the association between quality of sleep and chronic periodontitis.

    Materials and Methods: A total of 200 individuals participated in this study. All participants underwent a comprehensive clinical periodontal examination. Case-control were identified using the Centers for Disease Control and Prevention/American Academy of Periodontology case definitions for periodontal disease. The quality of sleep was assessed by Pittsburgh Sleep Quality Index. The univariate and multivariate logistic regression analysis was used to test the influence of variables (quality of sleep, age, sex, ethnicity, education, and socioeconomic status), in the occurrence of periodontitis. Odds ratio (OR) and respective confidence intervals (CIs) were calculated and reported. P =0.05 was considered statistically significant.

    Results: The prevalence of poor quality of sleep was 56.75% in cases (periodontitis group) and 43.24% in control group. There was positive association between quality of sleep and chronic periodontitis (OR = 3.04; 95% CI = 1.42-6.5; P = 0.004). In multivariate logistic regression analysis, only the age was significantly related to the periodontitis (OR = 1.11; 95% CI = 1.07-1.41; P < 0.001), other variables failed to reach the significant level.

    Conclusion: Poor quality of sleep was significantly associated with chronic periodontitis. Only the age was significantly related to periodontitis among the other covariable measured.
